Should we change our approach to resuscitating victims of femoral fracture?A clinical experience in a busy trauma hospital in Shiraz,Iran

Purpose Traumatic hemorrhagic shock is a life-threatening event *** brain trauma accompanying femoral fractures can trigger inflammatory responses in the body and increase pre-inflammatory cytokines such as TNF-α,*** primary treatment in these cases is hydration with crystalloids,which has both benefits and *** purpose of this study was to investigate the effects of fluid therapy on the hemodynamics,coagulation profiles,and blood gases in such *** In this cross-sectional study,patients were divided into two groups:femoral fracture group and non-femoral *** hemodynamic status,coagulation profile,and blood gases of patients in both groups were evaluated upon arrival at the hospital and again 2 h *** were analyzed by t-test and ANOVA with repeated data and paired samples *** A total of 681 trauma patients(605 men and 76 women)participated in this study,including 69(86.3%)men and 11(13.8%)women in femoral fracture group and 536 men(89.2%)and 65 women(10.8%)in non-femoral *** laboratory parameters were evaluated in response to the equal amount of crystalloid fluid given upon arrival and 2 h *** gases decreased in the fracture group despite fluid therapy(p<0.003),and the coagulation profile worsened although the change was not statistically *** The treatment of multiple-trauma patients with femoral bone fractures should be more concerned with the need for the infusion of vasopressors such as *** there is evidence of clinical shock,excessive crystalloid infusion(limited to 1 L)should be avoided,and blood and blood products should be started as soon as possible.

Phenylalanin Ammonia-lyase Activity,Total Phenolics and Flavonoids Contents in Flowers,Leaves,Hulls and Kernels of Three Pistachio(Pistacia vera L.) Cultivars

Phenylalanin ammonia-lyase(PAL) plays a pivotal role in the production of phenolic compounds,which are responsible for the success of the defense strategies in harsh environments in response to different *** of the PAL activity,total phenolics,total flavonoids and anthocyanin contents were performed in flowers,leaves and fruits of three pistachio cultivars "Ahmadaghaii","Ohadi" and "Kallehghuchi".The results showed that PAL activity was different in cultivars and in plant organs of pistachio trees(flowers,leaves and fruits).The highest activity rate of their compounds was observed in Ahmadaghaii cultivar.A positive correlation was observed between PAL activity,total phenolics and total flavonoids in leaves,and a negative correlation between PAL activity and anthocyanin contents in leaves and flowers of Ahmadaghaii *** activity and total phenolics in fruits of pistachio suffered a decrease when the maturation processes *** is suggested that the hulls of the pistachio fruits,containing high level of phenolic compounds(especially in Ahmadaghaii cultivar),may function as a protective layer of defense chemicals against ultraviolet radiation and *** final concentration of phenolic compounds,flavonoids and antocyanins in the kernel depend on PAL activity in the kernel's *** results led to the conclusion that increase in PAL activity,phenolic compounds and flavonoids in Ahmadaghaii can help the plant to cope with the stresses better than the other *** phenolic compounds are antioxidant and scavenge free oxygen,it is postulated that Ahmadaghaii is the most resistant cultivar to the environmental stresses.

An efficient, convenient and green method has been introduced for the preparation of 14-aryl（alkyl）-14H-dibenzo[a,j]xanthene and 1,8-dioxooctahydroxanthene derivatives by the reaction of aldehydes with 2-naphthol and 5,5-dimethyl-1,3-cyclohexanedione, respectively, in the presence of ZrO（OTf）2 as a reusable catalyst under solvent-free conditions. In addition, very short reaction times, excellent yields, straightforward procedure, and relatively non-toxicity of the catalyst are other noteworthy advantages of the present method.

Objective:To investigate Echinococcus(E.)granulosus genotypes as the causative agents of hydatidosis in humans in the southwest of Iran(Khuzestan province).Methods:In this study,isolates of 80 archived human paraffin embedded hydatid cysts were collected from pathology laboratories in Ahvaz city,Khuzestan *** was extracted and examined by nested-PCR of ribosomal DNA(rDNA)internal transcribed spacer 1(ITS1),and *** addition,the sequences of fragments of genes coding for Cox space1 and NADH dehydrogenase 1(ND1)were also ***:Of the 80 paraffin samples,44(55.0%)were from the liver,27(33.8%)from the lung,and the rest from other *** amplified hydatid genomic DNA showed that the cysts were *** *** results of PCR-RFLP and sequencing analysis revealed the presence of G1 genotype(sheep strain)in all human ***,no camel strain(G6)was detected among all samples in the regions ***:The molecular findings indicate that the predominant genotype involved in *** transmission in southwest of Iran is the common sheep strain(G1),which occurs in human *** results may have important implications for hydatid disease control in the studied areas.

T2* magnetic resonance imaging of the liver in thalassemic patients in Iran

AIM: To investigate the accuracy of T2*-weighted magnetic resonance imaging (MRI T2*) in the evaluation of iron overload in beta-thalassemia major patients. METHODS: In this cross-sectional study, 210 patients with beta-thalassemia major having regular blood transfusions were consecutively enrolled. Serum ferritin levels were measured, and all patients underwent MRI T2* of the liver. Liver biopsy was performed in 53 patients at an interval of no longer than 3 mo after the MRIT2* in each patient. The amount of iron was assessed in both MRI T2* and liver biopsy specimens of each patient. RESULTS: Patients’ ages ranged from 8 to 54 years with a mean of 24.59 ± 8.5 years. Mean serum ferritin level was 1906 ± 1644 ng/mL. Liver biopsy showed a moderate negative correlation with liver MRI T2* (r = -0.573, P = 0.000) and a low positive correlation with ferritin level (r = 0.350, P = 0.001). Serum ferritin levels showed a moderate negative correlation with liver MRI T2* values (r = -0.586, P = 0.000). CONCLUSION: Our study suggests that MRI T2* is a non-invasive, safe and reliable method for detecting iron load in patients with iron overload.

There is a high overvoltage in the oxidation of methanol in fuel cells,and so modified electrodes are used to decrease it.A modified electrode that used Ni(II) loaded analcime zeolite to catalyze the electrooxidation of methanol in alkaline solution was *** zeolite was synthesized by hydrothermal synthesis,and Ni(II) ions were incorporated into the analcime structure,which was then mixed with carbon paste to prepare modified *** electrocatalytic oxidation of methanol on the surface of the modified electrode in alkaline solution was investigated by cyclic voltammetry and *** effects of the scan rate of the potential,concentration of methanol,and amount of zeolite were *** rate constant for the catalytic reaction of methanol was 6 × 103 cm3 mol-1 s-1 from measurements using *** proposed electrode significantly improved the electron transfer rate and decreased the overpotential for methanol oxidation.
