Small bowel adenocarcinoma in neoterminal ileum in setting of stricturing Crohn’s disease:A case report and review of literature

BACKGROUND Small bowel adenocarcinomas(SBA) are rare malignancies with exceedingly low survival rates, with different presentation in Crohn’s disease(CD). CD-induced SBA poses diagnostic challenges given overlapping presentation with stricturing CD and lack of diagnostics for early detection. Moreover, guidance is lacking on the impact of recently approved therapeutics in CD on SBA management. Here, we aim to highlight the future of CD-induced SBA management and discuss the potential merit of balloon enteroscopy and genetic testing for earlier *** SUMMARY We report the case of a 60-year-old female with longstanding Crohn’s ileitis, presenting with acute obstructive symptoms attributed to stricturing phenotype. Her obstructive symptoms were refractory to intravenous(Ⅳ) steroids, with further investigation via computed tomography enterography not providing additional diagnostic yield. Ultimately, surgical resection revealed SBA in the neoterminal ileum, with oncologic therapy plan created. However, this therapy plan could not be initiated due to continued obstructive symptoms attributed to active CD. Ultimately, infused biologic therapy was initiated, but her obstructive symptoms continued to remain dependent on Ⅳ corticosteroids. Review of diagnostics by a multidisciplinary care team suggested metastatic disease in the peritoneum, lending to a shift in the goals of care to *** With the diagnostic and therapeutic challenges of concurrent SBA and CD, multidisciplinary care and algorithmic management can optimize outcomes.

A Simple Primary Key Algorithm Based Shade Dispersion Method for Maximizing PV Power Generation under Partial Shading Conditions

Maximum Power Point Tracking(MPPT)is crucial for maximizing the energy output of photovoltaic(PV)systems by continuously adjusting the operating point of the panels to track the point of maximum power production under changing environmental *** work proposes the design of an MPPT system for solar PV installations using the Differential Grey Wolf Optimizer(DGWO).It dynamically adjusts the parameters of the MPPT controller,specifically the duty cycle of the SEPIC converter,to efficiently track the Maximum Power Point(MPP).The proposed system aims to enhance the energy harvesting capability of solar PV systems by optimizing their performance under varying solar irradiance,temperature and shading *** results demonstrate the effectiveness of the DGWO-based MPPT system in maximizing the power output of solar PV installations compared to conventional MPPT *** research contributes to the development of advanced MPPT techniques for improving the efficiency and reliability of solar energy systems.

Combination of salicylic acid and ultrasonication for alleviating chilling injury symptoms of longkong

Objectives Chilling injury is a prominent physiological disorder in longkong fruit pericarp when stored under 13℃for a prolonged *** study aimed to investigate the effects of individual salicylic acid(SA)and ultrasonication(US)treatments and of the combination salicylic acid and ultrasonication(SA-US)on alleviating the chilling injury symptoms in longkong fruit pericarp when in prolonged cold *** and Methods SA(1 mmol/L)and US(40 kHz,10 min at 90%amplitude,350 W)were used as individual and combined(SA-US)treat-ments to control the chilling injury in longkong *** various quality measures were checked every 2 d in longkong for up to 18 d of cold storage(13℃,90%relative humidity).Results The results revealed that the control fruits treated with water exhibited severe chilling injury symptoms followed in rank order by US,SA,and SA-US *** such as US and SA alone were more effective in controlling chilling injuries than control,while only minimal signifi-cant differences were noticed between *** the other hand,the longkong pericarp treated with the SA-US combination had significantly increased antioxidant enzyme(superoxide dismutase and catalase)activities and decreased levels of membrane lytic(phospholipase D and lipoxygenase)enzymes and browning-inducing enzymes(phenylalanine ammonia lyase and polyphenol oxidase).Consequently,in the longkong pericarp,the chilling injury index,electrolytic leakage,respiration rate,weight loss,firmness,malondialdehyde content,changes in unsaturated and saturated fatty acid contents,and reactive oxygen species were significantly controlled by this *** The present study concludes that longkong fruit treatment with a combination of US and SA is an excellent alternative for control-ling the chilling injury symptoms and extending the shelf-life.

Building a Trust Model for Secure Data Sharing(TM-SDS)in Edge Computing Using HMAC Techniques

With the rapid growth of Internet of Things(IoT)based models,and the lack amount of data makes cloud computing resources ***,edge computing-based techniques are becoming more popular in present research domains that makes data storage,and processing effective at the network *** are several advanced features like parallel processing and data perception are available in edge ***,there are some challenges in providing privacy and data security over *** solve the security issues in Edge Computing,Hash-based Message Authentication Code(HMAC)algorithm is used to provide solutions for preserving data from various attacks that happens with the distributed network *** paper proposed a Trust Model for Secure Data Sharing(TM-SDS)with HMAC ***,data security is ensured with local and global trust levels with the centralized processing of cloud and by conserving resources ***,the proposed model achieved 84.25%of packet delivery ratio which is better compared to existing models in the resulting *** data packets are securely transmitted between entities in the proposed model and results showed that proposed TM-SDS model outperforms the existing models in an efficient manner.

Generalized Tonic Clonic Seizure(GTCS)is a form of epileptic seizure in which a patient loses control over their entire body,ultimately leading to loss of *** Vagus Nerve Stimulator(VNS)is a tool/method for treating epileptic episodes that sends counter-electrical stimulations to the Vagus Nerve in order to mitigate epileptic signals from the *** machine is a stand-alone device that depends on human *** proposed framework uses an IoT and Blockchain oversight mechanism to augment the device's *** system counteracts against false-activation by monitoring the patient's vitals through a smart watch and allows only legitimate *** nominal operating threshold is determined by preprocessing inferences that include an 18-year-old GTCS epileptic patient and a data set of 281 non-GTCS epileptic *** proposed system functions as a dual control lock where the IoT system and the manually activation system work in tandem to activate the *** on the values sensed by the IoT device,the deployed system is able to make deci-sions and regulate the use of the *** IoT-Blockchain framework is able to fully eradicate false activation by increasing accuracy and transparency,ensur-ing the device is used correctly and safely.

Weed Classification Using Particle Swarm Optimization and Deep Learning Models

Weed is a plant that grows along with nearly allfield crops,including rice,wheat,cotton,millets and sugar cane,affecting crop yield and *** and accurate identification of all types of weeds is a challenging task for farmers in earlier stage of crop growth because of *** address this issue,an efficient weed classification model is proposed with the Deep Convolutional Neural Network(CNN)that implements automatic feature extraction and performs complex feature learning for image *** this work,weed images were trained using the proposed CNN model with evolutionary computing approach to classify the weeds based on the two publicly available weed *** Tamil Nadu Agricultural University(TNAU)dataset used as afirst dataset that consists of 40 classes of weed images and the other dataset is from Indian Council of Agriculture Research–Directorate of Weed Research(ICAR-DWR)which contains 50 classes of weed *** effective Particle Swarm Optimization(PSO)technique is applied in the proposed CNN to automa-tically evolve and improve its classification *** proposed model was evaluated and compared with pre-trained transfer learning models such as GoogLeNet,AlexNet,Residual neural Network(ResNet)and Visual Geometry Group Network(VGGNet)for weed *** work shows that the performance of the PSO assisted proposed CNN model is significantly improved the success rate by 98.58%for TNAU and 97.79%for ICAR-DWR weed datasets.
