Dear Editor,We report the case of severe mixed-mechanism glaucoma observed in a monocular Axenfeld-Rieger syndrome(ARS) patient who was successfully managed with an ahmed glaucoma implant(AGI) followed by a novel technique described as augmented Micro Pulse transscleral cyclophotocoagulation(MP-TSCPC)[1]. ARS encompasses a heterogeneous spectrum of diseases involving anterior segment dysgenesis and systemic developmental anomalies[2].

Background and objective: Classically, diabetic subjects are at high risk of anaesthesia compared with general population. However, some recent publications have shown contrasting and sometimes contrary results. The aim of our study was to evaluate morbidity and mortality during and after anaesthesia in patients with versus without diabetes operated on at Monkole Hospital over the last ten years. Methods: Retrospective cohort study including all patients who underwent all-comers surgery excluding cardiac surgery between 2011 and 2021. Each diabetic patient was matched to 2 non-diabetic controls on age and sex. The evaluation criterion was the frequency of occurrence of at least one perioperative complication and/or death up to day 30. A multivariate analysis using a Cox model was used to determine the factors associated with the occurrence of this morbidity and mortality. The model was adjusted for comorbidities, preoperative hyperglycaemia, ASA score, type of anaesthesia and severity of surgery. Results: A total of 351 diabetic patients (mean age 53.3 ± 14.18 years) and 701 non-diabetic patients (mean age 53.52 ± 14.7 years) were included and analysed. Preoperatively, hyperglycaemia (blood glucose > 180 mg/dl) was observed in 24.3% of diabetic patients compared with 1.6% of non-diabetic patients. The incidence of overall perioperative complications was 25.6% in diabetic patients compared with 28.6% in non-diabetic patients (p = 0.27). The risk factors associated with this morbidity were general anaesthesia with oro-tracheal intubation vs loco-regional anaesthesia (OR = 3.06 [95%CI: 1.91 - 4.94];p Conclusion: This study shows that there is not significant increase in perioperative morbidity and mortality in diabetic patients compared with non-diabetic ones of similar severity. These results suggest that diabetes itself (excluding associated comorbidities) has only a minor impact on perioperative morbidity and mortality.

Given the current expansion of the computer visionﬁeld,several appli-cations that rely on extracting biometric information like facial gender for access control,security or marketing purposes are becoming more common.A typical gender classiﬁer requires many training samples to learn as many distinguishable features as ***,collecting facial images from individuals is usually a sensitive task,and it might violate either an individual's privacy or a speciﬁc data privacy *** order to bridge the gap between privacy and the need for many facial images for deep learning training,an artiﬁcially generated dataset of facial images is *** acquire a pre-trained Style-Generative Adversar-ial Networks(StyleGAN)generator and use it to create a dataset of facial *** label the images according to the observed gender using a set of criteria that differentiate the facial features of males and females *** use this manually-labelled dataset to train three facial gender classiﬁers,a custom-designed network,and two pre-trained networks based on the Visual Geometry Group designs(VGG16)and(VGG19).We cross-validate these three classiﬁers on two separate datasets containing labelled images of actual *** testing,we use the UTKFace and the Kaggle gender *** experimental results suggest that using a set of artiﬁcial images for training produces a comparable performance with accuracies similar to existing state-of-the-art methods,which uses actual images of *** average classiﬁcation accuracy of each classiﬁer is between 94%and 95%,which is similar to existing proposed methods.

The incidence of cardiogenic shock(CS)complicating acute myocardial infarction(AMI-CS)remains between 3%and 10%and in-hospital mortality is hardly less than 30%.[1-3]In addition,the economic cost of caring for these patients is high.[4]Revascularization in the acute phase is the only measure that has demonstrated to modify the prognosis of AMI-CS.

Transnational corporations(TNCs)act as unitary firms in an increasingly globalised economy,but taxes on their profits are levied by national ***,international tax rules have from the start been riven by contradictory approaches:ei-ther to determine the taxable profits attributable to each separate constituent entity of the TNC in their jurisdiction by comparing them with independent firms conducting a similar business,or to tax an appropriate share of the TNC’s global profits appor-tioned by factors reflecting its activities within the *** separate entity principle became dominant,especially with the adoption of the OECD Transfer Pricing Guidelines in 1995,but it gave a perverse incentive to TNCs to devise tax avoidance strategies,based on attributing high levels of profit to entities in countries where they would be taxed at low *** project on base erosion and profit shifting(BEPS)was mandated by the G20 to reform these rules so that TNCs could be taxed where their activities occur,signaling a return to the unitary *** latest proposals now adopt the principle of unitary taxation of TNCs,together with technical standards for formulary apportionment,but only as an overlay on top of existing rules based on the incompatible independent entity principle.A stable foundation for international tax depends on resolving this dilemma and agreeing a fair and balanced allocation of rights to tax TNCs’profits based on their real activities in each jurisdiction.
