Comparison of acid-detergent lignin,alkaline-peroxide lignin,and acid-detergent insoluble ash as internal markers for predicting fecal output and digestibility by cattle offered bermudagrass hays of varying nutrient composition

Background： The potential for acid-detergent insoluble ash （ADIA）, alkaline-peroxide lignin （APL）, and acid-detergent lignin （ADL） to predict fecal output （FO） and dry matter digestibility （DMD） by cattle offered bermudagrass [Cynodon doctylon （L） Pers.] hays of different qualities was evaluated. Eight ruminally cannulated cows （594 ± 35.5 kg） were allocated randomly to 4 hay diets： low （L）, medium low （ML）, medium high （MH）, and high （H） crude protein （CP） concentration （79, 111,131, and 164 g CP/kg on a DM basis, respectively）. Diets were offered in 3 periods with 2 diet replicates per period and were rotated across cows between periods. Cows were individually fed 20 g DM/kg of body weight in equal feedings at 08：00 and 16：00 h for a 10-d adaptation followed by a 5-d total fecal collection. Actual DM intake （DMI）, DMD, and FO were determined based on hay offered, ort, and feces excreted. These components were then analyzed for ADL, APL, and ADIA concentration to determine marker recovery and marker-based estimates of FO and DMD. Results： Forage DMI was affected by diet （P = 0.02）, and DMI from MH and H was greater （P 〈 0.05） than from L. Apparent DMD tended （P = 0.08） to differ among diets while FO （P = 0.20） was not affected by diet treatments. Average ADL recovery （1.16） was greater （P 〈 0.05） than that of ADIA （1.03） and APL （1.06）, but ADIA and APL did not differ （P = 0.42）. Estimates of FO and DMD derived using APL and ADIA were not different （P≥0.05） from total fecal collection while those using ADL differed （P 〈 0.05）. There was no diet by marker interaction （P≥ 0.22） for either FO or DMD. Conclusion： Acid-detergent insoluble ash and APL accurately predicted FO and DMD of cattle fed bermudagrass hay of varying nutrient composition. These internal markers may facilitate studies involving large numbers of animals and forages. Fusing Digital Elevation Models to Improve Hydrological Interpretations

Improving the accuracy of digital elevation is essential for reducing hydro-topographic derivation errors pertaining to, e.g., flow direction, basin borders, channel networks, depressions, flood forecasting, and soil drainage. This article demonstrates how a gain in this accuracy is improved through digital elevation model (DEM) fusion, and using LiDAR-derived elevation layers for conformance testing and validation. This demonstration is done for the Province of New Brunswick (NB, Canada), using five province-wide DEM sources (SRTM 90 m;SRTM 30 m;ASTER 30 m;CDED 22 m;NB-DEM 10 m) and a five-stage process that guides the re-projection of these DEMs while minimizing their elevational differences relative to LiDAR-captured bare-earth DEMs, through calibration and validation. This effort decreased the resulting non-LiDAR to LiDAR elevation differences by a factor of two, reduced the minimum distance conformance between the non-LiDAR and LiDAR-derived flow channels to ± 10 m at 8.5 times out of 10, and dropped the non-LiDAR wet-area percentages of false positives from 59% to 49%, and of false negatives from 14% to 7%. While these reductions are modest, they are nevertheless not only consistent with already existing hydrographic data layers informing about stream and wet-area locations, they also extend these data layers across the province by comprehensively locating previously unmapped flow channels and wet areas.

目的通过对澳大利亚和中国患者利用医疗服务的路线及其内外影响因素分析,讨论通过转诊行为来调节医疗服务利用行为的合理性。方法采用文献评价和政策文件分析,建立患者旅程的通用模型,并分析其在不同国家的变异。结果患者在医疗服务系统内的旅行路线,决定于社区医生、医院专家、患者的行为,而这些行为受制于他们各自的利益和价值取向,并与宏观上鼓励和(或)限制转诊行为的制度因素有关。结论缺乏对被调整者行为分析的政策理想,是难以在现实中实施的。而且仅仅调整社区医生的转诊行为,不足以改变医疗服务利用的方式和方向。应该通过对转诊行为主体,即社区医生、专科医生和患者的行为分析,探讨促成和阻碍有效医疗服务利用的内外因素,从而设计引导所有利益相关者的行为改变的政策。

Drainless Day Case Superficial Parotidectomy with ARTISS [Solutions for Sealant]

Aims: Superficial parotidectomy (SP) is a commonly performed procedure which traditionally requires a drain and overnight hospital stay. This series aimed to show the safety and efficacy of drainless day case parotidectomy using ARTISS [Solution for Sealant] fibrin glue. Materials and Methods: Patients with a superficial parotid lump with benign preoperative sampling underwent an SP. We initially used both ARTISS and a drain, which was removed when output was Objectives: We prospectively collected data and divided groups into ARTISS alone and ARTISS with a drain to compare patients’ length of stay and complication rates. Results: 88 patients were included;52 ARTISS alone, 22 as a day case. 42.3% of patients where ARTISS alone was used were discharged within 24 hours, with the remainder between 24 - 48 hours. Comparatively, no ARTISS and drain patients were discharged within 24 hours and 86.1% were discharged between 24 - 48 hours with the remainder over 48 hours. There were 3 postoperative haematomas and none in the drainless group;a statistically significant difference (p = 0.034). Of the 6 recorded salivary leaks, 4 were in the drain group and 2 in the ARTISS alone group (p > 0.05). Conclusions: Comparable complication rates and reduced length of stay suggest that ARTISS in SP is safe and effective. These findings stand to benefit both patients and the NHS by improving the patient journey and reducing overall costs.

目的:患者体验和满意调查已被认为是收集患者对医疗服务的评价信息和促进医疗服务质量改善的重要方法,2005年至2009年,北京和广西共50余家医院使用本土化的澳大利亚维多利亚州患者满意度监测工具(victorian patient satisfaction monitoring,VPSM)进行了满意度测量,比较分析当前本土化VPSM在我国医院应用情况,为其进一步应用提供参考建议。方法:采用文献研究方法,收集2005年至2009年以来公开或未公开发表的VPSM医院调查方案和监测报告,比较各次调查的操作过程和调查结果,分析优势与不足。结果:各次调查方法大致相同,但问卷结构、提醒感谢信发放、数据采集分析机构等略有差异;调查结果的稳定性和医院间可比性较好,但结果报告形式不够统一,总体上调查逐渐规范。长期监测能够发现医院服务质量的变化,对医院服务质量改进有参考意义。结论:各次调查验证了本土化VPSM工具的优势特点,但仍存在问题,有待进一步本土化和扩大调查范围。

Life-Cycle Cost Analysis of an Installed Multiunit Seasonal Thermal Energy Store

The financial viability of a solar STES （seasonal thermal energy store） installed in a mixed commercial and residential multiunit development of low-energy buildings located in Lysekil, Sweden, a maritime Scandinavian Climate has been investigated. Using recorded figures for the installation costs and performance, a financial life cycle analysis has been undertaken to determine the cost effectiveness of the system. The time value of money is considered and an LCC （life cycle cost） analysis undertaken to identify the cost-effectiveness of the solution. It shows that while a direct heating and hot water system incorporating STES can be economically viable in a Swedish maritime climate in the long term, assistance such as that provided by government incentives is required to assist with the high capital cost of the initial investment.
