Impact of topical nonsteroidal anti-inflammatory drugs in prevention of macular edema following cataract surgery in diabetic patients

AIM： To evaluate the efficacy of prophylactic administration of topical non-steroidal anti-inflammatory drugs（NSAIDs） on macular edema following cataract surgery in diabetic patients, and to compare between types of NSAIDs（ketorolac tromethamine 0.4% and nepafenac 0.1%）. MeTHODS： Group 1（control） received artificial tears substitute as a placebo group, group 2（nepafenac） received topical nepafenac 0.1%, and group 3（ketorolac） received topical ketorolac tromethamine 0.4%. Patients were examined postoperatively after completing one week, one month, two months and three months＇ intervals for evaluating cystoid macular edema（CMe） de.elopment. The main study outcomes were achieving the best corrected visual acuity（BCVA） and change in the central macular thickness（CMT） measured with optical coherence topography（OCT）.ReSULTS： eighty eyes of 76 patients were included in this study. BCVA showed a statistically significant difference at the third month postoperative follow up between the control group and the NSAIDs groups（P=0.04）. There was an increase in the CMT in all cases starting from postoperative first week until third month. CMT showed a statistically significant difference between control group and NSAIDs groups from postoperative first month until third month（P=0.008, 0.027, 0.004）. There was no statistically significant difference between nepafenac and ketorolac groups in BCVA and OCT CMT. CONCLUSION： Prophylactic preoperative and postoperative NSAIDs may have a role in reducing the frequency and severity of CMe in diabetic eyes following cataract surgery.

Functional and anatomical evaluation of the effect of nepafenac in prevention of macular edema after phacoemulsification in diabetic patients

AIM: To evaluate the effect of prophylactic administration of nepafenac in prevention of macular edema occurring in diabetic patients after phacoemulsification and to investigate the correlation between optical coherence tomography(OCT) foveal thickness and multifocal electroretinogram(MF-eRG) ***: The study included two groups. Group 1 included 50 diabetic patients with senile cataract(50 eyes, 30 females, 20 males, aged 55±7 y) received nepafenac 0.1% eye drop. Group 2 included another 50 diabetic patients with senile cataract(50 eyes, 22 female, 28 males, aged 53.8±8 y) did not receive nepafenac. All patients were followed up for 3 mo postoperatively. OCT and MF-eRG were done preoperative and at 1 wk, 1, 2 and 3 ***: The mean foveal thickness was statistically significantly lower in Group 1. Five eyes in Group 2 de.eloped clinical cystoid macular oedema(CMO)(10%), and no patients in Group 1 de.eloped central macular thickening more than 50 μm. There were insignificant differences in MF-eRG amplitudes and latencies between the two groups except in the five eyes that de.eloped CMO, there statistically significant reduction of MF-eRG amplitude with increase in foveal thickness. CONCLUSION: Perioperative nepafenac reduces the incidence of CMO following uncomplicated phacoemulsification significantly. Nepafenac has no side effects.

Visual performance after implantation of two types of phakic foldable intraocular lenses for correction of high myopia

