Passive expansion of sub-maximally dilated transjugular intrahepatic portosystemic shunts and assessment of clinical outcomes

AIM To assess for passive expansion of sub-maximally dilated transjugular intrahepatic portosystemic shunts(TIPS) and compare outcomes with maximally dilated *** Polytetrafluoroethylene covered TIPS(Viatorr) from July 2002 to December 2013 were retrospectively reviewed at two hospitals in a single institution. Two hundred and thirty patients had TIPS maximally dilated to 10 mm(m TIPS), while 43 patients who were at increased risk for hepatic encephalopathy(HE), based on clinical evaluation or low pre-TIPS portosystemic gradient(PSG), had 10 mm TIPS sub-maximally dilated to 8 mm(sm TIPS). Group characteristics(age, gender, Model for End-Stage Liver Disease score, post-TIPS PSG and clinical outcomes were compared between groups, including clinical success(ascites or varices), primary patency,primary assisted patency, and severe post-TIPS HE. A subset of fourteen patients with sm TIPS underwent follow-up computed tomography imaging after TIPS creation, and were grouped based on time of imaging(o and > 6 mo). Change in diameter and crosssectional area were measured with 3D imaging software to evaluate for passive *** Patient characteristics were similar between the sm TIPS and m TIPS groups, except for pre-TIPS portosystemic gradient, which was lower in the sm TIPS group(19.4 mm Hg ± 6.8 vs 22.4 mm Hg ± 7.1, P = 0.01). Primary patency and primary assisted patency between sm TIPS and m TIPS was not significantly different(P = 0.64 and 0.55, respectively). Four of the 55 patients(7%) with sm TIPS required TIPS reduction for severe refractory HE, while this occurred in 6 of the 218 patients(3%) with m TIPS(P = 0.12). For the 14 patients with follow-up computed tomography(CT) imaging, the median imaging follow-up was 373 d. There was an increase in median TIPS diameter, median percent diameter change, median area, and median percent area change in patients with CT follow-up greater than 6 mo after TIPS placement compared to follow-up within 6 mo(

Uncertainty quantification in the Permian Basin using conventional and modified bootstrap methodology

Various uncertainty quantification methodologies are presented using a combination of several deter-ministic decline curve analysis models and two bootstrapping *** probabilistic models are applied to 126 sample wells from the Permian *** are presented for 12-72 months of pro-duction hindcast given an average well production history of 103 *** on the coverage rate and the forecast error(with the coverage rate being more significant in our choice of the best probabilistic models)and using up to one-half of the available production history for a group of sample wells from the Permian Basin,we find that the CBM-SEPD combination is the best probabilistic model for the Central Basin Platform,the MBM-Arps combination is the best probabilistic model for the Delaware Basin,the CBM-Arps is the best probabilistic model for the Midland Basin,and the best probabilistic model for the overall Permian Basin is the CBM-Arps when early time data is used as hindcast and CBM-SEPD for when one-quarter to one-half of the data is used as *** three-quarters or more of the available production history is used for analysis,the MBM-SEPD probabilistic model is the best combination in terms of both coverage rate and forecast error for all the sub-basins in the *** novelty of this work lies in its extension of bootstrapping methods to other decline curve analysis *** work also offers the engineer guidance on the best choice of probabilistic model whilst attempting to forecast production from the Permian Basin.

The viscosity at the glass transition of a liquid lubricant

In the classical study of elastohydrodynamic lubrication(EHL) which does not employ real, measurable viscosity in analysis, the possibility of a glass transition has not been considered in many years. Indeed, the two rheological assumptions of classical EHL, the Newtonian inlet and the equivalence of a traction curve to a flow curve, would not have persisted so long had the pressure dependence of the viscosity been accurately stated. With the recent appearance of viscosity obtained from viscometers in EHL analysis, the possibility of a glass transition in the contact should be reexamined, especially for the fragile traction fluids. This article employs published data for a synthetic cycloaliphatic hydrocarbon to estimate the glass transition viscosity so that, when using real viscosities in EHL simulations, the state of the liquid may be assessed. Far into the glassy state the liquid should be treated as an elastic solid with a yield stress.

