Enhanced recovery after surgery protocol enhances early postoperative recovery after pancreaticoduodenectomy

Background: Enhanced recovery after surgery(ERAS) protocol is a multimodal, multidisciplinary and evidence-based approach to reduce surgical stress and enhance recovery in the postoperative period. This study aimed to analyze the outcome of ERAS protocol in patients after pancreaticoduodenectomy(PD). Methods: A total of 50 consecutive patients with pancreatic/periampullary cancer who underwent PD between January 2016 to August 2017 were included in the study. As per the institute ERAS protocol, nasogastric tube(NGT) was removed on postoperative day(POD) 1 if output was less than 200 mL and oral sips were allowed; oral liquids were allowed on POD2; semisolid diet by POD3; abdominal drain was removed on POD 4 if output was less than 100 mL with no evidence of postoperative pancreatic fistula(POPF); normal diet was allowed on POD5. Discharge criteria on POD6 were afebrile, tolerating oral normal diet, pain free and no surgery related complications(defined as per the ISGPS definitions). Results: NGT was removed on POD1 in 45(90%) patients, abdominal drain removed by POD4 in 41(82%) and 43(86%) patients were discharged on POD6. There was no 30-day postoperative mortality. Three(6%) patients had delayed gastric emptying(DGE). None had postoperative hemorrhage and POPF. Readmission rate was 8%. A significant relation was found between the length of hospital stay(LOS) with age( P < 0.05) and a marginal relation between LOS and postoperative albumin( P = 0.05). Conclusions: ERAS protocol can be safely followed in the perioperative care of patients who undergo PD. Early removal of NGT and allowing oral diet restore bowel function early. ERAS decreases the LOS and postoperative complications.

Patterns of species dominance, diversity and dispersion in ‘Khasi hill sal' forest ecosystem in northeast India

Background： The ＇Khasi hill sal＇ forest ecosystem in Meghalaya, India represents the easternmost limit of sal distribution. We tested if tree diversity and compositional heterogeneity of this ecosystem was higher than other sal-dominated forests due to moister environment. Methods： Vegetation was sampled in 11 transects of 10 m width and up to 500 m length covering 5.2 ha area. All stems ≥10 cm girth at breast height were enumerated. Results： We found a pattern of mixed dominance of Shored robusta （sal） and Schima wollichii and co-dominance of Pinus kesiyo and Careya arborea. The Shannon＇s diversity index （H＇） was 3.395 nats. This value is remarkably high and competitive to that of moist sal forests of eastern Himalayan foothills and sal-dominated forests of Tripura. A high value of H＇ was manifested by： a） high species richness （S = 123）, b） good equitability （70.6%）, c）＇fair＇ resource apportionment, and d） abundance of rare species （84% species with less than one per cent of total individuals, 67% species with two or less individuals ha-1 and 59% species with one or less individuals ha-1）. The compositional heterogeneity was ＇fair＇（Whittaker＇sβw = 3.15）. The presence of Fagaceae with six species commanding 4.3% of importance value （IVl） and of a pine （P. kesiya） in sal forest was remarkable. As many as 58 species showed ＇low density （〈 10 individuals ha-1）, uniform dispersion＇, five species achieved ＇higher density （〉 10 individuals ha-l）, uniform dispersion＇ and six of the top 10 species were ＇clumped＇. The forest showed an exponential demographic curve illustrating ＇good＇ regeneration of an expanding community. Vertical stratification was simple with a poor canopy and fair subcanopy, which together with low basal area （15.65 m2 . ha-1 for individuals ≥ 10 cm gbh） indicated logging of mature sal trees in the past. Tree species diversity and stand structure along major community types in lowland primary and secondary moist deciduous forests in Tripura,Northeast India

Tree species diversity and population structure at different community types were described and analyzed for primary and secondary lowland moist deciduous forests in Tripura. Overall 10,957 individual trees belonging to 46 family, 103 genera and 144 species were counted at ≥30 cm DBH （diameter at breast height） using 28 permanent belt transects with a size of 1 ha （10 m × 1000 m）. Four different tree communities were identified. The primary forests was dominated by Shorea robusta （mean density 464.77 ***^-1, 105 species） and Schima wallichii （336.25 ***^-1, 82 species）, while the secondary forests was dominated by Tectona grandis （333.88 ***^-1, 105 species） and Hevea brasiliensis （299.67 ***^-1, 82 species）. Overall mean basal area in this study was ***^-1; the maximum value was recorded in primary Shorea forest （26.21 ***^-1）. Mean density and diversity indices were differed significantly within four different communities. No significant differences were observed in number of species, genera, family and tree basal cover area. Significant relationships were found between the species richness and different tree population groups across the communities. Results revealed that species diversity and density were increased in those forests due to past disturbances which resulted in slow accumu- lation of native oligarchic small tree species. Seventeen species were recorded with 〈2 individuals of which Saraca asoka （Roxb.） de Wilde and Entada phaseoloides （L.） Men＇. etc. extensively used in local ethnomedicinal formulations. The present S. robusta Gaertn dominated forest was recorded richer （105 species） than other reported studies. Moraceae was found more speciose family instead of Papilionaceae and Euphorbiaceae than other Indian moist deciduous forests. Soil-Plant-Microbe Interactions in Stressed Agriculture Management:A Review

