瞄准: 用放射性免疫测定并且西方的弄污在 IGF-I 和 IGFBP-1 的导致乙醇的分泌物上评估甜菜硷的效果，分别地在主要有教养的老鼠 hepatocytes。方法：从男 Sprague-Dawley 老鼠孤立的 Hepatocytes 与乙醇和 PD98059 过程的各种各样的集中被孵化。hepatocytes 也与甜菜硷的不同剂量被对待(10 (-5), 10 (-4), 并且 10 (-3) mol/L ) 。我们用放射性免疫测定并且分别地的西方的弄污测量了 IGF-I 和 IGFBP-1。结果: IGF-I 分泌物的导致乙醇的抑制被甜菜硷在主要有教养的老鼠 hepatocytes 以一种集中依赖者方式稀释。在 10 点(-3) mol/L，甜菜硷显著地增加了 IGF-I 分泌物，但是减少 IGFBP-1 分泌物。另外， p42/44 激活 mitogen 的蛋白质激酶(MAPK ) 活动与 10 从 10 min 显著地被加速到 5 h 术后疗法(-3) mol/L 甜菜硷。而且，在 IGF-1 和 IGFBP-1 分泌物从增加的导致 betaine 的 p42/44 MAPK 活动导致主要有教养的老鼠 hepatocytes 的变化被处理与 MAPK 禁止者 PD98059 堵住。甜菜硷处理堵住了 IGF-I 的导致乙醇的抑制分泌物和 p42/44 MAPK 活动，和 IGFBP-1 的导致乙醇的增加分泌物。结论：甜菜硷在主要有教养的老鼠 hepatocytes 经由 p42/44 MAPK 的激活调制 IGF-I 和 IGFBP-1 的分泌物。甜菜硷也改变乙醇导致的 MAPK 激活。

Bispectral index monitoring as an adjunct to nurse-administered combined sedation during endoscopic retrograde cholangiopancreatography

AIM:To determine whether bispectral index (BIS) monitoring is useful for propofol administration for deep sedation during endoscopic retrograde cholangiopancreatography (ERCP). METHODS:Fifty-nine consecutive patients with a variety of reasons for ERCP who underwent the procedure at least twice between 1 July 2010 and 30 November 2010. This was a randomized cross-over study, in which each patient underwent ERCP twice, once with BIS monitoring and once with control monitoring. Whether BIS monitoring was done during the first or second ERCP procedure was random. Patients were intermittently administered a mixed regimen including midazolam, pethidine, and propofol by trained nurses. The nurse used a routine practice to monitor sedation using the Modified Observer's Assessment of Alertness/Sedation (MOAA/S) scale or the BIS monitoring. The total amount of midazolam and propofol used and serious side effects were compared between the BIS and control groups. RESULTS:The mean total propofol dose administered was 53.1 ± 32.2 mg in the BIS group and 54.9 ± 30.8 mg in the control group (P = 0.673). The individual propofol dose received per minute during the ERCP procedure was 2.90 ± 1.83 mg/min in the BIS group and 3.44 ± 2.04 mg in the control group (P = 0.103). The median value of the MOAA/S score during the maintenance phase of sedation was comparable for the two groups. The mean BIS values throughout the procedure (from insertion to removal of the endoscope) were 76.5 ± 8.7 for all 59 patients in using the BIS monitor. No significant differences in the frequency of < 80% oxygen saturation, hypotension (< 80 mmHg), or bradycardia (< 50 beats/min) were observed between the two study groups. Four cases of poor cooperation occurred, in which the procedure should be stopped to add the propofol dose. After adding the propofol, the procedure could be conducted successfully (one case in the BIS group, three cases in the control group). Sorafenib,a potent multikinase inhibitor,lead to a significant improvement in progression free survival and overall survival in patients with advanced hepatocellular carcinoma(HCC).Though sorafenib has proven its efficacy in advanced stage HCC,there are limitedreports on the role of sorafenib allowing for curative treatment by down-staging.We herein report a case of advanced HCC with vascular invasion,which showed treatment response by sorafenib therapy as to allow for radiofrequency ablation as curative treatment.The patient was followed-up for 6 mo without recurrence with continued sorafenib therapy.

