Long-Term Outcome and Risk Factor Analysis of Surgical Pulmonary Valve Replacement in Congenital Heart Disease

Objectives:To establish long-term outcome of surgical pulmonary valve replacement(PVR)in congenital heart disease(CHD)and to identify risk factors for overall mortality,operative mortality,and repetitive ***:This is a retrospective study of 375 surgical PVR in 293 patients who underwent surgical PVR for CHD between January 2000 and May *** only included patients with index PVR with previous open-heart surgery regardless of the number of *** previous surgical history of patients who underwent PVR during the study period was also *** who underwent the Rastelli operation,and those who underwent single PVR without previous open-heart surgery were ***:The median age of the patients at the time of surgical PVR was 14.9 years(Interquartile range,IQR,11.0–22.0).The median follow-up duration was 10.5 years(IQR,5.5–14.8 years).There were 3 patients with operative mortality(1.0%)and 15 patients with overall mortality(5.1%).The survival rate was 95.1%over 20 years follow-up *** analysis demonstrated that more than 3 times of previous open-heart surgeries before surgical PVR,older age at the first operation,longer cardiopulmonary bypass(CPB)time and longer intensive care unit(ICU)stay were predictors for overall *** who underwent surgical PVR after more than 3 times of previous open-heart surgeries had significantly higher mortality than those who underwent open-heart surgeries less than 3 times(P<0.001).Age younger than 10 years,male,multiple valve problems and longer ICU stay were significant predictors for repetitive PVR by multivariate ***:Though surgical PVR has excellent long-term outcome,it should be performed with caution for those who previously underwent multiple open-heart surgeries,especially if patient received more than 3 times of open-heart surgeries.

AIM：To investigate the antiangiogenic effects and safety of topically administered low-molecular-weight heparintaurocholate 7（LHT7） on corneal neovascularization（CoNV）.METHODS：Twenty-four Sprague-Dawley rats were randomly distributed into four groups of six rats *** central corneas were cauterized using a silver/potassium nitrate *** 2d after cauterization,12.5 mg/mL（low LHT7 group） or 25 mg/mL（high LHT7group） LHT7 was topically administered three times daily;12.5 mg/mL bevacizumab was topically administered as positive control（bevacizumab） group,with normal saline（NS） administered as negative control（NS group）.The corneas were digitally photographed to calculate the CoNV percentage from the neovascularized corneal area at 1 and ***：The 4 study groups did not have different CoNV percentages at 1wk after injury（P〉0.05）.However,the low LHT,high LHT,and bevacizumab groups had significantly lower CoNV percentages than the NS group at 2wk（all P〈0.05）.No significant differences in CoNV percentage were found among the low LHT,high LHT,and bevacizumab groups（all P〉0.05）.All groups except the NS group had lower CoNV percentages at 2wk postinjury than the levels observed at 1wk（all P〈0.05）.CONCLUSION：Topically-administered LHT7 inhibited CoNV without complication after chemical cauterization in the rat.

