BACKGROUND Posterior reversible encephalopathy syndrome(PRES)is characterized mainly by occipital and parietal lobe involvement,which can be reversible within a few ***,we report a rare case of PRES that developed after craniotomy for an unruptured intracranial aneurysm(UIA).CASE SUMMARY A 59-year-old man underwent clipping surgery for the treatment of UIA arising from the left middle cerebral *** surgery was performed uneventfully,and he regained consciousness quickly immediately after the *** the 4th hour after surgery,he developed a disorder of consciousness and *** resonance imaging revealed cortical and subcortical T2/FLAIR hyperintensities in the parietal,occipital,and frontal lobes ipsilaterally,without restricted diffusion,consistent with unilateral *** conservative treatment,his symptoms and radiological findings almost completely disappeared within *** our case,the important causative factor of PRES was suspected to be a sudden increase in cerebral perfusion pressure associated with temporary M1 *** Our unique case highlights that,to our knowledge,this is the second report of PRES developing after craniotomy for the treatment of *** must keep PRES in mind as one of the causes of perioperative neurological abnormality following clipping of an UIA.

The effects of 3 chairside polishing kits and mechanical brushing on the surface roughness of 3 different acrylic denture base resins were compared. Acrylic denture base resins （auto-polymerizing, heat-polymerizing, injected heat-polymerizing resins） were examined after a tungsten carbide bur, and after chairside polishing using 3 polishing kits and pumice. The specimens were subjected to mechanical brushing using a wear tester to simulate 30 000 strokes of brushing. The surface roughness of the acrylic denture base resin specimens was measured using a contact pro-filometer. After the test, the random polished acrylic resins were evaluated by scanning electron mi-croscopy （SEM） and atomic force microscopy （AFM）. Acrylic denture base resins polished using the 3 types of polishing kits had a smoother surface than those finished with the tungsten carbide bur （p〈0.05）. The surface of the resin polished by a TC cutter exceeded the Ra of 0.2 μm （p〈0.05）. The auto-polymerizing resin showed a significantly higher surface roughness than the heat-polymerizing resin and injected heat-polymerizing resin （p〉0.05）. In the case of polishing step wise, there was almost no change in surface roughness after brushing （p〉0.05）.

Background There may be concerns over disbenefits to patients who have chosen to undergo laparoscopic gastrectomy by experts in open gastrectomy, considering the disparity between the level of proficiency in open gastrectomy, at which they are already experts, and that in laparoscopic gastrectomy, at which they are beginners. The aim of this study was to compare surgical radicality and outcomes between laparoscopic gastrectomy and open gastrectomy during the learning period of laparoscopic gastrectomy for a senior surgeon who was already an expert in open gastrectomy. Methods Data of short-term surgical outcomes were obtained from patients following laparoscopy assisted distal gastrectomy （LADG） by a surgeon. The initial and following 30 experiences were grouped into LADG-I and LADG-II, respectively. Patients who underwent open distal subtotal gastrectomy （ODSG） and yet could have been candidates for LADG were grouped into ODSG. Known indicators of proficiency levels and the postoperative hospital course were compared. The consequences of extended lymphadenectomy, and the radicality of surgery by completing D2 lymphadenectomy were analyzed. Results The LADG group revealed longer operation time and less bleeding compared to the ODSG group （P 〈0.001）. The number of retrieved lymph nodes and the rate of complications were not significantly different. In the LADG-I group, the DI＋：D2 ratio was 4：1, showing significant differences from those in the LADG-II （0.36：1） and ODSG （0.16：1） groups （P 〈0.001）. The surgeon was able to complete D2 lymphadenectomy during LADG without significant change in the amount of bleeding and the rate of complications, but with a longer operation time （P=0.009）. The number of lymph nodes from the 12a station was not significantly different between the LADG and ODSG groups with D2 lymphadenectomy. Conclusions The surgical outcomes were comparable between LADG and ODSG even during the learning period of LADG, and the equivalence of radicality in

