The learning curve for transurethral enucleation with bipolar energy for benign prostate hyperplasia:a single-surgeon experience of 494 patients

This study was performed to investigate the learning curve of transurethral enucleation with bipolar energy(TUEB)for benignprostatic *** study involved 494 consecutive patients who underwent TUEB for benign prostatic hyperplasia fromAugust 2018 to March 2022 by one surgeon(SJJ,Seoul National University Bundang Hospital,seongnam,Korea).The patientswere followed up at 1 week,1 month,3 months,and 6 months *** evaluate the learning curve of TUEB,perioperativeparameters including the enucleation ratio(enucleated tissue weight/transitional zone volume),TUEB efficiency(enucleatedtissue weight/operation time),and enucleation efficiency(enucleated tissue weight/enucleation time)were *** and postoperative complications were also assessed,including the International Prostate Symptom Score(IPSS),IPSSquality-of-life(QoL)score,and uroflowmetry *** patients’median age was 72(interquartile range[IQR]:66–78)years,and the estimated prostate volume and transitional zone volume were 63.0(IQR:46.0–90.6)ml and 37.1(IQR:24.0–60.0)ml,*** enucleation ratio,TUEB efficiency,and enucleation efficiency were 0.60(IQR:0.46–0.54)g ml−1,0.33(IQR:0.22–0.46)g min−1,and 0.50(IQR:0.35–0.72)g min−1,respectively,plateauing after 70 *** functional outcomes,including total IPSS,IPSS QoL score,and uroflowmetry outcomes,significantly improved at 6 months after TUEB(all P<0.05),but without significant differences over the learning ***-five(13.2%)patients developed complications after TUEB,21.5%of whom experienced major complications(Clavien–Dindo grade≥3).The rate of major complications declined as the number ofTUEB cases increased(P=0.013).Our results suggest that the efficiency of TUEB stabilized within 70 procedures.

The intensity attenuation of a high-power laser is a frequent task in the measurements of optical *** intensity can be attenuated by inserting an optical element,such as a partial reflector,polarizer or absorption *** devices are,however,not always easily applicable,especially in the case of ultra-high-power lasers,because they can alter the characteristics of a laser beam or become easily *** this study,we demonstrated that the intensity of a laser beam could be effectively attenuated using a random pinhole attenuator(RPA),a device with randomly distributed pinholes,without changing the beam *** this device,a multi-PW laser beam was successfully attenuated and the focused beam profile was measured without any alterations of its *** addition,it was confirmed that the temporal profile of a laser pulse,including the spectral phase,was ***,the RPA possesses significant potential for a wide range of applications.

Specific Radius Change of Quantum Dot inside the Lipid Bilayer by Charge Effect of Lipid Head-Group

We studied the quantum dot-liposome complex (QLC), which is the giant unilamellar vesicle with quantum dots (QDs) incorporated in its lipid bilayer. A spin coating method in conjunction with the electroformation technique yielded vesicles with highly homogeneous unilamellar structure. We observed QD size dependence of the QLC formation: QLCs form with blue, green and yellow-emission QD (core radius ~1.05 nm, 1.25 nm and 1.65 nm) but not with red-emission QD (core radius ~2.5 nm). In order to explain this size dependence, we made a simple model explaining the QD size effect on QLC formation in terms of the molecular packing parameter and the lipid conformational change. This model predicts that QDs below a certain critical size (radius ≈ 1.8 nm) can stably reside in a lipid bilayer of 4 - 5 nm in thickness for Egg-PC lipids. This is consistent with our previous experimental results. In the case of red-emission QD, QD-aggregations are only observed on the fluorescent microscopy instead of QLC. We expected that the reduction of packing parameter (P) would lead to the change of specific QD radius. This prediction could be verified by our experimental observation of the shift of the specific QD size by mixing DOPG.

Severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)is a‘highly transmissible respiratory pathogen,leading to severe multiorgan ***,knowledge regarding SARS-CoV-2-induced cellular alterations is *** this study,we report that SARSCoV-2 aberrantly elevates mitochondrial bioenergetics and activates the EGFR-mediated cell survival signal cascade during the early stage of viral ***-CoV-2 causes an increase in mitochondrial transmembrane potential via the SARS-CoV-2 RNAnucleocapsid cluster,thereby abnormally promoting mitochondrial elongation and the OXPHOS process,followed by enhancing ATP ***,SARS-CoV-2 activates the EGFR signal cascade and subsequently induces mitochondrial EGFR trafficking,contributing to abnormal OXPHOS process and viral *** EGFR inhibitors remarkably reduce SARS-CoV-2 propagation,among which vandetanib exhibits the highest antiviral *** of SARS-CoV-2-infected cells with vandetanib decreases SARS-CoV-2-induced EGFR trafficking to the mitochondria and restores SARS-CoV-2-induced aberrant elevation in OXPHOS process and ATP generation,thereby resulting in the reduction of SARS-CoV-2 ***,oral administration of vandetanib to SARS-CoV-2-infected hACE2 transgenic mice reduces SARS-CoV-2 propagation in lung tissue and mitigates SARS-CoV-2-induced lung *** also exhibits potent antiviral activity against various SARS-CoV-2 variants of concern,including alpha,beta,delta and omicron,in in vitro cell culture *** together,our findings provide novel insight into SARS-CoV-2-induced alterations in mitochondrial dynamics and EGFR trafficking during the early stage of viral infection and their roles in robust SARS-CoV-2 propagation,suggesting that EGFR is an attractive host target for combating COVID-19.
