＜正＞We show that femtosecond （fs） laser enables us to produce true three-dimensional （3D） microstructures inside a photosensitive glass （Foturan）, which has superior properties on transparency, hardness, chemical and thermal resistances, and biological compatibility. Exposure to the focused fs laser beam writes latent images inside the glass. Modified regions are developed by a post baking process and then preferentially etched away in dilute hydrofluoric （HF） acid. In general, an embedded microchannel fabricated by scanning focal beam perpendicularly to the direction of laser propagation becomes a cross section of an elliptical shape with a large aspect ratio according to the longitudinal beam profile. We demonstrate that the aspect ratio of channel can be improved merely by placing a slit before the focusing system.

Purpose: Surfactant proteins exist in the digestive tract and may play an important role in the host defense. However, the expression of surfactant proteins in the premature digestive system remains unclear. The aim of this study was to investigate the expression of surfactant proteins in the intes-tines and pancreas of murine fetuses. Methods: Immunostaining for SP-A and SP-D was assessed in the small intestine and pancreas of ICR murine fetuses on days 15, 16, 17 and 18 of gestation (normal duration of pregnancy: 19 - 21 days). RT-PCR was performed to detect the expression of spa and spd mRNA in the small intestine and pancreas on day 16, 17 and 18 of gestation. Results: Immunoreactivity for SP-A and SP-D in the acinar cells of pancreas and intestinal mucosal surface were positive on day 16 of gestation onward. RT-PCR revealed that the expression of spa and spd mRNA was significant in the pancreas but weak in the small intestine. Conclusions: Our data revealed that surfactant proteins are present in the fetal intestines and pancreas and that a significant expression of spa and spd mRNA is detected in the fetal pancreas. Pancreas may be a possible organ involved in the synthesis and secretion of surfactant proteins into the intestinal lumen.

A Case of Male Anorgasmia with Cerebral Infarction

Objective: The deficit of male orgasm is an unusual clinical condition. This is a case report on secondary male anorgasmia with cerebral infarction. Material and Method: The patient was a 55-year-old man who had suffered from male anorgasmia, acquired type. The patient had a normal erectile response, but ejaculated without any sensation of orgasm. The patient underwent an evaluation by a psychiatrist and an urologist trained in sexual medicine. Result: The results of physical, psychological, and laboratory examination were normal. Brain MRI revealed cerebral infarction at bilateral parietal lobes and frontal lobes. Conclusion: There is a possibility that these brain regions are related to male orgasmic dysfunction.

The effect of monoethylphosphate （MEP, commercial available or synthesized） together with IL-2 on the selective proliferation of human γδT cells in Vitro from peripheral blood mononuclear cells （PBMC） of healthy donors and of cancer patients was investigated. The γδT cells were stimulated by MEP to proliferate in a dose-dependent manner. The effect of synthesized MEP was 10 times greater than that of commercial MEP. When the PBMCs of healthy donors were cultured for 25 d in the medium containing different concentrations of MEP, the total cell number increased about 1000-3000 fold; and the ratio of γδT cells reached to 70-80%. The selective expansion of γδT cells depended on the synergic action of MEP and IL-2. The bulk cultured γδT cells exhibited obvious cytotoxic activities against allogenic tumor cell lines （SQ-5,K562 alld Daudi） and autologous tumor cells. The culture system described here not only offers a simple method for obtaining a large number of γδT cells which may become a new effector in the adoptive immunotherapy, but also provides a useful model for the further studies of the structure and function of γδT cells in vitro.

Assessment of a Case of Immediate-Type Allergy against Human Insulin in a Type 2 Diabetic Patient and Allergic Reactions to Human Insulin in Japan

We report a 64-year-old female patient with an insulin allergy. She was treated with a combination of oral antihistamines, topical hydrocortisone cream, and moisturizing agents, which resulted in the improvement of eczema and intense pruritus. To evaluate insulin allergy, intradermal skin tests were performed with several insulin agents for clinical use and 0.9% NaCl. Skin testing with semisynthetic human insulin resulted in local, immediate skin reactions such as itchy erythema and wheals. Furthermore, we analyzed our case and 25 Japanese cases of insulin allergy previously reported in the literature as far as we know. Interestingly, the number of male patients was approximately two times higher than that of female, and the insulin-specific IgE antibody test was positive in 21 patients. We should keep the possibility of human insulin allergy in mind and prepare for it when initiating human insulin therapy.

A Case of Erythema Nodosum Associated with Subareolar Abscess

While erythema nodosum is a nodular, erythematous eruption predominantly affecting the extensor aspects of the legs, breast abscesses are the result of relatively common bacterial infections, and only 2 studies have reported an association between erythema nodosum and breast abscesses. Here, we report the case of a patient with repeated erythema nodosum associated with subareolar abscesses. The patient was a 34-year-old woman with painful erythematous nodules on her right shin, accompanied by an indurated lesion on her right breast 4 days before the onset of the nodules. Therefore, the patient underwent circumareolar incision, and consequently the painful erythematous nodules disappeared. However, after 39 days, the patient developed another tender, painful lesion in her right breast and painful erythematous nodules on her right shin. After another circumareolar incision, the painful erythematous nodules disappeared again. Therefore, we suggested a significant association between erythema nodosum and breast abscess in this patient. The most common underlying causative organism in breast abscess is Staphylococcus aureus;however, erythema nodosum has rarely been proven to be associated with staphylococcal infections. Therefore, the relationship between S. aureus and erythema nodosum is rather controversial. However, the resistance to the usual treatment methods and prolonged clinical course in our case suggest that the pathogenesis of erythema nodosum associated with breast abscesses might be different from that of the common form of erythema nodosum.

应用单克隆抗体１－２２－３（简称单抗），单次静脉注射Ｗｉｓｔａｒ大鼠，观察大鼠尿蛋白的排泄动力学变化，进行肾脏组织的免疫组化染色（ＳＰ染色）、直接免疫荧光、透射电镜分析以及常规ＨＥ、ＰＡＳ染色。结果发现：单次静脉注射单抗后３０分钟和３小时，肾脏有小鼠的ＩｇＧ在肾小球系膜区沉积，但在２４、４８小时时检查未再发现；出现一过性大量蛋白尿，高峰在注射后第６天，最高值为１８４．５ｍｇ／２４ｈ。肾脏病理检查发现注射单抗后２～４天有系膜细胞的溶解，肾小球内细胞总数较正常减少（Ｐ＜０．０５），但仍有系膜细胞增殖，ＰＣＮＡ染色有阳性细胞存在。４～６天，肾小球内细胞总数与正常比较明显增加（Ｐ＜０．０５），而且系膜细胞大量增殖，ＰＣＮＡ（＋）染色阳性细胞增加，与系膜细胞溶解期比较Ｐ＜０．０５。电镜和ＨＥＰＡＳ染色结果与上述一致。提示：用特异性单抗单次静脉注射，可选择性损害系膜细胞，建立稳定的系膜增殖性肾小球肾炎的动物模型。