Hepatocellular adenoma(HCA)is one of the important complications of glycogen storage disease typeⅠa(GSD-Ⅰa)because it can be transformed into hepatocellular carcinoma.Although surgical resection is a standard treatment of choice for solitary HCA,multiple HCAs in GSD-Ⅰa patients present as therapeutic challenges for curative treatment.Therefore,treatment strategy according to malignant potential is important in management of HCAs in GSD-Ⅰa.The authors present a case of histologically proven multiple HCAs withoutβ-catenin mutations occurred in a GSD-Ⅰa patient treated successfully with percutaneous radiofrequency ablation as a minimally invasive therapy.

Background:Administrative database provides valuable information for large cohort studies,especially when tissue diagnosis is rather difficult such as the diagnosis for bile duct cancer(BDC).The aim of this study was to evaluate the diagnostic accuracy of administrative database for BDC by International Classification of Diseases(ICD)-10 codes in a tertiary institute.Methods:BDC and control groups were collected from 2003 to 2016 at Seoul National University Bundang Hospital.Cases of BDC were identified in the National Health Insurance Service(NHIS)database by ICD 10-code supported by V code.The control group was selected from cases without ICD-10 codes for BDC.A definite or possible diagnosis was defined according to pathologic reports.Medical records,images,and pathology reports were analyzed to evaluate ICD-10 codes for BDC.Sensitivity,specificity,positive predictive value,and negative predictive value for BDC were analyzed according to diagnostic criteria and cancer locations.Results:A total of 1707 patients with BDC and 1707 controls were collected.Among those with BDC,1320(77.3%)were diagnosed by definite criteria.Most(99.4%)of them had adenocarcinoma.Rate of definite diagnosis was the highest for ampulla of Vater(88.9%),followed by that for extrahepatic(84.9%)and intrahepatic(68.3%)BDCs.False positive cases commonly had hepatocellular carcinomas.For overall diagnosis of BDC,sensitivity,specificity,positive predictive value,and negative predictive value were 99.94%,98.33%,98.30%,and 99.94%,respectively.Diagnostic accuracies were similar regardless of diagnostic criteria or tumor locations.Conclusions:Administrative database for BDC collected according to ICD-10 code with V code shows good accuracy.

Background In addition to hematopoietic effect, the erythropoietin is known as a multifunctional cytokine with anti-fibrosis and organ-protective activities. The purpose of this study was to evaluate the effect of recombinant human erythropoietin （rhEPO） on hepatic fibrosis and hepatic stellate cells （HSCs）. Methods Carbon tetrachloride （CCI4） induced hepatic fibrosis mice models were used for in vivo study and HSCs line for in vitro study. CCI4 and rhEPO （0,200 or 1000 U/kg） was injected intraperitoneally in BALB/c mice three times a week for 4 weeks. Immunohistochemistry and immunoblotting were performed to evaluate expressions of transforming growth factor-β1 （TGF-β1）, α-smooth muscle actin （α-SMA）, and fibronectin in explanted liver. Immunoblotting of α-SMA, phophorylated Smad-2 and Smad-2/3 was performed in HSCs treated with TGF-β1 and/or rhEPO. Results Expressions of TGF-β1, α-SMA, and fibronectin were increased in CCI4 injected mice livers, but significantly attenuated by co-treatment with CCI4 and rhEPO. Co-treatment of rhEPO markedly suppressed fibrosis in Masson＇s trichrome compared with treatment of only CCI4. TGF-β1 increased phosphorylated α-SMA, Smad-2 expressions in HSCs, which were decreased by rhEPO co-treatment. Conclusions Treatment of rhEPO effectively suppressed fibrosis in CCI4-induced liver fibrosis mice models. Anti-fibrosis effect of rhEPO could be related to inhibition of TGF-β1 induced activation of HSCs.

