Introduction: While exposure to environmental peanut during infancy appears to promote sensitization by the epicutaneous route, early and frequent peanut ingestion during infancy may prevent peanut allergy through oral tolerance. Due to parental occupation, the offspring of peanut farmers are likely exposed to peanut protein in their environment, but they may also have early introduction into their diet due to ready access. The purpose of this study was to determine the prevalence of peanut allergy in offspring of peanut farmers. Methods: A survey was mailed by the National Peanut Board to peanut farmers in the United States collecting information on peanut consumption, peanut reactions, physician-confirmed peanut allergy (PA), epinephrine auto-injector prescription and history of other allergies in offspring. Egg allergy (EA) served as a comparator to peanut allergy. Results: Of 10,349 households surveyed, 1050 responses were received (2493 offspring, participation rate 10%). The self-reported prevalence of peanut allergy in the general population in the USA is 0.9%, 1.3%, and 0.7% for children 0 - 5 years, 6 - 10 years, and 11 - 17 years, respectively. In offspring of peanut farmers, PA vs EA was reported in 3.69% (10/271) vs 2.6% (7/271), 2.8% (14/493) vs 2.0% (10/493), and 1.48% (37/2493) vs 1.4% (35/2493) of offspring ≤10 years, offspring ≤15 years and all offspring, respectively. Physician-confirmed PA was reported in 3.3% (9/271), 2.6% (13/493), and 1.24% (31/2493) in the three groups, respectively. Physician-confirmed PA with epinephrine auto-injector prescription was reported in1.8% (5/271), 1.8% (9/493), and 0.68% (17/2493) in the three groups, respectively. Conclusions: We found that the self-reported prevalence of peanut allergy in offspring of peanut farmers was similar to that reported in the general population. This is a unique population that may provide insight into factors that influence development of peanut allergy.

Risk Factors of First Acute Myocardial Infarction: Comparison of Elderly and Non-Elderly: A 24-Year Study

Although risk factors of Ischemic Heart Disease (IHD) are well known, knowledge about the distribution of these risk factors in different subgroups of patients can be used for designing preventive programs. The aim of this study was to assess differences of prevalence of IHD risk factors in elderly and non-elderly patients in their first Acute Myocardial Infarction (AMI). In this retros-pective study, elderly (age > 65;n = 1060) and non-elderly (age ≤ 65;n = 2228) patients with first AMI who were admitted into four teaching hospitals in Tehran between 1982 and 2005 were recruited. Risk factors containing male gender, systolic and diastolic hypertension (HTN), diabetes, hypertriglyceridemia, hypercholesterolemia, smoking and family history of IHD were compared between the two groups. Among different risk factors, diabetes and systolic HTN were more common in elderly than their non-elderly counterparts, and male gender, smoking, family history of IHD and dyslipidemia were more common in non-elderly patients than elderly ones (p < 0.05 for all comparison). Diastolic HTN showed no significant difference between the two groups. Findings of this study, stating difference of prevalence of risk factors in two elderly and non-elderly groups, can be used by health policy makers to conduct preventive programs for AMI in this country.

Self-Reported Assessment of Health Status and Psychological Condition among Hospital Medical Residents in Iran

Psychological stress and illness among hospital residences have been subject to increasing scientific scrutiny;however very few studies have been published to describe the extent of health status and psychological stresses in residents in different specialties especially in our country that our study was mainly focused on this subject. Methods: The study subjects were medical residents in public hospitals covering 14 different specialties in Tehran in 2007. A self-administered questionnaire elicited information related to socio-demographic profile, specialty, duty hours, sleep quality, physical activity level, number of night shifts per month, mood changes, sense of depression after night shifts, fatigue, use of antidepressant and tranquilizer, and their opinion regarding their health status on a 4-point Likert scale. Results: 66.7% of participants were male with the mean age of 32.9 ± 4.2 years. 62.1% of the residents felt mood changes after nightly shifts that could adversely affect their daily living, quality of work, and social relationships. The overall prevalence of the use of antidepressant drugs was 20.0% which was significantly higher in women than men. Also, 24.4% of them reported consumption of sedative and hypnotic drugs. 15.6% reported complete healthy status while 9.4% reported partial illness. Complete healthy status was more reported among the residents of anesthesiology and pediatrics while illness was more reported by residents in pathology field. Residents’ satisfaction with their status was positively correlated with the year of residency and marriage, while dissatisfaction was more reported in divorced ones as well as in those with higher number of nightly shifts. Conclusion: Notable number of hospital residents in Iran experience significant stressors and emotional and mental health problems. The occurrence of the historical and machine Earthquakes, near to the North Tabriz Fault in NW Iran is an evidence for the seismic activity of this fault, which records a historical earthquake with a magnitude more than 7. Using the existing experimental relations, seismicity, and the fault geometry, a Mw 7.7 earthquake scenario was defined. The stochastic finite-fault modeling based on a dynamic corner frequency shows good agreement with common attenuation patterns. The shake map illustrates that Baghmisheh, Roshtieh, Ellahieh, Valiamr, and Eram region on Tabriz are at high hazard areas, and the maximum acceleration is located at the north direction with the same azimuth similar to fault strike.

