Psychotherapy: A Way Forward to Improve the Quality of Life in Otalgia Patients

Introduction: Otalgia is not only an indicator of impact of disease but also impairs the daily activities and personal behaviors as is unpleasant and disturbing for the patient. It leads to decrease in functional status up to the complete disability. Nowadays the main focus of physician is to eradicate the disease or the symptoms, leaving behind the humanistic element. In diseased state, the most important point is to assess and improve the quality of life by using multidisciplinary approach. Objective: The objective is to assess the quality of life in patients having otalgia, due to any cause. Methodology: A descriptive cross sectional study was conducted by using modified form of WHOQOL-100 and COQOL. Data was accepted, if CFI was greater than 0.9. Qualitative data were presented as frequency and percentages and quantitative data as mean with standard deviation. ANOVA was used to assess the variations among groups. Change in behavior was assessed by using the Likert scale for each domain of WHOQOL and COQOL by a ten-point scale. Results: Majority of participants about 60.2% were male and were from the age group of 16 - 30 years. Most of the sample population were from lower socioeconomic class. Looking over the WHOQOL-100, higher score represented the higher quality of life. Variation among different socioeconomic status was non-significant for positive attitudes but was highly significant (p = 0.0020) for negative attitudes. Analyzing the COQOL scoring system, higher score reported the worse symptoms. The patient with high COQOL showed strong correlation with poor WHOQOL-100 score that was p value Conclusion: The current study highlights the need of multidisciplinary approach for the better management and improving the quality of life in patients with otalgia.

Ghost Haemoglobin Affecting the Efficacy of Phototherapy

Introduction: Phototherapy is the treatment of choice for neonatal hyperbilirubinemia. It converts the unconjugated bilirubin from polar and neurotoxic Z-Z-bilirubin to the more polar photobilirubin. It has been hypothesized that high level of hemoglobin or hematocrit interferes in the effectiveness of phototherapy. The objective of the current study is to find out the association of hemoglobin/hematocrit to change in total serum bilirubin concentration during phototherapy. Methods: A prospective cohort study was conducted on 296 neonates with uncomplicated hyperbilirubinemia. Before initiating the phototherapy, hemoglobin, hematocrit and total serum bilirubin levels were measured. After treating the neonates with phototherapy using LED-light for 24 hours, the total serum bilirubin level was measured again. The data were analyzed by using SPSS version-20. Results: The majority of study participants were male (60.8%), with mean postnatal age of 4.66 ±1.65, having mean birth weight of 2.41 ±0.41. Considering Pearson correlation, the hemoglobin had a significant inverse association with ΔTsB0-24 while birth weight and postnatal age also had inverse association but p-values were non-significant, while TsB0 had presented a very weak but significant positive association. The results might be evident of the fact that increase in hemoglobin concentration results in minimal change in total serum bilirubin concentration during phototherapy. Conclusion: Current study found a significant effect of hemoglobin on efficacy of phototherapy, as the change in total serum bilirubin concentration is inversely correlated with the level of hemoglobin. This is important for the physician in treating neonates with hyperbilirubinemia by using phototherapy.

Accidental Diagnosis of Infections among Blood Donors

ntroduction: Due to widespread incidence and prevalence, human immunodeficiency virus (HIV), hepatitis B (HBV) and C (HCV), syphilis and malaria are the most common viral infections transmitted via blood transfusions. Yet there is insufficient information available about the exact prevalence of these infections among blood donors in Pakistan. The objectives of current study are to highlight the importance of blood screening among blood donors by finding the prevalence of high-risk transmissible diseases such as hepatitis B, hepatitis C, HIV, syphilis and malaria in the population of Nawabshah (Sindh) and to compare it with other provinces. Methods: A descriptive type of cross-sectional study was conducted on 37,845 blood donor volunteers at the blood bank of a tertiary care hospital, Peoples University of Medical and Health Sciences Hospital, in Nawabshah Sindh from 1st January 2018 to 31st December 2018. The data was taken from the well-maintained yearly record of the hospital blood bank. Analysis of all the data regarding blood donors was done using Statistical Program of Special Sciences (SPSS) version 20. Results: The most common age group was between 18 - 40 years. Donors were predominantly male (90%). HBV is the most prevalent disease amongst the blood donors of Peoples Medical College with a prevalence of 6.52% while HIV, HCV and malaria were 6.33%, 5.83% and 1.24% respectively whereas Syphilis is the least prevalent disease with a prevalence of 1.11%. Comparing with other provinces, the highest prevalence of hepatitis B was seen in Peoples Medical College, Nawabshah, Sindh whereas for hepatitis C it was the highest in Isra University Hospital, Hyderabad, Sindh. Moreover, Hayatabad Medical Complex in Peshawar was the least burdened with Hepatitis B (1.46%) and C (1.34%). Conclusion: Accidental diagnosis of blood-borne infections including Hepatitis B, C, HIV, Syphilis and malaria highlights importance of blood screening among the blood donors. There is a need

