Porcine placenta extract (PPE) is widely accepted as an ingredient in complementary and alternative medicine and previous studies have reported its availability, however, its underlying action mechanism remains unclear. In this study, we investigated the underlying mechanism of PPE-induced ceramide synthase 3 upregulation. PPE enhanced the expression of ceramide synthase 3 at both the mRNA and protein levels in HaCaT cells. Moreover, PPE-induced ceramide synthase3 upregulation was suppressed by the Akt inhibitor, suggesting the involvement of Akt in the underlying mechanism. As the PI3K inhibitor did not affect PPE-induced ceramide synthase 3 upregulation, the factors upstream of Akt were estimated. Inhibition and small interfering RNA experiments demonstrated that the peroxisome proliferator-activated receptors δ (PPARδ) and integrin-linked kinase (ILK) are involved in the phosphorylation of Akt. Next, we explored the factors downstream of Akt and found that PPE induced phosphorylation of the STAT3 while PPE-induced upregulation of ceramide synthase 3 was significantly suppressed by the inhibitor of the mTOR, suggesting that the mTOR/STAT3 pathway is involved in the downstream of Akt. These results demonstrate that PPE upregulates CerS3 expression via the PPARδ/ILK/Akt/mTOR/STAT3 pathway.

Barrett’s adenocarcinoma is an esophageal cancer arising from Barrett’s esophagus. In this report, a metastatic epidural tumor from Barrett’s adenocarcinoma with paraplegia that required surgical treatment is documented. A 50-year-old man who had had a resection of the distal esophagus due to Barrett’s adenocarcinoma six month before presented with progressive weakness and numbness in lower limbs with urinary disturbance. Magnetic resonance imaging of the thoracic region revealed compression of the spinal cord at the T3-4 level by a dorsally located epidural mass. No signal changes were detected in vertebrae. Since the paraplegia and urinary disturbance were rapidly progressing, laminectomy and tumor resection was performed. Postoperatively, neurological function improved immediately and there was no recurrence of spinal region. This is the first report on an isolated metastatic Barrett’s adenocarcinoma of the thoracic epidural space.

AIM To compare the outcomes of endoscopic submucosal dissection(ESD) for gastric neoplasms using Clutch Cutter(ESD-C) or other knives(ESD-O).METHODS This was a single-center retrospective study. Gastric neoplasms treated by ESD between April 2016 and October 2017 at Kitakyushu Municipal Medical Center were reviewed. Multivariate analyses and propensity score matching were used to reduce biases. Covariates included factors that might affect outcomes of ESD, including age, sex, underlying disease, anti-thrombotic drugs use, tumor location, tumor position, tumor size, tumor depth, tumor morphology, tumor histology, ulcer(scar), and operator skill. The treatment outcomes were compared among two groups. The primary outcome was ESD procedure time. Secondary outcomes were en bloc, complete, and curative resection rates, and adverse events rates including perforation and delayed *** A total of 155 patients were included in this study; 44 pairs were created by propensity score matching. Background characteristics were quite similar among two groups after matching. Procedure time was significantly shorter for ESD-C(median; 49 min) than for ESD-O(median; 88.5 min)(P shorter procedure time without an increase in complication risk. Therefore, ESD-C could become an effective ESD option for gastric neoplasms.

Occult HBV infection status and its impact on surgical outcomes in patients with curative resection for HCV-associated hepatocellular carcinoma

Background:We sought to clarify the prevalence of occult hepatitis B virus(HBV)infection(OBI)and to determine whether OBI affects the surgical outcomes in curatively resected Japanese patients with hepatitis C virus(HCV)-related hepatocellular carcinoma(HCC).Methods:A total of 257 patients with HCV-related HCC who underwent curative surgical resection were *** enrolled patients were serologically negative for HBV surface antigen and positive for HCV *** was extracted from formalin-fixed paraffin-embedded liver *** was determined by the HBV-DNA amplification of at least two different sets of primers by TaqMan real-time polymerase chain *** outcomes were evaluated according to overall survival(OS),disease-specific survival(DSS),and disease-free survival(DFS).Results:OBI was identified in 15 of the 257(5.8%)*** the multivariate analyses,the factors significantly correlated with OS were BMI>25(P=0.0416),portal vein invasion(P=0.0065),and multiple tumors(P=0.0064).The only factor significantly correlated with DSS was T-stage(P=0.0275).The factors significantly correlated with DFS were liver fibrosis(P=0.0017)and T-stage(P=0.0001).The status of OBI did not show any significant correlation with OS,DSS or DFS,but a weak association with DSS(P=0.0603)was ***:The prevalence of OBI was 5.8%in 257 cases of HCV-related *** a weak association between DSS and OBI was observed,and statistical analyses were limited by small number of OBI cases,no significant correlation between OBI and surgical outcomes was detected.

The coccolithophorids have calcified scales called coccoliths on their surface that include abundant acid polysaccharides. To determine the localization and associative strength of acid polysaccharides Ph-PS-1, -2, and -3 incoccoliths of Pleurochrysis haptonemofera, we analyzed the acid polysaccharides extracted with urea from partially decalcified coccoliths. On treatment of coccoliths with8.0 Murea at pH8.0 inboiling water, Ph-PS-2 was not extracted, but parts of Ph-PS-1 and -3 were from the surface without the crystal morphology being affected. When coccoliths were partially decalcified at various pHs (pH 8.0 - 5.0), Ph-PS-1, -3, and -2 were extracted with urea in that order as the calcite crystals dissolved. Detection of the acid polysaccharides using fluorescein-isothiocyanate-labeled lectin (ConA) and anti-Ph-PS-2 antibodies demonstrated that Ph-PS-2 exists on the surfaces of both untreated and urea-treated coccoliths. Moreover, NaClO-treatment under moderate conditions extracted only Ph-PS-1, -3, and part of Ph-PS-2 from coccoliths, without separation of the crystal units, suggesting that at least some part of Ph-PS-2 acts as a glue connecting crystal units and/or a crystal unit and a base plate inside coccoliths. These results suggest that Ph-PS-2 exists not only on the surface but also inside of the coccoliths, most of it being strongly bound to coccoliths, while Ph-PS-1 and -3 are weakly associated close to the coccolith surface, in that order from the surface. This strongly supports a scheme in which PS-2 type acid polysaccharides play an important role in the crystal nucleation and PS-3 type functions during crystal growth.
