Prevalence of HIV in Hospitalized Patients with Venous Thrombosis of the Lower Limbs to the Abidjan Cardiological Institute

Background: The diagnosis of deep vein thrombosis (DVT) requires an etiological research of HIV infection. The objective was to identify the characteristics of patients with DVT of limbs without other risk factors in our context. Methods: We performed a comparative retrospective study from January 2005 to December 2012. We identified 162 cases of patients hospitalized in Medicine Department of Institute of Cardiology of Abidjan with 124 HIV-negative and 38 HIV positive patients. Results: DVT is more common in HIV positive young patients (57.8 ± 15.6 years vs 39.3 ± 10.6 years, p = 0.0001). The traditional risk factors were found in HIV negative patients. HIV positive patients had no predisposing factor for thrombosis. The ankle-femoral popliteal location (29% vs 73.7% p = 0.05) was most frequent in HIV positive patients. There was no significant difference in anticoagulant therapy: UFH (60.5% vs 52.6%;p > 0.05), LMWH (20.2% vs 7.9%;p > 0.05), AVk relay (99.2% vs 100%;p > 0.05) and general measures (elevation MI). (70.2% vs 65.8%;p > 0.05). None of the patients in both groups had worn stockings in hospitalization. The stockings were prescribed on discharge (70% vs 64.7%;p > 0.05). Conclusion: DVT may be the mode of revelation of HIV infection. The etiological research with HIV infection should be systematic in?young patients suffering from DVT in the absence of risk factors of thrombosis.

Despite the efforts made by the Malian government with technical and financial partners in the fight against HIV and AIDS, there are no data on the evolution of people with HIV in medical consultation in antiretroviral treatment sites. The objective of this work is to fill this gap by generating factual data describing the evolution of the number of people with HIV in medical consultation in Mali. To achieve this goal, we conducted a cross-sectional study using data from the Coordination Unit of the Sectoral Committee for AIDS Control of the Ministry of Health and Public Hygiene. Sampling was comprehensive including all people living with HIV. The data collected concerned those of the years 1999 to 2014 by the ESOPE software and analyzed with Stata version 12. A Total of 39,619 people were enrolled in the study. And 26,212 people living with HIV in the 17 ESOPE sites of Mali were female. Patients aged 25-34 and 35-44 years were strongly represented in consultation in the years 2007 and 2011, and those aged 15-24 were the least frequent. HIV1 was the most dominant with 97%. The majority of patients presented for initial consultation were clinical stages 2 and 3 in all years. Cases of lost sites seen in the more common in hospitals are 62.79% at the Hospital of Mopti and 57% at the University Hospital of Point G. The number of cases of people living with HIV in medical consultation has progressively augmented from 1999-2007. Then it dropped from 2008-2010. After this number has increased by 2011, it dropped again in 2012-2014. This study made it possible to note the evolution of the patients in medical consultation in ESOPE antiretroviral treatment sites according to the years and events (safe, subsidized by activities by the global fund). The activities of sensitization and communications, case research should be strengthened by involving the community.

针对微小钻头工作时容易折断的问题,依据转子动力学理论,利用Timoshenko梁-轴单元,建立了微孔钻床主轴系统的动力学模型。该模型考虑了钻头在切削时主轴-钻夹头-微钻头系统的偏心、钻削轴向力、系统转动惯性力、陀螺力矩和轴承等因素对微弯曲变形的影响。利用该模型确定了钻头的危险截面,分析了钻头夹持长度、轴承刚度、轴承阻尼、主轴转速、系统偏心、钻削轴向力以及钻削扭矩等因素对危险截面应力的影响。最后提出了避免微小钻头折断的措施,可用于指导微小孔钻床的设计和生产实践。

Effect of leachate recycle and inoculation on microbial characteristics of municipal refuse in landfill bioreactors

