背景:药物流产是通过口服药物来终止妊娠。另一种方法是手术流产,利用负压吸引原理,将妊娠物从宫腔内吸出来终止妊娠。流产常用药物是米非司酮联合米索前列醇。只要确认早期怀孕,就可以选择药物流产。实际上,怀孕的时间越短,药物流产的效果越好。口服米索前列醇数小时以内就会出现腹痛和阴道出血,但有时可能需要更长时间。腹痛的程度和出血量可能超过正常月经量,考虑药物流产的女性需要为此做好准备。目的:本研究旨在通过研究妊娠≦56天妇女口服一个疗程的药物来调查分析药物流产的可接受性与可行性,评估药物流产的成功率、安全性和有效性。方法:选入标准身体健康,妊娠天数≦56天,超声确诊为宫内早孕,自愿要求药物流产,如果药物流产失败,愿意接受人工流产,同意签署知情同意书,遵守访问时间表。本研究共选取新乡医学院第三附属医院产科门诊早孕孕妇70人。排除标准:可疑宫外孕,经确诊的慢性肾上腺皮质功能减退,有宫内节育器,慢性肾衰,长期使用糖皮质激素,药物过敏史,哮喘,青光眼,镰刀状细胞贫血,血栓性静脉炎,肝脏疾病,肾脏疾病,呼吸系统疾病,遗传性卟啉症。首诊时病人需要填写筛查表,告知药物服用方法,并将相关表格表1表2填写完整,临床上给入选者应用150mg米非司酮(25mg×6片)和米索前列醇600μg(200μg×3片),服用米非司酮时可以在医院或者在家,服用米非司酮前后空腹2小时,第一天建议患者早晨8点口服米非司酮50mg,12小时后口服25mg米非司酮,第二天,要求患者上午8时口服米非司酮25mg,然后在12小时后重复相同的剂量。第三天,要求患者严格的空腹,并且再次对表2进行填充。要求她们在早晨8点口服米非司酮25mg,1小时后一次性口服米索前列醇600μg(200μg×3片),要求患者在药物流产期间4-5小时之内留在医院观察。告知参与者在口服米索前列醇后会出现恶心呕吐、腹痛及阴道流血等症状,所以在口服米索前列醇期间患者需要有人帮助。如果患者出现大量或持续性出血,必须通知医护人员。评估流产状况后可以出院,并填写表3。流产1周后到门诊随诊,随诊时候填写相关表格4、5、8、9,所有参与者均被要求做超声检查评估流产状况,完全流产的可以离院,不完全流产的或者不确定流产情况的妇女,可以选择再等待一周或进行人工流产。对于选择等待一周的妇女,定期进行延期的随访。接受延期随访的妇女需要填写表4a。在延长随访期间对流产状况进行评估,如果完全流产即研究完成,不全流产者可以从手术回访中找到相关资料,如果流产期间出现严重并发症如大量阴道流血,需急诊行清宫手术。通过入选者口服150mg的米非司酮和600μg的米索前列醇,观察流产情况,从而来评估药物流产的可行性和可接受性。结果:共有70名妇女参与本次研究。研究组的平均年龄为26.57岁,从18岁到42岁不等。58.57%(41/70)的受试者为本科学历,68.57%(48/70)的研究对象是初产妇。研究组的妊娠时间从40天到55天不等,平均为43.47天。此次研究中药物流产有效率高达94.29%(66/70),然而也有5.71%(4/70)的疗效不确切。在这70名调查对象中,其中5名有药物流产史,2/5女性认为本次药物流产较之前药物流产好,2/5认为两次流产没有明显区别,也有1/5妇女说不知道。对于年龄分布是否影响流产满意度,我们研究发现,较先前流产相比,年龄分布对流产满意度有着显著的相关性(p=0.004)。这表明受试者的年龄差异一定程度上影响着她们的满意程度。选择药物流产最常见的原因有:私密性;惧怕手术;更安全;发生并发症的风险较低。药物流产最常见的缺点是腹痛47.14%(33/70),通过研究受试者年龄差异与药物流产过程中腹痛的发生率,发现他们的年龄差异(p=0.043)有统计学意义。这说明药物流产时15-24岁者较25-34岁和≥35岁者相比,更容易出现药物流产的不良症状和体征,这也表明年长者对药物流产有一定的了解。绝大多数女性(91.43%)对药物流产方法满意或非常满意。95.71%(67/70)的女性会再次选择这种方法,94.29%(66/70)的女性会给朋友或亲戚推荐这种方法。本次研究中还没有严重的不良事件,如大出血需要输血,过敏反应,败血症或死亡。结论:妊娠天数≦56天妇女给予口服150mg米非司酮和600μg米索前列醇来终止妊娠是可行的,药物流产成功率为高达94.29%。同时该方案安全有效,因为在本次研究中没有出现严重的不良事件:如大出血需要输血、过敏反应、败血症和死亡。

