Complement use of Chinese herbal medicine after percutaneous coronary intervention: a prospective observational study

BACKGROUND Chinese herbal medicine is widely used as a complement or alternative treatment in coronary artery disease(CAD)patients after percutaneous coronary intervention(PCI)in *** compared the incidence of the major adverse cardi-ovascular event(MACE)of CAD patients with or without the complement use of Chinese herbal medicine after *** In this prospective,observational study that was conducted from September 2016 to August 2019 in Fuwai Hospital(China),we followed up consecutive patients who received PCI treatment for two *** was defined as the composite all-cause mortality,revascularization,and myocardial infarction(MI)and was compared between those using(integrative medicine group)or those not using Chinese herbal medicine as an additional treatment to standard Western medicine,with unadjusted(Kaplan-Meier curves)and risk-adjusted(multivariable Cox regression)*** A total of 5942 patients after PCI were enrolled in this study,and 5453 patients were included in the final analysis(4189[76.8%]male;mean age:61.9±9.9%years).During the follow-ups,2932(53.8%)patients used only Western medicine while 2521(46.2%)patients had used Chinese herbal medicine as an additional treatment to standard Western *** in the integrative medicine group(IM group)were older than the Western medicine group(WM group),had more females and less pre-vious *** incidence of MACE was 15.3%(449/2932)in WM group and 11.54%(291/2521)in IM *** regression ana-lysis showed that cumulative incidence of MACE was 27%lower in patients of the IM group than those in WM group(hazard ra-tio=0.73;95%CI:0.63-0.85;PsiONS For CAD patients after PCI treatment,complement use of Chinese herbal medicine is associated with a lower 2-year MACE *** prospective studies are warranted to provide higher levels of benefit evidence in these pa-tients.

Qishen capsule safely boosts cardiac function and angiogenesis via the MEK/ERK pathway in a rat myocardial infarction model

Background Qishen(QS) capsules, a Traditional Chinese Medicine, has been widely used to treat coronary heart disease in China. However, evidence of its effectiveness remains unclear. Methods To explore whether QS has cardioprotective efficacy and/or promotes angiogenesis after myocardial infarction (MI), we performed experiments in a preclinical rat MI model. One month after left anterior descending coronary artery ligation, the rats received either QS solution (0.4 g/kg/day) or the same volume of saline by intragastric injection for four weeks. Results Echocardiographic and hemodynamic analyses demonstrated relatively preserved cardiac function in MI rats administered QS. Indeed, QS treatment was associated with reduced infarct scar size and heart weight index, and these beneficial effects were responsible for enhancing angiogenesis. Mechanistically, QS treatment increased phosphorylation of protein kinase B (Akt) and downregulated phosphorylation of mitogen-activated protein kinase/extracellular-regulated kinase (MEK/ERK). Conclusions QS therapy can improve the cardiac function of rats after MI by an underlying mechanism involving increased angiogenesis, at least partially via activation of the Akt signaling pathway and inhibition of MEK/ERK phosphorylation.

Production induced fracture closure of deep shale gas well under thermo-hydro-mechanical conditions

Deep shale gas reservoirs have geological characteristics of high temperature,high pressure,high stress,and inferior ability to pass through *** multi-stage fractured horizontal well is the key to exploiting the deep shale gas ***,during the production process,the effectiveness of the hydraulic fracture network decreases with the closure of fractures,which accelerates the decline of shale gas *** this paper,we addressed the problems of unclear fracture closure mechanisms and low accuracy of shale gas production prediction during deep shale gas *** we established the fluid—solid—heat coupled model coupling the deformation and fluid flow among the fracture surface,proppant and the shale *** the fluid—solid—heat coupled model was applied to the fracture network,it was well solved by our numerical method named discontinuous discrete fracture *** with the conventional discrete fracture method,the discontinuous discrete fracture method can describe the three-dimensional morphology of the fracture while considering the effect of the change of fracture surface permeation coefficient on the coupled fracture—matrix flow and describing the displacement discontinuity across the *** simulations revealed that the degree of fracture closure increases as the production time proceeds,and the degree of closure of the secondary fractures is higher than that of the primary *** creep and proppant embedment both increase the degree of fracture *** reduction in fracture surface permeability due to proppant embedment reduces the rate of fluid transfer between matrix and fracture,which has often been overlooked in the ***,it significantly impacts shale gas production,with calculations showing a 24.7%cumulative three-year yield *** study is helpful to understand the mechanism of hydraulic fracture ***,it provides the theoretical guidance for maintaining the long

Contrast-enhanced magnetic resonance imaging(CE-MRI) is a pivotal tool for global disease diagnosis and management. since its clinical availability in 2009, the off-label use of ferumoxytol for ferumoxytol-enhanced MRI(FE-MRI) has significantly reshaped CE-MRI practices. Unlike MRI that is enhanced by gadolinium-based contrast agents, FE-MRI offers advantages such as reduced contrast agent dosage, extended imaging windows, no nephrotoxicity, higher MRI time efficiency and the capability for molecular imaging. As a leading superparamagnetic iron oxide contrast agent, ferumoxytol is heralded as the next generation of contrast agents. This review delineates the pivotal clinical applications and inherent technical superiority of FE-MRI, providing an avant-garde medical-engineering interdisciplinary lens, thus bridging the gap between clinical demands and engineering innovations. Concurrently, we spotlight the emerging imaging themes and new technical breakthroughs. Lastly, we share our own insights on the potential trajectory of FE-MRI, shedding light on its future within the medical imaging realm.

Focusing on the customization techniques with COM components, the development method and process are illustrated with some case studies.

In this study,a novel equivalent damping ratio model that is suitable for reinforced concrete(RC)structures considering cyclic degradation behavior is developed,and a new equivalent linearization analysis method for implementing the proposed equivalent damping ratio model for use in seismic damage evaluation is *** this end,Ibarra’s peak-oriented model,which incorporates an energy-based degradation rule,is selected for representing hysteretic behavior of RC structure,and the optimized equivalent damping for predicting the maximum displacement response is presented by using the empirical method,in which the effect of cyclic degradation is ***,the relationship between the hysteretic energy dissipation of the inelastic system and the elastic strain energy of the equivalent linear system is established so that the proposed equivalent linear system can be directly integrated with the Park-Ang seismic model to implement seismic damage *** to the simplicity of the equivalent linearization method,the proposed method provides an efficient and reliable way of obtaining comprehensive insight into the seismic performance of RC *** verification demonstrates the validity of the proposed method.
