Clinical outcome after pulmonary metastasectomy from primary hepatocellular carcinoma:Analysis of prognostic factors

AIM： To review the surgical outcomes in terms of the surgical indications and relevant prognostic factors. METHODS： Sixteen patients underwent therapeutic lung surgery between March 1999 and May 2006. The observation period was terminated on May 31, 2007. The surgical outcomes and the clinicopathological factors were compared. RESULTS： There was no mortality or major morbidity encountered in this study. The mean follow-up period after metastasectomy was 26.7 ＋ 28.2 （range： 1-99 mo）, and the median survival time was 20 mo. The 1- and 5-year survival rates were 56% and 26%, respectively. At the end of the follow-up, 1 patient died from hepatic failure without recurrence, 6 died from hepatic failure with a recurrent hepatocellular carcinoma （HCC）, and 4 died from recurrent HCC with cachexia. Among several clinical factors, Kaplan Meier analysis revealed as a treatment for the that liver transplantation primary lesion, grade of cell differentiation, and negative evidence HBV infection were independent predictive factors. On Cox＇s proportional hazard model, there were no significant factors affecting survival after pulmonary metastasectomy in patients with HCC. CONCLUSION： A metastasectomy should be performed before other treatments in selected patients, Although not significant, patients with liver transplantation of a primary HCC survived longer, Liver transplantation might be the most beneficial modality that can offer patients better survival, A multi- institutional and collaborative study would be needed for identifying clinical prognostic factors predicting survival in patients with HCC and lung metastasis.

The supply of highly concentrated oxygen positively affects cognitive processing in normal young adults. However, there have been few reports on changes in cognitive ability in elderly subjects following highly concentrated oxygen administration. This study investigated changes in cognitive ability, blood oxygen saturation （%）, and heart rate （beats/min） in normal elderly subjects at three different levels of oxygen [21% （1 L/min）, 93% （1 L/min）, and 93% （5 L/min）] administered during a 1-back task. Eight elderly male （75.3 ＋ 4.3 years old） and 10 female （71.1 ＋ 3.9 years old） subjects, who were normal in cognitive ability as shown by a score of more than 24 points in the Mini-Mental State Examination-Korea, participated in the experiment. The experiment consisted of an adaptation phase after the start of oxygen administration （3 minutes）, a control phase to obtain stable baseline measurements of heart rate and blood oxygen saturation before the task （2 minutes） and a task phase during which the 1-back task was performed （2 minutes）. Three levels of oxygen were administered throughout the three phases （7 minutes）. Blood oxygen saturation and heart rate were measured during each phase. Our results show that blood oxygen saturation increased, heart rate decreased, and response time in the 1-back task decreased as the concentration and amount of administered oxygen increased. This shows that administration of sufficient oxygen for optimal cognitive functioning increases blood oxygen saturation and decreases heart rate.

The flowability of a root canal sealer is clinically important because it improves the penetration of the sealer into the complex root canal system. The purpose of this study was to compare the flowabilities of four root canal sealers, measured using the simple press method （ISO 6876）, and their viscosities, measured using a strain-controlled rheometer. A newly developed, calcium phosphate-based root canal sealer （Capseal） and three commercial root canal sealers （AH Plus, Sealapex and Pulp Canal Sealer EWT） were used in this study. The flowabilities of the four root canal sealers were measured using the simple press method （n= 5） and their viscosities were measured using a strain-controlled rheometer （n=5）. The correlation between these two values was statistically analysed using Spearman＇s correlation test. The flow diameters and the viscosities of the root canal sealers were strongly negatively correlated （p= -0.8618）. The viscosity of Pulp Canal Sealer EWT was the lowest and increased in the following order： AH Plus〈Sealapex〈Capseal （P〈0.05）. All of the tested root canal sealers showed characteristic time- and temperature-dependent changes in their rheological properties. The viscosities measured using the strain-controlled rheometer were more precise than the flowabilities measured using the simple press method, suggesting that the rheometer can accurately measure the rheological properties of root canal sealers.

