Assessing the Variability of Extreme Weather Events and Its Influence on Marine Accidents along the Northern Coast of Tanzania

The marine accidents are among the main components of the Zanzibar Disaster Management Policy (2011) and the Zanzibar Blue Economy Policy (2020). These policies aimed to institute legal frame works and procedures for reducing both the frequency of marine accidents and their associated fatalities. These fatalities include deaths, permanent disabilities and loss of properties which may result into increased poverty levels as per the sustainable development goal one (SDG1) which stipulates on ending the poverty in all its forms everywhere. Thus, in the way to support these Government efforts, the influence of climate and weather on marine accidents along Zanzibar and Pemba Channels was investigated. The study used the 10 years (2013-2022) records of daily rainfall and hourly wind speed acquired from Tanzania Meteorological Authority (TMA) (for the observation stations of Zanzibar, Pemba, Dares Salaam and Tanga), and the significant wave heights data, which was freely downloaded from Globally Forecasting System (GFS-World model of 13 km resolution). The marine accident records were collected from TASAC and Zanzibar Maritime Authority (ZMA), and the anecdotal information was collected from heads of quay and boat captains in different areas of Zanzibar. The Mann Kendal test, was used to determine the slopes and trends direction of used weather parameters, while the Pearson correlations analysis and t-tests were used to understand the significance of the underlying relationship between the weather and marine accidents. The paired t-test was used to evaluate the extent to which weather parameters affect the marine accidents. results revealed that the variability of extreme weather events (rainfall, ocean waves and wind speed) was seen to be among the key factors for most of the recorded marine accidents. For instance, in Pemba high rainfall showed an increasing trend of extreme rainfall events, while Zanzibar has shown a decreasing trend of these events. As for extreme wind

Peritoneo-Vaginal Leaks in Peritoneal Dialysis: A report of 3 Cases

Introduction: Peritoneal dialysis (PD) is an extra-renal purification technique indicated for patients with end-stage kidney disease (ESKD). Although it has many advantages, it also has a number of complications, such as peritoneo-vaginal leakage of dialysate. Cases reports: Three male patients, aged 70, 50 and 77 respectively, with ESKD were placed on PD. All three patients presented with inadequate drainage and the progressive appearance of bilateral hydrocele, which occurred one year (late leakage), two and four days (early leakage) respectively after the initiation of exchanges. The etiological investigation led to the conclusion of a peritoneo-vaginal leak associated with an inguinal hernia in only one case. The two cases of early leakage were treated by temporary suspension of PD, with a good response in one case and failure in the second, leading to hisdefinitive transfer to hemodialysis (HD);however, the hernia repair enabled immediate resumption of PD in the third patient. Discussion and Conclusion: Peritoneo-vaginal leakage is a rare mechanical complication in PD. The clinical examination plays an essential role in confirming the diagnosis and in the therapeutic decision. If the diagnosis is uncertain, or if there is a clinical need to show the anatomy of the leak, an imaging approach becomes desirable. This complication should not prevent the progression of PD in the management of patients with ESKD.

目的哮喘-慢性阻塞性肺疾病肺重叠症(Asthma-chronie obst ructive pulmonary disease overlap,A-CO)患者的生活质量较低,肺功能较差,病情经常加重。由于暴露于污染环境会加重其症状,增加发病率和病死率,环境管理对其至关重要。旨在通过系统性综述,寻找证据,以指导制订护理干预措施,促进ACO患者对空气质量的自我管理。方法2023年3月 ,系统检索CINAHL、Medline、Academie Search Complete .Scopus、WebofScience、Cochrane图书馆、乔安娜布里格斯研究所(JoannaBriggsInstitute,JBI)循证实践数据库,以及GoogleScholar。对纳人研究进行数据提取和综合,并对护理干预措施进行分类。采用JBI研究质量评估工具进行方法学质量评估。结果共纳入5篇来自不同国家的研究。促进ACO患者自我监控空气质量的干预措施包括:警觉性干预措施(医务人员定期巡诊、进行症状评估),监测干预措施(监测室内外诱发因素) ,教育干预措施(空气质量警报、避免接触过敏原),以及相关政策的影响,如无烟政策和改善空气质量的综合战略。尽管这些研究报告了不同的干预措施,方法质量也存在差异,但这些干预措施显示了预防患者病情恶化、尽量减少急诊就诊和住院治疗的潜力。这些关键点可作为护理干预措施的关键组成部分。结论有必要采取全面措施,让护士参与促进患者自我监控空气质量的多学科团队.提高认识、制订和实施护理干预,从而使ACO患者受益。同时,需要进--步开展研究,以扩大证据基础并完善这些干预措施。

Percutaneous nephrostomy versus ureteral stent in hydronephrosis secondary to obstructive urolithiasis:A systematic review and meta-analysis

