Background: There is a high degree of professional consensus that children dia gnosed with pharyngitis should only receive antibiotics if they have a positive test for group A streptococcus (GAS). Objectives: To develop and test the validity of a quality of care performance measure that examines GAS testing ra tes in children diagnosed with pharyngitis and prescribed an antibiotic. Design: The measure developed examines the annual rate of GAS testing in children aged 2 to 18 years with an episode of pharyngitis who were prescribed antibiotics. Th e measure was tested for feasibility of implementation and validity in 5 health plans in the United States. Health plan administrative data were used to identif y episodes of pharyngitis using International Classification of Diseases, ninth Revision (ICD-9) codes 462, 463, and 034.0. Pharmacy data (national Drug Codes) were used to determine if antibiotics were prescribed for the pharyngitis episo de. Laboratory claims data (Current Procedural Terminology codes) were used to d etermine whether a GAS test was performed. Rates of GAS testing in children with pharyngitis who received antibiotics were calculated for each health plan. Medi cal record abstractions were performed on a random sample (n = 465) of cases to assess percent agreement with laboratory claims data for GAS testing. Sensitivit y of the administrative data for accurately identifying when GAS tests were perf ormed was also assessed. Results: Of the 120 158 children aged 2 to 18 years who had at least 1 episode of pharyngitis during the measurement year, 51172 (43%) received antibiotics. Group A streptococcal testing rates for patients who were prescribed antibiotics varied widely among the participating health plans (59% -83%of cases; Pnt agreement between administrative and medical rec ords data for GAS tests was 86%. Purpose:The aim of the present study was to assess both the credibility and strength of evidence arising from systematic reviews with meta-analyses of observational studies on handgrip strength and health ***:An umbrella review of systematic reviews with meta-analyses of observational studies was *** assessed meta-analyses of observational studies based on random-effect summary effect sizes and their p values,95%prediction intervals,heterogeneity,small-study effects,and excess *** graded the evidence from convincing(Class I)to weak(Class IV).Results:From 504 articles returned in a search of the literature,8 systematic reviews were included in our review,with a total of 11 ***,nine of the 11 of the outcomes reported nominally significant summary results(pns surviving the application of the more stringent p value(pno outcome presented convincing *** associations showed Class II evidence(i.e.,highly suggestive):(1)higher handgrip values at baseline were associated with a minor reduction in mortality risk in the general population(n=34 studies;sample size=1,855,817;relative risk=0.72,95%confidence interval(95%CI):0.670.78),(2)cardiovascular death risk in mixed populations(n=15 studies;relative risk=0.84,95%CI:0.780.91),and(3)incidence of disability(n=7 studies;relative risk=0.76,95%CI:0.660.87).Conclusion:The present results show that handgrip strength is a useful indicator for general health status and specifically for early all-cause and cardiovascular mortality,as well as *** further inform intervention strategies,future research is now required to fully understand mechanisms linking handgrip strength scores to these health outcomes.

Homogenized Porcine Extracellular Matrix Derived Injectable Tissue Construct with Gold nanoparticles for Musculoskeletal Tissue Engineering Applications

A unique porcine extracellular matrix (ECM) derived injectable tissue construct with 100 nm or 20 nm gold nanoparticles (AunP) was developed for musculoskeletal tissue engineering applications. ECM has been shown to encourage cellularity and tissue remodeling due to its release of growth factors while AunP have been shown to reduce reactive oxygen species (ROS) levels. Injectable tissue constructs were created by homogenizing decellularized porcine diaphragm tendon conjugated with 100 nm or 20 nm AunP at 1x, 4x, and 8x concentrations. Extrusion force testing demonstrated that homogenized tissue constructs were injectable at an appropriate cannula size and force. L-929 murine fibroblasts were used to measure cell viability, cell proliferation, intracellular ROS levels, and cell migration in response to constructs. Enhanced cell viability and proliferation are observed on 1 × 20 nm AunP constructs. ROS assays demonstrate reduced cellular ROS concentrations from all 20 nm AunP constructs and from 8 × 100 nm AunP constructs compared with constructs without nanoparticles. Cellular migration is higher towards 4 × 20 nm AunP constructs compared with constructs without nanoparticles. Results support the potential use of a porcine ECM derived injectable tissue construct with AunP as an injectable tissue construct to reduce inflammation and to promote tissue remodeling in musculoskeletal tissue engineering applications.

Assessing Cowpea-Wheat Double Cropping Strategies in the Southern United States Using the DSSAT Crop Model

Information is limited on the potential of cowpea-wheat double cropping in the southern United States to enhance soil health and increase net returns. Using the Decision Support System for Agrotechnology Transfer (DSSAT) crop model and weather data spanning 80 years, we assessed the effects of soil type (Darco: Grossarenic Paleudults and Lilbert: Arenic Plinthic Paleudults), n application rate (0, 100, and 200 kg•ha−1), and El niño-Southern Oscillation (EnSO) on the grain yields of double-cropped cowpea (Vigna unguiculata L.) and wheat (Triticum aestivum L.) in this region. Yield differences were tested using the pairwise Wilcoxon rank sum test. Results showed that yields of wheat that followed cowpea (cwheat) were greater than those that followed fallow (fwheat). The soil type effects on cwheat and fwheat yields decreased with an increase in n rate. The soil type effect on cowpea yields was greater during La niña. The EnSO impact on cowpea yields was greater on the less fertile soil Darco. Yields of cwheat and fwheat increased with an increase in n rate up to 100 and 200 kg•ha−1, respectively. The yield response of cwheat to n rate was less than that of fwheat. The n rate effects on cwheat and fwheat yields were greater on Darco and under El niño. Yields of cowpea were greatest under El niño, whereas those of wheat were greatest under La niña. The EnSO effect on cowpea yields was greater on Darco. With an increase in n rate, the effect of EnSO was diminished.
