The efficacy of *** adjuvant chemotherapy on biliary tract cancer after R0 resection

Background: The optimal treatment strategy for biliary tract cancer(BTC) after curative?intent resection remains con?troversial. The purpose of this study was to evaluate the efficacy of fluoropyrimidine?based adjuvant chemotherapy for BTC patients undergoing microscopically margin?negative(R0) ***: We retrospectively analyzed the clinical data of BTC patients who underwent curative?intent R0 resection. Patients were eligible if they received either fluoropyrimidine?based adjuvant chemotherapy or observation after R0 ***: A total of 153 patients were included. In the entire patient cohort, no significant differences were observed in 5?year overall survival(OS) rates(48.4% vs. 39.6%, P = 0.439) or 3?year recurrence?free survival(RFS) rates(49.1% vs. 39.5%, P = 0.299) between patients who received fluoropyrimidine?based adjuvant chemotherapy or observation. However, for patients with stages Ⅱ and Ⅲ BTC, chemotherapy significantly improved 5?year OS rate(52.4% vs. 35.6%, P = 0.002) and 3?year RFS rate(55.5% vs. 39.1%, P = 0.021) compared with ***: Fluoropyrimidine?based adjuvant chemotherapy may prolong the survival of patients with stages Ⅱ and Ⅲ BTC after R0 resection.

Dietary composition of birds in isolated ecosystems:a case study of Black Wood Pigeon in Republic of Korea

We studied the feeding ecology of the Black Wood Pigeon(Columba janthina),a species endemic to the East Asian Pacific Islands,and determined the species’dietary composition and food preferences on Ulleung Island,South Korea,through field *** March 2016 to February 2018,the diversity of food items consumed was low,with the 10 most common plant species(from a total of 33)accounting for over 80%.Food availability varied depending on plant abundance,but this influence was minimized because of factors such as delayed fruit *** were the most favored food items(60-68%)but were replaced by nuts and weed seeds during the spring and winter *** composition and food preferences were related to the abundance of food items based on their phenology,but a steady,high preference for Aphananthe aspera was observed throughout the study *** abundance of Prunus takesimensis in June was correlated with changes in the Black Wood Pigeon *** study is the first to assess the feeding ecology of Black Wood Pigeons using field observations and suggests that the availability of different food items affects population changes on the East Asian Pacific islands.

Pretransplant absolute monocyte count in peripheral blood predicts posttransplant tumor prognosis in patients undergoing liver transplantation for hepatocellular carcinoma

BACKGROUND: Preoperative absolute monocyte count in peripheral blood(AMCPB) is closely associated with prognoses in not only various malignancies but also hepatocellular carcinoma(HCC). The purpose of this study was to evaluate whether pretransplant AMCPB predicts posttransplant outcomes in patients with HCC undergoing liver transplantation(LT).METHOD: We retrospectively analyzed relationships between clinicopathologic factors involving pretransplant AMCPB and tumor recurrence or survival in 256 patients who had undergone LT for HCC between january 2005 and April ***: ROC curve analysis showed that AMCPB >200/mm3was a risk factor for tumor recurrence; 43 patients showed higher AMCPB(>200/mm3), whereas 213 showed lower AMCPB(≤200/mm3) at the time of LT. On multivariate analysis,pretransplant high AMCPB, positive findings in pretransplant18F-FDG PET/CT, pathological maximal tumor size >5 cm,intrahepatic metastasis, moderately or poorly differentiated tumor and microvascular invasion were independent factors affecting recurrence-free survival. When we performed subgroup analysis based on the Milan criteria, high AMCPB was an independent factor for predicting HCC recurrence in patients with tumor beyond the Milan criteria(P=0.004), and not for patients within the ***: This study demonstrated that pretransplant AMCPB could predict tumor recurrence after LT for HCC,especially in patients with tumor beyond the Milan criteria.

Selecting and applying quantification models for ecosystem services to forest ecosystems in South Korea

There is growing interest in using ecosystem services to aid development of management strategies that target sustainability and enhance ecosystem support to humans. Challenges remain in the search for methods and indicators that can quantify ecosystem services using metrics that are meaningful in light of their high priorities. We developed a framework to link ecosystems to human wellbeing based on a stepwise approach. We evaluated prospective models in terms of their capacity to quantify national ecosystem services of forests. The most applicable models were subsequently used to quantify ecosystem services. The Korea Forest Research Institute model sat- isfied all criteria in its first practical use. A total of 12 key ecosystem services were identified. For our case study, we quantified four ecosystem functions, viz. water storage capacity in forest soil for water storage service, reduced suspended sediment for water purification service, reduced soil erosion for landslide prevention service, and reduced sediment yield for sediment regulation service. Water storage capacity in forest soil was estimated at 2142 t/ha, and reduced suspended sediment was estimated at 608 kg/ ha. Reduced soil erosion was estimated at 77 m^3/ha, and reduced sediment yield was estimated at 285 m^3/ha. These results were similar to those reported by previous studies. Mapped results revealed hotspots of ecosystem services around protected areas that were particularly rich in bio- diversity. In addition, the proposed framework illustrated that quantification of ecosystem services could be sup- ported by the spatial flow of ecosystem services. However, our approach did not address challenges faced when quantifying connections between ecosystem indicators and actual benefits of services described.

