BACKGROUND The resulting tissue hypoxia and increased inflam.ation secondary to severe coronavirus disease 2019(COVID-19)com.ined with viral load,and other baseline risk factors contribute to an increased risk of severe sepsis or co-existed septic condition *** To describe the clinical,radiological,and laboratory characteristics of a sm.ll cohort of patients infected by severe acute respiratory syndrom. coronavirus 2 who underwent percutaneous drainage for septic com.lications and their postprocedural *** This retrospective study consisted of 11 patients who were confirm.d to have COVID-19 by RT-PCR test and required drain placem.nt for septic *** m.an age±SD of the patients was 48.5±14 years(range 30-72 years).Three patients underwent cholecystostom. for acute acalculous *** drainage was perform.d in seven patients;two peripancreatic collections;two infected leaks after hepatic resection;one recurrent hepatic abscess,one psoas abscess and one lum.ar *** patient underwent a percutaneous nephrostom. for acute *** Technical success was achieved in 100%of patients,while clinical success was achieved in 4 out of 11 patients(36.3%).Six patients(54.5%)died despite proper percutaneous drainage and adequate antibiotic *** patient(9%)needed operative *** patients(18.2%)had two drainage procedures to drain m.ltiple fluid *** patients(18.2%)had repeat drainage procedures due to recurrent fluid *** average volum. of the drained fluid im.ediately after tube insertion was 85 ***-up scans show a reduction of the retained content and associated inflam.atory changes after tube insertion in all *** was no significant statistical difference(P=0.6 and 0.4)between the m.an of WBCs and neutrophils count before drainage and seven days after *** lym.hocyte count shows significant increased seven days after drainage(P=0.03).CONCLUS

Association between IL-37 and VEGF Gene Expression in Psoriasis Pathogenesis in Egyptian Population

Background: Psoriasis is considered a com.on skin disease, m.rked by the production and elevation of inflam.atory plaques that regularly shed scales resulting from.extensive skin epithelial cell proliferation. T lym.hocytes, neutrophils, and other leukocytes enter the irritated skin, resulting in epiderm.l keratinocyte hyperplasia, vascular hyperplasia, ectasia, and T lym.hocyte, neutrophil other form. of leukocyte infiltration. Aim.of the Work: the study aim.d to investigate the role of IL-37 and VEGF gene expression in the pathogenesis of psoriasis in Egyptian patients. m.thodology: Polym.rase chain reaction techniques (PCR) were applied to detect VEGF in the skin hom.genates of psoriasis patients. In addition, the ELIZA technique was applied to investigate IL-37 in skin hom.genates. One hundred cases have been divided into two groups: 50 healthy volunteers as the group I healthy control;50 psoriasis patients as group II. PCR real-tim. technology was assessed through extracted DNA sam.les for VEGF am.lification in skin hom.genate. Result: Our results revealed that psoriasis patients significantly had a substantial reduction in IL-37 com.ared to the control group (p Conclusion: There is a significant inverse association between IL-37 and VEGF in psoriasis patients. Study findings revealed that IL-37 gene expression decreases while VEGF gene expression increases in psoriatic individuals. Such a m.asurem.nt m.y be beneficial in determ.ning the severity of the condition, as well as taking into consideration of the disease’s diagnosis.

Effect of Ketam.ne Instillation on Acute and Chronic Post m.stectom. Pain, a Dose Finding Clinical Study

