Axillary Sentinel Lymph Node Biopsy for Breast Cancer and Melanoma Patients after Previous Axillary Surgery: A Systematic Review

Objective: Sentinel lymph node biopsy (SLNB) is a validated staging technique for breast carcinoma. Some women are exposed to have a second SLNB due to breast cancer recurrence or a second neoplasia (breast or other). Due to modified anatomy, it has been claimed that previous axillary surgery represents a contra-indication to SLNB. Our objective was to analyse the literature to assess if a second SLNB is to be recommended or not. Methods: For the present study, we performed a review of all published data during the last 10 years on patients with previous axilla surgery and second SLNB. Results: Our analysis shows that second SLNB is feasible in 70%. Extra-axillary SNs rate (31%) was higher after radical lymph node dissection (ALND) (60% - 84%) than after SLNB alone (14% - 65%). Follow-up and complementary ALND following negative and positive second SLNB shows that it is a reliable procedure. Conclusion: The review of literature confirms that SLNB is feasible after previous axillary dissection. Triple technique for SN mapping is the best examination to highlight modified lymphatic anatomy and shows definitively where SLNB must be performed. Surgery may be more demanding as patients may have more frequently extra-axillary SN only, like internal mammary nodes. ALND can be avoided when second SLNB harvests negative SNs. These conclusions should however be taken with caution because of the heterogeneity of publications regarding SLNB and surgical technique.

Distribution and fate of DCX/PSA-NCAM expressing cells in the adult mammalian cortex： A local reservoir for adult cortical neuroplasticity？

TCP-friendly rate control （TFRC） is a congestion control technique that trade-offs responsiveness to the network conditions for a smoother throughput *** take advantage of this trade-off by calculating the rate gap between the theoretical TCP throughput and the smoothed TFRC *** rate gain from this rate gap is then opportunistically used for video *** define a frame complexity measure to determine the additional rate to be used from the rate gap and then perform a rate negotiation to determine the target rate for the encoder and the final sending *** show that although this method has a more aggressive sending rate compared to TFRC,it is still TCP- friendly,does not contribute too much to network congestion and achieves a reasonable video quality gain over the conventional method.

Abstract The expression of early developmental markers such as doublecortin （DCX） and the polysialylated-neural cell adhesion molecule （PSA-NCAMIp has been used to identify immature neurons within canonical neurogenic niches. Additionally, DCX/PSA-NCAM ＋ immature neurons reside in cortical layer II of the paleocortex and in the paleo- and entorhinal cortex of mice and rats, respectively. These cells are also found in the neocortex of guinea pigs, rabbits, some afrotherian mammals, cats, dogs, non-human primates, and humans. The population of cortical DCX/PSA-NCAM ＋ immature neurons is generated prenatally as conclusively demonstrated in mice, rats, and guinea pigs. Thus, the majority of these cells do not appear to be the product of adult proliferative events. The immature neurons in cortical layer II are most abundant in the corlLices of young individuals, while very few DCX/PSA-NCAM ＋ cortical neurons can be detected in aged mammals. Maturation of DCX/PSA-NCAM ＋ cells into glutamatergic and GABAergic neurons has been proposed as an explanation for the age-dependent reduction in their population over time. In this review, we compile the recent information regarding the age-related decrease in the number of cortical DCX/PSA-NCAM ＋ neurons. We compare the distribution and fates of DCX/PSA-NCAM ＋ neurons among mammalian species and speculate their impact on cognitive function. To respond to the diversity of adult neurogenesis research produced over the last number of decades, we close this review by discussing the use and precision of the term ＂adult non-canonical neurogenesis.＂

The study was conducted to investigate the effect of sludge retention on bacterial community composition of membrane bioreactor(MBR)treating synthetic hospital *** removal of four pharmaceuticals,namely carbamazepine,estradiol,venlafaxine,and ibuprofen in MBR,was studied at varying sludge retention time(SRT)duration of 100,45,and 15 days and hydraulic retention time(HRT)of 18 *** removal of ibuprofen and estradiol was constant at varying SRT;however,a negligible removal of carbamazepine and low removal of venlafaxine was observed(<20%).The study suggested that the SRT of 45 days in MBR could provide maximum treatment efficiency via decreasing membrane *** effect of sludge age and pharmaceutical presence on the bacterial community was investigated via high-throughput *** study reveals that the variation in SRT affects the dynamics of the bacterial community *** instance,the dominant bacterium Caldimonas of SRT 100 was disappeared at lower ***,the profile of the dominant genus of the SRTs varied greatly from each other.

