Post-Traumatic Pyothorax: epidemiology, Management and Prognosis in the Thoracic Surgery Department of the Donka National Hospital

Pyothoraxes, also called thoracic empyemas, are defined by the presence between the two layers of the pleura, of a frankly purulent liquid, or of a shady or clear liquid but containing a majority of altered polynuclear cells with germs direct examination. They are said to be traumatic when they follow a thoracic trauma whatever the mechanism;usually a pre-existing post-traumatic pleural effusion. The general objective of this study was to help identify the epidemiological and prognostic factors in the management of post-traumatic pyothorax in the Thoracic Surgery Department of the Donka National Hospital. Methodology: This was a descriptive retrospective study covering a period of 2.5 years from 01/06/2016 to 31/12/2018 carried out in the Thoracic Surgery Department of the Donka National Hospital. Results: During the study period, 17 cases of post-traumatic pyothorax were observed among the 288 files of admitted and hospitalized patients. In the course of this study, a rate was 5.90%. The most affected age group was 1 to 5 years old. The male sex was in the majority (76.5%). The most common occurrence was the fall, i.e.e="font-family:Verdana;"> 41.2%. 47% of patients consult after 60 days. Chest pain and dyspnea were the main symptoms during our study (82.3%). Antibiotics and analgesics dominated the medical treatment used in 100% of the cases and pleural drainage was the first line surgical treatment practiced in 88.2% of the cases. The prognosis is favorable without sequelae in 52.9% of cases with an average hospital stay

Reproductive stage frost poses a major constraint for wheat production in countries such as ***,little progress has been made in identifying key genes to overcome the *** the present study,a severe frost event hit two large-scale field trials consisting of six doubled haploid(DH)wheat populations at reproductive stage(young microspore stage)in Western Australia,leading to the identification of 30 robust frost QTL on 17 *** major 18 QTL with the phenotype variation over 9.5%were located on 13 chromosomes including 2 A,2 B,2 D,3 A,4 A,4 B,4 D,5 A,5 D,6 D,7 A,7 B and7 *** frost QTL were closely linked to the QTL of anthesis,maturity,Zadok stages as well as linked to anthesis related *** of those,six QTL were repetitively detected on the homologous regions on 2 B,4 B,4 D,5 A,5 D,7 A in more than two *** showed that the frost damage is associated with alleles of Vrn-A1 a,Vrn-D1 a,Rht-B1 b,Rht-D1 b,and the high copy number of ***,anthesis QTL and anthesis related genes of Vrn-B1 a and Ta FT3-1 B on chromosomes 5 B and 1 B did not lead to frost damage,indicating that these early-flowering phenotype related genes are compatible with frost tolerance and thus can be utilised in *** results also indicate that wild-type alleles Rht-B1 a and Rht-D1 a can be used when breeding for frost-tolerant varieties without delaying flowering time.

Family Organization in the Care of Pediatric Heart Transplant Patients:The Genogram as an Investigative Tool

This study aimed to describe structure, functionality and development of pediatric heart transplant families, from the understanding of the experienced chronic condition. A descriptive, exploratory study using a qualitative approach was developed in a hospital situated in a city of Northeast Brazil, in 2012. The study was conducted with three families, using the Calgary Family Assessment Model (CFAM). In addition to the genogram, an analysis of the children’s/adolescents’ family members was conducted at the time of interview. Two families were binuclear families and one was mononuclear. From the evaluation, it was possible to determine the care and support network of children and adolescents with heart transplants, as well as some difficulties faced by the investigated families. Nurses are important in this family context since the visualization of the family genogram can generate a change in the nursing care to each family.

背景：孤立的股骨远端髁冠状面骨折（Hoffa骨折）十分罕见，其诊断困难，在治疗上也存在争议。股骨远端髁上骨折合并冠状面骨折也很少发生。此项研究的目的就是明确股骨远端髁上-髁间骨折合并股骨髁冠状面骨折的机率．并描述m这些损伤的影像学表现。方法：对189例患者的202个股骨远端髁上-髁间骨折进行临床和影像学回顾性评估。结果：在202个股骨远端髁上-髁间骨折中有77个（38．1％）诊断合并有冠状面骨折。在这些冠状面骨折中有59个（76．6％）只累及单髁，18个累及了股骨内和外髁累及单髁的冠状面骨折中有85％位于外髁。股骨远端开放骨折合并冠状面骨折的可能性是闭合骨折患者的2．8倍（95％可信区间，1．54～5．25）。在102个进行CT检查的膝关节中有47％被诊断出冠状面骨折，与其相比，在100个未行CT检查的膝关节中，其诊出率只有29％（p=0．008）。术前未被诊断出的10个冠状面骨折只是在术中固定股骨远端骨折时才明确诊断，且这些病例术前均未行过CT检查。结论：冠状面骨折经常与高能的股骨远端髁上-髁间骨折合并发生；在本研究中，合并冠状面骨折的发生率为38％。外髁较内髁更易受累。累及双髁的冠状面骨折也很常见，并且大多数冠状面骨折伴有开放伤口。加上了冠状面骨折的诊断使股骨远端髁上-髁间骨折的外科治疗的策略和方法发生改变，因此，术前对股骨远端损伤者，特别当合并有开放损伤时，竭力推荐进行CT扫描。

