VASCULAR TRAUMA IN THE AMAZON REGION: A TWO YEARS CASES REVIEW FROM A SINGLE INSTITUTION

Introduction: Vascular injuries are among the main mechanisms of death in trauma. In Brazil, the general surgeon is in charge of emergencies procedures in most hospitals, but many times these surgeons are not familiarized with more complex vascular surgical techniques that often require a vascular surgeon. Pará State is the most populated one in the Brazilian Amazon region and a single hospital with vascular surgeons permanently on call is available. Objectives: To evaluate demographic data on vascular trauma victims;to establish the mechanism, anatomic location, injury pattern and surgical techniques used to manage the vascular lesions treated at the Metropolitan Emergency Hospital;to ascertain associated non-vascular injuries and victim’s clinical outcome. Methods: Retrospective analysis of medical records of patients treated for vascular injuries from February 2011 to February 2013 at the institution in case. All arterial and venous injuries were analyzed. Patients who were not operated by the vascular surgery team, iatrogenic injuries and those who underwent primary or sustained traumatic amputation were excluded. Results: 173 cases;95.95% were male;54.90% were between 25 and 49 years;penetrating trauma mechanisms were found in 88.44%;lower limb was the most affected topography (41.50%);the most injured vessels were the superficial femoral and ulnar arteries (in 15.75% of cases each) and the superficial femoral vein (17.77 %);autologous vein graft was the most performed technique for arterial repair (36.57%) and ligature was performed in 85.00% of venous trauma;amputation rate was 15.60% and mortality rate was 6.35%. Conclusions: Vascular injuries occurred predominantly in men from 24 to 49 years old;penetrating mechanisms were most frequent;lower limb was the most affected topography;the most frequently injured vessels were the ulnar and superficial femoral arteries and the superficial femoral vein;complete vessel transection was the most common injury pattern;

Bacteria form biofilms as an adaptive mechanism in response to environmental changes. Streptococcus mutans is the biofilm-forming bacterium that is primarily associated with dental caries. The expression of important genes by bacteria in biofilms is different from that of planktonic cells. Lectins are proteins that bind specifically to carbohydrates and may have important biological activities on bacterial cells, acting as antibacterial and anti-biofilm agents. ConM (Canavalia maritima lectin) is a protein that is able to inhibit the planktonic growth and biofilm formation of S. mutans. In this context, this study aimed to evaluate the effects of ConM and concanavalin A (ConA) on the expression of genes related to virulence and biofilm formation in S. mutans. The results showed that ConM significantly reduced the expression of genes encoding enzymes related to adhesion, formation and regulation of biofilms. On the contrary, ConA did not alter the expression of the genes studied. Because the two lectins have a high degree of similarity, the differences in the actions of ConM and ConA may be explained by the small structural differences in the carbohydrate recognition domain of the lectins.

Crambe (Crambe abyssinica) Cultivation under Different Levels of Fertilization with Nitrogen, Phosphorus and Potassium

The effects of fertilization on crambe are poorly understood;in this sense, this study aimed to evaluate the crambe growth under different levels of nitrogen (0, 40, 80, 120 kg·ha-1), phosphorus (0, 50, 100, 150 kg·ha-1), and potassium (0, 30, 60, 90 kg·ha-1) in a greenhouse. The height of the plants, the production of dried biomass and that of 1000 grains of crambe were evaluated. The height of the plants was impaired as a function of increasing doses of nitrogen. The interaction of nitrogen and phosphorus in the crambe crop increased the number of grains and dry shoot biomass. The growth and yield of the crop were not influenced by the use of potassium in mineral fertilization.

The regime of disturbance with natural or anthropogenic origin can lead to the destabilization or even to a mass mortality of benthic communities. Due to the heterogeneity of the disturbance there is a formation of patches in different stages of ecological succession. The aim of this study is to follow and describe the resilience in artificial disturbed sediment in a polluted bay in Rio de Janeiro. The sediment was collected, sterilized and placed inside corers (10 cmdiameter ×15 cmheight) in the same place where it was collected. We allocated the samples in two structures, the difference between them was that one was placed closed to the bottom and the other was placed50 cmheight. We found 2352 organisms distributed in 14 taxons. The class Polychaeta and the order Amphipoda showed the highest density in all the survey and treatments. We identified the factors influencing the scenarium: different mechanisms of dispersal, the position of the structures and life history of each group. Disturbance can be the main cause of the patch’s diversity found in estuaries and coastal areas. Because of this, monitoring of selected areas becomes an important tool to understand the regime of disturbance as a key factor structuring benthic communities in soft sediment, also suggesting a metapopulation dynamics.