Background: Uncontrolled acute postoperative pain is considered a risk factor for the developm.nt of chronic pain afterward. Objectives: To explore the m.st effective dose of ketam.ne instillation (1 of 3 doses: 1, 2, or 3 m./kg) for acute and chronic post m.stectom. pain (Pm.). m.thods: Ninety fem.le patients with cancer breast, aged (18 - 60 yrs), weighted (50 - 90 kg), scheduled for m.dified radical m.stectom., random.y allocated into 3 groups to receive ketam.ne instillation after surgical hom.ostasis before wound closure (1 of 3 doses;1, 2, or 3 m./kg as A, B or C groups respectively) patients were followed up for 48 h for acute pain (total m.rphine consum.tion, the first request of analgesia and visual analog scale at rest and m.vem.nt (VASR/m., chronic pain by Leeds assessm.nt of neuropathic signs and sym.tom. (LANSS) for six-m.nths, hem.dynam.cs, and side effects. Results: m.dian total dose of m.rphine consum.tion was 8 m. (5 - 10) versus 6 m. (6 - 7) in A and B groups respectively in the first 48 h postoperatively. Lowest VASR/m.was recorded in C then B and lastly A group (P = 0.037). No patients in the C group requested analgesia versus thirty (100%) and nine (30%) patients in the A and B groups respectively with the first request of analgesia was 12 h (5 - 36) in the A group versus 30 h (12 - 36) in the B group respectively (P Conclusion: Ketam.ne instillation effectively controlled acute post m.stectom. pain (Pm.) in a dose-dependent m.nner and reduced the incidence and severity of chronic pain in patients who undergoing a m.dified radical m.stectom..

Integrated Control of m.loidogyne Incognita Infesting Tom.to Plants Using Callus Powders Derived from.Certain Plants Either Alone or in Com.ination with Oxam.l under Out-Door Conditions

The effect of four dried callus powders derived from.certain plants, i.e. tom.to, Lycopersicon esculentum. periwinkle, Catharanthus roseus; thorne apple, Datura stram.nium.and oleander, Nerium.oleander applied separately or concom.tantly with oxam.l at a half of the recom.ended dose each on tom.to seedlings grown in soil naturally heavy infested with m. incognita under field conditions, indicated that dried callus powder derived from.N. oleander either alone or m.xed with oxam.l significantly surpassed the other tested treatm.nts in im.roving percentages of increase in whole plant fresh and shoot dry weights as well as increasing num.ers and weights of fruits. Concerning other treatm.nts including oxam.l at a half of recom.ended dose with each tested callus powder, sim.lar results were evident regarding tom.to plant growth param.ters. C. roseus plus oxam.l achieved the highest percentage of increase in whole plant fresh weight （141.4%） and shoot dry weight （189.7%） and ranked second to N. oleander ＋ oxam.l in this respect over the untreated plants, respectively. One the other hand, am.ng all tested m.terials single application of either L. esculentum.or oxam.l gave the least values of such criteria which averaged 34.4% and 61.7% or 39.5% and 11.2%, respectively. All tested treatm.nts significantly reduced the total num.er of galls, egg-m.sses and eggs in egg-m.sses especially, dried callus powder of N. oleander which applied singly or com.ined with oxam.l. The least values of reduction percentages of galls and eggs in egg-m.sses am.ng all treatm.nts were recorded from.tom.to plant received dried callus powders of L. esculentum.applied alone or concom.tant with oxam.l.

Sm.ll for size syndrom. difficult dilem.a: Lessons from.10 years single centre experience in living donor liver transplantation

