Pancreatic cancer secondary to intraductal papillary mucinous neoplasm with collision between gastric cancer and B-cell lymphoma: A case report

＜正＞ In this paper, a novel slip prevention controller with a load torque disturbance observer using the signal of back EMF is proposed. When slip occurs, the equivalent inertia of EV system will change, and the acceleration of the drive wheels will also change. By constructing an observer, the value of the back EMF is acquired, which includes the information of wheel speed. Then a load torque observer is designed using the signal of back EMF to limit the torque （current） command, and keep the status of EV within a safe area. By selecting observer gain properly, the torque reduction can be adjusted freely in some range. The experimental results of the slip phenomenon simulator utilizing Motor-Generator set verified the effectiveness of the proposed approach.

AIM To examine whether second generation of colon capsule endoscopy(CCE-2) is acceptable for assessing the severity of mucosal inflammation and evaluating mucosal healing using CCE-2 is able to predict outcome in ulcerative colitis(UC) patients, especially in clinical *** A total of 30 consecutive UC patients in clinical remission were enrolled to undergo CCE-2. Clinical remission was defined as clinical activity index(CAI) ≤ 4 according to Rachmilewitz index. The rate of total colon observation and colon cleansing level were evaluated. Severity of mucosal inflammation in UC was assessed according to the Mayo endoscopic subscore(MES) and Ulcerative Colitis Endoscopic Index of Severity(UCEIS). Relapsefree survival was assessed. Acceptability of CCE-2 was assessed using a questionnaire *** The rate of total colon observation within its battery life was 93.3%. The proportion of "excellent" plus "good" cleansing level was 73.3%. The rate of mucosal healing(MES 0, 1) assessed by CCE-2 was 77.0%. The relapse-free survival rate was significantly higher in MES 0, 1 than in MES 2, 3(P = 0.0435), and in UCEIS 0-3 than in UCEIS 4-8(P = 0.0211), whereas there was no significant difference between CAI 0 and CAI 1-4 groups. A questionnaire survey revealed an overall acceptability of *** CCE-2 is acceptable for assessing the severity of mucosal inflammation in UC patients, especially in clinical remission. Evaluating mucosal healing using CCE-2 was able to predict outcome.

AIM The single nucleotide polymorphism(SNP) c.415C>T in exon 3 of NUDT15 affects thiopurine-induced leukopenia in Asian patients with Crohn's disease. Meanwhile, three additional genetic variants of NUDT15 were reported in patients with acute lymphoblastic leukemia. We evaluated the effects of these additional genetic variants of NUDT15 in patients with inflammatory bowel disease(IBD) treated with *** Ninety-six Japanese patients with IBD were enrolled. Genotyping for the NUDT15 and TPMT genes was performed using Custom Taq Man SNP genotyping assays or Sanger sequencing. The changes in white blood cell(WBC) count, mean corpuscular volume(MCV), platelet count, hemoglobin, CRP, amylase, albumin, AST, ALT, and ESR were *** Genetic variants of exon 1 and exon 3 of NUDT15 were identified in 24 of 96 patients(25.0%). C.52G > A and c.36_37 insG GAGTC in exon 1 were found in three patients each. All three patients with c.36_37 insG GAGTC in exon 1 were heterozygotes of ***139 Cys in exon 3. Eighteen patients had ***139 Cys in exon 3 alone. The WBC count gradually decreased after initiation of thiopurine treatment in the mutated cases(n = 24), and was significantly lower at 6, 8, 10, and 16 wk(P = 0.0271, 0.0037, 0.0051, and 0.0185, respectively). The WBC counts were also evaluated in patients with and without prednisolone treatment. In the patients with prednisolone treatment, the WBC count tended to show a greater decrease in the mutated cases, with significant differences at 8 and 10 wk(P = 0.012 and 0.029, respectively). In the patients without prednisolone treatment, the WBC count was significantly lower at 2, 4, 8, and 14 wk in mutated cases(P = 0.0196, 0.0182, 0.0237 and 0.0241, respectively). MCV increased after starting thiopurine treatment in the mutated cases, and was significantly higher at 10 wk(P = 0.0085). Scanning PIV Method and Its Application to the Calorimetry of Ground Source Heat Pump Systems

Ground source heat pump (GSHP) systems that use a direct expansion method?are expected to have higher energy-saving performance than conventional air conditioning systems. The heat transfer rate is evaluated by measuring the temperature, humidity, and flow rate at the indoor unit of the GSHP system. However, it is difficult to evaluate the flow rate by measuring the flow velocity distribution at the outlet of the indoor unit. In this study, the Scanning PIV method is proposed to improve the accuracy of the flow rate measured by hot wire anemometers. The flow rates obtained by the hot wire anemometers were 60.6% and 15.2% higher than those from the PIV method during cooling and heating operation, respectively. Compensation formulas are generated using the results of the Scanning PIV method to correct the measurements from the hot wire anemometers. This compensation formula reduced the error generated by the nonuniformity of velocity distribution. It was 60.6% to 2.5% in cooling operation and 15.2% to 0.9% in heating operation, respectively. The compensation formulas were applied to evaluate the performance of a GSHP system, and the result shows that the GSHP system provides improved performance stability compared to traditional air conditioning systems.