The expected rise in temperature and decreased precipitation owing to climate change and unabated anthropogenic activities add complexity and uncertainty to agro-industry. The impact of soil nutrient imbalance, mismanaged use of chemicals, high temperature, flood or drought, soil salinity, and heavy metal pollutions, with regard to food security, is increasingly being explored worldwide. This review describes the role of soil-plant-microbe interactions along with organic manure in solving stressed agriculture problems. Beneficial microbes associated with plants are known to stimulate plant growth and enhance plant resistance to biotic （diseases） and abiotic （salinity, drought, pollutions, etc.） stresses. The plant growth-promoting rhizobemteria （PGPR） and mycorrhizae, a key component of soil microbiota, could play vital roles in the maintenance of plant fitness and soil health under stressed environments. The application of organic manure as a soil conditioner to stressed soils along with suitable microbial strains could further enhance the plant-microbe associations and increase the crop yield. A combination of plant, stress-tolerant microbe, and organic amendment represents the tripartite association to offer a favourable environment to the proliferation of beneficial rhizosphere microbes that in turn enhance the plant growth performance in disturbed agro-ecosystem. Agriculture land use patterns with the proper exploitation of plant-microbe associations, with compatible beneficial microbial agents, could be one of the most effective strategies in the management of the concerned agriculture lands owing to climate change resilience. However, the association of such microbes with plants for stressed agriculture management still needs to be explored in greater depth.

Effect of Weld Parameters on Effective Notch Stress at Weld Root and Toe of Load Carrying Cruciform Joints

Cruciform joints in ships are prone to fatigue damage and the determination of type of weld plays a significant role in thefatigue design of the joint. In this paper, the effect of weld geometry on fatigue failure of load carrying cruciform joints inships is investigated using Effective Notch Stress (ENS) approach. A fictitious notch of 1 mm radius is introduced at theweld root and toe and fatigue stress is evaluated. The effect of weld leg length (l) and weld penetration depth (p) on ENS atweld root and toe are determined. The critical weld leg length (lcr) at which fatigue failure transitions from weld root toweld toe is investigated. An approximation formula for determination of the critical weld leg length considering weldpenetration depth (p) is proposed.

Hydrofluorosis is a major public health problem in India, which is aggravated by presence or absence of some minerals in water or food. Fluoride by itself or in combination with other elements like molybdenum （Mo） in diet or water changes excretion/retention of bone minerals. Sorghum and pearl millet grown and consumed in fluorosis endemic areas contained significantly higher amounts of Mo than that grown in non-fluorotic areas in India.

Stability of fluid flow in a Brinkman porous medium–A numerical study

The stability of fluid flow in a horizontal layer of Brinkman porous medium with fluid viscosity different from effective viscosity is investigated. A modified Orr-Sommerfeld equation is derived and solved numerically using the Chebyshev collocation method. The critical Reynolds number Re, the critical wave number ac and the critical wave speed cc are computed for various values of porous parameter and ratio of viscosities. Based on these parameters, the stability characteristics of the system are discussed in detail. Streamlines are presented for selected values of parameters at their critical state.

Sedimentary sources and processes in the eastern Arabian Sea:Insights from environmental magnetism,geochemistry and clay mineralogy

The spatial distribution patterns of surficial sediment samples from different sedimentary domains （shallow to deep-sea regions） of the eastern Arabian Sea were studied using sediment proxies viz. environmental magnetism, geochemistry, particle size and clay mineralogy. Higher concentrations of magnetic minerals （high Xlf） were recorded in the deep-water sediments when compared with the shallow water sediments. The magnetic mineralogy of one of the shallow water samples is influenced by the presence of bacterial magnetite as evidenced from the XARM/Xlf VS. XARM/Xfd biplot. However, the other samples are catchment-derived. The high correlation documented for Xlf, anhysteretic remanent mag- netisation （XARM） and isothermal remanent magnetisation （IRM） with Al indicates that the deep-sea surflcial sediments are influenced by terrigenous fluxes which have been probably derived from the southern Indian rivers, the Sindhu （the Indus） and the Narmada-Tapti rivers. A lower Mn concentration is recorded in the upper slope sediments from the oxygen minimum zone （OMZ） but a higher Mn/AI ratio is documented in the lower slope and deep-sea sediments. Clay minerals such as illite （24-48.5%）, chlorite （14.1-34.9%）, smectite （10.6-28.7%） and kaolinite （11.9-27.5%） dominate the sediments of shallow and deep-sea regions and may have been derived from different sources and transported by fluvial and aeolian agents. Organic carbon （OC） data indicate a low concentration in the shallow/shelf region （well oxygenated water conditions） and deeper basins （increased bottom-water oxygen concentration and low sedimentation rate）. High OC concentrations were documented in the OMZ （very low bottom-water oxygen concentration with high sedimentation rate）. The calcium carbonate concentration of the surface sediments from the continental shelf and slope regions （〈 1800 m） up to the Chagos-Laccadive Ridge show higher concentrations （average - 58%） when compared to deep basin sediments （average - 44%）.