Surgical Management of Obstructive Urolithiasis in Small Ruminants by Tube Cystostomy in Chittagong, Bangladesh

The present study was conducted to evaluate minimally invasive tube cystostomy technique in goats and calves suffering from obstructive urolithiasis having intact and ruptured urinary bladder. This clinical study was carried out on 84 clinical cases （61 goats and 23 calves） based on the history, clinical signs and physical examination along with common laboratory techniques at the Chittagong Veterinary and Animal Sciences University during January 2015-December 2016. Physical parameters like heart rate, respiratory rate, rectal temperature and dehydration status of animals were noted and corrected abnormality before surgery. Some cases had slightly higher haemoglobin （Hb）, packed cell volume （PCV） and uric acid and epithelial casts in urine. The study revealed that abnormal calcium-phosphorus ratio was predisposing the animals to urolithiasis. It was found that young ruminants （3-5 months） were most commonly affected in both species. All affected animals were male in this study, in which calves were not castrated but in goats 95.08% animals were castrated. Rupture of bladder was more common in calves as compared to goats. Postoperatively all cases were administered with broad-spectrum antibiotic, anti-inflammatory agent and caliculolytic agents like ammonium chloride. Tube cystostomy with Foley’s catheter was the most satisfactory technique for the management of obstructive urolithiasis in small ruminants. Postoperative complications were recorded only in four animals and remaining 80 animals had a normal recovery from tube cystostomy. Tube cystostomy is a simple, inexpensive and very effective procedure for the treatment of urolithiasis in ruminants.

A Hybrid Moth Flame Optimization Algorithm for Global Optimization

The Moth Flame Optimization(MFO)algorithm shows decent performance results compared to other meta-heuristic algorithms for tackling non-linear constrained global optimization ***,it still suffers from obtaining quality solution and slow convergence *** the other hand,the Butterfly Optimization Algorithm(BOA)is a comparatively new algorithm which is gaining its popularity due to its simplicity,but it also suffers from poor exploitation *** this study,a novel hybrid algorithm,h-MFOBOA,is introduced,which integrates BOA with the MFO algorithm to overcome the shortcomings of both the algorithms and at the same time inherit their *** performance evaluation,the proposed h-MFOBOA algorithm is applied on 23 classical benchmark functions with varied *** tested results of the proposed algorithm are compared with some well-known traditional meta-heuristic algorithms as well as MFO *** rank test and Wilcoxon signed rank test are employed to measure the performance of the newly introduced algorithm *** computational complexity has been ***,the proposed algorithm has been applied to solve one constrained and one unconstrained real-life problems to examine its problem-solving capability of both type of *** comparison results of benchmark functions,statistical analysis,real-world problems confirm that the proposed h-MFOBOA algorithm provides superior results compared to the other conventional optimization algorithms.

A Novel Variant of Moth Flame Optimizer for Higher Dimensional Optimization Problems

Moth Flame Optimization(MFO)is a nature-inspired optimization algorithm,based on the principle of navigation technique of moth toward *** to less parameter and easy implementation,MFO is used in various field to solve optimization ***,for the complex higher dimensional problems,MFO is unable to make a good trade-off between global and local *** overcome these drawbacks of MFO,in this work,an enhanced MFO,namely WF-MFO,is introduced to solve higher dimensional optimization *** a more optimal balance between global and local search,the original MFO’s exploration ability is improved by an exploration operator,namely,Weibull flight *** addition,the local optimal solutions have been avoided and the convergence speed has been increased using a Fibonacci search process-based technique that improves the quality of the solutions ***-nine benchmark functions of varying complexity with 1000 and 2000 dimensions have been utilized to verify the projected *** popular algorithms and MFO versions have been compared to the achieved *** addition,the robustness of the proposed WF-MFO method has been evaluated using the Friedman rank test,the Wilcoxon rank test,and convergence *** to other methods,the proposed WF-MFO algorithm provides higher quality solutions and converges more quickly,as shown by the ***,the proposed WF-MFO has been used to the solution of two engineering design issues,with striking *** improved performance of the proposed WF-MFO algorithm for addressing larger dimensional optimization problems is guaranteed by analyses of numerical data,statistical tests,and convergence performance.

