Role of endoscopic therapy in early esophageal cancer

Esophageal carcinoma is a highly lethal cancer associated with high morbidity and mortality. Esophageal squamous cell carcinoma and esophageal adenocarcinoma are the two distinct histological types. There has been significant progress in endoscopic diagnosis and treatment of early stages of cancer using resection and ablation techniques, as shown in several trials in the recent past. Earlier detection of esophageal cancer and advances in treatment modalities have lead to improvement in the 5-year survival from 5% to about 20% in the past decade. Endoscopic eradication therapy is the preferred modality of treatment in cancer limited to mucosal layer of the esophagus as there is very low risk of lymph node metastasis, leading to high cure rates, low risk of recurrence and with few adverse effects. The most common adverse events seen are strictures, bleeding and rarely perforation which can be endoscopically managed. In patients with recurrent advanced disease or invasive tumor, esophagectomy with lymph node dissection remains the mainstay of treatment. There is debate on post-endoscopic surveillance with some studies suggesting closer follow up with upper endoscopy every 6 mo for the first 1-2 years and then annually for the 3 years while others recommending the appropriate action only if symptoms or other abnormalities develop. Overall, the field of endoscopic therapy is still evolving and focus should be placed on careful patient selection using a multidisciplinary approach.

Occurrence of neurotoxic chemicals in the aquatic environment is on the rise posing a potential threat to aquatic biota including *** teleosts,zebrafish has become a popular model organism for toxicological studies and testing ***,over the decade,siluriformes(catfish)are also finding ever increasing application being robust as well as their adaptability to adverse ecological conditions,surgical interventions,and tolerant models for toxicity studies and *** information can infer potential effects occurring to other species exposed to neurotoxins in their aquatic environment and predicting potential risks of a chemical for the aquatic *** aim of this review is to compare and interpret recent results published concerning neuro-behavioral and morphological disturbances caused by toxicants/pollutants providing a holistic view of potential neurotoxic outcomes in ***,this review summarizes various effects of toxicants/pollutants in terms of neurotoxicity and neurodegeneration associated with behavioral phenotypes.

Down-conversion properties of Eu3+doped M2Y2Si2O9(M=Ba,Ca,Mg,Sr) phosphors have been investigated in *** phosphors were synthesized via the simple,fast and cost-effective sol-gel technique at a temperature of 950℃.Color coordinates and emission color can be altered by the varying concentration of dopant ion in *** luminescence intensity was obtained when doping 0.03 mol of Eu3+*** the excitation wavelength of 395 nm,these silicates showed strong red color,pure and sharp spectral peaks in visible region due to ~5 D0→F1-3transitions of Eu3+present in the *** of reaction temperature on luminescence was also analyzed for these *** sharp peaks in the X-ray diffraction pattern indicated the high crystallinity of prepared ***2Y2Si2O9has shown an orthorhombic crystal *** FTIR results confirmed the metal-oxygen vibrational modes available in the range of 400-1600 *** electron microscopy images have revealed that the variation of alkaline earth metal provided a very different crystal *** down conversion response of these phosphor materials can provide a great significance in the application of the coming solar devices.

Endocrine disruptors(EDs)are synthetic or natural chemical molecules occurring in environment that have the potential to impart adverse effects on homeostasis of endocrine axis leading to neurological,developmental,immunological and reproductive disarray at organismal level.A wide range of structurally diverse EDs such as,sex-steroid hormone mimics,pesticides and fertilizers,prevail in the environment originating from waste of industries,pharmaceutics,sewage treatment plants and *** addition,some metals,such as Cu,Hg and Zn,have endocrine disrupting potency in their metallic as well as synthesized nano-particulate *** is an increasing concern in research for the plausible threat posed by EDs that can disrupt the endocrine system in aquatic fauna as these compounds are frequently discharged or run-off into water *** are well known bio-indicators to understand toxicity of EDs as they are vulnerable to endocrine ***,EDs have the potential to affect fish-feeding higher vertebrates including mammals and subsequently human,as they make their way up on the food web pyramid due to *** light of this,several observations suggesting adverse effects of EDs and the mechanism contributing to endocrine disruption in fish are discussed extensively in this *** article highlights the necessity to choose a credible model for assessing the toxic effects exerted by ***,the toxic effects of EDs will be comprehensively reviewed with reference to sexual plasticity,neuroendocrine mechanisms,thyroid and immune modulation,gonadal development and maturation as well as changes in transcriptome/genome profile using fish models to imply ED-induced aquatic pollution in a larger *** decades now,studies on EDs have challenged traditional concepts in toxicology to develop new molecular markers to improve methodologies and to assess the ecological risks associated with field *** this regard,it is

Can Waist Circumference Be Used as an Anthropometric Parameter to Assess the Obesity Related Pregnancy Outcome: A Case Control Study

Introduction: The prevalence of obesity is increasing in women of reproductive age. Maternal obesity is associated with increased morbidity and mortality for both mother and offspring. BMI has been criticized as a limited measure of total obesity. Measurement of waist circumference can be useful in the assessment of abdominal obesity and disease risk. The study is aimed to evaluate the waist circumference as an anthropometric parameter in identifying women at risk of developing obstetric complications. Material and Methods: A prospective study was conducted at a tertiary health care centre on 200 antenatal women of age between 18 to 35 years with singleton pregnancy attending the antenatal clinic before 8 weeks of gestation. Women were divided into two groups. Group I included 100 women with waist circumference < 80 cm and Group II included 100 women with waist circumference ≥ 80 cm. Weight, height and waist circumference were measured and BMI was calculated. All the women were followed throughout their pregnancies as per the routine antenatal follow up. The fetomaternal outcome was recorded and analyzed statistically. Results: The two groups were comparable in age, parity and demographic profile. The maternal and neonatal complications—preeclampsia (p = 0.0052, RR 0.5062, 95% CI 0.2935 - 0.8728), gestational diabetes mellitus, preterm labor, postdatism, need for induction of labor (p value 0.0081, RR 0.6263, 95% CI 0.4314, 0.9091), instrumental vaginal delivery, cesarean delivery (p = 0.0072, RR 0.5745, 95% CI 0.3696, 0.8929), shoulder dystocia, PPH, macrosomia, neonatal asphyxia, admission to NICU were reported more in Group II women as compared to Group I. Conclusion: Assessment of waist circumference in early pregnancy provides a simple and practical parameter for predicting obesity related pregnancy outcome. All pregnancies in centrally obese women (waist circumference ≥ 80 cm) shall be acknowledged as high risk.
