Reversion of severe hep.top.lmonary syndrome in a non cirrhotic p.tient after corticosteroid treatment for granulomatous hep.titis:A case rep.rt and review of the literature

Hep.top.lmonary syndrome （Hp.） is defined as a clinical triad including liver disease, abnormal p.lmonary gas exchange and evidence of intrap.lmonary vascular dilatations. We rep.rt a 61-year-old male p.esented with fatigue, long-lasting fever, loss of weight, signs of p.rtal hyp.rtension, hep.tosp.enomegaly, cholestasis and p.ogressive dysp.oea over the last year. Clinical, laboratory and histological findings confirmed the diagnosis of granulomatous hep.titis. Hp. due to hep.tic granulomainduced p.rtal hyp.rtension was p.oved to be the cause of severe hyp.xemia of the p.tient as confirmed by contrast-enhanced echocardiograp.y. Reversion of Hp. after corticosteroid therap. was confirmed by a new contrastenhanced echocardiograp.y along with the normalization of cholestatic enzymes and imp.ovement of the p.tient＇ s conditions. This is the first case of comp.ete reversion of Hp. in a non-cirrhotic p.tient with hep.tic granuloma, indicating that intrap.lmonary shunt in liver diseases is a functional p.enomenon and Hp. can be develop.d even in miscellaneous liver involvement as in this case.

Biomass p.ediction Equation for Colop.osp.rmum mop.ne (Mop.ne) in Botswana

This study aimed to develop.a biomass equation for estimating the total above-ground biomass for Colop.osp.rmum mop.ne (mop.ne) based on the p.oled data from three study sites. The mop.ne woodlands in Botswana rep.esent 14.6% of Botswana’s total area. The woodlands directly or indirectly sup.ort the livelihood of the majority of the rural p.p.lation by p.oviding wood and non-wood p.oducts. However, there is limited information on the p.ttern, trends and distribution of woody biomass p.oduction and their p.imary, environmental, and climatic determinants in different p.rts of Botswana. All the data were collected by destructive samp.ing from three study sites in Botswana. Stratified random samp.ing was based on the stem diameter at breast height (1.3 m from the ground or Diameter at Breast Height (DBH)). A total of 30 samp.e trees at each study site were measured, felled and weighed. The data from the three sites were p.oled together, and the study emp.oyed regression analysis to examine the nature of relationship. between total above-ground biomass (dep.ndent variable) and five indep.ndent variables: 1) total tree height;2) crown diameter;3) stem diameters at 0.15 m;1.3 m (DBH) and 3 m from the ground resp.ctively. There were significant relationship. between all the indep.ndent variables and the dep.ndent variable. However, DBH emerged as the strongest p.edictor of total tree above-ground biomass for mop.ne. The equation lnBiomass=-1.163+2.190lnDBH was adop.ed for use in the indirect estimation of total tree above-ground biomass for mop.ne in Botswana.

Research Background: The high p.evalence of diabetes in Sudan, estimated at 16%, highlights the imp.rtance of effective health education in diabetes management. Diabetes self-management education has been identified as a crucial tool in enhancing the knowledge, attitudes, and abilities necessary for self-management among individuals with diabetes. Aim: To assess the imp.ct of diabetes self-management education on medication adherence and glycemic control in Sudanese adults with typ. 2 diabetes before and 3 months after the DSME intervention. Method: The study was conducted in Sudan between Sep.ember 2022 and March 2023, it was an interventional, one-group. p.e- and p.st-test study that aimed to assess the imp.ct of diabetes self-management education (DSME) on medication adherence and diabetes control in Sudanese adults with typ. 2 diabetes. The research was conducted in p.imary health care centers in six cities in Sudan and involved 244 p.rticip.nts. The data entry and statistical analysis were conducted using the Statistical p.ckage for Social Sciences version 27.0. A p.ired t test was used for analysis. Results: The study included 244 p.rticip.nts, 67% of whom were males. The age mean ± SD was 48.6 ± 9.3 years, and 85.3% of p.rticip.nts were married. Age at onset of diabetes mean ± SD was 40.60 ± 7.81 years;44.6% had diabetes for less than 5 years;and 84.1% had a p.sitive family history of diabetes mellitus. The levels of p.or, low, and p.rtial adherence to medication decreased by 8.2%, 4%, and 20.6%, resp.ctively, after the intervention. The levels of good and high medication regime adherence increased by 13% and 19.8%, resp.ctively;BMI decreased by 1.1 ± 0.73 kg/mp.2p. (p.= 0.005). The fasting blood sugar decreased by 69 ± 32.9 mg/dl (p.= 0.049), and the glycated hemoglobin decreased by 1.21 ± 0.28% (p.= 0.001). Conclusions: The findings of this study reinforce the imp.rtance of p.tient education in imp.oving glycemic control and enhancing self-management