Role of oxidative stress in endothelial insulin resistance

The International Diabetes Federation estimates that 316million people are currently affected by impaired glucose tolerance(IGT).Most importantly,recent forecasts anticipate a dramatic IGT increase with more that 470million people affected by the year *** insulin sensitivity is major feature of obesity and diabetes and is strongly linked with adverse cardiometabolic ***,the etiologic pathway linking impaired glucose tolerance and cardiovascular disease remains to be *** insulin resistance has been attributed to inflammatory programs starting in adipose tissue,emerging evidence indicates that endothelial dysfunction may represent the upstream event preceding peripheral impairment of insulin ***,suppression of reactive oxygen species-dependent pathways in the endothelium has shown to restore insulin delivery to peripheral organs by preserving nitric oxide(NO)*** we describe emerging theories concerning endothelial insulin resistance,with particular emphasis on the role oxidative *** molecular circuits including endothelial nitric oxide synthase,prostacyclin synthase,mitochondrial adaptor p66^(Shc),nicotinamide adenine dinucleotide phosphate-oxidase oxidase and nuclear factor kappa-B are ***,the review provides insights on the effectiveness of available compounds(i.e.,ruboxistaurin,sildenafil,endothelin receptor antagonists,NO donors)in restoring endothelial insulin *** together,these aspects may significantly contribute to design novel therapeutic approaches to restore glucose homeostasis in patients with obesity and diabetes.

Obesity and cardiometabolic disease risk factors among US adolescents with disabilities

AIM: To generate prevalence estimates of weight status and cardiometabolic disease risk factors among adolescents with and without ***: Analysis of the 1999-2010 National Health and Nutrition examination Survey data was conducted among 12-18 years old with(n = 256) and without disabilities(n = 5020). Mean values of waist circumference, fasting glucose, high-density-lipoprotein cholesterol, triglycerides, systolic and diastolic blood pressure and metabolic syndrome(Met S, ≥ 3 risk factors present) were examined by the following standardized body mass index(BMI) categories for those with and without disabilities; overweight(BMI ≥ 85th- ercentile for age and sex), obesity(BMI ≥ 95 th percentile) and severe obesity(BMI ≥35 kg/m2). Linear regression models were fit with each cardiometabolic disease risk factor independently as continuous outcomes to show relationships with disability status. ReSULTS: Adolescents with disabilities were significantlymore likely to be overweight(49.3%), obese(27.6%) and severely obese(12%) vs their peers without disabilities(33.1%, 17.5% and 3.6%, respectively, P ≤ 0.01 for all). A higher proportion of overweight, obese and severely obese children with disabilities had abnormal SBP, fasting lipids and glucose as well as Met S(18.9% of overweight, 32.3% of obese, 55% of severely obese) vs their peers without disabilities(9.7%, 16.8%, 36.3%, respectively). US adolescents with disabilities are over three times as likely to have Met S(OR = 3.45, 95%CI: 1.08-10.99, P = 0.03) vs their peers with no ***: Results show that adolescents with disabilities are disproportionately affected by obesity and poor cardiometabolic health vs their peers with no disabilities. Health care professionals should monitor the cardiometabolic health of adolescents with disabilities.

Cardiometabolic health among gastric bypass surgery patients with polycystic ovarian syndrome

AIM:To examine the effect of gastric bypass surgery on cardiometabolic health among women with polycystic ovarian syndrome(PCOS).MeTHODS:Retrospective medical chart review identified women(n = 389) with PCOS who underwent Roux-en-Y gastric bypass surgery from 2001-2009 in one surgical *** repeated measures linear mixed models were fit using the MIXeD procedureto assess mean change in cardiometabolic disease risk factors from before to 1-year after surgery and were evaluated by ethnicity [Hispanic,non-Hispanic black(NHB) and white(NHW)].ReSULTS:The majority of the sample was Hispanic(66%,25% NHB,9% NHW).Mean body mass index significantly improved 1 year post-surgery for all ethnic groups(45.5 to 35.5 kg/m2 for Hispanics,46.8 to 37.7 kg/m2 for NHB and 45.7 to 36.7 kg/m2 for NHW,P en mean total cholesterol(198.1 to 160.2 mg/dL),low-density lipoproteins(LDL) cholesterol(120.9 to 91.0 mg/dL),triglycerides(148.6 to 104.8 mg/dL),hemoglobin A1c(6.2% to 5.6%),alanine aminotransferase(28.1 to 23.0 U/L) and aspartate aminotransferase(23.5 to 21.6 U/L) decreased significantly(P ean total cholesterol(184.5 to 154.7 mg/dL),LDL cholesterol(111.7 to 88.9 mg/dL) and triglycerides(99.7 to 70.0 mg/dL) decreased significantly(P ean total cholesterol(200.9 to 172.8 mg/dL) and LDL cholesterol(124.2 to 96.6 mg/dL),decreased significantly(P e ethnic group comparisons of all cardiometabolic outcomes adjusted for age and type of surgery before and 1 year after surgery showed no statistical difference between the three groups for any ***:Cardiometabolic disease risk improvements vary by ethnicity and obesity may impact glucose tolerance and liver function changes more in Hispanic women with PCOS vs non-Hispanic women.
