Fatal and non-fatal injuries due to suspension trauma syndrome: A systematic review of definition, pathophysiology, and management controversies

BACKGROUND: Suspension trauma syndrome is a life-threatening event that occurs when a person is "trapped" in a prolonged passive suspension. It is most commonly seen in people who engage in occupational or sport activities that require harness suspension. The aim of this study is to identify the predisposing factors, pathophysiology, and management of suspension ***: A review and analysis of the literature published in English and Spanish from 1972 to 2020 on suspension trauma were performed. Search sources were Pub med, medline, Cochrane library, meSH, Up To Date, and Google Scholar. Articles referring to suspension trauma associated with other injury mechanisms(traumatic impact injuries, drowning, asphyxiation, or bleeding), case reports, and pediatric population were ***: Forty-one articles were identified. Of these, 29 articles related to mechanism, pathophysiology, and management of individuals who suffered prolonged suspension trauma without associated traumatic injuries were included in the study. We encountered several controversies describing the putative pathophysiology, ranging from blood sequestration in the lower extremities versus accum.lation of metabolic waste and hyperkalemia to dorsal hook-type harness as a trigger cause of positional asphyxia;to vascular compression of femoral vessels exerted by the harness causing decreased venous return. Pstients suspended in a full-body harness with dorsal hook showed more hemodynamic alterations in response to the compressive effect on the rib cage, causing a reduction in perfusion by presenting a decrease in pulse pressure. management strategies varied across ***: Progress has been made in individualizing the population at risk and in the management of suspension trauma. We recommend the formation of consensus definitions, larger cohort or registry studies to be conducted, and experimental anim.l models to better understand the mechanisms in order to develop management

AIm： TO evaluate the association between poly- morphisms XRCC1 Arg194Trp and Arg399GIn and XRCC3 Thr241met and the risk for chronic gastritis and gastric cancer, in a Southeastern Brazilian population. mETHODS： Genotyping by PCR-RFlP was carried out on 202 patients with chronic gastritis （CG） and 160 patients with gastric cancer （GC）, matched to 202 （Cl） and 150 （C2） controls, respectively. RESUlTS： No differences were observed among the studied groups with regard to the genotype distribution of XRCC1 codons 194 and 399 and of XRCC3 codon 241. However, the combined analyses of the three variant alleles （194Trp, 399Gin and 241met） showed an increased risk for chronic gastritis when compared to the GC group. moreover, an interaction between the polymorphic alleles and demographic and environmental factors was observed in the CG and GC groups. XRCC1 194Trp was associated with smoking in the CG group, while the variant alleles XRCC1 399Gin and XRCC3 241met were related with gender, smoking, drinking and Hpylori infection in the CG and GC groups. CONClUSION： Our results showed no evidence of a rela-tionship between the polymorphisms XRCC1 Arg194Trp and Arg399Gln and XRCC3 Thr241met and the risk of chronic gastritis and gastric cancer in the Brazilianpopulation, but the combined effect of these variants may interact to increase the risk for chronic gastritis, considered a prem.lignant lesion. Our data also indicate a gene-environment interaction in the susceptibility to chronic gastritis and gastric cancer.

用自发熔化渗透法制备高TiC含量 (～ 75vol% )的TiC/NiAl复合材料 ,研究了渗透温度和渗透时间对TiC/NiAl复合材料微观组织形成、硬度及断裂性能的影响。采用XRD和TEm /EDS分析了复合材料的相组成、微观结构和NiAl相与TiC颗粒在高温渗透过程中的互溶情况。结果表明 :自发熔渗法是制备致密的TiC/NiAl复合材料的有效方法 ,适当提高渗透温度 ,可大大缩短渗透时间。在完成渗透获得致密组织的前提下 ,渗透温度和渗透时间对TiC/NiAl复合材料的硬度及断裂韧性无显著影响。NiAl相和TiC颗粒结合良好 ,是渗透后复合物的仅有组成相。TiC少量地溶于NiAl,溶解度随渗透时间增长而增大 ,且在TiC颗粒外层 ,发现有少量的Al和微量的Ni。

Emerging concepts in liver graft preservation

The urgent need to expand the donor pool in order to attend to the growing demand for liver transplantation has obliged physicians to consider the use of suboptim.l liver grafts and also to redefine the preservation strategies. This review examines the different methods of liver graft preservation, focusing on the latest advances in both static cold storage and machine perfusion(mP). The new strategies for static cold storage are mainly designed to increase the fatty liver graft preservation via the supplementation of commercial organ preservation solutions with additives. In this paper we stress the importance of carrying out effective graft washout after static cold preservation, and present a detailed discussion of the future perspectives for dynamic graft preservation using mP at different temperatures(hypothermia at 4 ℃, normothermia a t 3 7 ℃ and subnormothermia at 20 ℃- 2 5 ℃). Finally, we highlight some emerging applications of regenerative medicine in liver graft preservation. In conclusion, this review discusses the "state of the art" and future perspectives in static and dynamic liver graft preservation in order to improve graft viability.

