The recent article by Zhou et al was highly interesting and thought provoking. The authors have clearly shown that triptolide administration is associated with upregulation of the Bax gene, resulting in an attenuating effect on cell growth in gastrointestinal malignancies such as pancreatic carcinomas. The article by Zhou et al is all the more important because it highlights the rapidly increasing role of triplodide in the management of systemic malignancies. For instance, triptolide acts on the PI3K/Akt/NF-κB pathway, thereby enhancing apoptosis secondary to the administration of bortezomib in multiple myeloma cells. Similar synergisms are seen when triptolide is administered along with 5-fluoruracil for the management of colonic carcinomas. Similarly, triptolide causes down-regulation of the Bcl-2 gene, resulting in control of cell growth in tumors, such as glioblastoma multiformes.

Magnesium alloys have many advantages as lightweight materials for engineering applications,especially in the fields of automotive and *** undergo extensive cutting or machining while making products out of *** cutting,a sustainable machining method,causes more friction and adhesion at the tool-chip *** of the promising solutions to this problem is cutting tool surface texturing,which can reduce tool wear and friction in dry cutting and improve machining *** paper aims to investigate the impact of dimple textures(made on the flank face of cutting inserts)on tool wear and chip morphology in the dry machining of AZ31B magnesium *** results show that the cutting speed was the most significant factor affecting tool flank wear,followed by feed rate and cutting *** tool wear mechanism was examined using scanning electron microscope(SEM)images and energy dispersive X-ray spectroscopy(EDS)analysis reports,which showed that at low cutting speed,the main wear mechanism was abrasion,while at high speed,it was *** chips are discontinuous at low cutting speeds,while continuous at high cutting *** dimple textured flank face cutting tools facilitate the dry machining of AZ31B magnesium alloy and contribute to ecological benefits.

The role of nm23H1 genetic instability is not limited to gastrointestinal malignancies.A similar close relationship exists between nm23H1 genetic instability and other non gastrointestinal systemic *** instance,in oral malignant melanomas with lymphoid metastasis,the nm23H1 expression is significantly lower in contrast to tumors with no lymphoid ***,increased metastasis is seen in non small cell lung cancers following down regulation of nm23H1 in conjunction with KAI-1 down regulation. There is an inverse relationship between tumor stage and metastasis and nm23H1 expression in individuals with prostate carcinomas and a similar relationship exists between microsatellite instability of the nm23H1 gene and ovarian *** instance,nearly 70.5%of stageⅠ-Ⅱovarian tumors express nm23H1 in sharp contrast to only 25%of stageⅢ-Ⅳovarian *** is clearly evident,nm23H1 has a major role in gastrointestinal and non-gastrointestinal *** coming few years will hopefully see the development of new strategies by virtue of which we can alter nm23H1 expression and thus decrease the risk of metastasis in malignant tumors.

The recent article by Liang et al. （2011）, pub- lished in Journal of Zhejiang University-SCIENCE B （Biomedicine ＆ Biotechnology）, is highly interesting. Interestingly, the past few years have seen the in- creasing application of berbamine as an anti-cancer agent in other systemic malignancies besides chronic myeloid leukemia. For instance, berbamine administration alters the Bcl-2/Bax ratio and mitigates cell migration and thereby augments the anti-proliferative ability of trichostatin A in non-small cell lung cancers （Duan et al., 2010）. Similarly, berbamine inhibits p65 trans- location, up-regulates A20 and increases the activa- tion of the GADD45/c-Jun N-terminal kinase （JNK） pathway, thereby enhancing cellular apoptosis in multiple myelomas （Liang et al., 2009a; 2009b）. Similarly, alteration of the nuclear factor-kappa B （NF-~B） pathway results in enhanced apoptosis in lymphomas （Duet al., 2010）.

A number of new, novel strategies for managing constipation in the elderly have emerged over the past few years. Prucalopride is one such new agent that is highly effi cacious in managing chronic constipation. In fact, Camilleri et al in a recent study reported that the average number of bowel movements increased by at least one in nearly 47% of the patients who were administered a dose of 4 mg. Lubiprostone is another new agent recently approved by the FDA that shows efficacy in managing the symptoms of constipation. Neostigmine has also been successfully used for the management of recalcitrant constipation. Most of these studies have used subcutaneous neostigmine. Symbiotic yogurt containig components, such as Bifidobacterium and fructoligosaccharide, have also been recently shown to be highly effective in improving symptoms of constipation. Elderly patients especially those in hospices and nursing homes are often on opiods for pain management. Constipation secondary to opioid use is extremely common in nursing homes. Subcutaneous methylnaltrexone has recently been shown to be highly effective in the management of opioid-related constipation, and was recently approved by the FDA. Sacral nerve stimulation is another emerging strategy. A recent analysis by Mowatt et al supports the eff icacy of this technique. Botulinum toxin is another agent that has already been successfully used for the management of chronic, refractory constipation in children and may be very effective for elderly constipation. Further larger studies are needed to confi rm the fi ndings noted in these studies. Constipation is clearly a major issue in the elderly and these new, emerging strategies will hopefully improve the quality of life and relieve the symptoms of constipation in this population.