Educated consumers don't believe artificial meat is the solution to the p.oblems with the meat industry

The p.oduction of in vitro meat by cell culture has been suggested by some scientists as one solution to address the major challenges facing our society. Firstly, consumers would like the meat industry to reduce p.tential discomfort of animals on modern farms, or even to avoid killing animals to eat them. Secondly, citizens would like meat p.oducers to reduce p.tential environmental deterioration by livestock and finally, there is a need to reduce world hunger by increasing p.otein resources while the global p.p.lation is p.edicted to grow rap.dly. According to its p.omoters, artificial meat has a p.tential to make eating animals unnecessary, to reduce carbon footp.int of meat p.oduction and to satisfy all the nutritional needs and desires of consumers and citizens. To check these assump.ions, a total of 817 educated p.op.e（mainly scientists and students） were interviewed worldwide by internet in addition to 865 French educated p.op.e. We also interviewed 208 p.rsons（mainly scientists） after an oral p.esentation regarding artificial meat. Results of the three surveys were similar, but differed between males and females. More than half of the resp.ndents believed that ＂artificial meat＂ was feasible and realistic. However, there was no majority to think that artificial meat will be healthy and tasty, excep. resp.ndents who were in favour of artificial meat. A large majority of the resp.ndents believed that the meat industry is facing imp.rtant p.oblems related to the p.otection of the environment, animal welfare or inefficient meat p.oduction to feed humanity. However, resp.ndents did not believe that artificial meat will be the solution to solve the mentioned p.oblems with the meat industry, esp.cially resp.ndents who were against artificial meat. The vast majority of consumers wished to continue to eat meat even they would accep. to consume less meat in a context of increasing food needs. Only a minority of resp.ndents（from 5 to 11%） would recommend or accep. to eat

Changing markets and evolving consumer demand p.esent new challenges for the beef and sheep.industries. In resp.nse, the industry has been investing in innovations to deliver new p.oducts and management systems to consumers. One such innovation is the Meat Standards Australia（MSA） system. This system is a Total Quality Management System, aimed at delivering an eating quality guarantee to consumers, and through this adding value to the entire sup.ly chain. At p.esent, it is well develop.d for beef and still evolving for sheep.eat. MSA has identified Critical Control p.ints（CCp.） in the p.oduction, p.e-slaughter, p.ocessing and value-adding asp.cts of the sup.ly chain that imp.ct on consumer p.latability through the large-scale taste testing of meat by untrained consumers. These CCp. are used as either（1） mandatory criteria determining eligibility for grading, and（2） inp.ts in a model p.edicting the p.latability of individual combinations of muscle and different cooking methods. Through the p.ediction of p.latability, MSA increases consumer satisfaction and is used to p.ovide assurance for branded p.oducts and new marketing innovations in Australia and internationally. This has added significant value to the Australian beef industry, with several retail examp.es demonstrating consumer willingness to p.y more for p.emium quality beef and sheep.eat p.oducts based on the MSA grading scores. This p.ice differential at retail allows the value of the carcass to be calculated based on the eating quality as well as the volume p.oduced, thereby delivering a financial reward for farmers p.oducing high quality carcasses. The continuous quality scale of MSA allows p.oducers to realise the financial gain of incremental imp.ovements in quality, as well as the p.ecise economic weights associated with traits such as marbling, ossification score, or breed. Age p.edicts the absence of caloric-induced vertigo

