BACKGROUND:Adequate airway management plays an important role in high-quality cardiopulmonary resuscitation(CPR).Airway management is usually performed using an endotracheal tube(ETT)during ***,no study has assessed the effect of ETT size on the fl ow rate and airway pressure during ***:We measured changes in peak inspiratory fl ow rate(PIFR),peak airway pressure(Ppeak),and mean airway pressure(Pmean)according to changes in ETT size(internal diameter 6.0,7.0,and 8.0 mm)and with or without CPR.A tidal volume of 500 mL was supplied at a rate of 10 times per minute using a mechanical *** compressions were maintained at a constant compression depth and speed using a mechanical chest compression device(LUCAS2,mode:active continuous,chest compression rate:102±2/minute,chest compression depth 2–2.5 inches).RESULTS:The median of several respiratory physiological parameters during CPR was significantly different according to the diameter of each ETT(6.0 vs.8.0 mm):PIFR(32.1 L/min[30.5–35.3]vs.28.9 L/min[27.5–30.8],P<0.001),Ppeak(48.84 cmH2O[27.46–52.11]vs.27.45 cmH2O[22.53–52.57],P<0.001),and Pmean(18.34 cmH2O[14.61–21.66]vs.13.66 cmH2O[8.41–19.24],P<0.001).CONCLUSION:The changes in PIFR,Ppeak,and Pmean were related to the internal diameter of ETT,and these values tended to decrease with an increase in ETT *** airway pressures were measured in the CPR group than in the no CPR group.

AIM: To determine the correlation between the hepatic venous pressure gradient and the endoscopic grade of esophageal ***: From September 2009 to March 2013, a total of 176 measurements of hepatic venous pressure gradient (HVPG) were done in 146 patients. Each transjugular HVPG was measured twice, first using an end whole catheter (EH-HVPG), and then using a balloon catheter (B-HVPG). The HVPG was compared with the endoscopic grade of esophageal varices (according to the general rules for recording endoscopic findings of esophagogastric varices), which was recorded within a month of the measurement of ***: The study included 110 men and 36 women, with a mean age of 56.1 years (range, 43-76 years). The technical success rate of the pressure measurements was 100% and there were no complication related to the procedures. Mean HVPG was 15.3 mmHg as measured using the end hole catheter method and 16.5 mmHg as measured using the balloon catheter method. Mean HVPG (both EH-HVPG and B-HVPG) was not significantly different among patients with different characteristics, including sex and comorbid factors, except for cases with hepatocellular carcinoma (B-HVPG, P = 0.01; EH-HVPG, P = 0.02). Portal hypertension (> 12 mmHg HVPG) occurred in 66% of patients according to EH-HVPG and 83% of patients according to B-HVGP, and significantly correlated with Child’s status (B-HVPG, P < 0.000; EH-HVGP, P < 0.000) and esophageal varies observed upon endoscopy (EH-HVGP, P = 0.003; B-HVGP, P = 0.006). One hundred and thirty-five endoscopies were performed, of which 15 showed normal findings, 27 showed grade 1 endoscopic esophageal varices, 49 showed grade 2 varices, and 44 showed grade 3 varices. Diatoms are a globally successful and eukaryotic photosynthetic organism with an ornamented silica external *** relationship between their valve morphology and habitat means that diatoms can be used as bioindicators to characterize the aquatic *** estimate the differential distribution and diversity of diatom assemblages along the coastal line,we collected phytoplankton samples from 114 coastal sites of waters of *** applied the unweighted pair-group technique using the arithmetic averages clustering method to cluster the sampling sites,apart from those where the biota consisted of other groups,such as dinoflagellates,into four regions:the Yellow Sea,South Sea,southern East Sea,and northern East *** species analysis in each region led to the selection of tychoplanktonic,chain-forming,attached species to substrates and psychrophilic indicator diatoms,respectively,each of which represented a planktonic lifestyle associated with one of the four *** study shows the diatom assemblages to serve as bioindicators of Korean coastal water in winter,and the subsequent seasonal survey will provide a starting point for the improved understanding of Korean diatom-based ecoregions,in both time and space.

Micro-light-emitting diodes（μLEDs） have gained significant interest as an activation source for gas sensors owing to their advantages, including room temperature operation and low power consumption. However, despite these benefits, challenges still exist such as a limited range of detectable gases and slow response. In this study, we present a blue μLED-integrated light-activated gas sensor array based on SnO2nanoparticles（NPs） that exhibit excellent sensitivity, tunable selectivity, and rapid detection with micro-watt level power consumption. The optimal power for μLED is observed at the highest gas response, supported by finite-difference time-domain simulation. Additionally, we first report the visible light-activated selective detection of reducing gases using noble metal-decorated SnO2NPs. The noble metals induce catalytic interaction with reducing gases, clearly distinguishing NH3, H2, and C2H5OH. Real-time gas monitoring based on a fully hardwareimplemented light-activated sensing array was demonstrated, opening up new avenues for advancements in light-activated electronic nose technologies.

