Clinical efficacy and safety of single cycle rituximab as induction therapy for aggressive neuromyelitis optica spectrum disorder in a resource limited center: a preliminary study

Aim: To analyse the efficacy of single dose rituximab (RTX) as induction therapy followed by conventional oral steroid-sparing agents (azathioprine, mycophenolate mofetil or methotrexate) in a cohort of patients with aggressive neuromyelitis optica spectrum disorder (NMOSD) without CD19, 20 and 27 biomarker ***: A retrospective analysis of clinical outcomes in eight patients with aggressive NMOSD treated with one course of RTX induction therapy in the Neurology Department at Kuala Lumpur Hospital from 2005 to 2018 was performed. The effectiveness of the treatment was determined by the number of relapses, expanded disability status scale, annualized relapsed rates, and modified Rankin Scale both before and after treatment. B cell enumeration testing was done instead of CD19, 20 and 27 biomarker testing. Results: There was a reduction in the mean annualized relapse rate from 4.7 to 0.5 attacks per year after treatment (P = 0.011). Mean expanded disability status scale and modified Rankin Scale values improved from 5.4 to 3.6 (P = 0.018) and 3.6 to 2.6 (P = 0.023), respectively. No patient developed any adverse effect. Conclusion: Single-course RTX induction therapy regime may be an alternative therapeutic option in resource limited hospitals to suppress NMOSD disease activity in the short term as pulse induction therapy whilst awaiting the effectiveness of conventional steroid-sparing agents. Further prospectively designed studies are required to prove efficacy.

A 70-day feeding trial was conducted to evaluate the efficacy of Chlorella vulgaris supplementation in diets of giant freshwater prawn(Macrobrachium rosenbergii)post *** of the prawns against Aeromonas hydrophila and several immune parameters(total hemocyte count and prophenoloxidase activity)were also assessed in the ***-nitrogenous,iso-lipidic,and iso-caloric prawn feeds were prepared utilizing a fishmeal based positive control diet(F0)and four dietary treatments with *** inclusion levels of 2(F2),4(F4),6(F6),and 8(F8)%.Post larvae of *** were stocked randomly(mean initial body weight of 0.39�0.38 g)in fifteen net cages(8.1 m�8.9 m with an average depth of 1 m)in an earthen pond for the assessment of growth *** growth indices for prawn fed Chlorella containing diets were enhanced in comparison to the control *** fed diets with Chlorella showed significantly higher(P<0.05)prophenol oxidase activity,total hemocyte counts,and survival rates post bacterial *** findings demonstrated that inclusion of 4%,6%,and 8%*** in prawn diets provided optimal growth rates and improved immunity of the post larvae.

Engagement in clinical learning environment among nursing students: Role of nurse educators

Student engagement in a clinical learning environment is a vital component in the curricula of pre-licensure nursing students, providing an opportunity to combine cognitive, psychomotor, and affective skills. This paper is significant in Arab world as there is a lack of knowledge, attitude and practice of student involvement in the new clinical learning environment. The purpose of this review article is to describe the experiences and perspectives of the nurse educator in facilitating pre-licensure nursing students’ engagement in the new clinical learning environment. The review suggests that novice students prefer actual engagement in clinical learning facilitated through diversity experiences, shared learning opportunities, student-faculty interaction and active learning. They expressed continuous supervision, ongoing feedback, interpersonal relationship and personal support from nurse educators useful in the clinical practice. However, the value of this review lies in a better understanding of what constitutes quality clinical learning environment from the students’ perspective of engagement in evidence-based nursing, reflective practice, e-learning and simulated case scenarios facilitated by the nurse educators. This review is valuable in planning and implementing innovative clinical and educational experiences for improving the quality of the clinical teaching-learning environment.

Thermal Behavior of Coal and Biomass Blends in Inert and Oxidizing Gaseous Environments