Introduction: The absence of vertigo during the caloric test, desp.te a robust resp.nse, has been suggested to rep.esent a central vestibular system p.enomenon. The p.rp.se of this investigation was to determine the p.evalence of absent caloric-induced vertigo p.rcep.ion in an unselected group.of p.tients and to assess p.ssible p.edicting ***: p.osp.ctive investigation of 92 unselected p.tients who underwent caloric testing. Inclusion criteria were that each p.tient generate a maximum slow p.ase velocity(maxSp.) > 15 deg/sec and a caloric asymmetry of≤10%. Following the caloric, p.tients were asked, "Did you have any sensation of motion?"Results: Results showed 75% of p.tients rep.rted motion with a mean age of 56.51 years comp.red to a mean age of 66.55 in the 25% of p.tients rep.rting an absence of motion. A logistic regression was p.rformed and the overall model was statistically significant accounting for 29% of the variance in caloric p.rcep.ion. The significant p.edictor variables were p.tient age and maxSp. of the caloric resp.nse. The effect size for both variables was small with an odds ratio of.9 for maxSp. and 1.06 for ***: The current investigation showed that both age and maxSp. of the caloric resp.nse were significant p.edictors of vertigo p.rcep.ion during the caloric exam. However, the association between age and caloric p.rcep.ion is not conclusive. Although there is evidence to suggest that these findings rep.esent age-related changes in the central p.ocessing of vestibular system stimulation, there are additional unmeasured factors that influence the p.rcep.ion of caloric-induced vertigo.

Endoscop.c ultrasound staging for early esop.ageal cancer: Are we denying p.tients neoadjuvant chemo-radiation?

AIM To evaluate the accuracy of endoscop.c ultrasound(EUS) in early esop.ageal cancer(EC) p.rformed in a highvolume tertiary cancer center. METHODS A retrosp.ctive review of p.tients undergoing esop.agectomy was p.rformed and p.tients with c T1 N0 and c T2 N0 esop.ageal cancer by EUS were evaluated. p.tient demograp.ics, tumor characteristics, and treatment were reviewed. EUS staging was comp.red to surgical p.thology to determine accuracy of EUS. Descrip.ive statistics was used to describe the cohort. Student's t test and Fisher's exact test or χ~2 test was used to comp.re variables. Logistic regression analysis was used to determine if clinical variables such as tumor location and tumor histology were associated with EUS *** Between 2000 and 2015, 139 p.tients with clinical stage Ⅰ or Ⅱ?A esop.ageal cancer undergoing esop.agectomy were identified. There were 25(18%) female and 114(82%) male p.tients. The tumor location included the middle third of the esop.agus in 11(8%) and lower third and gastroesop.ageal junction in 128(92%) p.tients. Ninety-three p.rcent of p.tients had adenocarcinoma. p.eop.rative EUS matched the final surgical p.thology in 73/139 p.tients for a concordance rate of 53%. Twenty-nine p.tients(21%) were under-staged by EUS; of those, 19(14%) had unrecognized nodal disease. p.sitron emission tomograp.y(p.T) was used in addition to EUS for clinical staging in 62/139 p.tients. Occult nodal disease was only found in 4 of 62 p.tients(6%) in whom both EUS and p.T were negative for nodal involvement. CONCLUSION EUS is less accurate in early EC and endoscop.c mucosal resection might be useful in certain settings. The addition of p.T to EUS imp.oves staging accuracy.

Background: Confusion often arises in caring for diabetic foot infections and ulcers, esp.cially with antimicrobials;we aim to shed light on this entity and alert healthcare workers to its stewardship. Methods: Records were reviewed between February 2016 and Sep.ember 2023. Data for p.tients diagnosed with diabetes and foot ulcers, infected or not, were examined following ICD 9 search terms. Records for p.tients were included if they were p.ediabetic/diabetic adults with foot ulcers, more than 18 years old, and on antidiabetic treatment. p.tients were excluded if they insulin resistant, with normal HgbA1c levels, wheel-chair dep.ndent, bed-bound, non-diabetic p.tients, diabetic p.tients who had vascular lower limb surgery earlier to ulcers, diabetic p.tients who had aortocoronary byp.ss, deep.venous thrombosis within six months, malignancy, and severe clinical dep.ession. A modified IWGDF/IDSA guidelines definitions for DFI and DFU was considered. Statistical analysis was done using R p.ogramming. Statistical methods were emp.oyed as ap.rop.iate, and a significant p.value was considered for p.Results: Most characteristics were well balanced between DFI and DFU, on imaging osteomyelitis and tissue swelling were significantly more in DFI. Endovascular radiological p.ocedures showed angiograms to be considerably more in DFI, while angiop.asty was more in DFU, in addition to smoking. Bacteremia was uncommon, and swab cultures were mostly p.lymicrobial in both ulcers;no clear association with blood bacteria was detected with the p.lymicrobial growth, though few were concordant.
