Areas of research and clinical approaches to the study of liver abscess

AIMTo analyze the research activity on liver abscess (LA) and identify the main topic clusters in the *** identified all documents using the medical subject heading “LA” indexed in the MEDLINE database between 2001 and 2015. We performed a descriptive bibliometric analysis, characterizing the evolution of scientific activity, the publication types of the documents, the document categories of clinical interest (case reports, clinical trials, evaluation studies, meta-analysis, observational studies, practice guidelines and validation studies) and the geographic distribution of the research. We also carried out an analysis of networks and research clusters in order to identify the main topic areas of *** search yielded a total of 1278 documents, showing a stable scientific production over the study period and a marked multidisciplinary nature. The research was dominated by case reports (65.9% of the documents analyzed). In terms of geographic distribution, researchers from the United States led in the number of signatures (n = 229), followed by those from Taiwan (n = 185), India (n = 145), Japan (n = 144), South Korea (n = 100), and China (n = 84). With regard to amebic LA, the top-producing countries were India and Mexico (n = 69 each), followed by the United States (n = 29). In the case of pyogenic LA, Taiwan Residents researchers led scientific production (n = 71), followed by the United States (n = 39) and China (n = 29). The most active areas of research in the field are diagnosis via computerized tomography scan, differential diagnosis with regard to liver cancer, treatment with antimicrobial agents, and Klebsiella infections (including bacteremia).CONCLUSIONClinical case reports associated with diagnosis and treatment are the main topic of study, highlighting the importance of this document type in advancing knowledge.

The intense study of coleopteran inclusions from Spanish （Albian in age） and French （Albian-Santonian in age） Cretaceous ambers, both of Laurasian origin, has revealed that the majority of samples belong to the Polyphaga suborder and, in contrast to the case of the compression fossils, only one family of Archostemata, one of Adephaga, and no Myxophaga suborders are represented. A total of 30 families from Spain and 16 families from France have been identified （with almost twice bioinclusions identified in Spain than in France）; 13 of these families have their most ancient representatives within these ambers. A similar study had previously only been performed on Lebanese ambers （Barremian in age and Gondwanan in origin）, recording 36 coleopteran families. Few lists of taxa were available for Myanmar （Burmese） amber （early Cenomanian in age and Laurasian in origin）. Coleopteran families found in Cretaceous ambers share with their modern relatives mainly saproxylic and detritivorous habits in the larval or adult stages, rather than wood-boring behavior. Fifteen of the coleopteran families occur in both the Lebanese and Spanish ambers; while only five are present in both Spanish and French. Considering the paleogeographic proximity and similarity of age of the Spanish and French ambers, the small number of taxa found in common at both areas is surprising. The ancient origin for the Lebanese and Spanish ambers, the paleogeography （including some barriers for terrestrial biota） and the local paleohabitats are factors that may explain the dissimilarity with the French specimens. Wildfires are believed to be a more likely cause of resin production during the Cretaceous than infestation by beetles. Current knowledge of the beetle species found in the Cretaceous ambers is introduced.

Pancreatic cancer remains one of the leading causes of cancer related death worldwide with an overall five-year survival of less than 5%. Potentially curative surgery, which alone can improve 5-year survival to 10%, is an option for only 10%-20% of patients at presentation owing to local invasion of the tumour or metastatic disease. Adjuvant chemotherapy has been shown to improve 5-year survival to 20%-25% but conflicting evidence remains with regards to chemoradiation. In this article we review the current evidence available from published randomised trials and discuss ongoing phase III trials in relation to adjuvant therapy in pancreatic cancer.

土壤含水层处理(SAT)逐渐被作为一种用作增强水资源和减少处理过的废水向水体中随意排放的有用的次要可靠手段的地理净化系统。研究目的是建立一个土壤含水层处理系统,以去除废水中的柴油污染物。使用90厘米长,内径为5厘米的PVC柱来模拟SAT系统中使用的土壤柱。小于2.00 mm的含水层材料被用来具有均匀的土壤柱填充材料,减少流动变形,尽量减小蚯蚓和碎片带来的代表性影响,并使用有助于SAT程序排出的粒径。在土壤柱中使用黄土作为含水层。色谱柱以1周为一个周期进行操作,包括3天的润湿,然后干燥4天(3w/4d),总共进行4个周期。该研究解释了从30℃到45℃的预处理废水温度变化对系统性能的影响。去除BOD和COD的过程效率分别高达99.12%和92.53%。在较高的温度和周期4下,出水的COD和BOD小,在较高的温度下后者的去除率较高。研究还集中在具有烃降解特性的细菌的分离,形态和生化特性上。旨在优化分离株的适宜培养条件,并筛选柴油的降解能力。鉴定出以下5种细菌。粪肠球菌-L1,溶菌杆菌-L2,芽孢杆菌-L3,马氏红球菌-L4和O-乳杆菌-L5。基于它们降解柴油的能力,选择了两个分离株并优化了它们的生长条件。在细菌菌株L2和L4的2%底物(柴油)浓度,p H 7-8和25℃至30℃的温度范围内记录了最佳细菌生长。

Non-cycloplegic spherical equivalent refraction in adults:comparison of the double-pass system,retinoscopy,subjective refraction and a table-mounted autorefractor