A roller rack pinion(RRP)system,which consists of a rack-bar and a cam pinion,transforms a rotation motion into linear *** rack-bar has a series of roller train and meshes with the cam ***,the exact tooth profile of the cam pinion and the non-undercut condition to satisfy the required performance have been proposed with the introduction of the profile shift ***,the load stress factors are investigated under the variation of the shape design parameters to predict the gear surface fatigue limit which is strongly related to the gear noise and vibration at the contact *** results show that the pitting life can be extended significantly with the increase of the profile shift coefficient.

Decadal changes in the subseasonal evolution and the phase-locked climatological intraseasonal fluctuation of summertime rainfall over the Korean Peninsula before and after the mid-1990s are *** activity and the migration speed of the monsoon rain band over the East Asian region are altered in the recent decade,resulting in the drier conditions in late spring and the earlier onset of *** early August when a climatological monsoon break was clear in the earlier decade,the precipitation has increased dramatically with a meridional *** response to the enhanced convection over the South China Sea and southeastern China provides a favorable condition for more precipitation in early August through the changes in moisture transport and tropical cyclone passage.

Graft-to-recipient weight ratio lower to 0.7% is safe without portal pressure modulation in right-lobe living donor liver transplantation with favorable conditions

BACKGROUND: The low graft-to-recipient weight ratio(GRWR) in adult-to-adult living donor liver transplantation(LDLT) is one of the major risk factors affecting graft survival. The goal of this study was to evaluate whether the lower limit of the GRWR can be safely reduced without portal pressure modulation in right-lobe LDLT. METHODS: From 2005 to 2011, 317 consecutive patients from a single institute underwent LDLT with right-lobe grafts without portal pressure modulation. Of these, 23 had a GRWR of less than 0.7%(group A), 27 had a GRWR of ≥0.7%, <0.8%(group B), and 267 had a GRWR of more than and equal to 0.8%(group C). Medical records, including recipient, donor, operation factors, laboratory findings and complications were reviewed retrospectively. RESULTS: The baseline demographics showed low model for end-stage liver disease score(mean 16.3±8.9) and high percentage of hepatocellular carcinoma(231 patients, 72.9%). Three groups by GRWR demonstrated similar characteristics except recipient body mass index and donor gender. For smallforsize syndrome, there were 3(13.0%) in group A, 1(3.7%) in group B, and 2 patients(0.7%) in group C(P<0.001). Hepatic artery thrombosis was more frequently observed in group A than in groups B and C(8.7% vs 3.7% vs 1.9%, P=0.047). However, among the three groups, graft survival rates at 1 year(100% vs 96.3% vs 93.6%) and 3 years(91.7% vs 73.2% vs 88.1%) were not different(P=0.539). In laboratory measurements,there was no group difference in total bilirubin and albumin. However, prothrombin time was longer in group A within postoperative 1 week and platelet count was lower in groups A and B within postoperative 1 month. CONCLUSION: A GRWR lower to 0.7% is safe and does not need to modulate portal pressure in adult-to-adult LDLT using the right-lobe in favorable conditions including low model for end-stage liver disease score.

AIM To investigate the efficacy and safety of endoscopic stent insertion in patients with delayed gastric emptying after *** In this study, we prospectively collected data from patients who underwent stent placement for delayed gastric emptying(DGE) after distal gastrectomy between June 2010 and April 2017, at a tertiary referral academic center. Clinical improvement, complications, and consequences after stent insertion were *** Technical success was achieved in all patients(100%). Early symptom improvement was observed in 15 of 20 patients(75%) and clinical success was achieved in all patients. Mean follow-up period was 1178.3 ± 844.1 d and median stent maintenance period was 51 d(range 6-2114 d). During the follow-up period, inserted stents were passed spontaneously per rectum without any complications in 14 of 20 patients(70%). Symptom improvement was maintained after stent placement without the requirement of any additional intervention in 19 of 20 patients(95%).CONCLUSION Endoscopic stent placement provides prompt relief of obstructive symptoms. Thus, it can be considered an effective and safe salvage technique for post-operative DGE.

Assessing vulnerability of forests to climate change in South Korea

This study demonstrated a framework to assess vulnerability of forests to climate change. We focused on how alterations of temperature and precipitation might affect forest type distributions and carbon-related functions. In particular, our framework considered three sectors of forest type distribution, net primary production, and soil carbon storage. Future projections were derived from mechanistic models for South Korean forests under the A1 B scenarios of the intergovernmental panel on climate change. Forest type distributions were simulated by the Hydrological and thermal analogy group model, while the MAPSS and CENTURY1 models estimated forest carbon flux/storage. We quantified normalized vulnerability indices for each sector. Our results indicate that the overall vulnerability of forest type distribution is greater in the west central regions and southeastern inlands. The vulnerabilities of carbon flux/storage show that net primary production of South Korean forests is relatively less susceptible to climate change, but a highly vulnerable area of soil carbon storage mainly spreads from the west central to the south east region. The spatio-temporal vulnerability map with a synoptic overview from this study might be useful for policy makers in preparing adaptive measures and identifying management priorities.