AIM:To evaluate the potential of thioredoxin (TXN) and thioredoxin-interacting protein (TXNIP) expression as biomarkers for predicting gastric cancer recurrence. METHODS:TXN and TXNIP expression levels were acquired from gene expression microarray data for 65 human gastric cancer tissues. We determined whether each gene expression level was associated with cancer recurrence and investigated the relationship between the two genes. For validation, the expression levels of TXN and TXNIP were measured by quantitative real- time reverse transcription polymerase chain reaction in 68 independent stage Ⅲ gastric cancer patients. The correlation between gene expression and cancer prognosis was evaluated. Immunohistochemical staining was performed to investigate the protein expression levels of TXN and TXNIP and to characterize the expression patterns of each protein. RESULTS:TXN was a prognosis-related gene (P = 0.009), whereas TXNIP, a TXN inhibitor, demonstrated a negative correlation with TXN in the gene expression microarray data. In the 68 stage Ⅲ patients, the expression levels of both TXN and TXNIP had a statistically significant effect on recurrence-free survival (RFS, P = 0.008 and P = 0.036, respectively). The low TXN and high TXNIP expression group exhibited a better prognosis than the other groups, and the high TXN and low TXNIP expression group exhibited a poorer prognosis (P < 0.001 for RFS and P = 0.001 for overall survival). More than half of the patients in the simulta-neously high TXN and low TXNIP expression group ex- perienced a recurrence within 1 year after curative surgery, and the 5-year survival rate of the patients in this group was 29%, compared with 89% in the low TXN and high TXNIP expression group. The TXN protein was overexpressed in 65% of the gastric cancer tissues, whereas the TXNIP protein was underexpressed in 85% of the cancer cells. In this study, the quality characteristics and residue analysis in circulated LPG fuel were investigated experimentally in Korea. Quality characteristics in circulated LPG fuel were examined with samples of LPG in the supply chain (refinery, petrochemical, and imported LPG), transport, gas stations, and vehicles. The experimental results showed that quality of all circulated LPG was well within the quality standard guideline of LPG in Korea. Especially, it has shown average 13 wt ppm in sulfur content over the full circulated LPG. The residue samples in LPG fuel were extracted on 2 L scale with acetonitrile and analyzed by gas chromatography-mass spectrometry (GC-MS). The components of residues in LPG were composed of 62 organic chemicals with C3 ~ C28 and the main ingredients of residue were plasticizers ((di-octyl phalate (DOP), di-octyl adiphate (DOA) etc.), lubricant oil and amine compounds. It was also showed that mass of residue in vehicles was increasing compared with supply (refinery, petrochemical, and imported LPG). It was presumed that this residue had been originated from automotive LPG fuel, vehicle components, and lubricant oil in infrastructure.

Previous studies have reported decreased cerebral volume as a result of smoking. However, little is known about accompanying changes in ventricular volume for healthy subjects who smoke, although ventricular volume is increased in patients with multiple sclerosis who smoke. The present study analyzed whether cerebral volume decreased with smoking through the use of magnetic resonance imaging. In addition, accompanying changes in ventricular volume that resulted from decreased cerebral volume and smoking were analyzed in healthy subjects. When multivariate analysis of covariance was performed by integrating the 2 age groups, aged 20-28 years and 40-49 years, with statistical significance, results showed that cerebral volume of smokers was smaller and ventricular volume was greater compared with the non-smokers. These findings suggest that ventricular volume changes could be utilized to characterize the effects of smoking.

AIM:To investigate M2 isoform of pyruvate kinase(PKM2) expression in gastric cancers and evaluate its potential as a prognostic biomarker and an anticancer ***:All tissue samples were derived from gastric cancer patients underwent curative gastrectomy as a primary *** and pathological information were obtained from the medical *** expression microarray data from 60 cancer and 19 noncancer gastric tissues were analyzed to evaluate the expression level of PKM2 *** microarrays were constructed from 368 gastric cancer *** was used to measure PKM2 expression and PKM2 positivity of cancer was determined by proportion of PKM2-positive tumor cells and staining *** between PKM2 expression and the clinicopathological factors was evaluated and the correlation between PKM2 and cancer prognosis was ***:PKM2 mRNA levels were increased more than 2-fold in primary gastric cancers compared to adjacent normal tissues from the same patients(log transformed expression level:7.6 ± 0.65 vs 6.3 ± 0.51,P < 0.001).Moreover,differentiated type cancers had significantly higher PKM2 mRNA compared to undifferentiated type cancers(log transformed expression level:7.8 ± 0.70 vs 6.7 ± 0.71,P < 0.001).PKM2 protein was mainly localized in the cytoplasm of primary cancer cells and detected in 144 of 368(39.1%) human gastric cancer ***2 expression was not related with stage(P = 0.811),but strongly correlated with gastric cancer differentiation(P < 0.001).Differentiated type cancers expressed more PKM2 protein than did the undifferentiated *** differentiated adenocarcinoma showed 63.6% PKM2-positive cells;in contrast,signet-ring cell cancers showed only 17.7% PKM2-positive ***,PKM2 expression was correlated with shorter overall survival(P < 0.05) independent of stage only in signet-ring cell ***:PKM2 expression might be an adverse prognostic factor for signet-rin
