AIM: To determine whether gastrointestinal (GI) symptoms are more frequent in type 2 diabetic patients and to examine which diabetic factors are associated with the symptoms. METHODS: Consecutive subjects with diabetes and age-/gender-matched normal controls were recruited for this study. GI symptoms were assessed using a structured questionnaire divided into two GI symptomcategories (upper and lower GI symptoms), and consisting of 11 individual symptoms. In the diabetic patient group, diabetic complications including peripheral neuropathy, nephropathy and retinopathy, glycosylated hemoglobin (HbA1c) level and diabetes duration were evaluated. RESULTS: Among the total 190 diabetic patients and 190 controls enrolled, 137 (72%) of the diabetic patients and 116 (62%) of the controls had GI symptoms. In the diabetic patient group, 83 (43%) had upper GI symptoms and 110 (58%) lower GI symptoms; in the control group, 59 (31%) had upper GI symptoms and 104 (55%) lower GI symptoms. This difference between the two groups was significant for only the upper GI symptoms (P = 0.02). Among the diabetic factors, the HbA1c level was the only independent risk factor for upper GI symptoms in the multiple logistic regression analysis (odds ratio = 2.01, 95% confidence interval: 1.02-3.95). CONCLUSION: Type 2 diabetes was associated with an increased prevalence of upper GI symptoms and these symptoms appeared to be independently linked to poor glycemic control, as measured by the HbA1c levels.

Distant skeletal muscle metastasis from intrahepatic cholangiocarcinoma presenting as Budd-Chiari syndrome

Intrahepatic cholangiocarcinoma is a malignant neoplasm arising from the biliary epithelium, which frequently invades adjacent organs or metastasizes to other visceral organs such as the lungs, bones, adrenals, and brain. However, distant skeletal muscle metastasis of cholangiocarcinoma has never been described before to the best of our knowledge and, furthermore, Budd-Chiari syndrome secondary to intrahepatic cholangiocarcinoma is also extremely rare. Here we present the first case overall of distant muscle metastasis from intrahepatic cholangiocarcinoma presenting as Budd-Chiari syndrome. A 44-year-old man admitted to the hospital with complaints of abdominal distension, edema of both legs, back pain and anorexia of 30 d' duration. Computed tomography and ultrasonography-guided percutaneous muscle biopsy established intrahepatic cholangiocarcinoma with disseminated thrombosis from inferior vena cava to bilateral iliac and femoral veins, and multiple skeletal muscle metastases in bilateral buttock and erector spinal muscle.

Efficacy of hepatic arterial infusion chemotherapy in advanced hepatocellular carcinoma

AIM:To investigate the efficacy of hepatic arterial infusion chemotherapy(HAIC) using floxuridine(FUDR) in patients with advanced hepatocellular carcinoma(HCC) confined to the ***:Thirty-four patients who had advanced HCC with unresectability or unsuccessful previous therapy in the absence of extrahepatic metastasis were treated with intra-arterial FUDR chemotherapy at ourhospital between March 2005 and May *** the 34 patients,9 patients were classified as Child class C,and 18 patients had portal vein tumor thrombus(PVTT).One course of chemotherapy consisted of continuous infusion of FUDR(0.3 mg/kg during day 1-14) and dexamethasone(10 mg on day 1,4,7 and 11),and this treatment was repeated every 28 ***:Two patients(5.9%) displayed a complete response,and 12 patients(35.3%) had a partial *** tumor control rate was 61.8%.The median overall survival times were 15.3 mo,12.4 mo and 4.3 mo for the patients who were classified as Child class A,Child class B and Child class C,respectively(P = 0.0392).The progression-free survival was 12.9 mo,7.7 mo and 2.6 mo for the patients who were classified as Child class A,Child class B and Child class C,respectively(P = 0.0443).The cumulative survival differed significantly according to the Child-Pugh classification and the presence of *** addition to hepatic reserve capacity and PVTT,the extent of HCC was an independent factor in determining a poor *** most common adverse reactions to HAIC were mucositis,diarrhea and peptic ulcer disease,but most of these complications were improved by medical treatment and/or a delay of ***:The present study demonstrates that intra-arterial FUDR chemotherapy is a safe and effective treatment for advanced HCC that is recalcitrant to other therapeutic modalities,even in patients with advanced cirrhosis.

目的以家庭复原力理论为基础探讨复原力因子对住院患儿家庭适应的影响。方法用家庭压力（FSI）、家庭价值观（FW）、家庭强韧性（FHI）、家庭适应量表（FWBI）对中国某8所医院的330位住院患儿的父母进行问卷调查，并对调查结果进行分析。结果多因素回归分析显示，对家庭影响最大的复原力因子是家庭强韧性，其次是家庭价值观。结论当家庭处于危机或压力事件中时，家庭强韧性，家庭价值观可对家庭适应起到保护作用，从而为今后的护理干预研究提供理论基础。

雪莲薯提取物及其成分的促精子发生作用

AIM: Smallanthus sonchifolius (Yacon) has been used as a food and folk medicine in the Andes region. It was screened the pharmacological effects of a 50% ethanol extract of Yacon on spermatogenesis in rats. This study is carried out to evaluate the possibility of using Yacon tubers extracts as a fertility agent to treat infertility caused by spermatogenic disorders. METHODS: The dried, pulverized Yacon tubers were extracted with 50% ethanol at 50 ℃ for 5 h under sonication. The Yacon extracts were analyzed by using HPLC system on a Kromasil C18 column (250 mm×4.6 mm). The Yacon tubers extracts, chlorogenic acid and ferulic acid were administrated to rats once daily by oral gavage. The number of sperm heads in the testis was assessed by counting the number of sperm heads using slight modification of the procedure reported by Toth et al (1989). RESULTS: After administration of chlorogenic acid and Yacon tuber extracts to rat for 5 weeks, the number of sperm in epididymis were increased by 20.33% and 34.48%, respectively. The ferulic acid which was reported as a metabolite of chlorogenic acid and constituent of Yacon to estimate the spermatogenic activity was also administered. After administration of ferulic acid and Yacon tubers extracts, the sperm number were increased by 37.87% and 43.33%, respectively. Spermatogenic effects of chlorogenic acid and ferulic acid were 0.59 times and 0.87 times of Yacon tubers extracts, respectively. CONCLUSION: The spermatogenic effect of Yacon tubers extracts may result from the combined effect of chlorogenic acid or its analogues and their metabolites.