Mercury bioaccumulation in aquatic biota along a salinity gradient in the Saint John River estuary

Although estuaries are critical habitats for many aquatic species, the spatial trends of toxic methylmercury（MeHg） in biota from fresh to marine waters are poorly understood. our objective was to determine if MeHg concentrations in biota changed along a salinity gradient in an estuary. Fourspine Stickleback（Apeltes quadracus）, invertebrates（snails,amphipods, and chironomids）, sediments, and water were collected from ten sites along the Saint John River estuary, New Brunswick, Canada in 2015 and 2016, with salinities ranging from 0.06 to 6.96. Total mercury（proxy for MeHg） was measured in whole fish and MeHg was measured in a subset of fish, pooled invertebrates, sediments, and water. Stable sulfur（δ34S）, carbon（δ13 C）, and nitrogen（δ15N） isotope values were measured to assess energy sources（S, C） and relative trophic level（N）. There were increases in biotic δ13C and δ34S from fresh to more saline sites and these measures were correlated with *** aqueous MeHg was higher at the freshwater than more saline sites, only chironomid MeHg increased significantly with salinity. In the Saint John River estuary, there was little evidence that MeHg and its associated risks increased along a salinity gradient.

Investigation on Liquid Holdup in Vertical Zero Net-Liquid Flow

Zero net-liquid flow (ZNLF) is a special case of upward gas-liquid two-phase flow. It is a phenomenon observed as a gas-liquid mixture flows in a conduit but the net liquid flow rate is zero. Investigation on the liquid holdup of ZNLF is conducted in a vertical ten-meter tube with diameter of 76 mm, both for Newtonian and nonNewtonian fluids. The gas phase is air. The Newtonian fluid is water and the non-Newtonian fluids are water-based guar gel solutions. The correlations developed for predicting liquid holdup on the basis of Lockhart-Martinelli parameter are not suitable to ZNLF. A constitutive correlation for the liquid holdup of vertical ZNLF was put forward by using the mass balance. It is found that the liquid holdup in ZNLF is dependent on both the gas flow rate and the flow distribution coefficient.

Investigation of genome structure of a cinnamyl alcohol dehydrogenase locus in a basal angiosperm hardwood species, Liriodendron tulipifera L., reveals low synteny

Basal angiosperms contain a wide diversity of floral and growth forms and gave rise to the largest recent angiosperm *** none of the basal angiosperm genomes has been sequenced,examining large bacterial artificial chromosome(BAC) inserts remains the main approach to providing a first glimpse of the structure and organization of their *** this study,we sequenced a 126.9-kbp BAC contig harboring a cinnamyl alcohol dehydrogenase gene(LtuCAD1) in a basal angiosperm species,Liriodendron tulipifera L.,an important timber tree species with significant ecological and economic values.A key enzyme in lignin biosynthesis,CAD catalyzes the final step in the synthesis of *** carried out phylogenetic analyses of seven full-length CAD family genes(LtuCAD1-7) obtained from a comprehensive Liriodendron expressed sequence tag *** phylogenetic tree suggests that LtuCAD1 is the primary CAD gene involved in lignifications as it is the only Liriodendron CAD grouped with the bona fide CADs *** well as the LtuCAD1,the BAC contig contained fragmented sequences for one integrase,eight hypothetical proteins,two gag-pol polyproteins,one RNase H family protein,and one chromatin binding *** analysis with other angiosperm species suggests that the genomic segment in this BAC has undergone frequent *** study is our initial step in identifying and understanding lignin biosynthesis genes from basal angiosperm *** knowledge can help bridge the information gap between hardwood(angiosperm) and softwood(gymnosperm) species and benefit potential breeding and biotechnology application for enhanced production of biomass and digestibility in ***.

Numerical Study on the Influence Mechanism of Inlet Distortion on the Stall Margin in a Transonic Axial Rotor

Drug induced acute pancreatitis: Does it exist?

As the incidence of acute pancreatitis continues to rise, establishing the etiology in order to prevent recurrence is important. Although the etiology of acute pancreatitis is not difficult in the majority of patients, almost a quarter of patients are initially labeled as having idiopathic acute pancreatitis. When confronted with a patient with acute pancreatitis and no clear etiology defined as an absence alcoholism, gallstones(ultrasound and/or MRI), a normal triglyceride level, and absence of tumor, it often appears reasonable to consider a drug as the cause of acute pancreatitis. over 100 drugs have been implicated by case reports as causing acute pancreatitis. While some of these case reports are well written, many case reports represent poorly written experiences of the clinician simply implicating a drug without a careful evaluation. over-reliance on case reports while ignoring randomized clinical trials and large pharmacoepidemiologic surveys has led to confusion about drug induced acute pancreatitis. This review will explain that drug induced acute pancreatitis does occur, but it is rare, and over diagnosis leads to misconceptions about the disease resulting in inappropriate patient care, increased litigation and a failure to address the true entity: idiopathic acute pancreatitis.