AIM: To evaluate the prevalence of preoperative and postoperative malnutrition and the relationships between objective and subjective nutritional assessment of gastric cancer patients. METhoDS: From October 2005 to July 2006, we studied 80 patients with no evidence of recurrent disease and no loss to follow-up after curative surgery for gastric cancer. In this group, 9 patients underwent total gastrectomy and 71 patients subtotal gastrectomy. At admission, 6 and 12 mo after surgery, the patients were assessed on the subjective global assessment (SGA), nutritional risk screening (NRS-2002), nutritional risk index (NRI) and by anthropometric measurements and laboratory data. Differences between the independent groups were assessed with the Student's t test and oneway analysis of variance. Spearman's rank correlation coefficients were calculated to evaluate the association between the scores and variables. RESULTS: The prevalence of malnutrition at admissionwas 31% by SGA and 43% by NRS-2002. At admission, the anthropometric data were lower in the malnourished groups defined by the SGA and NRS-2002 assessments, but did not differ between the groups using the NRI assessment. Body weight (BW), body mass index (BMI), triceps skin fold and midarm circumference were significantly reduced, but the total lymphocyte count, albumin, protein, cholesterol and serum iron levels did not decrease during the postoperative period. Six months after surgery, there was a good correlation between the nutritional assessment tools (SGA and NRS-2002) and the other nutritional measurement tools (BW, BMI, and anthropometric measurements). however, 12 mo after surgery, most patients who were assessed as malnourished by SGA and NRS-2002 had returned to their preoperative status, although their BW, BMI, and anthropometric measurements still indicated a malnourished status. CONCLUSION: A combination of objective and subjective assessments is needed for the early detection of the nutritional status

To date, the cortical effect of exercise has not been fully elucidated. Using the functional near infrared spectroscopy, we attempted to compare the cortical effect between shoulder vibration exercise and shoulder simple exercise. Eight healthy subjects were recruited for this study. Two different exercise tasks（shoulder vibration exercise using the flexible pole and shoulder simple exercise） were performed using a block paradigm. We measured the values of oxygenated hemoglobin in the four regions of interest： the primary sensory-motor cortex（SM1 total, arm somatotopy, and leg and trunk somatotopy）, the premotor cortex, the supplementary motor area, and the prefrontal cortex. During shoulder vibration exercise and shoulder simple exercise, cortical activation was observed in SM1（total, arm somatotopy, and leg and trunk somatotopy）, premotor cortex, supplementary motor area, and prefrontal cortex. Higher oxygenated hemoglobin values were also observed in the areas of arm somatotopy of SM1 compared with those of other regions of interest. however, no significant difference in the arm somatotopy of SM1 was observed between the two exercises. By contrast, in the leg and trunk somatotopy of SM1, shoulder vibration exercise led to a significantly higher oxy-hemoglobin value than shoulder simple exercise. These two exercises may result in cortical activation effects for the motor areas relevant to the shoulder exercise, especially in the arm somatotopy of SM1. however, shoulder vibration exercise has an additional cortical activation effect for the leg and trunk somatotopy of SM1.

The influence of the dehydration by metal oxides on the synthesis of dimethyl carbonate （DMC） via oxidative carbonylation of methanol was studied. A Cu/Y-zeolite catalyst was prepared by the ion exchange method from CuCl2.2H2O and the commercial NH4-form of the Y type zeolite, The catalyst was characterized by X-ray fluorescence （XRF）, N2 adsorption （BET method）, X-ray diffraction （XRD）, and temperature-programmed de- sorption of ammonia （NH3-TPD） to evaluate its Cu and Cl content, surface area, structure, and acidity. Reaction tests were carried out using an autoclave （batch reactor） for 18 h at 403 K and 5.5 MPa （2CH3OH ＋ 1/2O2 ＋ CO （CH3O）2CO ＋ H2O）. The influence of various dehydrating agents （ZnO, MgO, and CaO） was examined with the aim of increasing the methanol conversion （XMeOH, MeOH conversion）. The MeOH conversion increased upon addition of metal oxides in the order CaO 〉〉 MgO 〉 ZnO, with the DMC selectivity （SDMC） following the order MgO 〉 CaO 〉 ZnO. The catalysts and dehydrating agents were characterized before and after the oxidative carbonylation of methanol by thermogravimetric and differential thermogravimetric （TG/DTG）, and XRD to con- firm that the dehydration reaction occurred via the metal oxide （MO ＋ H2O →M（OH）2）. The MeOH conversion increased from 8.7% to 14.6% and DMC selectivity increased from 39.0% to 53.1%, when using the dehydrating azent CaO.