Frequency of Neurological Disorders and Related EEG Finding in a Tertiary Care Hospital of Karachi

Background: Electroencephalogram (EEG) is defined as a method of detecting brain waves signifying the electrical conductivity of the brain. Globally, EEG is used to further classify neuropsychiatric disorders. Objective: To evaluate the prevalence of abnormal EEGs and related neurological disorders and it’s correlation with age and gender. Methodology: A descriptive cross sectional study was conducted in Neurology department, Ziauddin hospital, Karachi, Pakistan from June 2018 to May 2019. A total of 440 individuals based on inclusion criteria were selected through Non-probability consecutive sampling. Informed consent was obtained from study participants. A self-designed structured questionnaire and EEG record were used for data collection. Data were entered and analyzed using SPSS v 20. Mean and standard deviation was calculated for numerical variable and frequency and percentages were calculated for categorical variable. Independent t-test and chi square was used to check association of abnormal EEG with age and gender. P value of ≤0.05 was considered statistically significant. Results: The mean age of study participants was 52.98 ± 22.68 years. There were 54.5% (n = 240) males. Approximately 45.2% (n = 199) participants had predisposing condition which can lead to abnormal EEG. EEG records of 39.8% (n = 175) of the patients was normal while 60.2% (n = 265) had abnormal records. Diffuse neuronal dysfunction consistent with encephalopathy was found in 45.2%. Mild neuronal dysfunction consistent with encephalopathy accounted for 33.5% of cases. Significant difference was found in Abnormal EEG among different age group (p = 0.01). Chi square shows an association between abnormal EEG and male gender (p = 0.025). Conclusion: EEG plays a vital role in the diagnosis of neuropsychiatric conditions in developing countries. EEGs can further help to determine the relationship of different neuropsychiatric conditions and can help in early diagnosis and better prognosis.

Nonlinear aspects of energy dissipation in wood-panel joints sara Casciati

The joints connecting vertical and horizontal elements are the ＂weak link＂ in structural systems assembled from wood panels. If they are too weak, local failures may occur, resulting in performance that is significantly below expectations. If they are too resistant, the joints may be unable to dissipate energy during vibrations, thus possibly initiating a fast progressive failure. This paper re-processes and re-elaborates the results of shaking table tests previously carried out by the author and other co-workers. The goal is to assess the feasibility of a joint which is able to dissipate energy during vibration, without degrading the connection performance.

Automatic characterization of spherical metal powders by microscope image analysis:a parallel computing approach

The microstructural characteristics of spherical metal powders play an important role in determining the quality of mechanical parts manufactured by powder metallurgy *** the individual powder particles from their microscopic images is one of the most convenient and cost-efficient methods for the analysis of powder *** numerous image processing algorithms have been developed for automating the powder particle identification process,they perform less accurately in identifying adjacent particles that are heavily overlapped in their image *** propose an automatic algorithm to robustly and accurately identify spherical powder particles,especially heavily overlapped particles,from their microscope images.A parallel computing framework is designed to further enhance the computational efficiency of the proposed *** characteristics such as particle size distribution and the location of potential satellite particles have been derived from our identification *** accuracy and efficiency of our algorithm are validated by real-world scanning electron microscope images,outperforming other existing methods and achieving both precision and recall above 99%.

Geological Controls and Prospectivity Mapping for Manganese Ore Deposits Using Predictive Modeling Comparison:An Integration of Outcrop and Remote Sensing Data,Sinai Microplate,Egypt

One of the most controversial minerals in their origin and occurrence around the world is manganese *** Abu Zenima area is rated one of the most economically important places where manganese ore deposits(Mn ODs)are located in the southwest Sinai microplate,*** deposits are confined with the Um Bogma Formation(UBF)and the reserves of this region are relatively *** this study,optical and radar data are used in a new challenge as an attempt to reach the closest controls and setting of Mn ***,Frequency Ratio(FR)and Logistic Regression(LogR)predictive models are applied to integrate different geospatial thematic maps,to predict new potential resource zones for increasing the ranges of mining ***8 OLI,Sentinel-2A Multi Spectral Instrument and Radar(Sentinel-1B)data are combined for mapping Mn ODs locations and their relationship with geological structures and the surrounding *** ratio,Principal and Independent Component Analysis techniques and four classification algorithms were implemented to the optical’VNIR and SWIR ***,the interferometric processing steps for Sentinel-1 data were made for understanding the tectonic features in the *** FR and LogR models are validated during fieldwork with known Mn ODs *** indicate that processed images are capable of differentiation of UBF which broadly distributed in the central and southern parts of the *** ODs possibly formed by thermal events that attributed to paleo-volcanic events before the rift *** high accuracy of LogR model(0.902)suggests that high Mn ODs potential zones are identified within the intersected fault zones near granitic *** integration is recommended for discriminating hydrothermally Mn ODs in other arid geographic regions.

A Voronoi diagram approach for detecting defects in 3D printed fiber-reinforced polymers from microscope images