BACKGROUND： An increasing number of studies have shown the effects of aging in basic cognitive processing and higher cognitive functions using functional magnetic resonance imaging （fMRI）. However, little is known about the aging effects in diverse cognitive abilities, such as spatial learning and reasoning. OBJECTIVE： To investigate the effect of aging on spatial cognitive performance and regional brain activation based on fMRI. DESIGN, TIME, AND SETTING： A block design for fMRI observation. This study was performed at the fMRI Laboratory, Brain Science Research Center, Korea Advanced Institute of Science and Technology from March 2006 to May 2009. PARTICIPANTS： Eight right-handed, male, college students in their 20s （mean age 21.5 years） and six right-handed, male, adults in their 40s （mean age 45.7 years）, who graduated from college, participated in the study. All subjects were healthy and had no prior history of psychiatric or neurological disorders. METHODS： A spatial task was presented while brain images were acquired using a 3T fMRI system （ISOL Technology, Korea）. The spatial tasks involved selecting a shape that corresponded to a given figure using four examples, as well as selecting a development figure of a diagram. MAIN OUTCOME MEASURES： The accuracy rate （number of correct answers/total number of items x 100%） of spatial tasks was calculated. Using the subtraction procedure, the activated areas in the brain during spatial tasks were color-coded by T-score. The double subtraction method was used to analyze the effect of aging between the two age groups （20s versus 40s）. RESULTS： The cerebellum, occipital lobe, parietal lobe, and frontal lobe were similarly activated in the two age groups. Increased brain activations, however, were observed in bilateral parietal and superior frontal lobes of the younger group. The time domain entombment of bacteria by intratubular mineralization following orthograde canal obturation with mineral trioxide aggregate（MTA） was studied by scanning electron microscopy（SEM）. Single-rooted human premolars（n560） were instrumented to an apical size #50/0.06 using ProF ile and treated as follows： Group 1（n510） was filled with phosphate buffered saline（PBS）; Group 2（n510） was incubated with Enterococcus faecalis for 3 weeks, and then filled with PBS; Group 3（n520） was obturated orthograde with a paste of OrthoM TA（BioM TA, Seoul, Korea） and PBS; and Group 4（n520） was incubated with E. faecalis for 3 weeks and then obturated with OrthoM TA–PBS paste. Following their treatments, the coronal openings were sealed with PBS-soaked cotton and intermediate restorative material（IRM）, and the roots were then stored in PBS for 1, 2, 4, 8 or 16 weeks. After each incubation period, the roots were split and their dentin/MTA interfaces examined in both longitudinal and horizontal directions by SEM. There appeared to be an increase in intratubular mineralization over time in the OrthoM TA-filled roots（Groups 3 and 4）. Furthermore, there was a gradual entombment of bacteria within the dentinal tubules in the E. faecalis inoculated MTA-filled roots（Group 4）. Therefore, the orthograde obturation of root canals with OrthoM TA mixed with PBS may create a favorable environment for bacterial entombment by intratubular mineralization.

＜正＞PURPOSE: This study was designed to determine the safety and clinical and functional results of coloanal anastomosis in the rectal cancer patients. METHODS: Between 1988 and 1998, 30 patients undergoing proctnsigmoidectomy and manual coloanal anastomosis for rectal cancer were reviewed. All of the patients had a straight hand-sewn coloanal anastomosis without diverting stoma. RESULTS: The mean age was 54（male:female, 20:11）. Mean length of follow up was 26.3 months. The distance from the anal verge was a mean of 4.0 （range, 2-6） cm and mean distal safety margin was 1.8 cm. According to Astler-Coller staging, at the time of surgical removal, 2 tumors had been sterilized completely with preoperative chemoradiation, 2 were A lesions, 13 were B lesions, 11 were C lesions, and 2 were D lesions. Anastomotic leakage occurred in one patient, and strictures were observed in 3 patients. There was no operative mortality. The locoregional relapse were in 3 patients. Postoperative daily frequence of bowel movement was decreased as the time passed [mean, 8.1（lmonth）, 5.8（3months）, 3.0（lyear）]. Mean grade of continence was 1.8 after 1 year. Seventy percent of patients defecated less than 3 times a day after Iyear. Satisfactory function was achieved in 86 percent of the patients. CONCLUSIONS: In the great majority, postoperative functional outcome is excellent. The coloanal anastomosis is a safe and acceptable operation that preserves anal sphincter and continence. The coloanal anastomosis is a reasonable alternative to abdominoperineal resection.

A control problem of a chain of integrator system with measurement noise on feedback sensor is considered. We propose a gain-scaling controller for compensating measurement noise of feedback sensor. Because control systems operate via feedback sensor＇s signal, the measurement noise in sensor＇ signal results in performance degradation or even system failure. Therefore, control systems often demand on compensating measurement noise. Our controller is equipped with a compensator and gain-scaling factor in order to reduce the effect of measurement noise on feedback sensor.
