Infectious Complications in Lupus Nephritis and Associated Factors: A Multicenter Study

Introduction: The prevalence of infections in patients with systemic lupus erythematosus ranges from 26% to 78%. Patients with systemic lupus erythematosus are particularly susceptible to infections due to a dysfunction of immune response and the immunosuppressive therapy. Patients and method: We carried out this study with the aim of describing the prevalence of infections in lupus nephritis and determining the associated risk factors. This was a multi-center, observational, retrospective, descriptive and analytical study over a 10-year period from November 1, 2010 to October 31, 2020. The study was carried out in the nephrology departments of six hospitals in Dakar. Results: During the study period, 98 patients were included. The mean age was 32.32 ± 11.33 years with a sex ratio of 0.21. Among the included patients, fifty-four (55.1%) had at least one infectious episode, of which 53.7% had 1 infection, 24.1% had 2 infectious episodes and 22.2% had 3 infectious episodes. The overall incidence was 55.1 infections per 100 patient-years. 57.2% of these infectious episodes occurred within the first six months after the lupus was diagnosed. The main sites of infection were urinary (30.7%), gastrointestinal (22.0%) and pleuropulmonary (16.5%). The incriminated germ was a bacterium in 78.18% of cases, a virus in 5.46%, a parasite in 9.09 and a fungus in 7.27. The most frequently germ found was Escherichia coli (29.09%). The evolution was marked by recovery in 93.4% of cases. Deaths occurred in 15 patients of which 33.3% were related to infections. Factor significantly associated with the onset of infection in multivariate analysis was the presence of a proliferative class of lupus nephritis (p = 0.013). Conclusion: Infections were common during lupus nephritis. The presence of a proliferative class was risk factors for infection.

为了探明自吸泵的自吸机理.以外混式自吸泵为研究对象,利用CFX仿真软件中的VOF(volume of fluid)两相流模型和k-ε湍流模型,并结合网格滑移技术进行自吸过程非稳态数值模拟,分析了不同时刻下泵内气液两相体积、液相速度、出口气体体积分数和压力的分布情况.同时改变非稳态模拟的时间步长及总时长,监测自吸过程叶轮及蜗壳内监测点的压力变化.结果表明:自吸泵的吸气与排气主要集中在启动过程的初期和中期,叶轮入口和泵出口的气体体积分数分别超过60%和75%;叶轮出口的气液混合带导致了叶轮出口到蜗壳内侧大片区域内的流动紊乱现象,降低了叶轮的做功能力,进而影响了泵的正常排气;叶轮监测点压力脉动幅值与距叶片工作面的大小有关,蜗壳内监测点压力脉动幅值与距叶轮外缘的大小有关.

Rectitis Radiation a Problem of Therapeutic Management in Senegal about Two Cases

Radiotherapy occupies an important place in the management of cancers of the pelvic-perineal organs, it is at the origin of the fact of the ionizing radiations of radiation proctitis which is most often revealed by hematochezia. Diagnosis is based on rectoscopy which highlights a telengiectatic proctitis aspect. The standard treatment is endoscopic and based on argon plasma electrocoagulation. We report two cases of radiation proctitis occurring a few months after radiotherapy. Case number 1: A 76-year-old hypertensive patient known to have a history of cancer of the cervix treated with radiochemotherapy, she was received in an array of hematochezia of medium abundance;total colonoscopy found an aspect of telengiectatic proctitis making retain radiation proctitis. She had benefited from medical treatment, however, after a two-month follow-up, there was a recurrence of rectal bleeding despite medical treatment, due to the unavailability of endoscopic treatment based on argon plasma, surgical treatment of a type of protectomy had been carried out on postoperative follow-up was marked by a regression of hemorrhage with the occurrence of a. Case number 2: An 87-year-old patient, hypertensive, with a history of prostate adenocarcinoma 3 years ago treated with brachytherapy and hormone therapy;he was hospitalized with hematochesia. Rectosigmoidoscopy found an aspect of telengiectatic proctitis. The diagnosis of radiation proctitis had been retained and the patient had received medical treatment. The short-term evolution was marked by an amendment of the hematochezia with a long-term recurrence requiring endoscopic destruction via the diathermic loop of the telengiectatic lesions with good evolution without relapse after the procedure. Conclusion: Radiation proctitis is a major side effect of pelvic radiotherapy. Management is very difficult in our regions due to the unavailability of endoscopic means, in particular argon plasma.

