BACKGROUND Diabetic foot(DMF)complications are common and are increasing in *** factors related to wound complications are yet to be established after transtibial amputation under the diagnosis of DMF *** To analyze the prognosis and risk factors related to wound complications after transtibial amputation in patients with *** This retrospective cohort study included seventy-two patients with DMF complications who underwent transtibial amputation between April 2014 and March *** groups were categorized based on the occurrence of wound complications,and we compared demographic data between the complication group and the non-complication group to analyze risk ***,a multivariate logistic regression analysis was performed to identify risk *** The average follow-up period was 36.2 *** the 72 cases,31(43.1%)had wound *** these,12 cases(16.7%)received further treatment,such as debridement,soft tissue stump revision,and re-amputation at the proximal *** a group that required further management due to wound complications after transtibial amputation,the hemoglobin A1c(HbA1c)level was 9.32,while the other group that did not require any treatment had a 7.54 HbA1c *** prevalence of a history of kidney transplantation with wound complications after transtibial amputation surgery in DMF patients was significantly greater than in cases without wound complications(p=0.02).Other factors did not show significant *** Approximately 43.1%of the patients with transtibial amputation surgery experienced wound complications,and 16.7%required additional surgical *** HbA1c levels and kidney transplant history are risk factors for postoperative wound complications.

Removal of intrahepatic bile duct stone could reduce the risk of cholangiocarcinoma: A single-center retrospective study in South Korea

BACKGROUND Intrahepatic duct(IHD)stones are among the most important risk factors for cholangiocarcinoma(CCC).Approximately 10%of patients with IHD stones develop CCC;however,there are limited studies regarding the effect of IHD stone removal on CCC *** To investigate the association between IHD stone removal and CCC *** We retrospectively analyzed 397 patients with IHD stones at a tertiary referral center between January 2011 and December *** CCC occurred in 36 of the 397 enrolled *** univariate analysis,chronic hepatitis B infection(11.1%vs 3.0%,p=0.03),carbohydrate antigen 19-9(CA19-9,176.00 vs 11.96 II/mL,p=0.010),stone located in left or both lobes(86.1%vs 70.1%,p=0.042),focal atrophy(52.8%vs 26.9%,p=0.001),duct stricture(47.2%vs 24.9%,p=0.004),and removal status of IHD stone(33.3%vs 63.2%,ppatients with and without *** the multivariate analysis,CA19-9>upper normal limit,carcinoembryonic antigen>upper normal limit,stones located in the left or both lobes,focal atrophy,and complete removal of IHD stones without recurrence were independent factors influencing CCC ***,the type of removal method was not associated with CCC *** Complete removal of IHD stones without recurrence could reduce CCC risk.

Efficient and cost-effective electrocatalysts that can operate across a wide range of pH conditions are essential for green hydrogen *** by biological systems,Fe_(7)S_(8)nanoparticles incorporated on polydopamine matrix electrocatalyst were synthesized by co-precipitation and annealing *** resulting Fe_(7)S_(8)/C electrocatalyst possesses a three-dimensional structure and exhibits enhanced electrocatalytic performance for hydrogen production across various pH ***,the Fe_(7)S_(8)/C electrocatalyst demonstrates exceptional activity,achieving low overpotentials of 90.6,45.9,and 107.4 mV in acidic,neutral,and alkaline environments,*** impedance spectroscopy reveals that Fe_(7)S_(8)/C exhibits the lowest charge transfer resistance under neutral conditions,indicating an improved proton-coupled electron transfer ***-wave electron paramagnetic resonance results confirm a change in the valence state of Fe from 3+to 1+during the hydrogen evolution reaction(HER).These findings closely resemble the behavior of natural[FeFe]-hydrogenase,known for its superior hydrogen production in neutral *** remarkable performance of our Fe_(7)S_(8)/C electrocatalyst opens up new possibilities for utilizing bioinspired materials as catalysts for the HER.

Anthropogenic Influence on Decadal Changes in Concurrent Hot and Dry Events over China around the Mid-1990s

The frequency and duration of observed concurrent hot and dry events(HDEs) over China during the growing season(April–September) exhibit significant decadal changes across the mid-1990s. These changes are characterized by increases in HDE frequency and duration over most of China, with relatively large increases over southeastern China(SEC), northern China(NC), and northeastern China(NEC). The frequency of HDEs averaged over China in the present day(pD,1994–2011) is double that in the early period(Ep, 1964–81);the duration of HDEs increases by 60%. Climate experiments with the Met Office Unified Model(MetUM-GOML2) are used to estimate the contributions of anthropogenic forcing to HDE decadal changes over China. Anthropogenic forcing changes can explain 60%–70% of the observed decadal changes,suggesting an important anthropogenic influence on HDE changes over China across the mid-1990s. Single-forcing experiments indicate that the increase in greenhouse gas(GHG) concentrations dominates the simulated decadal changes,increasing the frequency and duration of HDEs throughout China. The change in anthropogenic aerosol(AA) emissions significantly decreases the frequency and duration of HDEs over SEC and NC, but the magnitude of the decrease is much smaller than the increase induced by GHGs. The changes in HDEs in response to anthropogenic forcing are mainly due to the response of climatological mean surface air temperatures. The contributions from changes in variability and changes in climatological mean soil moisture and evapotranspiration are relatively small. The physical processes associated with the response of HDEs to GHG and AA changes are also revealed.

Compact flame-holders for afterburners are an increasing requirement for modern aero ***,flame-holder design is non-trivial since high inlet temperatures,velocities,and elaborate structures induce complex turbulence,combustion,and spray coupling in modern *** this work,the LES-pdf and stochastic fields-Lagrangian particle spray methods are used to investigate methane and aviation kerosene combustion structures formed by new-type concave *** flow pattern,combustion mode,and flame structure of gaseous and liquid fuel around a concave flame-holder are analyzed,discussed,and compared with experimental *** reveal that the flame stability of a concave flame-holder is better than that of the non-concave ***,when using liquid fuel,the concave flame-holder forms a stable and compact *** results suggest concave flame-holders are a promising design for compact afterburners.