Oblique continent—continent collision between Iranian microcontinent and Arabian plate is the main cause of transpression and transtension regimes in this area. Zagros orogeny in this area resulted in formation of various structures including thrust fault-related folds, dextral and sinistral, strike slip faults, normal fault related to dextral strike slip fault. Thrust faults within the area under study are duplex with general trend of NW-SE and dip toward the north-east with right slip component. Strike slip fault regime acts as tear fault and it is active yet. Therefore, simultaneous presence of thrusts and strike slip faults illustrates convergent dextral transpressional tectonic regime while this transpressional regime accompanied with transtension as well;since normal faults are also seen in alluvium around depression of Sirjan which can be as a result of extension stresses due to strike slip faults activity of the district. The results achieved from geometry and kinematic analysis of west of Sirjan structures indicate that structures of the area have characteristics of internal part of Zagros orogeny.

Production of cocoa butter replacer (CBR) from tea seed oil through common modification methods of oils (dry fractionation, partial hydrogenation, chemical and enzymatic interesterification) was evaluated. Some physico-chemical properties (iodine, saponification, acid and peroxide values) and fatty acid composition (FAC) of modified samples were analyzed and compared with a reference cocoa butter (CB). Solid and liquid fractions for large amounts of unsaturated fatty acids (approx. 80%) and thereby lower iodine values (81 - 85 gI2/100g) than that of CB (37% and 34 gI2/100 g, respectively), are not suitable as CBR. Among all ratios of chemically and enzaymatically interesterified oil blends (20%, 25% and 30% of hydrogenated tea seed oil with 80%, 75% and 70% of tea seed oil/liquid fraction/solid fraction), the samples with ratio of 30:70 from both chemical and enzymatic interesterification had FAC and iodine value closer to that of CB. A comparision between chemically and enzymatically interesterified samples (CISs and EISs, respectively), in terms of solid fat content (SFC) indicated that although the SFC values in EIS were much lower than that of CB, but the thermal behavior of this sample is comprable to CB at 20℃- 30℃ (sharp melting point of CB).

Introduction: Hypertension is one of the common problems associated with pregnancy that may be followed by eclampsia, acute renal failure, maternal death, premature delivery, intra-uterine growth restriction and other. This study was conducted to determine the results of pregnancies associated with hypertension in patients visiting in the Delivery Ward of Valiasr Hospital. Methods: A descriptive study was conducted on all the patients admitted to the aforementioned department and who possessed the inclusion criteria for hypertensive pregnancy. Results: Among the 1694 delivery cases examined, 173 cases had hypertension (9.8%). Among these, 75 (45%) had gesta-tional hypertension;24 (14.8%) had preeclampsia-eclampsia;30 (18%) had preeclampsia su-perimposed on chronic hypertension;21 (13.5%) cases had chronic hypertension;and 13 (8%) had pregnancy-aggravated chronic hypertension. Ninety-six point three percent (96.3%) had a systolic blood pressure (BP) of 140 - 190 mmHg, and 3.7% had a systolic BP greater than 190 mmHg. Whereas 61.1% of diastolic blood pressure 90 - 110 mmHg and 38.9% of the mothers had diastolic BP greater than 110 mmHg. The HELLP (Hemolysis, Elevated Liver enzymes & Low Platelet count) syndrome was present in 4.9% of cases;52.6% experienced premature delivery;7.4% had IUFD (intra uterine fetal death);9.9% had IUGR (intrauterine growth retardation);and 17.3% had LBW babies. Conclusions: Based on our results, hypertensive mothers who are younger and have lower weight babies at birth experience more perinatal complications. The unpleasant effects of hypertension in pregnancy warrant the need for training, routine prenatal care, the early detection and treatment of hypertension at younger ages of pregnancy, and follow-up after delivery.

Leukemia comprises a diverse group of malignancies which is accompanied with genetic disorderliness in hematopoietic cells. We evaluated effective risk factors in recovery process of under treatment patients suffering from acute myeloblastic leukemia （AML）. This study conducted a cross-sectional descriptive-analytical study on a population of 76 samples obtained non-randomly from patients in Taleghani Hospital （Tehran, Iran）. 30.3% patients resulted in death. According to logistic regression results, sexes [OR = 6.40, 95% CI = （0.27, 3.45）], ALT [OR = 1.03, 95% CI = （0.01, 0.05）] and HCT [OR = 0.55, 95% CI = （-1.12, -0.06）] were recognized as significant in prognoses. We predicted the probability of death with an error of 20.03% based on a prognoses system using support vector machine （SVM） classifier. Using this theory, we experienced an error of 20.03%. 46.6% patients with a positive and 20.8% patients without positive drug history resulted in death, which shows a significant correlation between patients＇ drug history and their death.
