Epidemiological and Computed Tomography Aspects of Facial Trauma at Kira Hospital in Bujumbura

Background: Maxillofacial trauma affects young adults more. The injury assessment is difficult to establish in low-income countries because of the imaging means, particularly the scanner, which is poorly available and less financially accessible. The aim of this study is to describe the epidemiological profile and the various tomodensitometric aspects of traumatic lesions of the face in patients received in the Radiology department of Kira Hospital. Patients and methods: This is a descriptive retrospective study involving 104 patients of all ages over a period of 2 years from December 2018 to November 2019 in the medical imaging department of KIRA HOSPITAL. We included in our study any patient having undergone a CT scan of the head and presenting at least one lesion of the facial mass, whether associated with other cranioencephalic lesions. Results: Among the 384 patients received for head trauma, 104 patients (27.1% of cases) presented facial damage. The average age of our patients was 32.02 years with extremes of 8 months and 79 years. In our study, 87 of the patients (83.6%) were male. The road accident was the circumstance in which facial trauma occurred in 79 patients (76% of cases). These injuries were accompanied by at least one bone fracture in 97 patients (93.3%). Patients with fractures of more than 3 facial bones accounted for 40.2% of cases and those with fractures of 2 to 3 bones accounted for 44.6% of cases. The midface was the site of the fracture in 85 patients (87.6% of cases). Orbital wall fractures were noted in 57 patients (58.8% of cases) and the jawbone was the site of a fracture in 50 patients (51.5% of cases). In the vault, the fractures involved the extra-facial frontal bone (36.1% of cases) and temporal bone (18.6% of cases). Cerebral contusion was noted in 41.2% of patients and pneumoencephaly in 15.5% of patients. Extradural hematoma was present in 16 patients and subdural hematoma affected 13 patients. Background: Abdominal parietal hernia, a temporary or permanent exit of viscera through an anatomically pre-existing zone of weakness, is a frequent pathology in surgery. So, the management of emergency hernias surgery should include some complications most often up after 30 days of the operation. Aim: To analyze the factors contributing to morbidity and mortality after 30 days of emergency hernia surgery in adults in the surgical departments of Bujumbura hospitals. Methodology: This is a prospective study over a period of one year that included all hernias operated on in emergency from January 2022 to February 2023. Results: During the period, 251 patients were admitted to the operating room for abdominal parietal hernias, including 49 for emergency hernia surgery. There were 43 men (87.76%) and 6 women (12.24%), i.e. a sex ratio of 7.1. The average age was 49.6 years, with extremes of 18 and 84 years. The occupation of strength (farmer, labourer, mechanic, mason, mason’s helper) represented 75.51% of the cases. Inguino-scrotal hernia was preponderant (65.31%) followed by inguinal hernia (25.58%), umbilical hernia (4.08%);femoral hernia represented 4.08%. Hernial strangulation represented 89.80% and engorged hernia 10.20%. Morbidity was minor, 2.04% of complications (suppuration, hematoma, urinary retention). No deaths were found. Altemeir stage and occupation were statistically related to morbi-mortality of emergency hernia surgery in adults at 30 days postoperative (p = 0.0028 and p = 0.0284 respectively). Conclusion: Abdominal parietal hernias are frequent, dominated by groin hernias. The high frequency of strangulation calls for awareness of cold hernia cures.

Characterisation of Human-Wildlife Conflicts around the Kahuzi-Biega National Park, Eastern Democratic Republic of Congo

The history of mankind contains so many illustrations of well-classified and violent struggles against various animals over crop production in agricultural fields, attacks on human life or competition in order to have access to certain natural resources. The aim of this study is to characterise the human-wildlife conflicts that occur around the KBNP, especially with regard to primates. Surveys were carried out in 260 households selected using the snowball method. The results showed that human-wildlife conflicts around the KBNP are characterised by the destruction of crops in riparian fields by monkeys, chimpanzees and gorillas, the destruction of houses, physical attacks and zoonoses. Despite the conservation and protection measures for wildlife and local populations put in place by the Park’s managers, conflicts between local populations and the Park’s wild animals are still visible. These conflicts lead to the detention of wild animals by local people roaming in the villages and to poaching on the Park’s boundaries. The weakness of the community management policy on the part of the park managers and the resentment of the indigenous peoples towards the restriction of their access to natural resources are the major constraints on the sustainable management of conflicts between the local populations and the wild animals in the KBNP. The strategies for the sustainable resolution of human-wildlife conflicts around the KBNP must be geared towards and integrated into a community conservation approach.

Objective. To determine the prevalence of HR- HPV DNA in ASC- US Pap smears following implementation of the Bethesda 2001 classification system. Methods. A computer database of Pap smears obtained within Department of the Army medical facilities was queried for the study period August 2002 to June 2004. All ASC- US Pap smears that underwent reflex testing for HR- HPV DNA were included. Additional clinical and demographic data were obtained from facilities within the US northeast region to evaluate the differences in ASC- US and SIL rates between the current and former Bethesda classification systems. Results. 550,000 Pap smears were collected during the study period. The HR- HPV prevalence was 40.8% (95% confidence interval [CI] = 40.3 to 41.3) among 40,870 patients with ASC- US Pap smears. Within the northeast region, the HR- HPV prevalence in ASC- US Pap smears decreased from 61.2% (95% CI = 57.4 to 64.8% ) in patients 18- 22 years old to 24.9% (95% CI = 23.1 to 26.8% ) in patients age 29 and older. When comparing the two classification systems, significant increases in both ASC- H and SIL and decreases in ASC- US were appreciated after the institution of Bethesda system 2001. Conclusion. In our large, diverse cohort, the implementation of the Bethesda II system has resulted in a decrease in ASC- US Pap smear results. Additionally, the prevalence of HR- HPV in theASC- US population was 40.8% , significantly lower than the rate noted in the ALTS trial under the Bethesda I classification system.
