Review of endoscopic techniques in the diagnosis and management of cholangiocarcinoma

Cholangiocarcinoma is a rare malignancy of the biliary tract. Key factors in determining therapeutic options include knowledge of tumor extent, anatomy and obtaining tissue diagnosis. Endoscopically, there are three modalities available to make the diagnosis of cholangiocarcinoma. These include endoscopic retrograde cholangiopancreatography, endoscopic ultrasound with fine needle aspiration and cholangioscopy. Management of cholangiocarcinoma endoscopically is typically confined to stent placement for palliative purposes or as a bridge to surgery. In this article, we will review the endoscopic techniques available for the diagnosis and management of cholangiocarcinoma.

Driven by rising energy demand and the goal of carbon neutrality,renewable energy generations(REGs),especially photovoltaic(PV)generations,are widely used in the urban power energy *** the intelligent control of microgrids(MG)brings economic and efficient operation,its potential stability problem cannot be *** date,most of the research on modeling,analyzing and enhancing the stability of MG usually assume the DC-link as an ideal voltage ***,this practice of ignoring the dynamics of DC-link may omit the latent oscillation phenomena of autonomous PV-based ***,this paper establishes a complete dynamic model of autonomous PV-based MG including PV panels and *** from previous conclusions of idealizing DC-link dynamics,participation factor analysis finds the potential impact of DClink dynamics on system dynamic performance,and different influence factors including critical control parameters and nonlinear V-I output characteristic of PV array are considered to further reveal oscillation ***,based on the average consensus algorithm,a distributed stabilization controller with strong robustness is proposed to enhance stability of the PVbased MG,which does not affect the steady-state performance of the ***,the correctness of all theoretical analysis and the effectiveness of the proposed controller are verified by time domain simulation and hardware-in-loop tests.

Thin film transistors （TFTs） of microcrystalline silicon （μc-Si） can provide higher mobility and stability than that of a-Si and better uniformity than that of poly-Si TFTs, and it would be more suitable to be applied to larger-area AMOLEDs. By using 2coYAG laser ann. ealing, crystalline μc-Si thin film on plastic substrate has been investigated and the proper laser energy needed for crystallization has been indicated. It has been found that the dehydrogenation process at 300-450℃ for a few of hours could be omitted by decreasing the H content in the crystallization precursor, which is suitable for laser crystallization on plastic substrates. The crystalline volume fraction （Xc） and the grain size of the resulted μc-Si could be adjusted by controlling the laser energy. By this method, the μc-Si on plastic substrate with Xc and grain size is respectively 85% （at the maximum） and 50 nm.

Hepatocellular carcinoma(HCC)is the most common type of primary liver *** treatment options are available for managing HCC patients,classified roughly as local,local-regional,and systemic *** high postmonotherapy recurrence rate of HCC urges the need for the use of combined modalities to increase tumor control and patient *** international guidelines offer treatment recommendations based on different points of view and classification ***(RT)is a well-known local-regional treatment modality for managing many types of cancers,including ***,only some of these treatment guidelines include RT,and the role of combined modalities is rarely ***,the present study reviewed clinical evidence for the use of different combined modalities in managing HCC,focusing on modern RT's *** RT has an increased utility in managing HCC patients,mainly due to two driving ***,technological advancement(e.g.,stereotactic body radiotherapy and advanced proton-beam therapy)enables precise delivery of radiation to increase tumor control and reduce side effects in the surrounding normal ***,the boom in developing target therapies and checkpoint-blockade immunotherapy prolongs overall survival in HCC patients,re-emphasizing the importance of local tumor ***,RT combines with systemic therapies to generate the systemic therapy augmented by radiotherapy effect,a benefit now being actively investigated.

AIM:To investigate the safety and effectiveness of endoscopic therapy with a paclitaxel-eluting balloon(PEB) for biliary anastomotic stricture(AS) after liver transplantation(LT).METHODS:This prospective pilot study enrolled 13 consecutive eligible patients treated for symptomatic AS after LT at the University Hospital of Münster between January 2011 and March *** patients were treated by endoscopic therapy with a PEB and followed up every 8 wk by endoscopic retrograde cholangiopancreatography(ERCP).In cases of re-stenosis,further balloon dilation with a PEB was ***-up was continued until 24 mo after the last ***:Initial technical feasibility,defined as successful balloon dilation with a PEB during the initial ERCP procedure,was achieved in 100% of ***-term clinical success(LTCS),defined as no need for further endoscopic intervention for at least 24 mo,was achieved in 12 of the 13 patients(92.3%).The mean number of endoscopic interventions required to achieve LTCS was only 1.7 ± *** failure,defined as the need for definitive alternative treatment,occurred in only one patient,who developed recurrent stenosis with increasing bile duct dilatation that required stent ***:Endoscopic therapy with a PEB is very effective for the treatment of AS after LT,and seems to significantly shorten the overall duration of endoscopic treatment by reducing the number of interventions needed to achieve LTCS.

AIM:To investigate the safety and diagnostic yield of colonoscopy in Chinese children in whom the procedure is not often ***:We conducted a retrospective review of all colonoscopies in consecutive children who underwent their fi rst diagnostic colonoscopy from Jan 2003 to ***:Seventy-nine children (48 boys,31 girls;mean age 9.2 ± 4.2 years) were identified and reviewed with a total of 82 colonoscopies *** caecal and ileal intubation rates were 97.6% and 75.6% *** patients (50.6%) had a positive diagnosis made in colonoscopy and that included colonic polyps (23),Crohn's disease (12),ulcerative colitis (1),and miscellaneous causes (4).80% of polyps were in the rectosigmoid *** but one were juvenile hamartomatous *** exceptionwas an adenomatous *** mean ages for children with inflammatory bowel disease (IBD) and polyps were 11.3 and 4.3 years *** was no procedure-related ***:Colonoscopy is a safe procedure in our Chinese *** increasing diagnosis of IBD in recent decades may reflect a rising incidence of the disease in our children.

Listeria monocytogenes （Lm） is an intracellular bacterium with unique predisposition for systemic maternal infection during pregnancy and morbid consequences for the developing fetus. Given the high mortality associated with prenatal Lm infection, strategies for augmenting protective immunity during the exceedingly vulnerable period of pregnancy are urgently needed. Herein, protection conferred by attenuated Lm administered before pregnancy against subsequent virulent Lm prenatal infection was evaluated. We show that protection against secondary Lm infection in non-pregnant mice is sharply moderated during allogeneic pregnancy because significantly more bacteria are recovered from maternal tissues, despite the numerical and functional preservation of pathogen-specific CD8^＋ T cells. More importantly, preconceptual priming does not protect against in utero invasion or fetal wastage because mice inoculated with attenuated Lm prior to pregnancy and naive pregnant controls each showed near complete fetal resorption and pathogen recovery from individual concepti after Lm infection during pregnancy. Remarkably, the lack of protection against prenatal Lm infection with preconceptual priming in allogeneic pregnancy is restored during syngeneic pregnancy. Thus, maternal-fetal antigen discordance dictates the ineffectiveness of preconceptual vaccination against fetal complications after prenatal Lm infection, despite the numerical and functional preservation of pathogen-snecific CD8^＋ T cells.
