Photodynamic therapy vs radiofrequency ablation for Barrett's dysplasia: Efficacy,safety and cost-comparison

AIM:To compare effectiveness,safety,and cost of photodynamic therapy(PDT)and radiofrequency ablation(RFA)in treatment of Barrett’s dysplasia(BD).METhODS:Consecutive case series of patients undergoing either PDT or RFA treatment at single center by a single investigator were ***-three patients with high-grade dysplasia(hGD)had treatment with porfimer sodium photosensitzer and 630 nm laser(130 J/cm),with maximum of 3 treatment ***-three patients with BD(47 with low-grade dysplasia-LGD,6 with hGD)had step-wise circumferential and focal ablation using the hALO system with maximum of 4 treatment *** groups received proton pump inhibitors twice *** biopsies were acquired at 2 and 12 mo after enrollment,with 4-quadrant biopsies every 1 cm of the original BE extent.A complete histological resolution response of BD(CR-D)was defined as all biopsies at the last endoscopy session negative for ***’s exact test was used to assess differences between the two study groups for primary *** all outcomes,a two-sided P value of less than 0.05 was considered to indicate statistical ***:Thirty(91%)PDT patients and 39(74%)RFA were men(P=0.05).The mean age was 70.7±12.2 and 65.4±12.7(P=0.10)year and mean length of BE was 5.4±3.2 cm and 5.7±3.2 cm(P=0.53)for PDT and RFA patients,*** CR-D was(18/33)54.5%with PDT vs(47/53)88.7%with RFA(P=0.001).One patient with PDT had an esophageal perforation and was managed with non-surgical measures and no perforation was seen with *** was five times more costly than RFA at our *** two groups were not randomized and had different BD grading are the limitations of the ***:In our experience,RFA had higher rate of CR-D without any serious adverse events and was less costly than PDT for endoscopic treatment of BD.

The future experimental campaign with the SAMURAI setup at RIKEN will explore a wide range of neutron-deficient nuclei with a particular focus on the most critical(p, γ) reaction rates relevant to the astrophysical rp-process in type-I X-ray bursts(XRB). Intense radioactive-ion(RI) beams at an energy of a few hundred Me V/nucleon will be deployed to populate proton-unbound states in the nuclei of interest through the Coulomb excitation or nucleon-removal processes. The decay of these states into a proton and a heavy residue will be measured using complete kinematics and the information about time reversal proton-capture process will be obtained. This method will provide the vital experimental data on the resonances, which dominate the stellar(p, γ) reaction rates, as well as on the direct proton-capture process for some other cases. The experimental setup will utilize for the first time the high-Resolution90?-mode of the SAMURAI spectrometer in combination with the existing detection systems, including custom-designed Si-strip detectors for simultaneous detection and tracking of heavy ions and protons emitted from the target. The details of the experimental method and the utilized apparatus are discussed in this paper.

A novel uranyl fluoride complex Na(UO)F(Oh)(hO)(Compound 1) has been prepared using hydrothermal method and characterized via single crystal X-ray *** 1 crystallizes in the monoclinic space group C12/c[a = 15.125(3) A,b = 6.941(1)(?),c = 11.257(2)(?),andβ= 94.76 (3)°]and contains an extended three dimensional structure which is constituted by the layers(Fig.1a).This is an unusual structure in uranium inorganic compounds;Further the axial oxo groups of each uranyl which has been well considered chemically inert[1]bridge other two layers up and down,respectively,by coordinating with sodium atoms,and form the so-called cation-cation interactions(CCIs).Similar compounds with CCIs were only reported by a few authors[2].In this work,the uranyl adopts the five-coordinate geometry in the equatorial planes defined by three F atoms and two O *** two uranyl pentagonal bipyramids link into a dimer via sharing an equatorial edge(two F atoms),and neighboring dimers connected together by sharing the same corner(the third F atom) forming zigzags as shown in Fig.1b,***,Na atoms adopt two different coordinate *** the one geometry, Na atoms bond with six O in which four locate in the same layer while other two in the layers up and down, respectively;In the other geometry,Na atoms bond with one F and five O atoms(Fig.1 a,b).In addition, the Compound 1 may likely find potential application in nuclear waste disposal and treatment,as well as serves as a good model to investigate the migration of radioactive actinides in the environment.

We report the radio observations of the eclipsing black widow pulsar J1720-0534, a 3.26 ms pulsar in orbit with a low mass companion of mass 0.029 to 0.034 M⊙. We obtain the phase-connected timing ephemeris and polarization profile of this millisecond pulsar(MSP) using the Five-hundred-meter Aperture Spherical radio Telescope(FAST), the Green Bank Telescope(GBT), and the Parkes Telescope. For the first time from such a system, an oscillatory polarization angle change was observed from a particular eclipse egress with partial depolarization, indicating 10-milliGauss-level reciprocating magnetic fields oscillating in a length scale of 5 ×10^(3)km(assuming an orbital inclination angle of 90°) outside the companion's magnetosphere. The dispersion measure variation observed during the ingresses and egresses shows the rapid raising of the electron density in the shock boundary between the companion's magnetosphere and the surrounding pulsar wind. We suggest that the observed oscillatory magnetic fields originate from the pulsar wind outside the companion's magnetosphere.