Diesel Consumption and CO2 Emissions in Agricultural Crops： An Estimation Model and a Forestry-Based Neutralization Proposal

This research presented a mathematical model to calculate the consumption of diesel oil and the respective emissions of CO2from 41 agricultural crops （38 permanent and 3 temporary） in Brazil. It contains data obtained between 2000 and 2012, from accreditedbibliographic sources. In addition to spreadsheets containing diesel consumption, resulting from the analysis of the productive processes,this research presents data on energy conversion and forest development used to subsidize CO2 emissions and mitigation *** for C sequestration of the atmosphere, four options of forest projects were systematized：（1） reforestation with fast growingspecies; （2） forest protection projects, with enrichment plantations; （3） implementation of agroforestry systems （1 st cycle）; and （4） urbanafforestation projects （streets and parks）. Such alternatives are in line with the proposals of the ＂sectorial plan for mitigation andadaptation to climate change for the consolidation of a low C emission in agriculture＂, of the Brazilian Agricultural ResearchCorporation （EMBRAPA）. The results show that three temporary products （soybean, sugarcane and cotton） are responsible, for at least,85% of all CO2 emissions, comparatively the low consumption of diesel oil verified in the management of the 38 permanent productsstudied in this research. Therefore, in order to contribute to the reduction of the release of CO2 into the atmosphere, perennial crops andecologically rational extractivism should be more encouraged and more supported by the public authorities.

BackgroundHuman rabies outbreak transmitted by bats continues to be a relevant public health problem not only in the Amazon region. The disease has affected one of the areas with the greatest poverty in southeastern Brazil, a region inhabited by the Maxakali indigenous *** presentationWe describe four cases of rabies among indigenous children that occurred in the indigenous village of Pradinho, municipality of Bertópolis, Minas Gerais, Brazil. Cases were notified between April and May 2022, all of whom died on average eight days after the first symptoms. All cases were observed in rural residents under 12 years of age. The probable form of exposure was through bat bites. The predominant symptoms were prostration, fever, dyspnea, sialorrhea, tachycardia, and altered level of consciousness. Half of the cases underwent late and/or incomplete post-exposure rabies prophylaxis, however, the other half underwent pre-exposure rabies prophylaxis, with only one case completing the scheme and another undergoing the adapted Milwaukee Protocol (Recife Protocol). All cases ended in *** was the first rabies outbreak among indigenous people in Brazil. Among the manifested clinical forms in the series, there was a disease atypical presentation in at least one case. We suggest active surveillance and an intercultural educational campaign to prevent new cases.

Effect of Copper, Zinc, Cadmium and Chromium in the Growth of Crambe

Crambe is a promising crop for biodiesel production. However, there is no much information available about techniques and research regarding the best performance in different regions to explore its potential. The objective of this study was to evaluate the effect of cadmium, chromium, copper and zinc in the development of crambe. The study was carried out in a greenhouse and it consisted of four independent experiments in completely randomized design, with three replications. Four metals with five levels were applied: 0, 10, 15, 20 and 25 mg·kg-1 for copper, cadmium, chromium;and 0, 20, 30, 40 and 50 mg·kg-1 for zinc, in order to evaluate the growth of crambe. Data were subjected to analysis of variance and comparison of means by Tukey test at 1 and 5% significance. despite doses of cadmium, chromium, copper and zinc in growing crambe have influenced in some of the evaluated variables of the plants, in general, their development was similar in relation to doses and the metals applied. The importance of cultivating crambe is related to grain production, based on this, copper and cadmium decreased this production.

