Importance of reporting segmental bowel preparation scores during colonoscopy in clinical practice

AIM: To evaluate the impact of reporting bowel preparation using Boston Bowel Preparation Scale(BBPS) in clinical ***: The study was a prospective observational cohort study which enrolled subjects reporting for screening colonoscopy. All subjects received a gallon of polyethylene glycol as bowel preparation regimen. After colonoscopy the endoscopists determined quality of bowel preparation using BBPS. Segmental scores were combined to calculate composite BBPS. Site and size of the polyps detected was recorded. Pathology reports were reviewed to determine advanced adenoma detection rates(AADR). Segmental AADR's were calculated and categorized based on the segmental BBPS to determine the differential impact of bowel prep on AADR. RESULTS: Three hundred and sixty subjects were enrolled in the study with a mean age of 59.2 years, 36.3% males and 63.8% females. Four subjects with incomplete colonoscopy due BBPS of 0 in any segment were excluded. Based on composite BBPS subjects were divided into 3 groups; Group-0(poor bowel prep, BBPS 0-3) n = 26(7.3%), Group-1(Suboptimal bowel prep, BBPS 4-6) n = 121(34%) and Group-2(Adequate bowel prep, BBPS 7-9) n = 209(58.7%). AADR showed a linear trend through Group-1 to 3; with an AADR of 3.8%, 14.8% and 16.7% respectively. Also seen was a linear increasing trend in segmental AADR with improvement in segmental BBPS. There was statistical significant difference between AADR among Group 0 and 2(3.8% vs 16.7%, P < 0.05), Group 1 and 2(14.8% vs 16.7%, P < 0.05) and Group 0 and 1(3.8% vs 14.8%, P < 0.05). χ2 method was used to compute P value for determining statistical ***: Segmental AADRs correlate with segmental BBPS. It is thus valuable to report segmental BBPS in colonoscopy reports in clinical practice.

Both benign and malignant tumors may arise from any of the structures contained within the parapharyngeal space. Parapharyngeal space is difficult to reach and formidable area to approach by any surgeon. Due to its deep placement and approximation to vital structures, not only is tumour involving this space a difficult task to manage for the surgeon, but even its diagnosis may elude the doctor. Even the battery of clinical, biochemical or radiological tests diagnosis remains a difficult objective. We aim to provide an adjunct mode of aiding the diagnosis in the form of tumour markers. Though tumour markers alone are not sufficient, with the help of other tests, the diagnosis can be reached in a fairly accurate measure.

Growth of Boron Nitride Nanotubes Having Large Surface Area Using Mechanothermal Process

Nanostructures of boron nitride (BN) including nanotubes, nanofibers and nanosheets having a large surface area are very useful in storing hydrogen and other gases. In the present paper we report the synthesis and characterization of these nanostructures of BN using mechanothermal process. Under this process elemental boron powder is first ball milled to about 50 h in an inert atmosphere and then annealed at 1100℃ - 1250℃ for 6 h in the presence of NH3 gas. By this treatment nanotubes and other nanostructures of BN were synthesized. The diameter of BN nanotubes varied from 20 to 50 nm and most of them exhibited spindle or bamboo like morphology. Because of the large surface area, these nanotubes may be explored for a better hydrogen gas storage device as compared to the crystallized nanotubes. The main advantage of this technique is that the nanotubes can be grown in large quantity. A possible growth mechanism towards the evolution of such fascinating nano-objects of boron nitride has been discussed employing high-resolution transmission electron microscopy and photoluminescence.