Kearns-Sayre syndrome （KSS） is an extremely rare form ofmitochondrial myopathy characterized by progressive external ophthalmoplegia, bilateral pigmentary retinopathy, and cardiac conduction abnormalities. In addition, KSS patients might present with proximal muscle weakness, diabetes mellitus, hearing loss, cerebellar ataxia, and elevated cerebrospinal fluid protein levels.

To clarify the role of neoadjuvant concurrent chemoradiotherapy (NACCRT) followed by surgical resection for localized or locally advanced perihilar cholangiocarcinoma (CCA).METHODSWe retrospectively reviewed 57 patients who underwent surgical resection with or without NACCRT for perihilar CCA; 12 patients received NACCRT and 45 patients did not received NACCRT. Patients with locally advanced perihilar CCA requiring NACCRT were defined as follows: (1) a mass involving unilateral branches of the portal vein or hepatic artery with insufficient volume of the anticipated remnant lobe; or (2) an infiltrating mass in the main portal vein that was too long for reconstruction, identified at preoperative *** median disease-free survival (DFS) durations of the neoadjuvant and non-neoadjuvant CCRT groups were 26.0 and 15.1 mo, respectively (P = 0.91). The median overall survival (OS) durations of the neoadjuvant and non-neoadjuvant CCRT groups were 32.9 and 27.1 mo, respectively (P = 0.26). The NACCRT group showed a downstaging tendency compared to the non-NACCRT group as compared with the tumor stage confirmed by histological examination after surgery and the tumor stage confirmed by imaging test at the time of diagnosis (P = 0.01).CONCLUSIONNACCRT does not prolong DFS and OS in localized or locally advanced perihilar CCA. However, NACCRT may allow tumor downstaging and improve tumor resectability.

Synthesis and characterization of mechanically alloyed Pt-5%ZrO2(volume fraction) for structural components in the glass industry were described. Zirconia(ZrO2) nanoparticles(<100 nm) were produced by the electrical explosion of zirconium(Zr) wires, and blended with platinum(Pt) powders(<44 ?m) for 2-72 h in ambient atmosphere. The Pt particle size followed the typical decreasing trend of the normal ball milling process up to 48 h, but particle agglomeration was observed at 72 h. The grain size evolution was similar to that of the particle size, dropping down to around 50 nm at 48 h. The root mean square strain of the Pt crystallites showed the opposite behavior, maximizing at 48 h with a subsequent relaxation process. For the 48 h ball milled powders, spark plasma sintering was carried out to form a bulk disk. The measured mass loss of the sintered bulk sample shows a decent thermal stability despite its relatively low density.

KML001, an arsenic compound, as salvage chemotherapy in refractory biliary tract cancers: A prospective study

Background: Sodium meta-arsenite(NaAsO_2, KML001) is a potential oral anticancer agent acting on telomerase and telomere length. This prospective study evaluated its safety, tolerability, and effectiveness as salvage chemotherapy in patients with advanced biliary tract cancer(BTC) resistant to gemcitabinebased chemotherapy. Methods: Forty-four patients(21 women and 23 men) with advanced BTC and failure history of gemcitabine-based chemotherapy, performance status(PS) 0–2, normal cardiac, hepatic, and renal function were enrolled. Daily dose of KML001(7.5 mg. p.o.) was administered to eligible subjects for 24 weeks divided into six treatment cycles. Response was evaluated bimonthly using CT. Results: After an average of 1.5 months of treatment(range: 0.5–10.0), 3 patients(6.8%) obtained progression-free status, 23 patients(52.3%) had disease progression, and 18 patients(40.9%) dropped out before evaluation. One patient(2.3%) completed six treatment cycles without progression. During the treatment, morphine dosage kept the same or decreased in 20 patients(47.6%). Nine patients(20.5%) experienced grade-3 adverse events(AEs), while no patient experienced grade-4 AEs. The most common AEs were liver enzyme elevation(11/44, 25%) and anemia(10/44, 22.7%). KML001 was discontinued in six patients(13.6%) due to AEs, including liver toxicity( n = 3), QTc prolongation( n = 2), and abdominal pain( n = 1). Conclusions: KML001 did not have enough anticancer effect on patients with advanced BTC resistant to gemcitabine. However, KML001 was safe and well-tolerable in terms of AEs and pain control when used as salvage therapy. Further studies are needed to establish arsenic agents as a reliable treatment option in patients with BTC.