对西藏拉萨地块超钾质岩石的研究是近10年来青藏高原研究的重要进展之一。本文对西藏拉萨地块中部当若雍错和许如错地区的超钾质火山岩进行了透长石和黑云母的^(40)Ar/^(39)Ar定年。当若雍错粗面岩的黑云母^(40)Ar/^(39)Ar等时线年龄为13．2±0．3Ma,3个透长石的等时线年龄分别为13．0±0．3Ma、13．7±0．3Ma和13．0±0．3Ma;许如错辉石粗面岩黑云母的^(40)Ar/^(39)Ar等时线年龄为11．2±0．3Ma。结合已有研究结果,探讨了拉萨地块碰撞后钾质和超钾质岩石的分布特征。超钾质岩石产出于大约东经87度以西的地区,岩石年龄介于8～25Ma之间,而钾质岩石则在东部和西部都有产出,时代为9～24Ma。超钾质和钾质岩石在野外产出的构造背景上,显示了与南北向裂谷、新生代盆地、南北延长的湖泊等分布的密切关系。拉萨地块碰撞后岩浆作用的时间与裂谷发育、岩脉侵入、埃达克岩形成等岩浆-构造事件的时间相互重叠不是偶然的,预示着可能存在一个深部岩石圈演化的统一事件,而最为可能的是高原南部岩石圈地幔的减薄作用。超钾质岩浆作用在拉萨地块与羌塘地区同时发育表明拉萨地块与藏北在构造和岩石圈演化方面具有一致性。

The Central Asia Orogenic Belt(CAOB) corresponds to the domain where Siberia and Mongolia were welded to North China. The eastern extension of the CAOB in Northeast China is disputed,since both suture location and timing are poorly documented. This paper reports for the first time the recognition of two suture zones in the southern part of Northeast China(Manchuria),between the Fushun Mishan and Yilan-Yitong *** the Jilin Province,west-directed thrust sheets involving successively,from west to east,passive continental margin rocks,metamorphic rocks and ophiolites,block-in-matrix formations and arc plutons indicate a Permian-Early Triassic *** the Liaoning Province,arc plutonism and top-to-the-north ductile shearing,coeval with the emplacement of an ophiolitic nappe,suggest a Palaeozoic *** two sutures are correlated with the Ondor Sum and Solonker sutures,described in Inner Mongolia.A new geodynamic model involving rifting and collision of the southern part of the Xilinhot Block with North China is proposed.

Introduction: Fractures of the femoral head are rare injuries which generally follow a traumatic dislocation of the hip or in a poly-trauma scenario. A fracture dislocation of the femoral head is an orthopedic emergency. The most frequent complications after a fracture of the femoral head are osteonecrosis, post traumatic arthritis and heterotopic ossification. Objective: To focus on the therapeutic aspect of fracture-dislocations of femoral head and their short- and long-term prognoses. Materials and Methods: This is a prospective study conducted at Kamenge teaching hospital and Rohero Christian medical and surgical clinic from January 2013 to August 2020. All patients diagnosed with fracture-dislocations of the femoral head were included in this study. Results: Five patients were admitted for fracture dislocation of the femoral head. The mean age was 40.4 years and varied between 25 and 55 years. The dashboard injury was the most common mechanism and was found in four patients (80%). A standard x-ray was performed for the five patients as well as CT scans in two cases. All patients had posterior iliac dislocations with fractures of the femoral head classified as Pipkin I in three patients and Pipkin III in two patie
