Does the discrepancy in histologic differentiation between a forceps biopsy and an endoscopic specimen necessitate additional surgery in early gastric cancer?

To investigate the clinicopathological variables in early gastric cancer (EGC) patients in relation to differentiation discrepancy. METHODSThe data of 265 specimens from 240 patients with EGC, who had undergone radical operation at Hallym University Sacred Heart Hospital from 2010 to 2015, were retrospectively analyzed. We evaluated clinical, endoscopic, and histopathological data according to histological discrepancy. RESULTSClinically significant discrepancy rate showed the difference in differentiated type (well and moderately differentiated) and undifferentiated type (poorly differentiated and signet ring cell) between endoscopic biopsies and postoperative specimens was 9.4% (25/265). There were no differences in tumor location, size, gross pattern, and number of biopsies. Specimens having histological discrepancy showed more submucosal invasion (72.0% vs 49.6%, P = 0.033) and lymph node involvement (24.0% vs 7.9%, P = 0.009) than specimens having non-discrepancy. The rate of a positive epidermal growth factor receptor status was higher in specimens having discrepancy than in specimens having non-discrepancy (81.0% vs 55.4%, P = 0.035). CONCLUSIONThe discordance of histologic differentiation is associated with higher submucosal invasion and lymph node metastases in EGC. Patients have histological discrepancy may require additional surgical treatments.

Dear Editor,Ocular toxoplasmosis,caused by Toxoplasma gondii infection,is one of the most common causes of posterior uveitis worldwide[1].It typically manifests as white focal retinitis with overlying vitreous inflammation that results in a chorioretinal scar and associated scotoma[2].However,the diagnosis of ocular toxoplasmosis can be challenging when it presents as an isolated papillitis without other typical signs of retinochoroiditis or vitritis[2].Although optic nerve involvement in ocular toxoplasmosis was present in 5.3%of cases,isolated papillitis was extremely rare(3 eyes out of 926 patients)and was only considered a presumed diagnosis in such cases given the presence of old toxoplasmic retinochoroiditis lesions[3].Few reports have described optic nerve involvement preceding toxoplasmic retinochoroiditis,while perineuritis,an uncommon form of orbital inflammatory disease involving the optic nerve sheath,has not been reported at all[4-5].Herein,we describe a case of ocular toxoplasmosis that initially presented with optic perineuritis followed by typical retinochoroidal *** our knowledge,this is the first case report of ocular toxoplasmosis with perineuritis in an immunocompetent individual.

Evaluation of defects in aluminum piston castings by using ultrasonics and computer tomography

Pistons used in automobile engines are made of aluminum alloy; and endurance of high pressure and high temperature is required. Recently, high strength pistons are needed to cope with the increase of pressure and temperature in the engine cylinder. A high strength piston is possible when the piston casting has little or no casting defects, such as micro-pores and cracks. Generally the defects can be evaluated by non-destructive testing (NDT), and the most efficient ways for evaluation are using ultrasonics and computer tomography. In the present study, two NDT methods were compared and evaluated to investigate the defects in the aluminum piston castings. Artificial defects were machined by using very small drill bits with diameters 1, 0.5, 0.3, 0.2 and 0.1 mm. The defects were investigated by using an ultrasonic tester and a computer tomography system. Defects smaller than 0.3 mm may not be found by using the ultrasonic test, but the defects may be found by using the computer tomography system. The investigation conditions for detecting small defects and the scanning time for in-line defect analysis are discussed in the present study.

Thioredoxin and thioredoxin-interacting protein as prognostic markers for gastric cancer recurrence

