Short-term effectiveness of radiochemoembolization for selected hepatic metastases with a combination protocol

AIM： To introduce the combination method of radio- chemoembolization for the treatment of selected he- patic metastases. METHODS： Twenty patients with biopsy proven hepat- ic metastases were selected from those who underwent transarterial radiochemoembolization, a novel combina- tion protocol, between .lanuary 2009 and July 2010. Patients had different sources of liver metastasis. The treatment included transarterial administration of three chemotherapeutic drugs （mitomycin, doxorubicin and cisplatin）, followed by embolization with large （50-150 μm） radioisotope particles of chromic 32R Multiphasic computer tomography or computer tomography stud- ies, with and without contrast medium injections, were performed for all patients for a short-term period before and after the treatment sessions. The short-term effec- tiveness of this procedure was evaluated by modified response evaluation criteria in solid tumors （mRECIST）, which also takes necrosis into account. The subjective percentage of necrosis was also assessed. The re- sponse evaluation methods were based on the changes in size, number, and the enhancement patterns of the lesions between the pre- and post-treatment imaging studies. RESULTS： Patients had liver metastasis from colorectal carcinomas, breast cancer, lung cancer and carcinoid tumors. The response rate based on the mRECIST criteria was 5% for complete response, 60% for par- tial response, 10% for stable disease, and 25% for progressive disease. Regarding the subjective necrosis percentage, 5% of patients had complete response, 50% had partial response, 25% had stable disease, and 20% had progressive disease. Based on traditional RECIST criteria, 3 patients （15%） had partial response, 13 patients （65%） had stable disease, and 4 patients （20%） had disease progression. In most patients, colorectal carcinoma was the source of metastasis （13 patients）. Based on the mRECIST criteria, 8 out of these 13 patients had partial responses, while one remained stable

T2* magnetic resonance imaging of the liver in thalassemic patients in Iran

AIM: To investigate the accuracy of T2*-weighted magnetic resonance imaging (MRI T2*) in the evaluation of iron overload in beta-thalassemia major patients. METHODS: In this cross-sectional study, 210 patients with beta-thalassemia major having regular blood transfusions were consecutively enrolled. Serum ferritin levels were measured, and all patients underwent MRI T2* of the liver. Liver biopsy was performed in 53 patients at an interval of no longer than 3 mo after the MRIT2* in each patient. The amount of iron was assessed in both MRI T2* and liver biopsy specimens of each patient. RESULTS: Patients’ ages ranged from 8 to 54 years with a mean of 24.59 ± 8.5 years. Mean serum ferritin level was 1906 ± 1644 ng/mL. Liver biopsy showed a moderate negative correlation with liver MRI T2* (r = -0.573, P = 0.000) and a low positive correlation with ferritin level (r = 0.350, P = 0.001). Serum ferritin levels showed a moderate negative correlation with liver MRI T2* values (r = -0.586, P = 0.000). CONCLUSION: Our study suggests that MRI T2* is a non-invasive, safe and reliable method for detecting iron load in patients with iron overload.

Optimization of Liver Iron Load Assessment by Pixel-Based T2* MRI in Thalassemic Patients

Purpose: To improve liver iron load assessment by investigating the precision of different approaches of T2* Measurement. Background: Iron overload is a major problem in the treatment of thalassemic patients. Liver iron concentration (LIC) is an important index toward the management of body iron load. The accuracy of iron load estimation may suffer from the methodology of T2* measurement and there is no complete agreement upon the best approach of T2* calculation. Methods: 32 β-thallasemic patients (18 male) with the mean age of 20.0 ± 6.5 years were involved in this study. A multi-echo fast gradient-echo technique on a 1.5 T MRI system was used to measure liver iron overload and the T2* map of liver was reconstructed on a pixel-by-pixel basis. The T2* map and MRI images were utilized to deter- mine accurate location of ROI (region of interest). The mean of T2* were computed from the ROIs. The reproducibility of calculated T2* values in two methods were obtained. Moreover, the mean of the pixel’s T2* was calculated in the entire liver parenchyma of one slice. The T2* value of the entire slice was compared with the ROI approach. Results: In the ROI based method, the CoV for the intra-observer reproducibility was 8.5% and for the inter-observer was 9.78%. In the pixel based method, the CoVs for intra-observer and inter-observer reproducibility were 2.79% and 3.91%. There was an acceptable correlation (r = 0.96) between the T2* values calculated by the ROI and the entire slice. Conclusions: The pixel-based approach is more precise to determine the appropriate placement of the ROI. The assessment of T2* in the entire slice reduces the user-based errors significantly.

