Chronic methadone use,poor bowel visualization and failed colonoscopy:A preliminary study

AIM:To examine effects of chronic methadone usage on bowel visualization,preparation,and repeat ***:In-patient colonoscopy reports from October,2004 to May,2009 for methadone dependent(MD) patients were retrospectively evaluated and compared to matched opioid naive controls(C).Strict criteria were applied to exclude patients with risk factors known to cause constipation or gastric *** reports of all eligible patients were analyzed for degree of bowel visualization,assessment of bowel preparation(good,fair,or poor),and whether a repeat colonoscopy was *** visualization was scored on a 4 point scale based on multiple prior studies:excellent = 1,good = 2,fair = 3,or poor = *** of variance(ANOVA) and Pearson χ 2 test were used for data *** analysis included correlation between methadone dose and colonoscopy *** variables significantly differing between MD and C groups were included in both univariate and multivariate logistic regression analyses.P values were two sided,and < 0.05 were considered statistically ***:After applying exclusionary criteria,a total of 178 MD patients and 115 C patients underwent a colonoscopy during the designated study period.A total of 67 colonoscopy reports for MD patients and 72 for C were included for data *** and gender matched controls were randomly selected from this population to serve as controls in a numerically comparable *** average age for MD patients was 52.2 ± 9.2 years(range:32-72 years) years compared to 54.6 ± 15.5 years(range:20-81 years) for C(P = 0.27).Sixty nine percent of patients in MD and 65% in C group were males(P = 0.67).When evaluating colonoscopy reports for bowel visualization,MD patients had significantly greater percentage of solid stool(i.e.,poor visualization) compared to C(40.3% vs 6.9%,P < 0.001).Poor bowel preparation(35.8% vs 9.7%,P < 0.001) and need for repeat colonoscopy(32.8% vs 12.5%,P = 0.004)

Hydrodynamics of gas-solid fluidization of a homogeneous ternary mixture in a conical bed:Prediction of bed expansion and bed fluctuation ratios

Hydrodynamic characteristics of fluidization in a conical or tapered bed differ from those in a columnar bed because the superficial velocity in the bed varies in the axial direction. Fixed and fluidized regions could coexist and sharp variations in pressure drop could occur, thereby giving rise to a noticeable pressure drop-flow rate hysteresis loop under incipient fluidization conditions. To explore these unique properties, several experiments were carried out using homogeneous, well-mixed, ternary mixtures with three dif- ferent particle sizes at varying composition in gas-solid conical fluidized beds with varying cone angles. The hydrodynamic characteristics determined include the minimum fluidization velocity, bed fluctuation, and bed expansion ratios. The dependence of these quantities on average particle diameter, mass fraction of the fines in the mixture, initial static bed height, and cone angle is discussed. Based on dimensional analysis and factorial design, correlations are developed using the system parameters, i.e. geometry of the bed （cone angle）, particle diameter, initial static bed height, density of the solid, and superficial velocity of the fluidizing medium. Experimental values of minimum fluidization velocity, bed fluctuation, and bed expansion ratios were found to agree well with the developed correlations.

Testing the intraplate origin of mega-earthquakes at subduction margins

The disastrous Mw 9.3 （seismic moment 1.0 × 10^30 dyn/cm） earthquake that struck northwest Sumatra on 26 December 2004 and triggered -30 m high tsunami has rejuvenated the quest for identi- fying the forcing behind subduction related earthquakes around the world. Studies reveal that the stron- gest part （elastic core） of the oceanic lithosphere lie between 20 and 60 km depth beneath the upper （- 7 km thick） crustal layer, and compressive stress of GPa order is required to fail the rock-layers within the core zone. Here we present evidences in favor of an intraplate origin of mega-earthquakes right within the strong core part （at the interface of semi-brittle and brittle zone）, and propose an alternate model exploring the flexing zone of the descending lithosphere as the nodal area for major stress accumulation. We believe that at high confining pressure and elevated temperature, unidirectional cyclic compressive stress loading in the flexing zone results in an increase of material yield strength through strain hardening, which transforms the rheology of the layer from semi-brittle to near-brittle state. The increased compres- sive stress field coupled with upward migration of the neutral surface （of zero stress fields） under non- coaxial deformation triggers shear crack. The growth of the shear crack is initially confined in the near-brittle domain, and propagates later through the more brittle crustal part of the descending oceanic lithosphere in the form of cataclastic failure,

Diagnostic role of Xpert-MTB RIF assay in osteoarticular tuberculosis:A retrospective study

BACKGROUND Osteoarticular tuberculosis(OATB) is a severe form of extrapulmonary TB, which causes notable morbidity and warrants a high index of suspicion for prompt management. The diagnosis of OATB poses a challenge, because of the difficulty of collecting the samples and, secondarily, the paucibacillary nature of lesion, which gives poor sensitivity and reproducibility, with long turnaround time of routine/conventional laboratory tests and the requirement for invasive procedures and expertise. The Xpert MTB/RIF assay has been approved by the World Health Organization as a rapid diagnostic tool for diagnosing pulmonary and extrapulmonary *** To emphasize the diagnostic efficiency of gene Xpert for OATB in suspected patients in a tertiary care hospital of Eastern *** This retrospective study was conducted in the Department of Microbiology and Orthopaedics by analyzing the data of the gene Xpert assay over a 3-year duration from January 2018 to February 2021. Demographic and clinical data were recorded. The diagnostic efficiency of gene Xpert was evaluated against the composite reference standard(CRS).RESULTS A total of 37 cases fell into positive, probable, and possible categories of osteoarticular TB out of 112 patients included in the study by CRS;gene Xpert result was positive in 35 out of the 37 different CRS categorized cases. Of the 112 cases, culture was put in 40 cases, and, of these cultures, 5 cases showed the growth of MTB. Of these, 4 cases were included in the 35 cases diagnosed by gene *** microscopy was positive in 6 out of 37 CRS categorized cases. When compared with CRS, the sensitivity of gene Xpert assay, culture, and smear was found to be 94.6%, 13.5%, and 16.2%, respectively, while specificity in all the three types of tests was 100%. Statistical modeling and optimization of a multistage gas-solid fluidized bed for removing pollutants from flue gases

The present paper describes the statistical modeling and optimization of a multistage gas-solid fluidized bed reactor for the control of hazardous pollutants in flue gas. In this work, we study the hydrodynamics of the pressure drop and minimum fluidization velocity. The hydrodynamics of a three-stage fluidized bed are then compared with those for a single-stage unit. It is observed that the total pressure drop over all stages of the three-stage fluidized bed is less than that of an identical single-stage system. However, the minimum fluidization velocity is higher in the single-stage unit. Under identical conditions, the minimum fluidization velocity is highest in the top bed, and lowest in the bottom bed. This signifies that the behavior of solids changes from a well-mixed flow to a plug-flow, with intermediate behavior in the middle bed.