Background:To identify biometric and im.lantable collamer lens(ICl)-related risk factors associated with suboptim.l postoperative vault in eyes im.lanted with phakic ***:This study reports a retrospective case series of the first operated eye in 360 patients im.lanted with myopic spherical or toric ***,white-to-white(WTW),central keratometry(Kc)and central corneal thickness(CCT)were measured using the ***-segment optical coherence tomography(AS-OCT,Visante)was applied preoperatively for measuring the horizontal anterior-chamber angle-to-angle distance(ATA),internal anterior chamber depth(ACD),crystalline lens rise(ClR),anterior-chamber angle(ACA)and postoperatively the *** were divided into three vault groups:low(lVG:≤250μm),optim.l(OVG:>250 andm)and high(HVG:≥1000μm).m.ltinomial logistic regression(mlR)was used to find the sub-optim.l vault ***:mlR showed that ClR,ICl size minus the ATA(ICl size-ATA),age,ICl spherical equivalent(IClSE)and ICl size as contributing factors for sub-optim.l vaults(pseudo-R2=0.40).Increased ClR(OR:1.01,CI:1.00-1.01)and less myopic IClSE(OR:1.22,CI:1.07-1.40)were risk factors for low *** ICl size-ATA(OR:41.29,CI:10.57-161.22)and the 13.7mm ICl(OR:7.08,CI:3.16-15.89)were risk factors for high vaults,whereas less myopic IClSE(OR:0.85,CI:0.76-0.95)and older age(OR:0.92,CI:0.88-0.98)were protective ***:High ClR and low IClSE were the major risk factors in eyes presenting low *** the opposite direction,ICl size-ATA was the major contributor for high *** relationship was more critical in higher myopic IClSE,younger eyes and when 13.7mm ICl were *** findings show that factors influencing the vault have differentiated weight of influence depending on the type of vault(low,optim.l or high).

The therapeutic promise of sm.ll interfering RNAs （siRNAs） for specific gene silencing is dependent on the successful delivery of functional siRNAs to the cytoplasm. Their conjugation to an established delivery platform, such as gold nanoparticles, offers tremendous potential for treating diseases and advancing our understanding of cellular processes. Their success or failure is dependent on both the uptake of the nanoparticles into the cells and subsequent intracellular release of the functional siRNA. In this study, utilizing gold nanoparticle siRNA-mediated delivery against C-mYC, we aimed to determine if we could achieve knockdown in a cancer cell line with low levels of intracellular glutathione, and determine the influence, if any, of polyethylene glycol （PEG） ligand density on knockdown, with a view to determining the optim.l nanoparticle design to achieve C-mYC knockdown. We demonstrate that, regardless of the PEG density, knockdown in cells with relatively low glutathione levels can be achieved, as well as the possible effect of steric hindrance of PEG on the availability of the siRNA for cleavage in the intracellular environment. Gold nanoparticle uptake was demonstrated via transmission electron microscopy and mass spectroscopy, while knockdown was determined at the protein and physiological levels （cells in S-phase） by in-cell westerns and BrdU incorporation, respectively.

AIm To determine if manipulation of dietary advanced glycation end product(AGE), intake affects nonalcoholic fatty liver disease(NAFlD) progression and whether these effects are mediated via RAGE. mETHODS m.le C57Bl6 mice were fed a high fat, high fructose, high cholesterol(HFHC) diet for 33 wk and compared with anim.ls on norm.l chow. A third group were given a HFHC diet that was high in AGEs. Another group was given a HFHC diet that was marinated in vinegar to prevent the formation of AGEs. In a second experiment, RAGE KO anim.ls were fed a HFHC diet or a high AGE HFHC diet and compared with wildtype controls. Hepatic biochemistry, histology, picrosirius red morphometry and hepatic mR NA were determined. RESUlTS long-term consumption of the HFHC diet generated significant steatohepatitis and fibrosis after 33 wk. In this model, hepatic 4-hydroxynonenal content(a marker of chronic oxidative stress), hepatocyte ballooning, picrosirius red staining, α-smooth muscle actin and collagen type 1A gene expression were all significantly increased. Increasing the AGE content of the HFHC diet by baking further increased these markers of liver damage, but this was abrogated by pre-marination in acetic acid. In response to the HFHC diet, RAGE-/-anim.ls developed NASH of sim.lar severity to RAGE+/+ anim.ls but were protected from the additional harmful effects of the high AGE containing diet. Studies in isolated Kupffer cells showed that AGEs increase cell proliferation and oxidative stress, providing a likely mechanism through which these compounds contribute to liver injury. CONClUSION In the HFHC model of NAFlD, manipulation of dietary AGEs modulates liver injury, inflammation, and liver fibrosis via a RAGE dependent pathway. This suggests that pharmacological and dietary strategies targeting the AGE/RAGE pathway could slow the progression of NAFlD.