Oxy-fuel combustion and gasification (pre-combustion) may have potential for capturing carbon dioxide at lower costs for power generation. Oxy-co-firing and co-gasifying coal with biomass could further reduce effective CO2 emissions and utilize renewable energy resources. A key feature of these two approaches is that they process fuel in concentrated CO2 or O2/CO2 instead of N2 or O2/N2. Accurate predictive models of these processes using blends of coal and biomass can be used in process simulation and could aid in the development and implementation of these technologies. To develop these accurate predictive models, it is important to understand the conversion routes and thermal behavior of these fuels in appropriate gas environments. The objectives of this study are to investigate the impact of inert and oxidative gaseous environments on thermal behavior and reactivity of coal and biomass blends and to study the effect of biomass percentage on coal/biomass blend co-utilization. Fuel samples included a Powder River Basin (PRB) sub-bituminous coal, yellow pine wood sawdust pellets, and mixtures of 10 and 20 weight percent wood in coal. The samples were tested under N2, CO2, and 10% O2 in CO2 by volume using a non-isothermal thermogravimetric method for temperatures up to 1000℃. Fuel weight losses of both coal and wood are essentially the same in CO2 as in N2 in the low temperature range, but higher in 10% O2 in CO2 compared to N2 and CO2. However, total weight losses at 1000℃ under CO2 and 10% O2 in CO2 are similar and higher than in N2 due to char gasification by the CO2 and combustion by O2. The char combustion in 10% O2 in CO2 takes place at lower temperature than char gasification in CO2. Coal and wood blends have higher reactivity compared to coal alone in the lower temperature range due to the high volatile matter content of wood. Interactions of wood and coal in these gas environments and blend percentage are discussed.

目的评价骨性安氏Ⅲ类患者术前正畸去代偿的效果。方法选取32例在中国医科大学附属口腔医院进行正畸-外科联合治疗的骨性安氏Ⅲ类患者,对其术前正畸去代偿前后的X线头影测量片进行测量分析。结果经过术前正畸治疗后上下前牙有明显的去代偿改变。L1-MP平均值由79.0°变为88.1°,下切牙平均唇向直立了9.1°,（P＜0.001）,其变化有统计学意义。U1-SN平均值由111.8°变为109.0°,上切牙平均舌向直立了2.8°,（P＞0.001）,其变化无统计学意义。经术前正畸去代偿后,L1-MP值接近正常值。治疗前拔牙组与非拔牙组的U1-SN平均值均高于正常值,治疗后,达到正常值范围。结论正确的术前正畸治疗为外科手术前移上颌或后退下颌至理想位置打下良好的基础,是治疗骨性安氏Ⅲ类畸形的重要步骤,也是获得高质量手术结果的可靠保证。

花粉受体激酶LePRKs被认为在显花植物有性生殖中具有重要的作用:花粉萌发和花粉管的正确生长需要胞外信号的引导,LePRKs可能通过识别胞外的特异信号,并将其转化为胞内特异信号而发挥这一作用。在花粉中瞬时过表达LePRK1使生长的花粉管顶端出现异常泡状结构,而稳定过表达LePRK1则使花粉不能萌发。这些证据显示LePRK1在LePRKs信号通路中起着重要的作用。我们拟通过基因枪介导的瞬时过表达及农杆菌介导的稳定过表达和RNAi技术获得LePRK1在遗传学水平上影响LePRKs信号通路的证据,并将该信号转导通路与微丝骨架动态等细胞事件联系起来,进一步研究LePRKs信号通路的机制及其对花粉管生长发育的影响。

Immunohistochemical expression of thymidylate synthase and prognosis in gastric cancer patients submitted to fluoropyrimidine-based chemotherapy

Objective： Adjuvant chemotherapy with 5-fluorouracil （5-FU） has been widely used in gastric cancer （GC） patients to prevent relapse after curative resection. 5-FU acts by inhibiting thymidylate synthase （TS）, and high levels of TS correlate with resistance to treatment with fluoropyfimidines. The aim of this study was to evaluate the expression of TS in GC patients, and its relation with clinicopathological characteristics and prognosis in adjuvant chemotherapy with 5-FU. Methods： We retrospectively evaluated 285 patients who underwent D2-gastrectomy with curative intent. TS expression was determined by immunohistochemistry （IHC） in tumor cells by tissue microarray （TMA）. TS level was evaluated according to the intensity and percentage of cells marked by a score system. Patients were divided in three groups according to their TS-score： negative, low and high. Results： TS expression was positive in 92.3% of GC. TS-high, TS-low and TS-negative were observed in 46.3%, 46.0% and 7.7% of patients, respectively. High-TS GC were associated with older age （P=0.007）, high neutrophil/lymphocyte ratio （P=0.048）, well/moderately differentiated histology （P=0.001）, intestinal Lauren type （P〈0.001） and absence of perineural invasion （P=0.003）. Among 285 patients, 133 stage IUIII patients （46.7%） received chemotherapy with 5-FU. In survival analysis, TS-high was associated with worse disease-free survival （DFS） in stage III GC patients who received 5-FU-based chemotherapy （P=0.007）. Multivariate analysis revealed that total gastrectomy, poorly differentiated tumors and high TS-score were associated with worse DFS in stage III GC patients. Conclusions： High TS-score in stage III GC was associated with poor DFS in patients treated with fluoropyrimidine-based chemotherapy.
