Endoscopic submucosal dissection(ESD)and related procedures are minimally invasive and cost-effective alternates to ***,there is no approved or listed current procedural terminology(CPT)for *** aimed to review the current reimbursement process hurdles for ESD procedures in private practice model in United *** reviewed the data of two advanced endoscopists(one in New York and other in Pennsylvania State)performing ESD in their private practice *** found the reimbursement process was complex,with number of refusals varied from 0-9 for ESD *** was not paid at all in 8.3%of cases by the medical *** mucosal resection,which is considered inferior as compared to ESD,but has a listed CPT,was denied in only 0.83%*** data highlights the billing hurdles by the endoscopists to adopt ESD-related procedures in private practice model.

About 20000 gastrointestinal endoscopies are performed annually in America in pregnant women. Gastrointestinal endoscopy during pregnancy raises the critical issue of fetal safety in addition to patient safety. Endoscopic medications may be potentially abortifacient or teratogenic. Generally, Food and Drug Administration category B or C drugs should be used for endoscopy. Esophagogastroduodenoscopy(EGD) seems to be relatively safe for both mother and fetus based on two retrospective studies of 83 and 60 pregnant patients. The diagnostic yield is about 95% when EGD is performed for gastrointestinal bleeding. EGD indications during pregnancy include acute gastrointestinal bleeding, dysphagia > 1 wk, or endoscopic therapy. Therapeutic EGD is experimental due to scant data, but should be strongly considered for urgent indications such as active bleeding. One study of 48 sigmoidoscopies performed during pregnancy showed relatively favorable fetal outcomes, rare bad fetal outcomes, and bad outcomes linked to very sick mothers. Sigmoidoscopy should be strongly considered for strong indications,including significant acute lower gastrointestinal bleeding, chronic diarrhea, distal colonic stricture, suspected inflammatory bowel disease flare, and potential colonic malignancy. Data on colonoscopy during pregnancy are limited. One study of 20 pregnant patients showed rare poor fetal outcomes. Colonoscopy is generally experimental during pregnancy, but can be considered for strong indications: known colonic mass/stricture, active lower gastrointestinal bleeding, or colonoscopic therapy. Endoscopic retrograde cholangiopancreatography(ERCP) entails fetal risks from fetal radiation exposure. ERCP risks to mother and fetus appear to be acceptable when performed for ERCP therapy, as demonstrated by analysis of nearly 350 cases during pregnancy. Justifiable indications include symptomatic or complicated choledocholithiasis, manifested by jaundice, cholangitis, gallstone pancreatitis,

Cobalt-silicon based carbon composites(Co–Si/C)have established a noteworthy consideration in recent years as a replacement for conventional materials in the automotive and aerospace *** achieve the composite,a reactive melt infiltration process(RMI)is used,in which a melt impregnates a porous preform by capillary *** method promises a high-volume fraction of reinforcement and can be steered in such a way to get the good“near-net”shaped components.A mathematical model is developed using reaction-formed Co–Si alloy/C composite as a prototype system for this *** wetting behavior and contact angle are discussed;surface tension and viscosity are calculated by Wang’s and Egry’s equations,*** radii of 5μm and 10μm are set as a reference on highly oriented pyrolytic *** graphs are plotted using the model,to study some aspects of the infiltration *** highlights the possible connections among the various *** this attempt,the Co–Si(62.5 at.%silicon)alloy’s maximum infiltration at 5μm and 10μm radii are found as 0.05668 m at 125 s and 0.22674 m at 250 s,respectively.

Many countries developed and increased greenery in their country sights to attract international *** planning is now significantly contributing to their *** next task is to facilitate the tourists by sufficient arrangements and providing a green and clean environment;it is only possible if an upcoming number of tourists’arrivals are accurately *** accurate prediction is not easy as empirical evidence shows that the tourists’arrival data often contains linear,nonlinear,and seasonal *** traditional model,like the seasonal autoregressive fractional integrated moving average(SARFIMA),handles seasonal trends with *** contrast,the artificial neural network(ANN)model deals better with nonlinear time *** get a better forecasting result,this study combines the merits of the SARFIMA and the ANN models and the purpose of the hybrid SARFIMA-ANN ***,we have used the proposed model to predict the tourists’arrival inNew Zealand,Australia,and *** results showed that the proposed hybrid model outperforms in predicting tourists’arrival compared to the traditional SARFIMA and ANN ***,these results can be generalized to predict tourists’arrival in any country or region with a complicated data pattern.