Rifaximin ameliorates hepatic encephalopathy and endotoxemia without affecting the gut microbiome diversity

AIM To determine the efficacy of rifaximin for hepatic encephalopathy(HE) with the linkage of gut microbiome in decompensated cirrhotic *** Twenty patients(12 men and 8 women; median age, 66.8 years; range, 46-81 years) with decompensated cirrhosis(Child-pugh score > 7) underwent cognitive neuropsychological testing, endotoxin analysis, and fecal microbiome assessment at baseline and after 4 wk of treatment with rifaximin 400 mg thrice a day. HE was determined by serum ammonia level and number connection test(NCT)-A. Changes in whole blood endotoxin activity(EA) was analyzed by endotoxinactivity assay. Fecal microbiome was assessed by 16 S ribosome RNA(rR NA) gene *** Treatment with rifaximin for 4 wk improved hyperammonemia(from 90.6 ± 23.9 μg/d L to 73.1 ± 33.1 μg/dL; P < 0.05) and time required for NCT(from 68.2 ± 17.4 s to 54.9 ± 20.3 s; P < 0.05) in patients who had higher levels at baseline. Endotoxin activity was reduced(from 0.43 ± 0.03 to 0.32 ± 0.09; P < 0.05) in direct correlation with decrease in serum ammonia levels(r = 0.5886, P < 0.05). No statistically significant differences were observed in the diversity estimator(Shannon diversity index) and major components of the gut microbiome between the baseline and after treatment groups(3.948 ± 0.548 at baseline vs 3.980 ± 0.968 after treatment; P = 0.544), but the relative abundances of genus Veillonella and Streptococcus were *** Rifaximin significantly improved cognition and reduced endotoxin activity without significantly affecting the composition of the gut microbiome in patients with decompensated cirrhosis.

Transcript Level Analysis of Lignin and Flavonoid Biosynthesis Related Genes in <i>Eucalyptus globulus</i>

We have investigated a correlation of transcript abundances of key genes that influence the quality of wood and flavonoid biosynthesis, such as the two p-hydroxycinnamoyl-CoA:quinate shikimate p-hydroxycinnamoyl transferase (HCT) and the two chalcone synthases (CHS) from Eucalyptus globulus grown in a greenhouse. The EglHCT1 and EglHCT2 transcripts accumulated in stems of all ages, but to a lesser extent in leaves. On the other hand, EglCHS3 and EglCHS4 exhibited high transcript levels in leaves, roots and shoots, but low levels in the stem. A positive correlation (R2 > 0.70) was observed between the transcript levels of the EglHCT1, EglHCT2 genes and Klason lignin (KL) content. In addition, the sum of transcript levels of EglHCT1 and EglHCT2 genes were highly correlated to KL contents (R2 > 0.85). However, there is no relationship between transcript levels of two CHS genes and, KL or flavonoid contents. This may imply that lignin biosynthesis and flavonoid biosynthesis are independently regulated in E. globulus.

Cooperative Particle Swarm Optimization in Distance-Based Clustered Groups

TCPSO (Two-swarm Cooperative Particle Swarm Optimization) has been proposed by Sun and Li in 2014. TCPSO divides the swarms into two groups with different migration rules, and it has higher performance for high-dimensional nonlinear optimization problems than traditional PSO and other modified method of PSO. This paper proposes a particle swarm optimization by modifying TCPSO to avoid inappropriate convergence onto local optima. The quite feature of the proposed method is that two kinds of subpopulations constructed based on the scheme of TCPSO are divided into some clusters based on distance measure, k-means clustering method, to maintain both diversity and centralization of search process are maintained. This paper conducts numerical experiments using several types of functions, and the experimental results indicate that the proposed method has higher performance than the TCPSO for large-scale optimization problems.

Development and Evaluation of an All Weather-Type Solar Drying House to Make for Wood Pellet Material

To suppress the global environment pollutions, we tried to develop a new-type solar drying house by improving a typical agricultural green-house, so that an all weather-type solar drying house was invented ultimately. This house is capable to dry raw wood materials (Ogako) into suitable moisture content (Mc) to make a wood pellet. The all weather-type solar Ogako drying house is covered with a triple transparent film, and an open/close free-type shield sheet is spread along with house’s inner surface with a small space, which is opened when solar radiation is incident on the house in daytime and closed to prevent heat loss from the house while out of sun shining in night. Inside of the all weather-type solar Ogako drying house, there are four belt-conveyors over which four top radiation panels are hanged, and on which four Ogako agitators are touched, a turn-table, two hoppers, four small fans, and besides, a floor heating is molded in concrete floor. Also on the north wall outside the house, two insulated cylinders (chimney) are stood up vertically to exhaust inside moist air passively. Then, to make clearly the operation performance of the house, the drying tests for the proof examination were conducted nineteen times at first test site in Ashoro where is located east-central part of Hokkaido, Japan. As a result of the drying test for the proof examination, it was made clear that the all weather-type solar Ogako drying house is practically useful as a supplementary apparatus to produce the dried Ogako, and consequently to suppress CO2 exhaustion.
