Objective:Concurrent chemoradiotherapy using cisplatin was thought to be standard treatment for squamous cell carcinoma of cervix,but it had not been effective for *** chemoradiotherapy using irinotecan hydrochloride(CPT-11)had been effective for colorectal cancer,thus,we chose CPT-11 as a candidate for gynecologic *** evaluate the maximum tolerated dose(MTD)of weekly CPT-11 with external pelvic radiotherapy,a phase 1/2 study was conducted according to modified Fibonacci ***:Eligible patients were advanced uterine cancer with measurable diseases[performance score(PS):0-2].Study period was from August 1 st,2002 to December 31 st,*** starting dose level(DL)of CPT-11 was 30 mg/m2(DL1)given weekly for 4 ***,dose escalation was scheduled in 10 mg/m2 increments to 60 mg/m^2(DL4).The fixed radiotherapy consisted of whole pelvic 1.8 Gy/d,once a day in weekday for five weeks and it amounted to 45 Gy(25 fractions)in ***:Seventeen patients were *** for toxicities,one(1/17:5.9%)grade(G)4 neutropenia lasting 7 days had been seen in DL4.G2 diarrhea was identified in 35.3%(6/17)of the patients,and 11.8%(2/17)G3 diarrhea was observed in DL3 and ***,the MTD of CPT-11 was defined as dose of 60 mg/m^*** recommended dose was decided as 50 mg/m^*** response rate was 88.2%[9 complete response(CR),3 partial response(PR),3 stable disease(SD),2 not evaluable(NE)].Disease control rate at 1 month after treatment completion was 100%but distant metastases were found in 24%(4/17)in longer ***:MTD was 60 mg/m^2 and recommended dose was set as 50 mg/*** concurrent chemoradiation using weekly CPT-11 was feasible at 50 mg/m^2,and it might be effective even in adenocarcinoma of the uterus.

The purpose of this research was to elucidate the relationship between tonsillar hypertrophy and Obstructive Sleep Apnea Syndrome (OSAS) in terms of cephalometric analysis, as well as to determine factors that affect OSAS severity. 25 patients were enrolled in this study. Lateral cephalograms of the subjects were taken. Disease severity was evaluated by the Apnea Hypopnea Index (AHI). Cephalometric measurements (angle between line S-N and line N-A (SNA), angle between line S-N and line N-B (SNB), distance between the Mandibular Plane and the Hyoid (MPH), posterior airway space, the width of the airway behind the base of tongue along the B-Go line (PAS), upper airway space, the distance parallel to long axis of the airway, between a horizontal plane tangent to the superior aspect of the hyoid and a horizontal plane tangent to the posterior palate (UAL), multiplication of PAS and UAL (PAS × UAL)) were analyzed and compared between the groups with and without tonsillar hypertrophy. PAS and PAS × UAL were lower in the no hypertrophy group. SNB was significantly lower in the severe apnea group than mild to moderate group in no hypertrophy group (p = 0.035). In conclusion, patients with tonsillar hypertrophy had severe obstruction, but they had a relatively larger pharyngeal space on cephalometric analysis. After exclusion of the tonsillar hypertrophy group, cephalometric analysis could be more effective for analyzing OSAS severity.

The first excited 2^(+)state(885 keV)of ^(32)Mg and its corresponding deformation parameterβ_(2) are studied with proton inelastic scattering at 190 MeV/nucleon using in-beamγtechnology with a thick *** distributions of the inelastic scattering cross sections are analyzed with coupled-channel *** deformation parameterβ_(2)=0.41(3)obtained in this work agrees with the earlier experimental results obtained at lower beam *** work demonstrates that the structure of unstable nuclei can be studied with proton inelastic scattering at high incident energies in the vicinity of 200 MeV/nucleon using thick targets.

Fluorescent-SiC （f-SiC）, which contains donor and acceptor impurities with optimum concentrations, has high conversion efficiency from NUV to visible light caused by donor-acceptor-pair （DAP） recombination. This material can be used as a substrate for a near UVlight-emitting diode （LED） stack, and leads to monolithic white LED device with suitable spectral property for general lighting applications. In this paper, we describe basic technologies of the white LED, such as optical properties of f-SiC substrate, and epitaxial growth of NUV stack on the f-SiC substrate.