Trends of Seroprevalence, Epidemiology and Clinical Presentation of HIV in North India: A Tertiary Care Hospital Based Study

Introduction: HIV infection represents a major public health problem for both developing and developed countries as it has grown to global pandemic. Spectrum of clinical presentation of HIV can greatly vary with geopolitical, socioeconomic and cultural environment. Aims: The aim of this study was to estimate the prevalence, socio-demographic conditions, clinical presentations, opportunistic infections and the possible associated risk factors for acquiring HIV infection. Materials and Methods: An observational prospective study was conducted from January 2010 to December 2014 at our centre situated in north India. As per the strategy and policy prescribed by NACO, tests were performed on the serum samples. Results: Out of the total 35369 clients tested for HIV infection, 292 were found to be HIV-1 seropositive. Two HIV-2 cases were found among the studied population. The seroprevalence of HIV was found to be 0.83% (294/35369). Mean age of the study group was 30 ± 5.65 (range: 02 - 80) years. Overall positivity rates among attendees were found to be 0.97% (138/14098), 0.96% (35/3610), 0.82% (7/850), 0.78% (22/2810) and 0.65% (92/14001) in the years 2010, 2011, 2012, 2013 and 2014 respectively. Heterosexual route of transmission was the major route of infection in 78.2 % patients. At the time of presentation, it was observed that Tuberculosis (14.9%) was the most common opportunistic infection. Conclusion: The study shows that there is a decreasing trend in the HIV prevalence in North India. However, the number of HIV cases is still significant and suggests the need for focused prevention efforts in high-risk groups.

A compact narrowband non-degenerate dual-mode microstrip filter with square shape cuts is presented. The structure is developed by loading the conventional non-degenerate dual-mode resonator by open circuit stubs at two opposite corners. The filter bandwidth is controlled by only decreasing the higher cutoff frequency of the conventional type. With Square shape cuts, return loss is improved. A 20% fractional bandwidth filter is designed and implemented on FR4 material with 4.4 dielectric constant and 1.6 mm thickness at center frequency of 1.5 GHz with passband of 1.3 GHz to 1.6 GHz. Analysis has been achieved using the IE3D simulator. Experimental results do agree with simulations.

Resurgence of mycobacterial diseases particularly tuberculosis has caused a renewed interest to unravel the strategies employed by mycobacteria for intracellular survival. In spite of advancement in mycobacterial research, our knowledge about genes and their corresponding functional proteins involved during the interaction of mycobacterium with host’s macrophages is fragmentary. This study pertains to development of a suitable in vitro model using murine macrophages and Mycobacterium bovis BCG to study proteins expressed during macrophage-myco bacterium interactions. Peritoneal macrophages from BALB/c mice were infected with M. bovis BCG and intracellular replication was assessed by {3H} thymi- dine uptake assay which was maximal when macrophage to mycobacterium ratio was 1:10. SDS-PAGE was employed to study the proteins expressed and selected proteins were subjected to mass spectrometry. Seven proteins found to be upregulated during macrophage-mycobacterium interaction were identified by MALDI-TOF. The results indicate that the present in vitro infection model was able to support the growth of M. bovis BCG in murine macrophages and is an ideal model to determine the pattern of functions of gene expression during the interaction of mycobacterium with macrophages. The differentially expressed proteins will help in understanding the mycobacterial molecular basis of adaptation to intracellular macrophage environment.

Non-steroidal anti-inflammatory drugs(NSAIDs)(aspirin,ibuprofen,naproxen,ketoprofen,and diclofenac)were determined in three selected wastewater treatment plants(WWTPs)in southwestern India and the Gurupura *** concentrations of the NSAIDs in the influents of the WWTPs ranged 125-184 mg/L for aspirin,5e22 mg/L for ibuprofen,11-217 mg/L for naproxen,3-41 mg/L for ketoprofen and 12-68 mg/L for *** the effluents,concentrations ranged 0.4-0.7 mg/L for aspirin,0.1-2 mg/L for ibuprofen,3-14 mg/L for naproxen,0.6-0.8 mg/L for ketoprofen and 2-26 mg/L for *** NSAIDs in the WWTPs were found in the order of aspirin>naproxen>diclofenac>ketoprofen>*** the Gurupura river,aspirin(0.02 mg/L),ibuprofen(0.17 mg/L),naproxen(8.8 mg/L),ketoprofen(1.5 mg/L)and diclofenac(1.6 mg/L)were *** quotient(HQ)for various aquatic organisms were calculated for the effluents of WWTPs and Gurupura river *** results showed medium risk of ibuprofen and naproxen to polyp Hydra *** discharge of NSAIDs into the river can result in adverse effects to the resident organisms.