Multiple Myeloma Secondary to HIV Infection, Revealed by Renal Failure: About a Case

Multiple myeloma is on the list of neoplasia that may be associated with human immunodeficiency virus infection. It is an affection that aggravates the prognosis in these particular patients. We present the case of a patient with multiple myeloma and HIV infection, revealed by renal failure. This was a 59-year-old patient who was received to the Department of nephrology for renal failure associated with severe aregenerative pancytopenia. In etiological investigations, multiple myeloma associated with HIV1 infection was found. The evolution was unfavorable, marked by the death of the patient caused by digestive haemorrhage before the start of antiretroviral treatment and chemotherapy.

Eosinophilic Gastroenteritis in Tropical Area: about 6 Cases from Senegal

The aetiologies of hypereosinophilia are dominated by digestive parasitosis and drug intake. In the tropics, because of the frequency of parasitosis, eosinophilic digestive pathologies of primary origin, which are rare, may be overlooked. We report 6 cases of eosinophilic gastroenteritis with polymorphic digestive manifestations testifying to the different locations of eosinophilic infiltration in the digestive parietal layer. Three patients had ascites rich in eosinophils, indicative of serous involvement, while the other two had a muscular form, and the last a mucosal involvement. The evolution was favourable in all cases with corticosteroid therapy, but a recurrence was noted in 4 cases when treatment was stopped.

呕吐毒素（vomitoxin）,又名脱氧雪腐镰刀菌烯醇（deoxynivalenol,简称DON）,化学名为3α,7α,15-三羟基草镰孢菌-9-烯-8-酮,属于B型单端孢霉烯族化合物,是目前世界范围内谷物及其制品中污染最为普遍、含量最高的一种真菌毒素。除DON外,谷类食品和饲料还经常被其隐蔽型衍生物如3-acetyl-DON,15-acetyl-DON和DON-3-β-D-glucoside等共同污染。DON及其隐蔽型衍生物污染严重威胁我国粮食安全、食品安全和人民生命健康,并造成巨大经济损失,受到国内外科学家的高度关注。本文研究了鸡不同肠段部位（十二指肠、空肠、回肠、盲肠）内容物对DON和乙酰基DON毒素的代谢转化规律;采用宏基因组测序研究了不同肠段部位的菌群组成;对鸡肠道菌群代谢转化DON和乙酰基DON毒素的规律进行了初步探究。主要研究结论如下:（1）取肉鸡十二指肠、空肠、回肠和盲肠内容物,在厌氧培养箱内用PBS缓冲液稀释后,与DON毒素共同孵育培养。培养温度37℃,培养时间分别为0、2、4、6、24和48 h。实验结果表明:鸡空肠、回肠和盲肠内容物能够将DON毒素去环氧化转化为去环氧-脱氧雪腐镰刀菌烯醇（DOM-1）。空肠内容物样品孵育48 h后,34.34±0.04%的DON转化为DOM-1;回肠内容物样品孵育24 h和48 h之后,分别35±0.03%和36.08±1.34%的DON转化为DOM-1;盲肠内容物样品对DON的转化效率最高,在孵育24 h后,全部的DON毒素转化为DOM-1。（2）在上述孵育培养条件下,将乙酰基DON毒素与肠道内容物共同孵育培养。实验结果表明:肉鸡十二指肠、空肠、回肠和盲肠内容物均能将3-乙酰基-DON毒素转化为DON,不同部位转化速率差异显著,依次为十二指肠>盲肠>回肠>空肠。对于15-乙酰基-DON毒素,十二指肠、回肠和盲肠内容物能将15-乙酰基-DON毒素转化为DON,转化速率依次为盲肠>十二指肠>回肠。不同肠段部位对3-乙酰基-DON毒素的转化效率大于15-乙酰基-DON毒素。（3）为了阐明肉鸡肠道不同部位微生物群的组成,对十二指肠、空肠、回肠和盲肠部位的内容物样品进行了 16S rDNA扩增测序分析。盲肠部位内容物α和β多样性丰度最高,优势门为厚壁菌门（Firmicutes）、拟杆菌门（Bacteroidota）、广泛古菌门（Euryarchaeota）,优势属为乳酸菌属（Lactobacillus）、拟杆菌属（Bacteroides）、甲烷杆菌属（Methanobrevibacter）,优势种为乳酸菌属（Lactobacillusviarius）、拟杆菌属（bacteroidesparseile-p3 166）和乳酸菌属（Lactobacillusalivarius）。不同肠段部位菌群组成存在显著差异。

