Objective:To compare the diagnostic value of the ABCD2 and ABCD3-栺scoring systems in predicting the development of a 30-day neurological event in adult patients with transient ischemic attack(TIA)presenting to the emergency ***:The study was observational and prospective and was conducted in a single *** diagnostic values of the ABCD2 and ABCD3-栺scores in predicting a neurological event within one month were compared in patients diagnosed with TIA in the emergency ***:A statistically significant difference was observed between groups with or without stroke within one month in terms of both the ABCD2 and ABCD3-栺scores(P=0.044 and P=0.029,respectively).There was no statistically significant difference between the patients with and without a recurrent TIA within one month in relation to the ABCD2 score(P=0.934),but a statistically significant difference was found in the ABCD3-栺scores of these groups(Plusions:Both the ABCD2 and ABCD3-栺scoring systems could predict ischemic stroke within 30 days of TIA,the ABCD3-栺score is more effective than the ABCD2 score in the prediction of TIA recurrence.

Air guns and rifles are usually considered to be harmless. Even though these firearms are categorized as toys, injuries with these are quite common in children and adolescents. The most common reason for these types of injuries is accidents and negligence. In this case report we present a 17-year-old male, admitted to the emergency department with a 5 mm incision below his left eye. In the CT imaging a pellet was seen lodged in the cribriform plate. It pierced through the ethmoid sinus. The patient was admitted to the inpatient clinic and managed conservatively with antibiotherapy. A review of literature was performed to find out the best strategy to reach an optimized outcome and evaluate probable complications.

Evaluation of leukocyte esterase and nitrite strip tests to detect spontaneous bacterial peritonitis in cirrhotic patients

AIM: To investigate the diagnostic eff icacy of leukocyte esterase and nitrite reagent strips for bedside diagnosis of spontaneous bacterial peritonitis (SBP). METHODS: A total of 63 consecutive patients with cirrhotic ascites (38 male, 25 female) tested between April 2005 and July 2006 were included in the study. Bedside reagent strip testing was performed on ascitic ? uid and the results compared to manual cell counting and ascitic fluid culture. SBP was defined as having a polymorphonuclear ascites count of ≥ 250/mm3. RESULTS: Fifteen samples showed SBP. The sensitivity, specificity, positive and negative predictive values of the leukocyte esterase reagent strips were; 93%, 100%, 100%, and 98%, respectively. The sensitivity, specifi city, positive and negative predictive value of the nitrite reagent strips were 13%, 93%, 40%, and 77%, respectively. The combination of leukocyte esterase and nitrite reagents strips did not yield statistically signif icant effects on diagnostic accuracy. CONCluSION: Leukocyte esterase reagent strips may provide a rapid, bedside diagnostic test for SBP.