AIM:To evaluate the potential of thioredoxin (TXN) and thioredoxin-interacting protein (TXNIP) expression as biomarkers for predicting gastric cancer recurrence. METHODS:TXN and TXNIP expression levels were acquired from gene expression microarray data for 65 human gastric cancer tissues. We determined whether each gene expression level was associated with cancer recurrence and investigated the relationship between the two genes. For validation, the expression levels of TXN and TXNIP were measured by quantitative real- time reverse transcription polymerase chain reaction in 68 independent stage Ⅲ gastric cancer patients. The correlation between gene expression and cancer prognosis was evaluated. Immunohistochemical staining was performed to investigate the protein expression levels of TXN and TXNIP and to characterize the expression patterns of each protein. RESULTS:TXN was a prognosis-related gene (P = 0.009), whereas TXNIP, a TXN inhibitor, demonstrated a negative correlation with TXN in the gene expression microarray data. In the 68 stage Ⅲ patients, the expression levels of both TXN and TXNIP had a statistically significant effect on recurrence-free survival (RFS, P = 0.008 and P = 0.036, respectively). The low TXN and high TXNIP expression group exhibited a better prognosis than the other groups, and the high TXN and low TXNIP expression group exhibited a poorer prognosis (P < 0.001 for RFS and P = 0.001 for overall survival). More than half of the patients in the simulta-neously high TXN and low TXNIP expression group ex- perienced a recurrence within 1 year after curative surgery, and the 5-year survival rate of the patients in this group was 29%, compared with 89% in the low TXN and high TXNIP expression group. The TXN protein was overexpressed in 65% of the gastric cancer tissues, whereas the TXNIP protein was underexpressed in 85% of the cancer cells. AIM:There is some evidence of functional superiority of colonic J-pouch over straight coloanal anastomosis (CM) in ultralow anterior resection (ULAR) or intersphincteric resection. On the assumption that colonic J-pouch anal anastomosis is superior to straight CM in ULAR with upper sphincter excision (USE: excision of the upper part of the internal sphincter) for low-lying rectal cancer, we compare functional outcome of colonic J-pouch vsthe straight CM. METHODS: Fifty patients of one hundred and thirty-three rectal cancer patients in whom lower margin of the tumors were located between 3 and 5 cm from the anal verge received ULAR including USE from September 1998 to January 2002. Patients were randomized for reconstruction using either a straight (n = 26) or a colonic J-pouch anastomosis (n = 24) with a temporary diverting-loop ileostomy. All patients were followed-up prospectively by a standardized questionnaire [Fecal Inco-ntinence Severity Index (FISI) scores and Fecal Incontinence Quality of Life (FIQL) scales]. RESULTS: We found that, compared to straight anastomosis patients, the frequency of defecation was significantly lower in J-pouch anastomosis patients for 10 mo after ileostomy takedown. The FISI scores and FIQL scales were significantly better in J-pouch patients than in straight patients at both 3 and 12 mo after ileostomy takedown. Furthermore, we found that FISI scores highly correlated with FIQL scales. CONCLUSION: This study indicates that colonic J-pouch anal anastomosis decreases the severity of fecal incontinence and improves the quality of life for 10 mo after ileostomy takedown in patients undergoing ULAR with USE for low-lying rectal cancer.

Upgrading and upstaging of low-risk prostate cancer among Korean patients： a multicenter study

Only 54% of prostate cancer cases in Korea are localized compared with 82% of cases in the US. Furthermore, half of Korean patients are upgraded after radical prostatectomy （41.6%-50.6%）. We investigated the risk factors for upgrading and/or upstaging of low-risk prostate cancer after radical prostatectomy. We retrospectively reviewed the medical records of 1159 patients who underwent radical prostatectomy at five hospitals in Honam Province. Preoperative data on standard clinicopathological parameters were collected. The radical prostatectomy specimens were graded and staged and we defined a ＂worsening prognosis＂ as a Gleason score ≥ 7 or upstaging to ≥ pT3. Multivariate logistic regression models were used to assess factors associated with postoperative pathological upstaging. Among the 1159 patients, 324 were classified into the clinically low-risk group, and 154 （47.5%） patients were either upgraded or upstaged. The multivariable analysis revealed that the preoperative serum prostate-specific antigen level （odds ratio [OR], 1.131; 95% confidence interval [CI], 1.007-1.271; P = 0.037）, percent positive biopsy core （OR： 1.018; 95% CI： 1.002-1.035; P = 0.032）, and small prostate volume （≤30 ml）（OR： 2.280; 95% CI： 1.351-3.848; P = 0.002） were predictive of a worsening prognosis. Overall, 47.5% of patients with low-risk disease were upstaged postoperatively. The current risk stratification criteria may be too relaxed for our study cohort.