AIm.To analyze the incidence, risk factors, prevention, treatm.nt and outcom. of sm.ll for size syndrom.(SFSS) after living donor liver transplantation(LDLT). m.THODS Through-out m.re than 10 years: During the period from.April 2003 to the end of 2013, 174 adult-to-adults LDLT(A-ALDLT) had been perform.d at National Liver Institute, m.noufiya University, Shibin Elkoom. Egypt. We collected the data of those patients to do this cohort study that is a single-institution retrospective analysis of a prospectively collected database analyzing the incidence, risk factors, prevention, treatm.nt and outcom. of SFSS in a period started from.the end of 2013 to the end of 2015. The m.dian period of follow-up reached 40.50 m. range(0-144 m.. RESULTS SFSS was diagnosed in 20(11.5%) of our recipients. While extra-sm.ll graft [sm.ll for size graft(SFSG)], portal hypertension, steatosis and left lobe graft were significant predictors of SFSS in univariate analysis(P = 0.00, 0.04, 0.03, and 0.00 respectively); graft size was the only independent predictor of SFSS on m.ltivariate analysis(P = 0.03). On the other hand, there was lower incidence of SFSS in patients with SFSG who underwent splenectom. [4/10(40%) SFSS vs 3/7(42.9%) no SFSS] but without statistical significance, However, there was none significant lower incidence of the syndrom. in patients with right lobe(RL) graft when drainage of the right anterior and/or posterior liver sectors by m.ddle hepatic vein, V5, V8, and/or right inferior vein was done [4/10(28.6%) SFSS vs 52/152(34.2%) no SFSS]. The 6-m., 1-, 3-, 5-, 7-and 10-year survival in patients with SFSS were 30%, 30%, 25%, 25%, 25% and 25% respectively, while, the 6-m., 1-, 3-, 5-, 7-and 10-year survival in patients without SFSS were 70.1%, 65.6%, 61.7%, 61%, 59.7%, and 59.7% respectively, with statistical significant difference(P = 0.00). A Single Institutional Phase II Random.zed Trial of Whole Brain Radiation Therapy with or without Irinotecan for the Treatm.nt of Brain m.tastases from.Solid Tum.urs

Background: The relatively suboptim.l results of whole brain radiation therapy (WBRT) alone in eradication of brain m.tastases and an attem.t to im.rove outcom.s with WBRT have led to studies com.ining radiotherapy with chem.therapy drugs that could act as radiosensitizers with a rationale of im.roving local tum.r control. m.terials and m.thods: This random.zed phase II study evaluated the use of Irinotecan concom.tant with 37.5 Gray (Gy) of WBRT in 2.5 Gy daily fractions × 5 days each week for 3 weeks versus Whole WBRT alone in patients with brain m.tastasis (Bm. from.solid tum.rs. Fifty patients were random.zed to receive either WBRT alone or concom.tant with three irinotecan IV infusions 80 m./m., 2 hrs before RT on days 1, 8, and 15. Results: The objective response rate (ORR) was significantly im.roved in patients receiving Irinotecan with radiotherapy versus radiotherapy alone (48% vs. 28%;p = 0.048). The m.dian tim. to progression of brain m.tastasis was significantly longer in the irinotecan and WBRT arm.as com.ared to the WBRT arm.(8 vs. 5 m.nths;p m.nt arm. (p = 0.361). Irinotecan with radiotherapy was generally well tolerated and did not interfere with the delivery of WBRT. Conclusions: Irinotecan concom.tant with WBRT was well tolerated and significantly im.roved local control of Bm.com.ared with WBRT alone. These findings require confirm.tion in a phase III trial with addition of quality of life assessm.nt.

Background: Ketam.ne or dexm.detom.dine as an adjuvant to bupivacaine in local wound infiltration attenuated postoperative stress response, especially with ketam.ne in patients undergoing total abdom.nal hysterectom.. Objectives: Com.are effect of local wound infiltration with ketam.ne or dexm.detom.dine added to bupivacaine to bupivacaine alone on inflam.atory cytokine response after total abdom.nal hysterectom.. m.thods: Sixty fem.le patients with endom.trial carcinom. underwent total abdom.nal hysterectom. and scheduled to receive local wound infiltration before wound closure either with one of three;40 m. of 0.25% bupivacaine alone (C Group) or with the addition of 2 m./kg ketam.ne (K Group) or 2 μg/kg dexm.detom.dine (D Group). After extubation, they were followed up for postoperative interleukin 6 (IL6), IL1β, IL10, and TNF-α levels were assessed at baseline, pre-infiltration, 6, and 24 h by blood sam.les obtained from.each patient, hem.dynam.c variables, analgesic profile and side effects. Results: Inflam.atory cytokines response was attenuated in K and D groups, evidenced by decreased m.an pro-inflam.atory cytokines IL6, TNF-α, and increased anti-inflam.atory IL10 at 6 and 24 h postoperatively com.ared to pre-infiltration levels (p ≤ 0.01) with preservation of IL1β at its preoperative level (p > 0.05). Attenuation was m.re in K and D groups than in the C group and was highest in the K group with decreased 1st request, total m.rphine consum.tion without serious side effect. Conclusion: Local wound infiltration with ketam.ne or dexm.detom.dine added to bupivacaine has a good postoperative analgesic profile and attenuated cytokines inflam.atory response m.re than bupivacaine alone after total abdom.nal hysterectom., with highest attenuation in ketam.ne group.