AIM: To compare Visian lens(model V4 c) and Artiflex lens regarding quality(contrast sensitivity) and quantity(efficacy, predictability, safety and stability) of vision in correcting high myopia with recording and analysis of complications. MeTHODS: The comparative prospective study included 39 eyes of 23 patients with high myopia, 19 eyes had Visian lens implantation(model V4 c) and 20 eyes had Artiflex lens implantation. The inclusion criteria were high myopia(higher than 6.0 D) and stable refraction(e over one year). Outcomes included assessment of safety and efficacy indices, predictability, stability, contrast sensitivity and analysis of complications at postoperative 1 d, 1 wk and 1, 3, 6 and 12 mo. Selection of the type of phakic intraocular lens for patients was based on surgeons' preferences, which was no specific selection criteria. ReSULTS: After 12 mo of follow up, difference in uncorrected and corrected distant visual acuity(CDVA) between both groups was statistically insignificant(UDVA for VisianV4 c and Artiflex lens were 0.33±0.2 logMAR and 0.37±0.2 logMAR respectively, P=0.59, CDVA for VisianV4 c and Artiflex lens were 0.155±0.1 logMAR and 0.147± 0.1 logMAR respectively, P=0.87). The efficacy index was 1.25 for VisianV4 c lens and 0.8 for Artiflex lens, 78.9% of eyes were within one diopter spherical equivalent in Visian V4 c lens group compared to 70% in the Artiflex lens group. No eye lost lines of CVDA proving a good safety index for both lenses(safety index was 1.67 for VisianV4 c lens and 1.34 for Artiflex lens). Difference in contrast sensitivity between both groups was statistically insignificant(P=0.15, 0.88, 0.27, 0.32 and 0.82 at five spatial frequencies). CONCLUSION: Both Visian ICL V4 c and Artiflex lensesare safe and effective with stable and predictable refraction and they have comparable contrast sensitivity outcomes with no vision threatening complications.

Launching a paradigm for first and redo-surgery in primary congenital glaucoma: institutional experience

AIM: To evaluate the outcome of the initial and the redo-surgeries for primary congenital glaucoma(PCG) correlated to its degree of severity. MeTHODS: A retrospective study involved patients with PCG presented between 2010 and 2018. Medical records were reviewed to assess the degree of the preoperative severity according to the intraocular pressure(IOP), corneal diameter and corneal edema. Success and failure rates were calculated for both first and redo-surgeries at 6 and 12 mo respectively then correlated to the severity of the cases. ReSULTS: Complete records were retrieved for 272 eyes(153 patients) with PCG: 43 eyes were mild, 136 moderate and 93 severe. Combined trabeculotomy and trabeculectomy(CTT) had the highest success rate in moderate(96.4%) and severe cases(59.3%) while trabeculotomy had the highest success rate in mild cases(96.3%). Medical records of 88 eyes(63 patients) with recurrent PCG were analyzed, most with severe presentation(59 eyes). Ahmed glaucoma valve(AGV) was used in 67(76%) eyes and augmented trabeculectomy in 21(24%) eyes. At 12 mo, there was no statistically significant difference between both surgeries in total success rate(P=0.256). For mild cases, success rate was 100% for both surgeries. Severe cases had higher success rates following AGV(87%) than augmented trabeculectomy(20%). Preoperative severity of the disease was an independent factor affecting the failure rate in secondary trabeculectomy but not in AGV. Patients younger than 24 mo had higher probabilities of failure following both redo-surgeries with hazard ratio =1.325 and 0.37 for augmented trabeculectomy and AGV respectively. CONCLUSION: Preoperative assessment of the severity of eyes with PCG helps in the selecting the optimal primary and secondary surgery. For first surgery, trabeculotomy is more effective in mild cases whereas; CTT and augmented subscleral trabeculectomy(SST) are appropriate for moderate and severe cases. A new technique of monsplasty as an adjunctive procedure in cases of abdominoplasty:A prospective clinical trial

Background:No standard monsplasty technique has existed until *** various monsplasty methods have been described,they exhibit high complication rates with sparse postoperative *** that used pubic suspension techniques did not describe exact anatomical landmarks for the mons *** study aimed to improve the aesthetic and functional appearance of the pubic region with long-term results for all grades of pubic ptosis or bulging with abdominoplasty and provide exact anatomical points to anchor the mons to the rectus ***:This non-randomized prospective clinical trial included 30 patients with various degrees of abdominal wall laxity between December 2017 and September *** surgical procedure was performed for female patients with pendulous abdomen,pubic ptosis or bulging,body mass index≤35 kg/m^(2),and age 18–60 *** patients were followed for up to one year;patient satisfaction was assessed before and after surgery regarding aesthetic results and impact on outfits,hygiene,and sexual ***:The marginal homogeneity test revealed a statistically significant patient satisfaction rate regarding outfits,sexual function,hygiene,and *** postoperative complications,seroma(one case)was managed by aspiration,and one case of wound dehiscence was managed conservatively with ***:This study recommends a quick and reproducible monsplasty technique with a low complication rate,a technique that helps determine specific anatomical landmarks for anchoring the mons to the rectus *** this technique with abdominal contouring is advisable for optimal aesthetic results and maintained lymphatic drainage in the pubic region.

