An 81-year-old Japanese man with jaundice was strongly suspected clinically of having primary sclerosing cholangitis based on clinical examinations and later died of hepatic failure. The entire course of the disease lasted about 10 mo. The autopsy revealed extensive fibrinoid necrosis in the liver, kidney, spleen, pancreas, lung, lymph nodes, and pleura. Particularly extensive fibrinoid necrosis in the portal tracts of the liver induced severe stenoses of the intrahepatic bile ducts, resulting in cholestasis in association with prominent liver injury. There were no findings indicating primary sclerosing cholangitis. The hepatic lesions in this case did not coincide with any known disease including collagen diseases. To clarify the cause of irregular stenoses of the intrahepatic biliary trees on cholangiographic findings, we postulate that some form of immunological derangement might be involved in pathogenesis of fibrinoid necrosis. However, the true etiology remains unknown.

The enantioselectivity of α-chymotrypsin in the esterification of N-acetyl-tryptophan with ethanol was examined in aprotic ionic liquids. The enantioselectivity was found to be strongly affected by a kind of solvent, water content, and reaction temperature. The (kcatKM)L/(kcat/KM)D ratio in 1-ethyl-3-methylimidazolium bis(fluorosulfonyl)imide ([C2mim][FSI]) containing 1.0% (v/v) water at 25°C exhibits 32,000, while that in 1-ethyl-3-methylimidazolium tetrafluoroborate ([C2mim][BF4]) containing 1.0% (v/v) water at 25°C shows 190. Antimicrobial therapy using sulfamethoxazole trimethoprim for Kawasaki disease patients unresponsive to intravenous immunoglobulin

Our previous study suggested that the production of superantigens and heat-shock protein 60 by small intestinal bacteria might play a role in Kawasaki disease (KD). We demonstrated that they were all resistant to commonly used antibiotics, except for sulamethoxazole trimethoprim (SMX-TMP). We used SMX-TMP for 7 cases of KD that were unresponsive to intravenous immunoglobulin (IVIG) and studied the antipyretic potency of this treatment. In 6 out of the 7 cases, we demonstrated that antipyretic potency was observed without side effects within 2 days of the initial administration. Antimicrobial therapy using SMX-TMP might represent a novel strategy for cases of KD that are unresponsive to IVIG.

Removal of diminutive colorectal polyps: A prospective randomized clinical trial between cold snare polypectomy and hot forceps biopsy

AIMTo compare the efficacy and safety of cold snare polypectomy (CSP) and hot forceps biopsy (HFB) for diminutive colorectal *** prospective, randomized single-center clinical trial included consecutive patients ≥ 20 years of age with diminutive colorectal polyps 3-5 mm from December 2014 to October 2015. The primary outcome measures were en-bloc resection (endoscopic evaluation) and complete resection rates (pathological evaluation). The secondary outcome measures were the immediate bleeding or immediate perforation rate after polypectomy, delayed bleeding or delayed perforation rate after polypectomy, use of clipping for bleeding or perforation, and polyp retrieval rate. Prophylactic clipping after polyp removal wasn’t routinely *** hundred eight patients were randomized into the CSP (102), HFB (106) and 283 polyps were evaluated (CSP: 148, HFB: 135). The en-bloc resection rate was significantly higher with CSP than with HFB [99.3% (147/148) vs 80.0% (108/135), P < 0.0001]. The complete resection rate was significantly higher with CSP than with HFB [80.4% (119/148) vs 47.4% (64/135), P < 0.0001]. The immediate bleeding rate was similar between the groups [8.6% (13/148) vs 8.1% (11/135), P = 1.000], and endoscopic hemostasis with hemoclips was successful in all cases. No cases of perforation or delayed bleeding occurred. The rate of severe tissue injury to the pathological specimen was higher HFB than CSP [52.6% (71/135) vs 1.3% (2/148), P < 0.0001]. Polyp retrieval failure was encountered CSP (7), HFB (2).CONCLUSIONCSP is more effective than HFB for resecting diminutive polyps. Further long-term follow-up study is required.

The effects of ethephon and abscisic acid(ABA) application on ripening-related genes of pre-harvest ‘Kohi' kiwifruit(Actinidia chinensis) were studied to clarify the possibility of the fruit ripening on the vine. The fruits were treated on the vine at mature stage with 250 μL·L^(-1)ethephon or 100 μmol·L^(-1)ABA, and the expression levels of chlorophyll synthase(Ac CLS), chlorophyllase1(Ac CLH1), polygalacturonase(Ac PG), expansin(Ac EXP),β-amylase(Acβ-AM), sucrose synthase(Ac SUSY), MADS-box SEPALLATA4/RIPENING INHIBITOR-like(Ac SEP4/RIN) and FRUITFUL-like(Ac TDR4/FUL) genes were analyzed. The expression levels of Ac PG, Ac EXP, Acβ-AM, and Ac SUSY increased in the ethephon-treated fruit, but those of Ac CLH1 at 6and 9 days after treatment and Ac CLS decreased. Moreover, the expression levels of Ac SEP4/RIN and Ac TDR4/FUL, the latter of which is associated with ethylene biosynthesis, were higher in the ethephon-treated fruit. The expression level of each gene in ABA-treated fruit was not significantly different from that of the untreated control. The results suggest that ethephon application increases the expression levels of Ac PG,Ac EXP, Acβ-AM, Ac SUSY, Ac SEP4/RIN, and Ac TDR4/FUL in ‘Kohi' kiwifruit on the vine.

Producing enough tomato to meet market demand sustainably has not been feasible in the tropics like Ghana. Attempts to improve production using greenhouse facilities have not addressed the challenge because of high-temperature conditions in the greenhouse, which are difficult to manage. Heat stress, arising from high temperatures, hinder the performance of tomato in terms of fruit set and yield. Moreover, the impending climate change is expected to impose more unfavorable environmental conditions on crop production. An experiment was conducted in (greenhouse at Chiba University, Japan) summer period, which has similar high-temperature conditions like Ghana. This work sought to increase the yield of a heat-tolerant tomato using a state-of-the-art hydroponic system through high-density planting. The outcome of this work was intended for adoption and practice in Ghana. A Heat-tolerant tomato “Nkansah HT” along with Lebombo and Jaguar cultivars, were grown at high and low plant densities (4.1 and 2.7 plants m-2 respectively).