Since the commercialization of the fuel cell electric vehicles (FCEVs), the polymer electrolyte membrane fuel cell system has been actively improved as a powertrain for ultimate environment-friendly vehicle. During the FCEV operation, various transient conditions such as start-up/shut-down and fuel starvation occur irregularly, which deteriorates the durability of the membrane electrode assembly. In particular, when fuel starvation occurs, the carbon support in the anode is oxidized within few minutes, thus the mitigation of this phenomenon is essential for securing the durability. This short review introduces the concept of reversal tolerant anode (RTA), which is a mitigation method using an oxygen evolution catalyst and reviews some previous reports. In addition, new approach for RTA suggested by authors recently, which is the replacement of Pt anode catalyst by multifunctional IrRu alloy catalyst that simultaneously exhibits the activities for the hydrogen oxidation reaction and the oxygen evolution reaction is introduced.

BACKGROUND Colonic perfusion status can be assessed easily by indocyanine green(ICG)angiography to predict ischemia related anastomotic complications during laparoscopic colorectal ***,various parameter-based perfusion analysis have been studied for quantitative evaluation,but the analysis results differ depending on the use of quantitative parameters due to differences in vascular anatomical ***,it can help improve the accuracy and consistency by artificial intelligence(AI)based real-time analysis microperfusion(AIRAM).AIM To evaluate the feasibility of AIRAM to predict the risk of anastomotic complication in the patient with laparoscopic colorectal cancer *** The ICG curve was extracted from the region of interest(ROI)set in the ICG fluorescence video of the laparoscopic colorectal ***-processing was performed to reduce AI performance degradation caused by external environment such as background,light source reflection,and camera shaking using MATLAB 2019 on an I7-8700k Intel central processing unit(CPU)*** learning and evaluation were performed by dividing into a training patient group(n=50)and a test patient group(n=15).Training ICG curve data sets were classified and machine learned into 25 ICG curve patterns using a self-organizing map(SOM)*** predictive reliability of anastomotic complications in a trained SOM network is verified using test *** AI-based risk and the conventional quantitative parameters including T1/2max,time ratio(TR),and rising slope(RS)were consistent when colonic perfusion was favorable as steep increasing ICG curve *** the ICG graph pattern showed stepped rise,the accuracy of conventional quantitative parameters decreased,but the AI-based classification maintained accuracy *** receiver operating characteristic curves for conventional parameters and AI-based classification were comparable for predicting the anastomotic complication *** pe

Cystoid macular edema(CME) is the abnormal collection of intraretinal fluid in the macular region, especially in the inner nuclear and outer plexiform layers. CME leads to severe visual impairment in patients with various retinal diseases, such as diabetic retinopathy, retinal vascular occlusion, choroidal neovascularization, and uveitis. Although various retinal conditions lead to CME, a shared pathogenesis of CME is involved in these diseases. Accordingly, the pathogenesis of CME based on vasogenic mechanisms is first discussed in this review, including vascular hyperpermeability, leukostasis, and inflammation. We then describe cytotoxic mechanisms based on retinal Müller cell dysfunction. This comprehensive review will provide an understanding of the pathogenesis of CME for potential therapeutic strategies.
