Background:Contralateral delayed endolymphatic hydrops(CDEH)is a clinical entity characterized by fluctuating low frequency hearing loss and/or vertigo,mimicking Meniere’s disease(md),that manifests after the appearance of severe non-hydropic hearing loss(NHHL)at the other ***:to describe the clinical features and the course of 57 patients affected by ***:this is a retrospective study;57 patients affected by CDEH,out of 1065 patients seen in the same period and affected by md,were subjected to otoscopy,PTA threshold evaluation,impedance testing,ABR,research of positioning nystagmus,vestibular function evaluated by means of bithermal caloric test under video-oculographic,and MRI with ***:the CDEH was definite in 24 cases(42%),probable in 2(4%)and possible in 31(54%).The mean PTA threshold at the hydropic ear was 41 *** the last follow-up,40 patients(70%)did not report vertigo or fluctuating hearing *** the 17 patients who still reported symptomatology,11(64%)were affected by fluctuating hearing loss alone,4(23%)reported a subjective worsening of hearing loss and 2(12%)an acute vertigo ***:contralateral delayed endolymphatic hydrops is a relatively rare form of Meniere disease that manifests more frequently as a definite form or with fluctuating low-frequency hearing *** prognosis at a long term follow-up is relatively good in terms of vertigo *** delayed endolymphatic hydrops rarely determines a severe hearing loss in the better ear.

AIM: To determine existing correlates among diffusion tensor imaging(DTI)-derived metrics in healthy brains and brains with glioblastoma multiforme(GBM). METHODS: Case-control study using DTI data from brain magnetic resonance imaging of 34 controls(mean, 41.47; SD, ± 21.94 years; range, 21-80 years) and 27 patients with GBM(mean, SD; 48.41 ± 15.18 years; range, 18-78 years). Image postprocessing using FSL software calculated eleven tensor metrics: fractional(FA) and relative anisotropy; pure isotropic(p) and anisotropic diffusions(q), total magnitude of diffusion(L); linear(Cl), planar(Cp) and spherical tensors(Cs); mean(md), axial(AD) and radial diffusivities(RD). Partial correlation analyses(controlling the effect of ageand gender) and multivariate Mancova were ***: There was a normal distribution for all metrics. Comparing healthy brains vs brains with GBM, there were significant very strong bivariate correlations only depicted in GBM: [FA?Cl(+)], [FA?q(+)], [p?AD(+)], [AD?md(+)], and [md?RD(+)]. Among 56 pairs of bivariate correlations, only seven were significantly different. The diagnosis variable depicted a main effect [F-value(11, 23) = 11.842, P ≤ 0.001], with partial eta squared = 0.850, meaning a large effect size; age showed a similar result. The age also had a significant influence as a covariate [F(11, 23) = 10.523, P < 0.001], with a large effect size(partial eta squared = 0.834).CONCLUSION: DTI-derived metrics depict significant differences between healthy brains and brains with GBM, with specific magnitudes and correlations. This study provides reference data and makes a contribution to decrease the underlying empiricism in the use of DTI parameters in brain imaging.

SARS-CoV-2 infection is a viral disease that causes respiratory infections that can cause multiple complications. Patients with adjacent morbidities have a higher chance of complications and mortality. The National Health and Nutrition Survey (ENSANUT) in 2018 reported that in Mexico, 75.2% of adults aged 20 years and over are overweight and obese, both situations are closely related to the cases reported as severe COVID and even with a higher risk of mortality. The main objective was to evaluate the risk and mortality groups in patients with SARS-CoV-2 from family medicine in the south of México. The study was carried out during the 2020 year, in the Family Medicine Unit No. 43 of Villahermosa, Tabasco, a cross-sectional, retrospective investigation was carried out with an analytical approach, and the sample was obtained using the formula of n for infinite populations that correspond to 215, patients were included who met the selection criteria;3 groups were integrated: DM2, HAS and obesity. Descriptive statistics andχ2 formula were used, using the SPSS version 25 Windows program. The female gender presented the highest number of infections 122 (56.7%) and mortality: 5 (2.32%), obesity comorbidity presented the highest number of infections: 194 (90.2%), HAS: 153 (71.2%), and md: 44 (20.46%). Complications were observed in 23 (10.69%), distributed in: SIRA: 7 (3.3%), multiple organ failure: 6 (2.8%), heart failure: 6 (2.8%) and pneumonia 4 (1.9%). The observed mortality was 8 (3.72%). The comorbidities of patients with the highest mortality were: SAH: 6 (75%) (χ2 = 9.169;df = 1;p = 0.010) and obesity: 6 (75%) (χ2 = 9.169;df = 1;p = 0.010). The group with the highest risk for SARS-CoV-2 was patients with overweight/obesity comorbidities. Mortality was 3.7%. The patients who died had at least one or more comorbidities. The main complications were ARIS, heart failure, and multiple organ failure.

BACKGROUND For palliation of malignant biliary obstruction(MBO), the gold-standard method of biliary drainage is endoscopic retrograde cholangiopancreatography(ERCP)with the placement of metallic stents. Endoscopic ultrasound(EUS)-guided drainage is an alternative that is typically reserved for cases of ERCP ***, however, there have been robust randomized clinical trials(RCTs)comparing EUS-guided drainage and ERCP as primary approaches to *** To compare EUS guidance and ERCP in terms of their effectiveness and safety in palliative biliary drainage for *** This was a systematic review and meta-analysis, in which we searched the MEDLINE, Excerpta Medica, and Cochrane Central Register of Controlled Trials databases. Only RCTs comparing EUS and ERCP for primary drainage of MBO were eligible. All of the studies selected provided data regarding the rates of technical and clinical success, as well as the duration of the procedure, adverse events, and stent patency. We assessed the risk of biases using the Jadad score and the quality of evidence using the Grading of Recommendations Assessment,Development and Evaluation *** The database searches yielded 5920 records, from which we selected 3 RCTs involving a total of 222 patients(112 submitted to EUS and 110 submitted to ERCP). In the EUS and ERCP groups, the rate of technical success was 91.96%n and 91.81%, respectively, with a risk difference(RD) of 0.00%(95%CI:-0.07, 0.07;P = 0.97; I^2 = 0%). The clinical success was 84.81% and 85.53% in the EUS and ERCP groups, respectively, with an RD of-0.01%(95%CI:-0.12, 0.10; P = 0.90; I^2 =0%). The mean difference(md) for the duration of the procedure was-0.12%(95%CI:-8.20, 7.97; P = 0.98; I^2 = 84%).