BACKGROUND Few studies have been conducted on sex differences in the incidence, pathophysiology, and prognosis of gastric cancer(GC).AIM To analyze the differences in GC characteristics according to sex in patients who underwent surgical treatment for GC.METHODS A total of 2983 patients diagnosed with gastric adenocarcinoma who received surgical treatment at the Seoul National University Bundang Hospital between 2003 and 2017 were included.Baseline clinicopathological characteristics, histologic type of GC, overall and GC-specific survival rates, and associated risk factors were analyzed.RESULTS Among the 2983 patients, 2005(67.2%) and 978(32.8%) were males and females, respectively.The average age of the female group(59.36 years) was significantly younger than that of the male group(61.66 years;P younger than 60 years of age, with a significantly higher proportion of diffuse-type cancer than in the male group.Regardless of sex, diffuse-type GC was more common in younger patients, and the proportion of intestinal-type GC increased with age.The overall survival rate was significantly higher in females(P < 0.001).However, this difference disappeared for GC-specific survival(P = 0.168), except for the poor GC-specific survival rate in advanced-stage cancer(stage Ⅲ or above) in females(P = 0.045).The risk factors for GC-related mortality were older age, upper location of GC, and diffuse-or mixed-type histology.In terms of comorbidities, more males died from diseases other than GC, including other malignancies such as lung cancer, hepatocellular carcinoma, and pancreatic cancer, and respiratory diseases such as interstitial lung disease and chronic obstructive pulmonary disease, while there were relatively more cardiovascular or cerebrovascular deaths in females.CONCLUSION Sex-based differences in GC were observed in clinicopathological features, includ

Coordinated morphogenic adaptation of growing plants is critical for their survival and propagation under fluctuating environments.Plant morphogenic responses to light and warm temperatures,termed photomorphogenesis and thermomorphogenesis,respectively,have been extensively studied in recent decades.During photomorphogenesis,plants actively reshape their growth and developmental patterns to cope with changes in light regimes.Accordingly,photomorphogenesis is closely associated with diverse growth hormonal cues.Notably,accumulating evidence indicates that light-directed morphogenesis is profoundly affected by two recently identified phytochemicals,karrikins(KARs)and strigolactones(SLs).KARs and SLs are structurally related butenolides acting as signaling molecules during a variety of developmental steps,including seed germination.Their receptors and signaling mediators have been identified,and associated working mechanisms have been explored using gene-deficient mutants in various plant species.Of particular interest is that the KAR and SL signaling pathways play important roles in environmental responses,among which their linkages with photomorphogenesis are most comprehensively studied during seedling establishment.In this review,we focus on how the phytochemical and light signals converge on the optimization of morphogenic fitness.We also discuss molecular mechanisms underlying the signaling crosstalks with an aim of developing potential ways to improve crop productivity under climate changes.

BACKGROUND Korean National Health Insurance(NHI)claims database provides large-cohort.However,studies regarding accuracy of administrative database for pancreatic cancer(PC)have not been reported.We aimed to identify accuracy of NHI database regarding PC classified by international classification of disease(ICD)-10 codes.AIM To identify the accuracy and usefulness of administrative database in PC and the accurate ICD codes for PC with location.METHODS Study and control groups were collected from 2003 to 2016 at Seoul National University Bundang Hospital.Cases of PC were identified in NHI database by international classification of diseases,10th revision edition(ICD-10 codes)supported with V codes.V code is issued by medical doctors for covering 95%of medical cost by Korean government.According to pathologic reports,definite or possible diagnoses were defined using medical records,images,and pathology.RESULTS A total of 1846 cases with PC and controls were collected.Among PC,only 410(22.2%)cases were identified as specific cancer sites including head in 234(12.7%)cases,tail in 104(5.6%)cases and body in 72(3.9%)cases.Among PC,910(49.3%)cases were diagnosed by definite criteria.Most of these were adenocarcinoma(98.0%).The rates of definite diagnosis of PC were highest in head(70.1%)followed by body(47.2%)and tail(43.3%).False-positive cases were pancreatic cystic neoplasm and metastasis to the pancreas.In terms of the overall diagnosis of PC,sensitivity,specificity,positive predictive value,and negative predictive value were 99.95%,98.72%,98.70%,and 99.95%,respectively.Diagnostic accuracy was similar both in terms of diagnostic criteria and tumor locations.CONCLUSION Korean NHI claims database collected according to ICD-10 code with V code for PC showed good accuracy.