Petrography, Geochemistry and Relative Chronology of Quaternary Volcanic Formations in the Mermoz and Fann Sectors, West Senegal

Detailed work on Quaternary volcanism has been carried out in the Mermoz and Fann sectors of western Senegal. In the Mermoz sector, the main emission zone is a collapsed crater located at the intersection of three major fractures: NE-SW, NW-SE and N-S. The lithological succession in this Mermoz sector comprises, from bottom to top: 1) a substratum with at its base Eocene limestones on which lie Quaternary sands surmounted by stratified tuffs;2) a vesicular ball dolerite which deforms the stratified tuffs;3) a dark early breccia;4) two generations of basanites: the first is vesicular, the second non-vesicular;5) a clear intermediate breccia and finally 6) a late breccia. The Fann sector contains several emission zones, most of which are currently located in the ocean. The lavas may have reached the coast through E-W and NE-SW faults. The lithological succession includes from bottom to top: 1) scoria-rich early volcanic breccias;2) a first generation of non-vesicular mesocrate dolerite (D1);3) a second generation of melanocrate vesicular dolerite (D2);4) basanites and finally 5) a late breccia. The geochemical characteristics of the lavas studied are compatible with a very enriched and very deep magmatic source of the garnet lherzolite type located in the lower mantle. The magma from this source would have risen in the form of mantle plumes through major NE-SW and E-W faults in a continental intraplate context.

Clinical Characteristics of COVID-19 Patients with Digestive Symptoms: A Retrospective Study from Dakar, Senegal

Introduction: As of February 2022, coronavirus disease 2019 (COVID-19) has caused a pandemic affecting all countries, with a total of 399 million patients and 5 million deaths. The lungs are the major organs involved in COVID-19. COVID-19 infection is not limited to the respiratory system but can affect multiple organs including the gastrointestinal tract. The aim of our study was to assess the prevalence of gastrointestinal (GI) symptoms on admission in patients with COVID-19 and their association with adverse outcomes, including mortality. Methods: In a retrospective study, we examined medical record data from patients with SARS-CoV-2 infection admitted to epidemic treatment center of hospital Aristide Le Dantec, Dakar (Senegal) between May 1, 2020, and June 31, 2021. SARS-CoV-2 infection was diagnosed by real-time polymerase chain reaction on nasopharyngeal and throat swabs. We included all patients with SARS-CoV-2 infection and GI symptoms. Result: The study identified 472 hospitalized patients with confirmed SARS-CoV-2 infection during the study period. We recruited 222 patients with gastrointestinal symptoms. The mean age of patients with GI symptoms was 56 years [17 - 90 years], and 54.9% were male. Patients with GI symptoms had comorbidities in 66.2% of cases. Hypertension, diabetes mellitus and chronic kidney disease were the most common comorbidities at 29.3%, 23.9%, and 6.7%, respectively. Patients with GI had cough in 69.4%, shortness of breath in 61.7%, ageusia in 57.6%, and fever in 53.1%. At presentation among patients with GI symptoms, 32.4% had mild disease, 27.5% had moderate disease, and 40.1% had severe disease. The prevalence of digestive manifestations was 47%, and the main digestive manifestations were nausea (27%), diarrhea (17.1%), abdominal pain (17.1%) and vomiting (14.4%).