Effects of delay in immunological response of HIV infection

In this paper, a human immunodeficiency virus （HIV） infection model with both the types of immune responses, the antibody and the killer cell immune responses has been introduced. The model has been made more logical by including two delays in the acti-vation of both the immune responses, along with the combination drug therapy. The inclusion of both the delayed immune responses provides a greater understanding of long-term dynamics of the disease. The dependence of the stability of the steady states of the model on the reproduction number R0 has been explored through stability theory. Moreover, the global stability analysis of the infection-free steady state and the infected steady state has been proved with respect to R0. The bifurcation analysis of the infected steady state with respect to both delays has been performed. Numerical simulations have been carried out to justify the results proved. This model is capable of explaining the long-term dynamics of HIV infection to a greater extent than that of the existing model as it captures some basic parameters involved in the system such as immunological delay and immune response. Similarly, the model also explains the basic understanding of the disease dynamics as a result of activation of the immune response toward the virus.

mLBOA:A Modified Butterfly Optimization Algorithm with Lagrange Interpolation for Global Optimization

Though the Butterfly Bptimization Algorithm(BOA)has already proved its effectiveness as a robust optimization algorithm,it has certain ***,a new variant of BOA,namely mLBOA,is proposed here to improve its *** proposed algorithm employs a self-adaptive parameter setting,Lagrange interpolation formula,and a new local search strategy embedded with Levy flight search to enhance its searching ability to make a better trade-off between exploration and ***,the fragrance generation scheme of BOA is modified,which leads for exploring the domain effectively for better *** evaluate the performance,it has been applied to solve the IEEE CEC 2017 benchmark *** results have been compared to that of six state-of-the-art algorithms and five BOA ***,various statistical tests,such as the Friedman rank test,Wilcoxon rank test,convergence analysis,and complexity analysis,have been conducted to justify the rank,significance,and complexity of the proposed ***,the mLBOA has been applied to solve three real-world engineering design *** all the analyses,it has been found that the proposed mLBOA is a competitive algorithm compared to other popular state-of-the-art algorithms and BOA variants.

Because of their superior problem-solving ability,nature-inspired optimization algorithms are being regularly used in solving complex real-world optimization *** academics have recently focused on meta-heuristic algorithms to solve various optimization *** the state-of-the-art algorithms,Differential Evolution(DE)is one of the most successful algorithms and is frequently used to solve various industrial *** the previous 2 decades,DE has been heavily modified to improve its *** DE variations secured positions in IEEE CEC competitions,establishing their ***,to our knowledge,there has never been a comparison of performance across various CEC-winning DE versions,which could aid in determining which is the most *** this study,the performance of DE and its eight other IEEE CEC competition-winning variants are ***,the algorithms have evaluated IEEE CEC 2019 and 2020 bound-constrained functions,and the performances have been *** unconstrained problem from IEEE CEC 2011 problem suite and five other constrained mechanical engineering design problems,out of which four issues have been taken from IEEE CEC 2020 non-convex constrained optimization suite,have been solved to compare the *** analyses like Friedman's test and Wilcoxon's test are executed to verify the algorithm’s ability *** analysis exposes that none of the DE variants can solve all the problems *** of SHADE and ELSHADE-SPACMA are considerable among the methods used for comparison to solve such mechanical design problems.

Abiotic stresses including drought,salinity,heat,cold,flooding,and ultraviolet radiation causes crop losses *** recent times,preventing these crop losses and producing more food and feed to meet the demands of ever-increasing human populations have gained unprecedented ***,the proportion of agricultural lands facing multiple abiotic stresses is expected only to rise under a changing global climate fueled by anthropogenic *** the mechanisms developed and deployed by plants to counteract abiotic stresses and maintain their growth and survival under harsh conditions thus holds great *** investigations have shown that phytohormones,including the classical auxins,cytokinins,ethylene,and gibberellins,and newer members including brassinosteroids,jasmonates,and strigolactones may prove to be important metabolic engineering targets for producing abiotic stress-tolerant crop *** this review,we summarize and critically assess the roles that phytohormones play in plant growth and development and abiotic stress tolerance,besides their engineering for conferring abiotic stress tolerance in transgenic *** also describe recent successes in identifying the roles of phytohormones under stressful *** conclude by describing the recent progress and future prospects including limitations and challenges of phytohormone engineering for inducing abiotic stress tolerance in crop plants.