A novel approach towards parametric assessment of reliability and resilience of high voltage mica-based insulation systems by statistical analysis of experimental failure data

This study aimed to investigate the osteo-inductive capacity of a fucoidan polysaccharide network derived from brown algae on human adipose-derived stem cells(HA-MSCs)for bone *** physiochemical properties of the scaffold including surface morphology,surface chemistry,hydrophilicity,mechanical stiffness,and porosity were thoroughly *** in vitro and in vivo measurements implied a superior cell viability,proliferation,adhesion,and osteo-inductive performance of obtained scaffolds compared to using specific osteogenic induction medium with increased irregular growth of calcium crystallites,which mimic the structure of natural *** scaffold was highly biocompatible and suitable for cell *** examinations,such as quantification of mineralization,alkaline phosphatase,gene expression,and immunocytochemical staining of pre-osteocyte and bone markers confirmed that HAD-MSCs differentiate into osteoblasts,even without an osteogenic induction *** study provides evidence for the positive relationship and synergistic effects between the physical properties of the decellularized seaweed scaffold and the chemical composition of fucoidan in promoting the osteogenic differentiation of ***,the natural matrices derived from brown seaweed offers a sustainable,cost-effective,non-toxic bioinspired scaffold and holds promise for future clinical applications in orthopedics.

Game Theory Based Model for Predictive Analytics Using Distributed Position Function

This paper presents a game theory-based method for predicting the outcomes of negotiation and group decision-making problems. We propose an extension to the BDM model to address problems where actors’ positions are distributed over a position spectrum. We generalize the concept of position in the model to incorporate continuous positions for the actors, enabling them to have more flexibility in defining their targets. We explore different possible functions to study the role of the position function and discuss appropriate distance measures for computing the distance between the positions of actors. To validate the proposed extension, we demonstrate the trustworthiness of our model’s performance and interpretation by replicating the results based on data used in earlier studies.

Considerable research has been conducted into the efficacy of individual probiotics in broiler production,however information on the most effective combinations of synergistic Bacillus probiotic is *** study investigated the impact of different Bacillus strain combinations in broiler chickens,as well as in vitro enzyme *** experiment one,a total of 576 Ross 308 broilers at 1 d old were grown for 21 d across 6 treatments of maize-soybean diets(n=12 pens per treatment)to compare three different strain combinations(formulation 1[F1]:3 strains Bacillus amyloliquefaciens;F2:Bacillus coagulans and 2 strains ***;F3:***,Bacillus licheniformis and 2 strains ***;F5:Bacillus subtilis,*** and 2 strains ***),positive control(PC),and a negative control antibiotic treatment group(NC).In Exp.2,a total of 360 one-day-old ROSS308 broilers were used to test five treatments(n=9)including PC,NC,F1 and F5(selected from Exp.1),and F4(Bacillus pumilis and 2 strains ***)in a maize-soybean *** F1 demonstrated a significant improvement in feed conversion ratio(FCR)compared to F2 at d 14(1.49 vs 2.10;P=0.038)and the body weight(BW)at d 21(847.0 g vs 787.4 g)compared to other combinations(P=0.027).The FCR at d 21 tended to be lower in birds fed F1(1.46 vs 1.66)compared to the control(P=0.068).Probiotic treatments had significantly improved nutrient digestibility compared to the PC and ***,probiotic treatments supported the growth of Streptococcus,a common commensal genus and reduced the abundance of genera that correlated with low weight gain such as *** two revealed that F4 improved FCR(P<0.001)and BW at 28 d(P=0.014).In vitro testing showed a high production of protease and amylase by ***,the addition of Bacillus probiotics,particularly containing *** strains and Bacillus pumilus,into the diet of broiler chickens improves

Domestic PV System with Feedback Linearization-based Control Strategy for Module-level MPPT under Partial Shading Condition

A partial shading condition can adversely affect the energy conversion efficiency of domestic photovoltaic(PV) systems. Connecting each PV module to a microinverter and performing module-level maximum power point tracking(MPPT)are proposed as promising solutions. In this paper, a feedback linearization-based control strategy is designed for the nonlinear system by a novel straightforward approach. The obtained nonlinear control law can independently govern each microinverter, providing module-level MPPT for PV modules without DC optimizer. Moreover, PV modules can be easily connected or disconnected due to the lug-and-play ability of the proposed controller. As a result, the proposed PV system can be easily maintained and extended even by non-expert users. Moreover,any module failure in the proposed PV system can be tolerated without impacts on the normal operation of other PV *** advantages of the proposed control strategy are verified by the simulation of a test PV system in MATLAB/Simulink under various partial shading conditions as well as adding or removing PV modules.