A sensing system in the near infrared region has been developed for ammonia sensing based on the wavelength modulation spectroscopy （WmS） principle. The WmS is a rather sensitive technique for detecting atomic/m.lecular species, presenting the advantage that it can be used in the near-infrared region by using the optical telecommunications technology. In this technique, the laser wavelength and intensity were modulated by applying a sine wave signal through the injection current, which allowed the shift of the detection bandwidth to higher frequencies where laser intensity noise was typically lower. Two m.lti-pass cells based on free space light propagation with 160 cm and 16 cm of optical path length were used, allowing the redundancy operation and technology validation. This system used a diode laser with an emission wavelength at 1512.21nm, where NH3 has a strong absorption line. The control of the NH3 gas sensing system, as well as acquisition, processing and data presentation was performed.

Effect of Fe_(3)O_(4) nanoparticles on magnetic xerogel composites for enhanced removal of fluoride and arsenic from aqueous solution

Fe_(3)O_(4)magnetic xerogel composites were prepared by polycondensation of resorcinol(R)-form.ldehyde reaction via a sol-gel process in an aqueous solution through varying the m.lar ratio of Fe_(3)O_(4)nanoparticles(mNPs),catalyst(C),and water(W)*** were obtained by co-precipitation(mC),oxidation of iron salts(mO),or solvotherm.l synthesis(mS).Both mNPs and magnetic xerogels were examined regarding the performance of arsenic and fluoride removal in a batch *** mC-based mNPs had higher adsorption capacities for both fluoride(202.9 mg/g)and arsenic(3.2 mg/g)than other mNPs in optimum *** X-ray diffraction,Fourier transform infrared spectroscopy,and energy-dispersive X-ray spectroscopy confirmed that Fe was composed into the polymeric matrix of magnetic xerogels that contained 0.59%-4.42%of Fe with a m.lar ratio of mNPs(m)to R between 0.01 and *** low R/C and optimum m/R ratios,an increase in the surface area of magnetic xerogels affected the fluoride and arsenic adsorption *** magnetic xerogel composites with the mC-based mNPs prepared at a fixed R/C ratio(100)and at different R/W(0.05-0.06)and m/R(0.07-0.10)ratios had a high arsenic removal efficiency of 100%at an As(V)concentration of 0.1 mg/l and pH of *** maximum adsorption capacities of magnetic xerogels were approximately five times higher than those of the xerogels without mNP ***,Fe_(3)O_(4)nanoparticles enhanced the adsorption of arsenate and *** variations of alkaline catalyst and water content significantly affected the resulting properties of textural and surface chemistry of magnetic xerogel composites.

m.lt Infiltration Ability and microstructural Evolution of Fe40Al/ TiC Composites System

Pressureless m.lt infiltration is an economic route f or preparation of high-density ceramic/m.lt composites. In this study, the Fe40 Al iron aluminide intermetallic, a low cost material of excellent oxidation and corrosion resistance, was used as binder for fabricating Fe40Al/TiC composites b y pressureless m.lt infiltration. The wetting ability of liquid Fe40Al in porous TiC pre-form was studied by in-situ monitoring the m.lting and infiltration p rocess. The infiltration ability was investigated by observing the distance of l iquid Fe40Al intrusion in porous TiC pre-forms at different infiltration temper atures and times by using optical microscope. Porous TiC per-forms with density of 60%～88%TD (theoretical density), prepared under pre-defined sintering temp e rature cycles, were used for fabricating Fe40Al/TiC composites in the range of 1 2%～40% metal content by volume. Almost full dense Fe40Al/TiC composites were su c cessfully fabricated by this technique. liquid Fe40Al exhibited excellent infilt ration ability, the distance of com.lete intrusion of liquid Fe40Al in the TiC s intered pre-form with density of 88%TD was over 7 mm after 5 min at the inf iltration temperature of 1 450 ℃. microstructural observation by SEm and TEm also showed that liquid Fe40Al filled the very narrow gaps among TiC particles, the interfaces of TiC particles and F e40Al plastic ligaments being metallurgical bonded. TEm revealed that high densi ty of dislocations formed in Fe40Al ligaments during solidification, which favor the mechanical properties. Ti decomposed from TiC particles and dissolved into Fe40Al during infiltration. According to the compositional analysis of TEm-EDS, the concentration of Ti in Fe40Al ranges at 1at%～4at% depending on composite f a bricating conditions and the distance from the measuring point to the closest Ti C particles.