AIM:To evaluate the accuracy of spherical equivalent(SE) estimates of a double-pass system and to compare it with retinoscopy,subjective refraction and a table mounted ***:Non-cycloplegic refraction was performed on 125 eyes of 65 healthy adults(age 23.5±3.0 years) from October 2010 to January 2011 using retinoscopy,subjective refraction,autorefraction(Auto kerato refractometer TOPCON KR-8100,Japan) and a double pass system(Optical Quality Analysis System,OQAS,Visiometrics S.L.,Spain).Nine consecutive measurements with the double-pass system were performed on a subgroup of 22 eyes to assess *** evaluate the trueness of the OQAS instrument,the SE laboratory bias between the double pass system and the other techniques was ***:The SE mean coefficient of repeatability obtained was *** correlations could be established between the OQAS and the SE obtained with retinoscopy(r=0.956,P<0.001),subjective refraction(r=0.955,P<0.001) and autorefraction(r=0.957,P<0.001).The differences in SE between the double-pass system and the other techniques were significant(P<0.001),but lacked clinical relevance except for retinoscopy;Retinoscopy gave more hyperopic values than the double-pass system-0.51±0.50D as well as the subjective refraction-0.23±0.50D;More myopic values were achieved by means of autorefraction 0.24±0.49D. CONCLUSION:The double-pass system provides accurate and reliable estimates of the SE that can be used for clinical *** technique can determine the correct focus position to assess the ocular optical ***,it has a relatively small measuring range in comparison with autorefractors(-8.00 to +5.00D),and requires prior information on the refractive state of the patient.

To facilitate the implementation of controlled donation after circulatory death(cDCD)programs even in hospitals not equipped with a local Extracorporeal Membrane Oxygenation(ECMO)team(Spokes),some countries and Italian Regions have launched a local cDCD network with a ECMO mobile team who move from Hub hospitals to Spokes for normothermic regional perfusion(NRP)implantation in the setting of a cDCD *** ECMO teams have been clearly defined by the Extracorporeal Life Support Organization,regarding composition,responsibilities and training programs,no clear,widely accepted indications are to date available for NRP *** existing NRP mobile networks were developed due to the urgent need to increase the number of cDCDs,there is now the necessity for transplantation medicine to identify the peculiarities and responsibility of a NRP team for all those centers launching a cDCD ***,in the present manuscript we summarized the character-istics of an ECMO mobile team,highlighting similarities and differences with the NRP mobile *** also assessed existing evidence on NRP teams with the goal of identifying the characteristic and essential features of an NRP mobile team for a cDCD program,especially for those centers who are starting the *** were identified between the mobile ECMO team and NRP mobile *** common essential feature for both mobile teams is high skills and experience to reduce complications and,in the case of cDCD,to reduce the total warm ischemic *** training programs should be developed for the launch of de novo NRP teams.

A method based on micellar liquid chromatography has been developed to simultaneously monitor four pesticides largely post-harvest applied to citrus：thiabendazole,pyrimethanil,o-phenylphenol and *** samples were filtered and directly injected without other treatment,thus avoiding extraction *** composition of the mobile phase was optimized using a chemometrical approach to achieve and excellent resolution to 0.07 mol/L SDS/5%,V/V 1-pentanol buffered at p H *** phase run through a C18 column at 1 m L/min at room *** detection was performing by UV–Visible absorbance using a wavelength program：0–10 min,305 nm（for thiabendazole）;10–12;265 nm（for pyrimethanil）and 12–18,220 nm（o-phenylphenol and imazalil）.The developed method was validated following the guidelines of the US Environmental Protection Agency in terms of：quantitation range,（0.5–4 to 15μg/m L）,linearity（r2〉0.9995）,sensitivity（LOD,0.18–1.4μg/m L）,precision（〈9.2%）,trueness（93.9%–103.7%）,and ruggedness（〈9.9%）.It was found that the fungicides remain up to eight days in surface water at outdoor *** method was used to screen the presence of the analytes in several waste water samples,and was proved to be useful in routine analysis.

Donor management is the key in the complex donation process,since up to 20%of organs of brain death donors(DBD)are lost due to hemodynamic *** challenge is made more difficult due to the lack of strong recommendations on therapies for hemodynamic management in DBDs and more importantly to the epidemiologic changes in these donors who are becoming older and with more comorbidities(marginal donors).In the present manuscript we aimed at summarizing the available evidence on therapeutic strategies for hemodynamic management(focusing on vasoactive drugs)and monitoring(therapeutic goals).Evidence on management in elderly DBDs is also *** management continues critical care but with different and specific therapeutic goals since the number of donor goals met is related to the number of organs retrieved and *** monitoring of selected parameters(possibly including serial echocardiography)is the clinical tool able to guarantee the achievement and maintaining of therapeutic *** worldwide differences,norepinephrine is the vasoactive of choice in most countries but,whenever higher doses(>0.2 mcg/kg/min)are needed,a second vasoactive drug(vasopressin)is *** therapy(desmopressin,corticosteroid and thyroid hormone)are suggested in all DBDs independently of hemodynamic *** the single patient,therapeutic regimen(imprimis vasoactive drugs)should be chosen also according to the potential organs retrievable(*** vs liver and kidneys).

Objective: To study the regulation of blood pulse volume via photoplethysmography (PPG) signal detected from toe, while the lower limb is passively raised in different height positions. Methods: Use a modified non-invasive PPG technique to detect the blood pulse signal on toe with infrared (IR) photo sensor. A protocol consisting of two postures, i.e., supine and 45° reclining, was designed to conduct laboratory trial in this study. During the period of performing the protocol of these postures, the lower limb was passively raised from the heights of 10 cm to 60 cm randomly and individually with sponge blocks underneath the foot. Results: In the supine posture, the higher the foot was passively raised, the more the blood PPG signal decreased. In the 45° reclining posture, the blood PPG signal increased at the beginning and then decreased in the foot height position from 10 cm to 60 cm. In both postures the normalized AC signal changes significantly while the normalized DC signal changes little. Conclusion: The toe PPG signals can obviously indicate the regulated blood volume change with the designated postural procedures due to the heart level position.