Vitamin D and Secondary Hyperparathyroidism in HIV Infected Patients Taking Antiretroviral Therapy

Objective: Due to the lack of studies assessing hypovitaminosis D and secondary hyperparathyroidism in Brazilian HIV-infected population, especially in the northeastern population, this study aimed to determine the profile of these conditions in patients infected with HIV and its correlation with immuno-virological, sociodemographic data and associated comorbidities. Methods: Comparison studies were obtained from routine clinical samples of HIV infected patients submitted for 25-OH Vitamin D, PTH and alkaline phosphatase determination. Results: A total of 78 patients were included, 42 (53.8%) males, mean age 45.7 years. Antiretroviral regimens most used in this study were Zidovudine/Lamivudine/Efavirenz 17.9%, Tenofovir/Lamivudine/Efavirenz 17.9%,Tenofovir/Lamivudine/Atazanavir-r 15.4%. The mean value CD4 count was 592.1 ± 247.2 cells/mm3, CD8 cell count was 1026.5 ± 467.3 cells/mm3, mean detectable viral load was 2220 ± 15703 copies and CD4/CD8 ratio was 0.63 ± 0.33. A total of 34 vitamin D dosages were collected with 41.2% representing sufficient amount and 58.8% insufficient. Alkaline Phosphatase (ALP) dosage was elevated in 49.3% (N=35) of the patients. Parathormone (PTH) was elevated in 18% (N = 11). Among patients with elevated PTH levels, 81.9% had elevated levels of ALP (p = 0.01). In the group of patients with high levels of ALP, 45.7% had a CD4 count 3 (p = 0.02). There was no significant difference in vitamin D related to gender (p = 0.21), age (p = 0.23), CD4 count (p = 0.26), suppressed viral load (p = 0.44) or blood glucose (p = 0.45). Conclusions: This study evidenced a high prevalence of Vitamin D insufficiency in Northeast Brazil, which suggests HIV infection correlation. A high prevalence of Hyperparathyroidism was detected and related with inflammatory condition persistence and low CD4 count. We suggest improve vitamin D follow up and measurements in this population with better CD4 count control to avoid future osteoarticular complications of HIV

A New Biomaterial for Urinary Catheters

Several studies argue that an ideal biomaterial for urinary catheters is utopian. Based in literature review it seems to be true. However, research advances: the biomaterial itself, new designs, new coatings, associated drugs, etc. Once implanted and interacting with urine, two old problems persist: encrustation and bacterial colonization. In this context, an extracellular product from bacterial synthesis on sugarcane molasses biomaterial has been studied in several experimental and clinical studies. Based on its high biocompatibility, the aim of this study is to evaluate its performance in an in vivo model as an endourologic prosthesis implanted in the bladder of Wistar rats. We evaluate physical, chemical and biological phenomena in comparison to an already established biomaterial, polyurethane. Even though it is not a finished product, the sugarcane biopolymer presented similar performance compared to polyurethane in several analyzed parameters and has an important characteristic: low cost.

Background: COVID-19 is a rapidly transmissible pneumonia-like illness caused by SARS-CoV-2 that out broke in China in 2019 and is currently circulating worldwide. In the current context of the SARS-CoV-2 pandemic, complications are observed in clinical settings for the treatment of severe COVID-19 disease in nosocomial settings, due to cases of fungal co-infections. Objective: To carry out a review on fungal infections associated with respiratory infections caused by COVID-19 (Sars-Cov-2) and their aggravation. Methodology: The purpose of this study is to inform the reader about the characteristics of SARS-CoV-2 and the main fungal species that are affecting patients undergoing treatment for severe COVID-19, provoking discussion of the importance of the proposed topic, in relation to co-infections by different fungal microorganisms. Result: 80 scientific studies were selected, resulting from patients with COVID-19 and most commonly observed in patients with a history of comorbidities such as diabetes mellitus, hypertension, kidney disease, severe liver disease, oncological diseases, obesity and with severe COVID-19. These data do not represent the total number of records of the disease in the world, but cases reported by researchers in their series, showing the overlapping of fungal co-infection through the compromised immune status due to the use of therapeutic drugs, dysregulation of the microbiota, age of patients and the severity itself of the severe inflammation caused by COVID-19. Conclusion: The immunosuppression caused by the infection of COVID-19 concomitant with its therapy through corticosteroid therapy and comorbidities of the patients made fungal infections more susceptible, and these interfere in the evolution of the case and in the treatment of COVID-19, being relevant to distinguish secondary infections to therapy and the best possible reconstitution of the clinical picture.
