Comparison of Established Risk Factors among Type 2 Diabetic Patients with or without Retinopathy in Golestan Hospital, Ahvaz 2010

The aim of this study was to establish diabetic retinopathy correlation with several risk factors. A prospective study including 256 type II diabetic patients who referred to diabetes’s clinic was designed. All patients underwent physical & retina examination and then completed questionnaire including duration of DM, cardiovascular disease, stroke, chronic kidney disease, smoking, and kind of treatment, blood pressure, BMI, abdominal & hip circumference. Results of HbA1C, total cholesterol, HDL & LDL cholesterol, TG, FBS, creatinine, 24-h urine microalbumin were recorded. Data were analyzed by SPSS version 17 with K2, independent samples T, and logistic regression. Of 256 types II diabetic patients 81 & 175 were men and women respectively. Mean age of subject was 54 ± 10.27. Prevalence of diabetic retinopathy was 22.3% & macula edema 0.5% & PDR 6% & NPDR 14%;diabetic retinopathy was associated with duration of DM & BMI (P = 0, P = 0.43 respectively). Mean of FBS 191 ± 83 vs. 165.7 ± 74.5 (P = 0.03), HbA1c 9.1 ± 2 vs. 8.5 ± 1.9 (P = 0.02), 24-h urine microalbumin 181.2 ± 404.7 vs. 60.2 ± 236 (P = 0.03), Systolic blood pressure 124.3 ± 21.3 vs. 118.5 ± 17.8 (p = 0.04) differed between diabetic patients with or without diabetic retinopathy. There were no significant difference between subject with & without diabetic retinopathy in total cholesterol LDL, HDL, TG, Cr, Diastolic BP, types of treatment, cardiovascular disease, chronic kidney disease, stroke & smoking. This study showed prevalence of diabetic retinopathy was high and it associated with duration of DM, BMI, FBS, HbA1C, 24-h urine albumin, and systolic blood pressure.

Background: Erectile dysfunction is common in patients with diabetes mellitus. In addition, reduced testosterone itself is considered as a risk factor for diabetes;therefore hypogonadism was studied in diabetes. Objective: This study was done to determine the prevalence of hypo- and hypergonadotropic hypogonadism in the type 2 diabetes male patients in Mashhad in north-east of Iran. Methods: This study was done on type 2 diabetic men aged 40 - 60 years in the endocrine clinic, Endocrinology Research Center, Mashhad University of Medical Sciences, Iran. Fasting blood samples were collected at 8 am for measurement of fasting blood sugar (FBS), HbA1C, total serum testosterone, FSH, Sex Hormone Binding Globulin (SHBG), LH, prolactin, thyroxin-stimulating hormone (TSH), and immediately was sent to laboratory. Results: Out of total 96 type 2 diabetic males (mean age of 51.4 ± 11.26 years, range of 40 - 60 years), 11 (12.94%) patients were excluded because of inadequate samples, insufficient information and fulfillment of the exclusion criteria of the study. Hypogonadism based on Testosterone, Calculated free testosterone (CFT), and boiavailable testosterone (BT) were observed in 10 (11.8%), 31 (36.6%), and 30 (35.3%) of the patients, respectively. Libido was decreased in 55 (64.7%) of the patients. Based on the obtained SHBG values there were 7 (8.2%), 52 (61.2%), and 26 (30.6%) cases of low, normal and high values, respectively. According to TSH observed values there were 6 (7.1%) patients and 1 case of sub-clinical hypothyroidism and hyperthyroidism, respectively, and the rest 78 (91.8%) cases were euthyroid. Prolactin level was normal in all cases. Conclusion: Hypogonadotropic hypogonadism is common in type 2 diabetic men, and whether its treatment is useful for erectile dysfunction or not, needed additional investigation.

Estimation of Species Richness of Permian Foraminifera in Non-Parametric Methods and Investigation of Its Change Trend in Central Alborz, Western Tethys

Species richness of foraminifera assemblages in the Permian succession, contains Dorud, Ruteh and Nessen Formations, in Central Alborz—North of Iran, was estimated and studied based on lithostratigraphy and microbiostratigraphy of Permian. We used four non-parametric estimators to investigate the species richness: Chao 2, Jackknife 1, Jackknife 2 and bootstrap. These methods estimates the species richness based on the presence/absence data of each taxon identified in the samples. We use the submenu of quadrat richness in “Past” [1] software to estimate richness in regional chronostratigraphic *** results show that the estimated diversity of foraminiferal assemblages with the exception of late Yakhtashian, increased constantly from Asselian to Murgabian with the highest diversity of foraminifera seen in the Murgabian. The main decrease in foraminiferal species richness happened during the Midian which corresponds to the kamura cooling event.

Effect of Bromocriptine on Insulin Resistance in Patients with PCO

Introduction: Poly cystic ovary (PCO) is one of the most common endocrine disorders in women. All patients with PCO are at risk of insulin resistance, IFG and diabetes. Recently, bromocriptine is used in treatment of diabetes mellitus type II to improve insulin resistance. Objective: The aim of this study is the evaluation of bromocriptine on insulin resistance in PCO people. Patients and Methods: In this single-blind controlled clinical trial with placebo, 44 patients with PCO referring to endocrinology clinic were evaluated. Inclusion criteria were BMI > 25 kg/m2 and diagnosed PCO patients according to Rotterdam criteria and rule out other causes. Blood samples were obtained for FBS, Fasting Insulin, Prolactin, TSH and 17(OH)P. They divided two groups: Case group was given bromocriptine 2.5 mg daily and placebo was given to control group. Patients were treated for 8 days and in day 9th blood sample was obtained for FBS, Fasting insulin, HOMA-IR index. Mann-Whitney method is used for mean comparison. Results: Data analysis using showed in pre diabetes range, mean changes of FBS, insulin level and IR in Groups 1 and 2 had significant differences(P = 0.004), but no significant different was found in FBS 100 mg/dl with (P = 0.036) were significant, but no significant difference showed in mean change of diastolic blood pressure in FBS > 100 mg/dl (P = 0.99). In FBS < 100 mg/dl mean changes in systolic and diastolic blood pressures were not significant (P = 0.6). Age showed no difference changes in effect on treatment in Groups 1 and 2 (P = 0.1).