Objective:To evaluate the predatory potential of the Odonata nymph on freshwater snails that serve as intermediate hosts for Schistosoma species(Bulinus truncatus and Biomphalaria alexandrina).Methods:Observations on the searching,attacking and devouring of the two snail types with series of laboratory-based predation experiments,whose aims were to determine daily predation rate,differential predation,prey preference considering small-,mediumand large-sized snails were ***:Laboratory evaluation revealed that,the Odonata nymph could kill and consume the two intermediate *** number of snails consumed differed according to the snail type,size and *** times taken for searching and handling times were dependent on the snail size,type and satiation of the *** predation rate varied also with respect to snail type,size and *** study also evaluated that Odonata nymphs consumed more Bulinus truncatus than Biomphalaria alexandrina per unit time,and that there may be a preference for smaller than larger ***:According to our observation,the predator,Hemianax ephippiger nymph may be a suitable biocontrol agent in connection with Schistosoma intermediate hosts.

Detection of virulent strains of Helicobacter pylori and its relation to symptoms and signs in children

Objective:To detect the prevalence of Helicobacter polori(***) in a group of pediatric patients complaining of gastrointestinal disorders,and to identify the virulent strains of H. pylori and its relation to peptic ***:The study included 60 patients(41 males and 19 females),aged from 6 months to 14 years,with complaining of different gastrointestinal *** IgG antibodies were *** examination of the oesophagus, stomach and duodenum was done in all patients using a fibreoptic *** addition, gastric antral biopsies were taken for diagnosis of *** infection in the following test: rapid urease test,histopathologic examination and *** cytotoxin assay of virulent strains was *** infection was diagnosed if 3 or more tests for diagnosis were found to be ***:33 out of 60 patients(55%) was positive.12(20%) patients had positive toxin producing strains(virulent strains).Virulent strain had a significantly closer relationship with recurrent abdominal pain,while no difference was found in other symptoms. Age of patients showed a highly significant positive correlation with virulent *** strain. Histopathological finding revealed that oesophagitis was significandy common in patients with negative *** and histopathological findings revealed that virulent strain had a significandy higher gastritis and oesophagitis ***:Infection by virulent strains of *** in children is not uncommon:It is associated with endoscopic and histopathologic *** cases infected with virulent strain showed positive results by using rapid urease test,culture and IgG antibodies.

Fundamental solutions for axi-symmetric translational motionof a microstretch fluid

The fundamental solution for the axi-symmetric translational motion of a microstretch fluid due to a concen- trated point body force is obtained. A general formula for the drag force exerted by the fluid on an axi-symmetric rigid par- ticle translating in it is then deduced. As an application to the obtained drag formula, this paper has discussed the problem of creeping translational motion of a rigid sphere in a mi- crostretch fluid. The slip boundary condition on the surface of the spherical particle is applied. The drag force and the other physical quantities are obtained and represented graph- ically for various values of the micropolarity and slip param- eters.

exact solution for the unsteady flow of a semi-infinite micropolar fluid

The unsteady motion of an incompressible micropolar fluid filling a half-space bounded by a horizontal infinite plate that started to move suddenly is considered. Laplace transform techniques are used. The solution in the Laplace transform domain is obtained by using a direct approach. The inverse Laplace transforms are obtained in an exact manner using the complex inversion formula of the transform together with contour integration techniques. The solution in the case of classical viscous fluids is recovered as a special case of this work when the micropolarity coecient is assumed to be zero. Numerical computations are carried out and represented graphically.