Population development of key groups of anaerobic and aerobic bacteria involved in municipal refuse decomposition under laboratory landfill bioreactors with and without leachate recycle and inoculation was measured since modeling municipal refuse was landfilled in bioreactors for about 210 days. Hydrolytic fermentative bacteria (HFB), hydrogen-producing acetogenic bacteria (HPAB), methane-producing bacteria (MPB), sulfate-reducing bacteria (SRB), anaerobic and aerobic cellulolytic bacteria and denitrabacteria were enumerated by the most probable number technique. The results showed that the dominant microorganism groups were the methanogenic bacteria including hydrolytic fermentative, hydrogen-producing acetogenic and methane-producing bacteria. They were present in fresh refuse but at low values and positively affected by leachate recycle and refuse inoculation. The amounts of HFB or HPAB in digesters D4 and D5 operated with inoculation and leachate recycle reached their maximum values of 1010 - 1012 cells/g dry refuse for HFB or 105 - 106 cells/g dry refuse for HPAB on day 60, in digester D3 operated with leachate recycle on day 120 for HFB (109 cells/g dry refuse) or on day 90 for HPAB (105 cells/g dry refuse), and in digesters D1 and D2 on day 210 for HFB (109 cells/g dry refuse) or on day 90 for HPAB (104 - 106 cells/g dry refuse). The population of methane-producing bacteria in digesters D4 and D5 sharply increased on days 60 and 90 respectively, however in digesters D1, D2 and D3 on day 120. Leachate recycle and inoculation changed the cellulolytic microorganisms composition of refuse ecosystem, the higher amounts of anaerobic cellulolytic bacteria were measured in digesters D4 and D5 (107 cells/g dry refuse), followed by digesters D3 (106 cells/g dry refuse), D2 or D1 (104 cells/g dry refuse). Supracondylar fractures of the humerus are the most common traumatic elbow injury in children. Several techniques are described in their support. Blount’s method is one of the widely used orthopedic means. The objective of our study was to evaluate the results of supracondylar fractures of the humerus treated by this method in our department. This was a descriptive, longitudinal study with the prospective collection over a period of 12 months from May 2018 to April 2019. It focused on children aged 0 to 15 years treated in the Orthopedic Traumatology Department of the CHU of kati for a supracondylar fracture of the humerus in extension. A minimum follow-up period of 6 months was necessary for inclusion. 12 patients were collected. The sociodemographic, anatomopathologic, therapeutic and evolutionary aspects were studied. The results were evaluated according to Flynn’s criteria. The male sex was predominant with a ratio of 2. The average age was 5.5 years. The average admission time was 12.5 hours. Domestic accidents were the main etiology (66.66%) and Type II fracture was the most frequent with 58.33%. The duration of immobilization was 42 days for 11 patients. Complications were 1 case of secondary displacement and 1 case of 12˚ flexion deficit. The average Baumann angle was 72˚. The result was satisfactory for all our patients. Supracondylar fractures of the humerus are common in children, especially boys. The method of Blount when it is well carried out by respecting the indications gives good results.

Outcomes of Surgical Treatment of Skin Cancer at Surgical Oncology Unit of Donka, Conakry University Hospital

Aim: The main aim of this study was to evaluate outcomes after surgical treatment of skin cancer. Methods: This retrospective cohort study concerned 46 patients who presented 43 skin cancers lesions surgically treated at surgical oncology unit, Conakry University Hospital. There were 29 (61.7%) squamous cell carcinoma, 12 (25.5%) melanoma, 4 (8.5%) sarcomas and 1 (2.1%) porocarcinoma. Surgical indications and prognosis factors were analyzed. Results: Surgical treatment included simple excision in 2 cases (3.8%), wide excision in 38 cases (71.7%) and amputation/disarticulation in 13 cases (24.5%). Inguinal lymph node dissection was performed in 16 patients (34.3%). Postoperative complications were wound suppuration (5 cases), lower limb lymphedema (4 cases), seroma (2 cases) and skin flap necrosis (1 case). Surgical margins were free in 28 (60.9%) patients, infiltrated in 3 patients (6.5%) and unspecified in 15 patients (32.6%). The median follow-up after surgery was 29 months. During the follow-up, 13 patients (28.3%) had a relapse. The relapse was influenced by surgical margins (p = 0.012) and iterative resection (p = 0.04). Overall survival was 65.2%. Factors related to survival in univariate analysis were: iterative resection (p = 0.008), fungated tumor (p = 0.037), the status of surgical margins (p = 0.002) and the occurrence of relapse (p = 0.0000). In multivariate analysis, the status of surgical margins was the only independent prognostic factor. Conclusion: The prognosis after surgical treatment of cutaneous cancers depends on the resection margins.