A perioperative multimodal strategy including combination chemotherapy and radiotherapy, in addition to surgical resection, has been acknowledged to improve patient prognosis. However chemotherapy has not been actively applied as an immunomodulating modality because of concerns about various immunosuppressive effects. It has recently been shown that certain chemotherapeutic agents could modify tumor microenvironment and host immune responses through several underlying mechanisms such as immunogenic cell death, local T-cell infiltration and also the eradication of immune-suppressing regulatory cells such as regulatory T cells(Tregs) and myeloid-derived suppressor cells. With the better understanding of the cell components in the tumor microenvironment and the effect of chemotherapy to improve tumor microenvironment, it has been gradually clear that the chemotherapeutic agents is two-edged sword to have both immune promoting and suppressing effects. The cellular components of the tumor microenvironment include infiltrating T lymphocytes, dendritic cells, regulatory T cells, tumor associated macrophages, myeloid derived suppressor cells and cancer associated fibroblasts. Based on the better understanding of tumor microenvironment following chemotherapy, the treatment protocol could be modified as personalized medicine and the prognosis of pancreas cancer would be more improved utilizing multimodal chemotherapy. Here we review the recent advances of chemotherapy to improve tumor microenvironment of pancreatic cancer, introducing the unique feature of tumor microenvironment of pancreatic cancer, interaction between anti-cancer reagents and these constituting cells and future prospects.

In this study,we attempted to elucidate the carrier dynamics behind the abnormal characteristics of photogenerated current and voltage(IPhand VPh)at cryogenic temperature inⅢ-nitride optoelectronic semiconductors by employing space-charge *** this end,we carefully investigated and analyzed excitation-powerdependentⅠ-Ⅴ(PDIV)curves operated by quasiresonant excitation of an Al Ga In N-based p-i-n junction semiconductor at 300 K and 15 *** 300 K,the curves exhibited typical characteristics and were well described by the conventional ***,the PDIV curves at 15 K could no longer be described by the conventional *** elucidate the mechanism behind this phenomenon,we proposed a model in which the space-charge effect(SCE)plays a key *** on this model,we proposed the modified Shockley diode equation,which can explain the PDIV characteristic at 15 K,including the *** also discussed the SCE on the efficiency of devices.

AIM:To assess adalimumab's efficacy with concomitant azathioprine (AZA) for induction and maintenance of clinical remission in Japanese Crohn's disease (CD) patients. METHODS:This retrospective, observational, singlecenter study enrolled 28 consecutive CD patients treated with adalimumab (ADA). Mean age and mean disease duration were 38.1 ± 11.8 years and 11.8 ± 10.1 years, respectively. The baseline mean Crohn's disease activity index (CDAI) and C-reactive protein were 177.8 ± 82.0 and 0.70 ± 0.83 mg/dL, respectively. Twelve of these patients also received a concomitant stable dose of AZA. ADA was subcutaneously administered:160 mg at week 0, 80 mg at week 2, followed by 40 mg every other week. Clinical response and remission rates were assessed via CDAI and C-reactive protein for 24 wk. RESULTS:The mean CDAI at weeks 2, 4, 8, and 24 was 124.4, 120.2, 123.6, and 135.1, respectively. The CDAI was significantly decreased at weeks 2 and 4 with ADA and was significantly suppressed at 24 wk with ADA/AZA. Overall clinical remission rates at weeks 4 and 24 were 66.7% and 63.2%, respectively. Although no statistically significant difference in C-reactive protein was demonstrated, ADA with AZA resulted in a greater statistically significant improvement in CDAI at 24 wk, compared to ADA alone. CONCLUSION:Scheduled ADA with concomitant AZA may be more effective for clinical remission achievement at 24 wk in Japanese Crohn's disease patients.

Transient Behavior in Water Distillation Tower for Tritium Separation and Its Long-Time Operation Test Results

Transient separation behavior in tritiated water distillation tower packed with materials having ability to adsorb water is investigated analytically and experimentally for nuclear reactor safety. Analytical equations based on the stage model are set up for simulation of the transient behavior of tritium (T) separation from wastewater. It is found that a dimensionless time defined in terms of the inside vapor flow rate and the liquid holdups in tower, reboiler and condenser can correlate variations over time to achieve a steady-state T concentration. However, when mixing with different T concentrations at a feed point occurs, the transition time period becomes longer than expected. Effects of the reflux ratio, the stage separation factor and the total stage number on the transient and steady-state T concentrations are numerically calculated. Variations over time to achieve each steady-state value are compared with experimental data using a small-scale tower. Long time distillation experiment for one month has been completed, and it is confirmed that a distillation column packed with ceramic Raschig rings coated with zeolite 13X adsorbent is hardly affected by water corrosion.