Objective: To assess if there is a preferable intervention between retrograde ureteral stent (rUS) and percutaneous nephrostomy (PCN) tube, in cases of upper urinary tract stone obstruction with complications requiring urgent drainage, by evaluating outcomes regarding urinary symptoms, quality of life (QoL), spontaneous stone passage, and length of hospital stays, since there is no literature stating the superiority of one modality over the ***: We searched MEDLINE and other sources for relevant articles in June 2019 without any date restrictions or filters applied. The selection was done first by the title and abstract screening and then by full-text assessment for eligibility. Only randomized controlled trials or cohort studies in patients with hydronephrosis secondary to obstructive urolithiasis that presented comparative data between PCN and rUS placement concerning at least one of the defined outcome measures were included. Lastly, MEDLINE database and PubMed platform were screened again using the same terms, from June 2019 until November ***: Of 556 initial articles, seven were included in this review. Most works were considered of moderate-to-high quality. Three studies regarding QoL showed a tendency against stenting, even though only one demonstrated statistically significant negative impact on overall health state. Two works reported significantly more post-intervention urinary symptoms in stenting patients. One article found that PCN is a significant predictor of spontaneous stone passage, when adjusted for stone size and location. Findings on length of hospital stays were not consistent among ***: PCN appears to be the intervention better tolerated, with less impact on the patient’s perceived QoL and less post-operative urinary symptoms, in comparison with rUS. Nevertheless, further studies with larger samples and a randomized controlled design are suggested.

Seroprevalence Survey of HIV and Hepatitis B Virus and Behavioral Characteristics among Heavy Truck Drivers along Port Sudan-Khartoum Highways

The prevalence of human immunodeficiency virus (AIDS) and hepatitis B virus among heavy truck drivers and their assistants has been well documented globally in correlation with their behavioral characteristics. The present study aimed to screen for human immunodeficiency virus (HIV), hepatitis B virus (HBV), and behavioral characteristics among heavy truck drivers in Port Sudan. A cross-sectional study was conducted on 274 heavy truck drivers and their assistants who used the highway Port Sudan-Khartoum in Port Sudan city during 2019-2021. Data on behavioral characteristics and substance use habits were collected using a structured questionnaire, and an ELISA test was used to screen for HIV and HBV infections in the study participants. The chi-square test, odds ratio, and confidence intervals were used to find the association between behavioral characteristics and seropositive HIV/HBV. Of the 274 enrolled participants, the seroprevalence rates of HIV were 2.7% and HBV was 23.7%. Ninety-four (34.3%) of them had a history of high-risk sexual behavior outside of marriage;only two (0.7%) used condoms;14.2% of participants reported alcohol use;and 1.1% reported drug use. Univariate analysis revealed that having a sex history outside of marriage with ≥1 sex partner and never using a condom with a spouse or casual partner were significant risk factors for HIV and HBV among drivers. Fortunately, we found that most of the drivers reported low alcohol and drug use. Concerning this study, the seroprevalence of HIV and HBV is highly associated with a history of having sex outside of marriage and sexual behavior among truck drivers and assistances. Additional studies are needed to further investigate other STIs and behavioral characteristics associated with factors in truck drivers/assistance in different truck stop regions in Sudan.

Parallel Image Processing: Taking Grayscale Conversion Using OpenMP as an Example

In recent years, the widespread adoption of parallel computing, especially in multi-core processors and high-performance computing environments, ushered in a new era of efficiency and speed. This trend was particularly noteworthy in the field of image processing, which witnessed significant advancements. This parallel computing project explored the field of parallel image processing, with a focus on the grayscale conversion of colorful images. Our approach involved integrating OpenMP into our framework for parallelization to execute a critical image processing task: grayscale conversion. By using OpenMP, we strategically enhanced the overall performance of the conversion process by distributing the workload across multiple threads. The primary objectives of our project revolved around optimizing computation time and improving overall efficiency, particularly in the task of grayscale conversion of colorful images. Utilizing OpenMP for concurrent processing across multiple cores significantly reduced execution times through the effective distribution of tasks among these cores. The speedup values for various image sizes highlighted the efficacy of parallel processing, especially for large images. However, a detailed examination revealed a potential decline in parallelization efficiency with an increasing number of cores. This underscored the importance of a carefully optimized parallelization strategy, considering factors like load balancing and minimizing communication overhead. Despite challenges, the overall scalability and efficiency achieved with parallel image processing underscored OpenMP’s effectiveness in accelerating image manipulation tasks.

Nonlinear aspects of energy dissipation in wood-panel joints sara Casciati

The joints connecting vertical and horizontal elements are the ＂weak link＂ in structural systems assembled from wood panels. If they are too weak, local failures may occur, resulting in performance that is significantly below expectations. If they are too resistant, the joints may be unable to dissipate energy during vibrations, thus possibly initiating a fast progressive failure. This paper re-processes and re-elaborates the results of shaking table tests previously carried out by the author and other co-workers. The goal is to assess the feasibility of a joint which is able to dissipate energy during vibration, without degrading the connection performance.