Introduction: A fracture is a solution in the continuity of a bone. Pelvic limb fractures may involve one or more of the bones. They constitute a real public health problem, which requires the identification of the factors inherent to this phenomenon for better prevention, but also for quality management of fractures and sequelae. Objectives: Were to describe the epidemiological, clinical and therapeutic profile of pelvic limb fractures in Timbuktu Hospital. Patients and Method: We conducted a prospective, descriptive study in the Surgery Department of Timbuktu Hospital, covering a period of one year from January 1 to December 31, 2017. We collected 39 patients who presented with fractures of the pelvic limbs, who were hospitalized and monitored throughout our study. Results: We obtained a hospital frequency of 2.86% limb fractures. Among the 39 patients included in our study, the male sex was predominant in 69.2% of cases, the average age of our patients was 20.5 years. Pupils and students were in the majority in 48.7% of cases. Road traffic accidents were the most common cause of fracture with 59.0% of cases. The tibia was the most affected segment in 38.5% of cases. Surgical treatment was predominant in 64.0% of cases. We obtained very good results in 94.87% of cases. Conclusion: Limb fractures remain frequent due to road traffic accidents. Osteosynthesis treatment provides a good result with fewer complications.

Situs inversus is a rare congenital anomaly referring to the mirror image of the abdominal and thoracic viscera. It can be discovered routinely or on the occasion of a complication related or not to situs inversus. We report a case of Situs inversus discovered incidentally during surgery for acute intestinal obstruction caused by an iatrogenic uterine perforation.

Introduction: Appendicular abscess is a progressive complication of acute appendicitis in which the spread of infection is contained by the greater omentum and the slender loops resulting in the formation of a true septate abscess of the large peritoneal cavity. Materials and Methods: This was a retrospective study from January 2010 to December 2019 carried out at the Bocar Sidi Sall University Hospital in kati (CHU BSS in kati) in general surgery. It concerned all patients operated on for appendicular abscess in the department. Results: 75 cases of appendicular abscess were collected, which represented 5.76% of surgical emergencies and 25% of acute appendicitis. The average age of the patients was 29 years. The male sex represented 67% of our patients with a sex ratio of 2. The average consultation time was 5 days. Abdominal pain was noted in all patients. It was localized in the right iliac fossa in 80% (n = 60) and diffuse in 2.7% of cases (n = 2). It was accompanied by nausea and vomiting in 93% of cases (n = 70), urinary disorders in 20% (n = 15), fever in 94% (n = 71), cessation of materials and gas in 1.33% (n = 1). Abdominal ultrasound was performed in 86% (n = 65). It made it possible to suggest a peri-appendicular effusion. Biological examination revealed a neutrophilic hyperleukocytosis greater than 20,000/mm3 in 47 patients, or 63%. All the patients were operated on by laparotomy (Marc Burney or midline subumbilical) under general anesthesia. The length of hospitalization was 6 days. We have not recorded any deaths. Morbidity was 8% (n = 6) represented by parietal suppuration. The postoperative course was straightforward in 92% of cases (n = 69). Conclusion: Appendicular abscess is a frequent medico-surgical emergency, the prognosis of which depends greatly on early diagnosis and adequate and immediate management.