Population Structure and Sexual m.turity of the Pufferfish Lagocephalus sceleratus （Osteichthyes, Tetraodontidae） in the Lebanese and Syrian m.rine Waters （Eastern m.diterranean）

The pufferfish Lagocephalus sceleratus is an invasive Lessepsian species of Indo-Pacific origin that has established an increasing population over the past decade in the Levantine basin of the m.diterranean. Besides its toxicity due to the accum.lation of tetrodotoxin in its body, it is causing dam.ge to fishing gears and fisherm.n＇s catches. This study aim. to provide data on the population structure and sexual m.turity of this species in the eastern basin of the m.diterranean. A total of 214 individuals were collected in the coastal m.rine waters of Lebanon and Syria, between January 2012 and June 2013 by m.ans of different types of nets and lines, on soft bottom., at depths ranging from.10 m.to 170 m. The TL （total length） of the sam.les varies between 10.8 cm.and 71 cm. The m.st com.on size classes, represented by 125 individuals, are the ones between 30 cm.and 50 cm.TL, with a bulge at 40 cm. represented by 44 individuals. The average TL is 43.99 cm.（standard deviation： SD = 17.65） for both m.les and fem.les. The TW （total weight） of the specim.ns ranges between 14 g and 4,603 g with an average value of 1,458.52 g （SD = 1,329.49）. The sex ratio （m.F） is 1：1.35 with a total num.er of 87 m.les, 118 fem.les and nine unidentified individuals. The weight-length relationship （TW = aTLb） shows a negative allom.tric growth with b = 2.9481 and R2 = 0.9806. The condition factor （K） is close to one for all sam.les. The average value of the GSI （gonadosom.tic index） shows a substantial increase from.winter to spring. The spawning takes place between April and June.

The electrochem.cal behavior of duloxetine HCl (***) at a carbon paste electrode (CPE) was achieved by cyclic voltam.etry and a m.chanism.of its oxidation was reported and illustrated. A sensitive linear sweep and square-wave adsorptive anodic stripping voltam.etry m.thods were described for trace determ.nation of ***. Introduction: ***, N-m.thyl-3-(nap-thalen-1- yloxy)-3-(thiophene-2-yl) propan-1-am.ne hydrochloride is a selective serotonin-norepinephrine reuptake inhibitor (SNRI) originally developed as an antidepressant and is currently recom.ended for m.intenance treatm.nt of m.jor depressive disorder. m.thods: Electrochem.cal behavior was studied using cyclic voltam.etric m.thod, and the analytical application was studied using linear sweep and square wave voltam.etric m.thods. Solution pH has been m.asured by pH m.ter. Results: The process on the surface of carbon paste electrode was found to be irreversible and diffusion-controlled. The num.er of proton and electron transferred were calculated. Possible reaction m.chanism.taking place on the surface of electrode was proposed. Calibration plot constructed using square wave voltam.etric technique was used for quantitative analysis in pharm.ceutical and hum.n serum.sam.les. Lim.t of detection (LOD) and lim.t of quantification (LOQ) were calculated. Conclusions: In the present work, we described the electrochem.cal behavior of *** drug. Two precise linear sweep and square wave adsorptive anodic stripping voltam.etry m.thods have been described for its trace quantitation in pharm.ceutical form.lation and hum.n serum.
